【発明の詳細な説明】
【0001】
【発明の属する技術分野】本発明は、被検体の頭部を固
定する頭部固定具に係り、特に磁気共鳴イメージング装
置(以下、MRI装置という)により撮影を行ないながら
手術や治療を行なうInterventional MR(以下、I-MRと
いう)に好適な技術に関する。

【従来の技術】MRI装置では、静磁場中に置かれた被検
体に対し高周波パルスを照射することで、被検体中の原
子核の核スピンが励起され核磁気共鳴信号が発生する。
このとき、傾斜磁場を所定のシーケンスで印加すること
により、核磁気共鳴信号は位置的な情報を与えられ、こ
の核磁気共鳴信号を再構成することにより被検体内の断
層像を得ることができる。MRI装置は、X線等による被曝
がなく、また傾斜磁場の印加の仕方により任意の断面像
を得ることができ、さらには内臓や脳等の軟部組織の分
解能に優れているため、現在様々な用途に用いられてい
る。
【0003】また、最近では、静磁場を発生するための
磁石(超電導磁石、永久磁石等)を上下に配置したオー
プン型のMRI装置を手術室内において、被検体を配置し
た空間の側方から手術や治療等を行なうI-MR手技が行な
われている。
【0004】例えば、頭部の手術を行なう場合、脳定位
固定具によって頭部(脳)を固定し、内視鏡や顕微鏡を
使用して腫瘍の摘出手術等が行なわれる。そして、途中
で腫瘍の摘出状況を確認するため手術を一時中断し、脳
定位固定具から頭部を外し清潔部分を保持するためのビ
ニール等を覆った受信コイルを巻き付けてMRI装置に移
動して撮影を行なう。

【発明が解決しようとする課題】上記従来技術では、手
術からMRI装置への撮影へ移動する際、脳定位固定具を
一度外さなければならず、被検体に影響を与えないよう
慎重な作業になり、手術から撮影、撮影から手術へ移行
する時間がかかるものであった。これに対し、特開2000
-300580号公報では脳定位固定具に受信コイルを取り付
けたものが開示されている。これによれば、脳定位固定
具から外すことなく受信コイルを取り付けることができ
る。
【0006】しかしながら、撮影毎に脳定位固定具に触
れる部分をビニール等で覆う作業が発生する共に、受信
コイルは脳定位固定具の外側を囲う形となり頭部に直接
巻き付けた場合に比べ感度が低いものであった。
【0007】本発明は、頭部を固定したままで受信コイ
ルの取り付けを容易に行ない手術から撮影、撮影から手
術への移行をスムーズに行なうための頭部固定具を提供
することにある。

【課題を解決するための手段】上記目的を達成するため
に、本発明は寝台に寝載された被検体の頭部を固定する
頭部固定具において、前記寝台に取付けられ被検体を固
定し一部を非磁性かつ導電性を有する材質で構成したも
のである。

【発明の実施の形態】本発明の実施形態を図を用いて説
明する。
【0010】まず、本発明が適用されるMRI装置の構成
を図1により説明する。手術室兼撮影室内にはMRI装置
1が配置されその近傍には被検体3を寝載する寝台2が
配置される。この寝台2は、撮影用の寝台と手術用の寝
台が兼用されるものであり、MRI装置1内に搬送されて
も影響のないよう非磁性体(ニッケルクロム合金やチタ
ン合金等)で構成されている。また、寝台2の端部には
被検体3の頭部を固定するための脳定位固定具8が取り
付けられている。手術の際は、MRI装置1の磁場の影響
がないところまで退避させて内視鏡(図示省略)や顕微
鏡4等を用いて行なわれる。内視鏡や顕微鏡4を使用す
る場合には、MRI装置1の磁場の影響がない所で行なわ
れるが、MRI装置1の磁場への影響を考慮して上述と同
様に非磁性体で構成されている。
【0011】このような被検体3が寝台2に寝載された
状態で手術等を行ない、その状況を確認するため寝台2
をMRI装置2内に搬送する。その際、脳定位固定具8に
後述する上部受信コイルを取り付け、脳定位固定具8を
含めた全体を受信コイルとして使用する。この脳定位固
定具8は磁場を乱さずかつ受信コイルとして使用するた
め良導電性である必要があり、本実施形態においては受
信コイルを形成する箇所にはアルミを使用している。そ
して、MRI装置1からアーム7を介して取り付けられた
インルームモニタ6によって、手術による摘出等の状況
を確認する。
【0012】次に、本発明の脳定位固定具並びに受信コ
イルの詳細について説明する。
【0013】まず、手術時における脳定位固定具の説明
を図2により行なう。被検体を寝台(図示省略)に寝載
させた状態で、被検体頭部9を脳定位固定具8に固定す
る。脳定位固定具8は、被検体頭部9の形状に沿うよう
な半円状のベース部16と、ベース部16を円周方向の
任意の角度で保持する保持部17と、保持部17を被検
体体軸方向に平行移動可能に保持すると共に寝台へ固定
する固定部18で構成される。ベース部16には所定の
間隔を置いて複数個のねじ穴が設けられており、ねじ穴
には被検体頭部9と接触して固定するボルト10を備え
る。また、ベース部16は後述するように受信コイルを
形成させるため非磁性でかつ導電性の良いアルミで構成
され、それ以外の保持部17、固定部18はベース部1
6と絶縁するため非磁性かつ非導電性のカーボンファイ
バーやグラスファイバー等で構成される。また、ボルト
10も非磁性である必要があるが、ある程度の強度が必
要となるためチタン合金等を用いる。その際、ボルト1
0とベース部16とは電気的に非導通にしておく必要が
あるため、ボルト10若しくはベース部16のねじ穴を
絶縁体で鍍金、塗布等で覆うようにすればよい。手術の
内容や頭部の位置、向きに応じて保持部17、ベース部
16の位置を調整し、またボルト10で固定する位置や
個数を適宜調整する。被検体9の頭部がボルトで固定さ
れた状態で、被検体頭部9と脳定位固定具8を滅菌した
布12で覆い、無用な接触によって感染しないよう清潔
部分を保つ。また、ベース部16の端部にはコネクタ1
1を設け、布12にはコネクタ11が貫通する穴を設け
る。そして、端部に貫通孔を設けたヘッドフレーム13
をコネクタ11に差し込み脳定位固定具8に固定する。
ヘッドフレーム13は被検体頭部9を開頭したさいの皮
膚を固定しておいたり、術者の腕受けとして使用する。
ヘッドフレーム13を固定すると、布12がヘッドフレ
ーム13と脳定位固定具8の間に挟まれるため、不用意
に外れることはない。この状態で、腫瘍の摘出等の手術
を行なう。
【0014】次に、撮影時における脳定位固定具の説明
を図3により行なう。被検体9は上述のように脳定位固
定具8に固定されており、この状態で半円状の上部受信
コイル15を滅菌された中継コネクタ14を介してコネ
クタ11に取付ける。コネクタ11及び中継コネクタ1
4は両者とも電気的に接続させるものである。前述のよ
うに非磁性かつ良導電性のアルミで構成された脳定位固
定具8のベース部16が下側の受信コイルを形成するた
め、上部受信コイル15とベース部16をコネクタ11
並びに中継コネクタ14で接続することにより被検体頭
部9全体を囲うループ状の受信コイルが形成される。こ
の状態で撮影を行ない、腫瘍の摘出状況等を確認する。
そして、撮影が終了し再度手術を行なう場合、上部受信
コイル15並びに中継コネクタ14を取り外して手術に
移行する。また、再度撮影を行なう場合には、中継コネ
クタ14を滅菌済の他の中継コネクタ14に変えて上部
受信コイル15を取付ける。
【0015】以上のように、本実施形態では脳定位固定
具8のベース部16をアルミで構成し、コネクタ11、
中継コネクタ14を介して上部受信コイル15と接続す
ることで、ループ状の受信コイルを形成することができ
るため、被検体を移動させなくともコイルを装着できる
と共に、受信コイルの径をより被検体に近付いた位置に
配置できる。
【0016】なお、脳定位固定具8のベース部16全体
を非磁性かつ良導電性のアルミで構成させたが、ベース
部16を非磁性のもので構成しベース部16の内部に非
磁性かつ良導電性のものを入れて構成してもよい。

【発明の効果】本発明によれば、頭部固定具の一部を非
磁性かつ導電性のよい材質のもので構成したため、頭部
固定具に導電体を接続することで頭部固定具を含めた全
体を受信コイルとして構成させることができる。これに
より、被検体の移動をせずに受信コイルを取付けること
