酸化窒素シンターゼの選択的阻害剤を同定する方法発明の分野
本発明は、酸化窒素シンターゼ(NOS)を選択的に阻害する化合物を同定する方
法に関する。それは、そのような化合物の治療法における使用にも関する。発明の背景
敗血症性ショックは、難治性の低血圧であり、米国で毎年100,000を超える生
命を奪う。血管拡張は、急性全身性炎症に対する心血管系の遍在性で保存性の反
応であり、敗血症性ショックに関連する低血圧の主要な原因である。酸化窒素(N
O)の過剰産生は、齧歯類およびヒトの両方において、敗血症性ショックの病態生
理に関連している。NO合成は、酵素である酸化窒素シンターゼ(NOS)によって触
媒される。3つの公知のタイプの哺乳動物NOSがある:誘導性NOS(iNOS)、内皮性
NOS(eNOS)およびニューロン性NOS(nNOS)。
齧歯類細胞では、iNOSの発現は、細菌内毒素または特定の炎症性サイトカイン
に反応して血管平滑筋細胞で容易に起こり、実験的な敗血症性ショックで見られ
る低血圧の大きな理由となる。齧歯類モデルでiNOSが関連する
証拠にもかかわらず、ヒト血管細胞および組織における機能的に活性なiNOSの再
現性のある持続性の誘導は、インビトロでは達成されていない。
NOの過剰産生はヒトの敗血症性ショックの病態生理に関連し、iNOSは薬剤療法
の標的であるけれども、iNOSが、敗血症の患者の血管系で発現されるという直接
的証拠はない。さらに、敗血症患者のNOxの血漿濃度は、齧歯類モデルで見られ
る10倍上昇と比較すると、約0.5倍の上昇である。これらの観察は、異なる種で
のiNOS間の重要な機能的差異に関する分子的証拠によって支持される。発明の概要
本発明は、NO生成の増加によって引き起こされるヒトでのサイトカインに誘発
される血管拡張が、eNOSの活性化によるものであり、iNOSの誘導によるものでは
ないという驚くべき発見に基づく。この現象は、NOSの3つの公知のイソ型に関
する補助因子である、プテリンであるテトラヒドロビオプテリン(BH4)レベルの
増加をもたらすGTPシクロヒドロラーゼ1の転写調節の帰結としてあるように見え
る。BH4レベルの増加は、次に、eNOS活性の増加をもたらす。
NOSsによりNO生成がもたらされる生理学的経路がプテリン濃度の変化によって
調節され得るという発見は、敗
血症性低血圧および炎症性血管拡張を治療する薬剤をデザインするための具体的
な暗示を有する。それはまた、それによって過剰のNOが、ヒトの全身性炎症の際
に複数の細胞タイプで、並びにプテリン合成が変化する広範囲の炎症性または腫
瘍性条件で生成される経路に、より一般的に関連するかもしれない。
従って、本発明は、酸化窒素シンターゼ(NOS)を選択的に阻害する化合物を同
定する方法を提供し、該方法は、
(a)候補化合物およびBH4の第1濃度の存在下にNOS活性の阻害を測定すること;
(b)候補化合物およびBH4の第2濃度の存在下にNOS活性の阻害を測定すること;
(c)候補化合物が(a)においては(b)においてよりも異なる程度までNOS活性を阻害
するかどうか決定すること、
を包含する。
好ましくは、工程(a)は、
(i)候補化合物の非存在下およびBH4の第1濃度の存在下に、該NOS活性を測定す
ること;
(ii)候補化合物およびBH4の第1濃度の存在下に、該NOS活性を測定すること;
(iii)工程(i)および(ii)における該NOS活性を比較すること、
を包含する。
好ましくは、工程(b)は、

工程(b)で使用されるBH4の第2濃度は、好ましくは、工程(a)で使用されるBH4
の第1濃度よりも低い。NOSは、好ましくはヒトNOS、より好ましくはヒトeNOSで
ある。
酸化窒素シンターゼ活性を選択的に阻害する、本発明方法で同定される化合物
は、酸化窒素の過剰産生が関わる様々な状態、例えば、敗血症性ショック、喘息
、関節炎、炎症性腸疾患、心不全および急性全身性炎症を治療するのに使用され
得る。さらに、このアプローチは、nNOSが関連する神経学的な疾病状態(例えば
、発作、痴呆およびパーキンソン病)の治療に価値があるかもしれない。従って
、本発明は、ヒト酸化窒素シンターゼを選択的に阻害するのに使用される医薬製
造における、本発明方法で同定される化合物の使用も提供する。
本発明は、さらに、本発明方法で同定される化合物を、薬学的に許容される担
体または希釈剤とともに含む薬学的組成物を提供する。
NOSによるNO産生のサイトカインに誘発される刺激は、BH4合成の律速酵素であ
るGTPシクロヒドロラーゼ1活性の増加を介して、仲介されるように見える。BH4
は、NOS活性に必要とされ、従って、サイトカインに反応したBH4レベルの増加は
NOS活性の増大をもたらすので、GTPシクロヒドロラーゼ活性の阻害は、NOSの任
意のイソ型によるNO産生を阻止または減少させるかもしれない。
従って、本発明はさらに、ヒト酸化窒素シンターゼを阻害するのに使用される
医薬製造における化合物の使用を提供し、ここで、該化合物は、GTPシクロヒド
ロラーゼ1によるBH4合成を阻止または減少させ得る。好ましくは、該化合物は、
(i)GTPシクロヒドロラーゼ1を候補化合物と接触させること;および
(ii)該化合物によるGTPシクロヒドロラーゼに仲介されるBH4合成の阻害を決定す
ること
を包含する、アッセイ法によって同定される。発明の詳細な説明
用語「酸化窒素シンターゼ」(NOS)は、eNOS、iNOSおよ
びnNOSの全ての天然に生じる形態、ならびにNOS活性を保持する変異種、例えば
、iNOS遺伝子の更なるコピーによりコードされるiNOSの機能性形態または突然変
異誘発技術によって産生される変異種を含むことが意図される。NOSは、好まし
くは、哺乳動物起源のもの、例えば、齧歯類(ラットおよびマウスを含む)または
霊長類(ヒトなど)のものである。好ましくは、NOSは、ヒト起源のものである。
アッセイで使用されるNOSは、哺乳動物細胞抽出物から得られるか又は、組換え
的に、例えば、細菌、酵母あるいは哺乳動物細胞系および昆虫細胞系を含む高等
真核細胞から産生され得る。好ましくは、アッセイで使用されるNOSは、組換え
のものである(例えば、Charlesら、1996を参照)。
好適な候補化合物は、アルギニンのアナローグ、アミノグアニジンまたはその
アナローグ、グアニジン化合物、イソチオウレア、L-N6-(1-イミノエチル)リシ
ン(L-NMMA)、2-ニトロアリールおよび2-シアノアリール化合物またはδ-(S-メチ
ルイソチオウレイド)-L-ノルバリンの誘導体を含み得る。アッセイ方法論
本発明者は、異なる阻害剤に対するNOSの感受性が、生理学的プテリン濃度に
依存して変化し得ることを発見し
た。これは、BH4修飾イソ型を生じるNOSのコンホメーションに対するアロステリ
ック効果によるかもしれない。特に、実施例のように、本発明者は、サイトカイ
ン/内毒素の刺激によって引き起こされる生理学的BH4レベルの上昇がeNOSのアミ
ノグアニジンに対する感受性を増加することを見い出した。従って、最大の特異
的および臨床的効果を提供するために、治療方法に使用されるための可能性のあ
る阻害剤を、生理学的または病態生理学的に関連するプテリン濃度でスクリーニ
ングすることは、非常に重要である。
該アッセイ方法論の背後の原理は、特定の生理学的または病態生理学的に関連
するBH4濃度または濃度範囲で特定のNOSイソ型を阻害するが、他の濃度では減少
した阻害効果を有する化合物をスクリーニングすることである。実施例によると
、正常のBH4濃度で存在するヒトeNOSイソ型は、恒常的圧力を維持するのに必須
である。このイソ型を劇的に阻害するどの化合物も、患者に対して潜在的に有害
な効果を有する。しかしながら、例えば、炎症性サイトカインによって誘発され
る増加したBH4濃度で、eNOSは、恐らくアロステリックなコンホメーション変化
によって或る形態に変換され、該変化は難治性低血圧を生じるNO生成の原因とな
る。標的化される必要があるのは、
この後者の形態である。従って、好適な阻害剤は、例えば、炎症性サイトカイン
の作用の帰結として存在するBH4の病態生理学的濃度でeNOSを阻害するであろう
が、正常のBH4濃度ではeNOSを阻害しないであろう。
本発明方法の工程(a)は、BH4の第1濃度の存在下に、候補化合物によるNOS活
性の阻害を測定することを包含する。代表的には、アッセイで使用されるNOS量
は、10〜600pmol/分の比活性を有する。代表的には、アッセイで使用されるBH4
濃度は、1μM〜1000μM、好ましくは10μM〜1000μM、より好ましくは100μM〜1
000μMまたは500μM〜1000μM、最も好ましくは500μM〜1000μMである。代表的
には、アッセイで使用される候補化合物の濃度は、1μM〜1000μM、好ましくは1
μM〜100μM、より好ましくは1μM〜100μMである。
一般に、NOS活性の阻害は、もしあるとして、候補化合物の非存在下に、しか
しBH4の第1濃度の存在下に、NOS活性を先ず測定することによって測定される。
これは、アッセイ条件下で、NOS活性に関するコントロール値を提供する。次に
、NOS活性を、候補化合物およびBH4の第1濃度の両方の存在下に、測定する。候
補化合物によるNOS活性の阻害は、コントロール値を、候補化合物の存在下に得
られる値と比較することによって測定され得る。こ
れらのアッセイ工程は、NOSを、候補化合物および/または適切な場合にはBH4と
接触させることによって為される。反応の範囲および/または速度は、定量的、
半定量的または定性的に測定され得る。もし、例えば、迅速なスループット・ス
クリーニング・アッセイ形式が要求される場合、候補化合物の非存在下のコント
ロール反応は、テストされる全ての候補化合物に為される必要はない。
NOS活性は、多くの方法(Packer、1996を参照)で測定され得る。例えば、NOS活
性は、放射性標識されたアルギニンのシトルリンへの変換によって直接的に、ま
たはNO産生をアッセイすることにより間接的に測定され得る。NOのアッセイは、
化学的アッセイまたはバイオアッセイを含む。NOの代表的な化学的アッセイは、
NOがオキシヘモグロビンをメトヘモグロビンに酸化するとき、405nmおよび420nm
での吸光度の変化を利用する。代表的には、反応混合物は、3μMオキシヘモグロ
ビン、200μM CaCl2(eNOSおよびnNOSに関して)、1mM MgCl2、1μM FAD、1μM FM
N、100μM NADPH、0.1μMカルモジュリン、30μM L-アルギニン(NOSの基質)、1
μM〜1000μM BH4および100μM DTT;100mM HEPES緩衝液、pH7.4中、および250
μlの最終反応体積中(Charlesら、1996)を含む。このアッセイは、マイクロタイ
ター・プレート形式で使用されるの
に適している。
3つのイソ型の全てが、活性のために、L-アルギニン、NADPH、FMN、FAD、BH4
およびカルモジュリンを必要とする。eNOSおよびnNOSは、カルシウムも必要とす
る(Charlesら、1996)。
本発明方法の工程(b)は、NOS活性がBH4の第2濃度の存在下に、候補化合物に
より阻害される程度を測定することを包含する。この工程は、使用されるBH4の
濃度が工程(a)で使用されるものとは異なり、好ましくはより低いものであるこ
とを例外として、工程(a)と同様である。代表的には、アッセイで使用されるNOS
量は、10〜600pmol/分の比活性を有する。代表的には、アッセイで使用されるBH4
の第2濃度は、1μM〜1000μM、好ましくは1μM〜100μM、より好ましくは1μM
〜10μである。代表的には、アッセイで使用される候補化合物の濃度は、1μM〜
1000μM、好ましくは1μM〜100μM、より好ましくは1μM〜10μMである。一般
に、定常的な阻害は、15〜30分間のインキュベーション後に測定される。
本発明方法の工程(c)は、候補化合物が、NOS活性を、(a)では(b)においてより
も異なる程度まで阻害するかどうか測定することを包含する。これは、コントロ
ール値を第2の値と比較して示されるように、工程(a)での阻害
を、コントロール値を第2の値と比較して同様に示される工程(b)での阻害程度
と単純に比較することによって達成され得る。従って、例えば、候補化合物は、
eNOSを、高い濃度のBH4の存在下では70%阻害し得るが、低い濃度のBH4の存在下
では5%しか阻害しない。そのような化合物は、それが恒常的圧力を維持するのに
関連するeNOSの正常な生理学的形態を阻害しないが、炎症性サイトカイン/内毒
素によって引き起こされる高レベルのBH4存在下で生成されるeNOSのイソ型は阻
害するので、有用な治療剤となるであろう。
換言すれば、阻害剤の存在下でのNOS活性は、BH4の濃度に依存する(反比例す
る)。阻害剤は、好ましくは、異なるNOSのイソ型、例えば、同じ生物に由来する
か異なる生物に由来するeNOS、iNOSおよびnNOSに関して選択的である。例えば、
阻害剤は、ヒトeNOSを阻害し得るが、ヒトiNOSを阻害しない。
数値形態の例によって、より高いレベルのBH4での阻害化合物によるeNOS活性
の阻害は、より低いレベルのBH4での阻害化合物によるeNOS活性の阻害よりも、
代表的には、少なくとも20%、好ましくは少なくとも30%、より好ましくは少なく
とも40、50、75、90または95%高い。代表的には、より高い及びより低いレベル
のBH4に関して意図され
る実際的濃度は、上記で議論されている。
心血管疾患の治療においてNOS阻害に使用する候補化合物をスクリーニングす
るとき、eNOSをアッセイで使用することが好ましい。反対に、神経学的疾患の治
療においてNOS阻害に使用する候補化合物をスクリーニングするとき、nNOSをア
ッセイで使用することが好ましい。
他のタイプのアッセイは、WO95/34534に記載されるもののようなインビボ・ア
ッセイを含み得る。例えば、ラット大動脈輪(aortic ring)でのin situでのeNOS
およびiNOSの阻害は、NOシンターゼ阻害によって生じる輪圧力(ring tension)の
増加を測定することによって評価され得る。基礎的緊張(basal tone)を研究する
ために、無傷の内皮を有する胸大動脈の輪を、代表的には、Reesら(1989)に記載
されるように調製し、累積性濃度曲線を、閾値濃度のフェニレフリン(ED10=10nM
)の存在下に、阻害剤について得る。誘発された平滑筋緊張を研究するために、
代表的には、約ED90のフェニレフリン存在下に、Reesら(1990)に記載されるよう
に、LPS(S.thphosaからの0.1μg/ml)に6時間暴す。この時間の間、緊張の漸進
的な損失が、通常、iNOS誘発ゆえに起こる。一般に、累積性濃度曲線が、次に阻
害剤に対して得られる。
eNOSおよびiNOSのインビボでの阻害は、正常または内
毒素性ショック下の意識のあるマウスのいずれかで、血圧に対する阻害剤の効果
によっても評価され得る。例えば、一つの好適な方法は、次の通りである。マウ
スを、イソフロウラン(isoflourane)(2%)で簡単に麻酔する。カニューラ・ライ
ンを、大腿静脈にインプラントし、背中の頂部で出るように皮下にトンネルを作
り、血圧を連続的にモニターすること及び阻害剤投与それぞれのために、旋回つ
なぎシステム(swivel tether system)に接続する。手術から回復した後、平均血
圧が正常範囲(90-110mmHg)にある動物を使用して、さらなる治療をしないか(「
正常マウス」)またはショックを誘発するためにリポポリサッカリド投与(E.coli
026:B6からの12.5mg/kgのLPSを静脈内に30秒かけて)した7時間後に(「ショッ
ク状態のマウス」)のいずれかで、血圧に対する阻害剤の累積性濃度曲線を得る
。NOS活性の阻害剤を、実施例に記載されるようにインビボで(或いは、実際には
インビトロで)測定するために、他の適切なアッセイが使用され得ること、およ
び上記アッセイが実施例であることが認識される。
本発明者は、NOSによるサイトカインに誘発されるNO産生の刺激が、BH4合成に
おいて律速酵素であるGTPシクロヒドロラーゼ1活性の増加を介して仲介されるよ
うに見えることも見い出した。BH4はNOS活性に必要とされ、従っ
て、サイトカインに反応したBH4レベルの増加は増大したNOS活性を生じるので、
GTPシクロヒドロラーゼ活性の阻害は、NOSによるNO産生を阻止または減少させ得
る。従って、GTPシクロヒドロラーゼ1活性を阻害する化合物は、BH4産生の阻害
または減少の帰結として、NOS活性も阻害し得る。
一般に、GTPシクロヒドロラーゼ1の阻害は、もしあるとして、候補化合物の非
存在下に、GTPシクロヒドロラーゼ1活性を先ず測定することによって測定される
。これは、アッセイ条件下で、GTPシクロヒドロラーゼ1活性に関するコントロー
ル値を提供する。次に、GTPシクロヒドロラーゼ1活性を、候補化合物の存在下に
測定する。候補化合物によるGTPシクロヒドロラーゼ1活性の阻害は、コントロー
ル値を、候補化合物の存在下に得られた値と比較することによって決定され得る
。このアッセイは、GTPシクロヒドロラーゼ1を、候補化合物と接触させることに
よって為される。反応の程度および/または速度は、定量的、半定量的または定
性的に測定され得る。もし、例えば、迅速なスループット・スクリーニング・ア
ッセイ形式が要求される場合、候補化合物の非存在下でのコントロール反応は、
テストされる全ての候補化合物に為される必要はない。
代表的には、アッセイで使用されるGTPシクロヒドロラーゼ1の量は、1〜10ng
の純粋タンパク質である。代表的には、アッセイで使用される候補化合物の濃度
は、1μM〜1000μM、好ましくは1μM〜500μMまたは1μM〜100μM、より好ま
しくは1μM〜10μMである。
GTPシクロヒドロラーゼ1活性は、例えば、BH4産生またはGTP消費を評価するこ
とにより測定され得る。BH4産生は、例えば、ヨウ素を用いるビオプテリンに対
する酸化またはHPLC(移動相は、1.5ml/分、10ミクロンODS逆相カラム上(4.6mmx2
5cm)で5%メタノールでランされ、350nmでの励起および410nmでの放出を有する)
を用いるビオプテリン測定によって測定され得る。治療的使用
例えば、細菌内毒素の影響の帰結としての酸化窒素の過剰産生は、潜在的に致
命的な血管拡張および低血圧をもたらし得る。本発明方法によって同定される化
合物による酸化窒素シンターゼの選択的阻害は、この潜在的に致命的な反応を緩
和するのに有用であり得る。本発明方法によって同定される化合物は、NOの過剰
産生が関連する状態、例えば、敗血症性ショック、喘息、慢性関節リウマチ、炎
症性腸疾患、心不全または急性全身性炎症を治療的に処置するのに使用され得る
。NOSは、発作、痴呆
およびパーキンソン病を含む様々な神経学的疾患状態にも関連する。
同様に、GTPシクロヒドロラーゼ1によってBH4合成を阻害または減少させる化
合物は、NOSが関連する上述の状態を治療的に処置するのにも使用され得る。好
ましくは、該化合物は、eNOSが関連する疾患を治療するのに使用される。
剤型は、同定される化合物の性質に依存するが、代表的には、該化合物は、そ
れらを薬学的に許容される担体または希釈剤と混合することにより臨床的投与の
ために剤型化され得る。例えば、それらは、局所的、非経口的、静脈内、筋肉内
、皮下、眼内または経皮的投与用に剤型化され得る。好ましくは、該化合物は、
注射可能な形態で使用される。それは、従って、注射可能な剤型用に、好ましく
は治療される部位での直接的注射用に、薬学的に許容される任意のビヒクルと混
合され得る。薬学的に許容される担体または希釈剤は、例えば、滅菌もしくは等
張溶液であり得る。その化合物を経口的に活性な形態で剤型化するのも好ましい
。
使用される化合物の用量は、様々なパラメーターに従って、特に、使用される
化合物、治療される患者の年齢、体重および状態、使用される投与様式および要
求される
臨床的生活規制に従って、調節され得る。指針として、注射により投与される化
合物の量は、好適には、0.01mg/kg〜30mg/kg、好ましくは、0.1mg/kg〜10mg/kg
である。
記載される投与経路および投与量は、熟練した開業医が、任意の特定の患者お
よび状態のための至適な投与経路および投与量を容易に決定できるので、指針と
してのみ意図される。
