【発明の詳細な説明】
粒状組成物
本発明は、非経口投与が可能な粒状組成物、特に造影剤、例えば、核磁気共鳴
画像診断法(MRI)、X線、超音波、光画像または核画像の造影剤、特にX線
造影剤、さらに特に小胞を含有するX線造影剤に関する。
造影剤は、得られる画像のコントラストを増強するために、例えば、異なる臓
器間もしくは正常組織と病変組織の差の識別を助けるために、画像診断療法(例
えば、CT、MRI、超音波等)において広く使用されている。
X線画像診断の分野において、市販の造影剤は、主に次の2つに分類される。
すなわち、胃腸(GI)管の画像診断のための不溶性無機重金属塩類(例えば、
バリウム硫酸塩)、および非経口投与用もしくは同じくGI画像診断用の可溶性
ヨウ化有機化合物(通常、トリヨードフェニルモノマー類およびダイマー類、例
えば、イオヘキソールおよびイオジキサノール)に分類される。
親水性で小分子量の性質から、可溶性ヨウ化有機化合物類は、非経口投与後の
体内分布および生体からの排出パターンがどれも類似しており、また、腎臓から
排出される前の比較的早いうちに細胞外液(ECF)部に分布する。したがって
、患者の不快感および忍容性の点でより良好であることから非経口投与としては
、非イオン性化合物(例えば、イオヘキソール)がイオン性化合物(例えば、メ
トリゾエート)より著明に好ましいにもかかわらず、それらの臨床的指示は同じ
である。
しかし、臓器特異性を有する新しい造影剤、特に、肝臓(肝臓造影剤)、血管
系(血液貯留造影剤)およびリンパ節(リンパ造影剤)等の特定の臓器のコント
ラストを増強する標的物質への需要が高まっている。
肝臓の需要に応えるために、粒状性の性質をもつために網内皮系によって血液
から抽出され、肝臓に蓄積される粒状造影剤を製造するための様々な試みがなさ
れてきた。血液貯留造影剤の需要に応えるために、ヨードフェニル造影剤に高分
子量の物質もしくは構造を付与し、その血中半減期を延長してきた。リンパ節画
像診断については、粒状造影剤を局所投与(例えば、皮下投与)する試みがなさ
れてきた。しかし、粒状造影剤を静脈投与してリンパ節を画像化することも可能
である。
有望な肝臓造影剤としては、生分解性粒子(WO89/00988およびWO90/07491
参照)、リポソーム含有X線造影剤(WO95/26205参照)および乳濁液(EP-
A-294534参照)に基にした造影剤が製造されているが、いずれもまだ市販され
ていない。したがって、効率性、安定性をもつ非毒性の臓器特異的なX線造影剤
を達成する必要がある。
小胞を含有する造影剤、例えば、WO95-26205に記載のリポソーム造影剤は、
動物実験において有望な肝臓画像診断結果を示し、前臨床試験において良好な安
全性プロファイルを示している。しかし、診断的に望ましい用量を用いたヒト志
願者での試験では、程度は低いが不快な副作用が発生した。
したがって、リン脂質リポソーム(平均(Z平均)粒径約350nm、組成物の体
積全体の約40%を構成)に取り込まれ、ほぼ同濃度の前記リポソーム用水性懸濁
媒体に溶解した80mg I/mLの水溶性非イオン性X線造影剤を有するリポソーム造
影剤(用量70〜100mg I/kg(体重))は、発熱、寒気、頭痛、吐気および嘔吐と
いった副作用を引き起こした。これらの副作用は、患者に投与した粒子数に関連
するとの報告がなされている。
本発明は、診断的効果のある用量のヨウ素では、そのような副作用をきたさず
、または、副作用を忍容レベルにまで減らした粒状造影剤を提供することを目的
とする。
例えば、より高濃度のヨウ素を使用して、および/または、より高容量の小胞
を使用して、組成物中に含まれるヨウ素の粒子数(個/mg)を減らすことによっ
て、本発明は上記目的を達成することができる。
したがって、一つの態様によれば、本発明は造影剤組成物、好ましくは水性媒
体の懸濁液中に小胞(例えば、ミセルもしくはリポソーム)を含むX線造影剤で
あって、前記小胞、および任意に前記水性媒体は、ヨウ化X線造影剤を含有し、
前記小胞に取り込まれた100mgのIを含有する前記組成物は、一用量に8×1012以
下の小胞(好ましくは、6×1012未満、特に好ましくは4×1012未満、さらに好ま
しくは2×1012未満、そしてさらに特に1012未満、例えば3〜9×1011、特に好ま
しくは、1×107〜9×1011未満、例えば1×107〜9.8×109の小胞)を含有するこ
とを特徴とする造影剤組成物を提供する。カプセル化されたヨウ素と小胞膜物質
(例えば、脂質)の重量比は、好ましくは1.8:1以上、例えば2:1以上、例えば
2〜5:1、より好ましくは2.5〜4.8:1、特に好ましくは3.0〜4.5:1である。前
記組成物において、前記小胞中のヨウ素含率は40%以上であることが好ましい。
カプセル化されたヨウ素の含有量は好ましくは30mg/mL以上(組成物全体に対す
る)、より好ましくは60mg/mL以上、特に好ましくは100mg/mL以上である。
さらに別の態様によれば、本発明は、コントラストが増強された画像、例えば
、X線画像を生成する方法であって、本発明の造影剤組成物をヒトまたはヒト以
外の動物(好ましくは、哺乳動物)の体に非経口(例えば、経静脈もしくは経動
脈)投与し、前記体の少なくとも一部、好ましくは少なくとも肝臓の画像、好ま
しくはX線(例えば、CT)画像を生成する方法を提供する。
本発明の組成物において、前記小胞は、ユニラメラであってもマルチラメラで
あってもよい。前記組成物は、バイラメラまたはトリラメラリポソーム、および
/または、ユニラメラ小胞を高いレベルで有することが好ましい。小胞(ここで
使われている小胞という語は、ユニラメラおよびマルチラメラの両方を示す)に
おいて、前記ヨウ素は、例えば、小胞核内にカプセル化することによって、およ
び/または、小胞膜内に組込むことによって、全体に、またはほぼ全体に小胞を
伴うことができる。
しかし、中に小胞が懸濁している水性媒体は、溶解したヨウ化有機造影剤を、
特に小胞の核内のヨウ素濃度またはそれに近い濃度で含むことが特に好ましい。
このように、前記造影剤は、3重の造影効果を有する、すなわち、前記小胞は、
血液貯留およびRES(例えば、肝臓やリンパ節)のコントラストを増強し、ま
た、水性懸濁液媒体中の前記ヨウ化造影剤は、血液貯留および他のECFのコン
トラストを作り出す。一般的に、前記組成物中のヨウ素濃度は、好適には100〜5
00mg I/mL、特に100〜460mg I/mL、さらに150〜420mg I/mL、特に200〜400mg I/
mL、さらに特に225〜370mg I/mLである。小胞でカプセル化された造影剤の濃度
は、組成物全体の体積に対して、40mg I/mL以上(例えば、40〜160mg I/m
L)、特に40〜100mg I/mL以上であることが好ましい。時間をかけて、もしくは
動的に肝臓を画像化するために投与された場合、その用量は、好ましくは体重1k
g当り60〜600mg I(合計)、好ましくは150〜500mg I/kg、より好ましくは200〜
400mg I/kgである。このようにヨウ素濃度を上昇させて、特に診断的効果のある
ヨウ素濃度で、注射もしくは注入、特にボーラス注射によって、造影剤を血管内
に投与する。
小胞の内部とその周りのヨウ素濃度が実質的に同じであることは、造影剤の製
造の簡略化、滅菌、特に高圧滅菌の点でさらに好都合である。なぜなら、取り込
まれていないヨウ素を除去する必要がなく、時間をかけた分散(したがって保存
性)がヨウ素の分布を大幅に変化させることがないからである。
本発明の造影剤の粒子数、例えば、mL当りの小胞数は、それ自体は重要なパ
ラメータではない。用量(つまり、特に患者の体重1kg当りの用量)当りの粒
子数が重要であり、したがって、本発明の特徴をなしているのである。
従来技術の非経口投与の粒状X線造影剤の副作用および撮像効果の低さは、肝
臓で粒状吸収システムが過剰に蓄積することから生じること、また、したがって
、肝臓のコントラストを増強させるために、忍容できない副作用をきたす程の吸
収飽和を引き起こすことなく、診断的効果のある用量のヨウ素を肝臓に送達する
ことが重要であると本発明者らは考えている。
本発明の組成物に使用されるX線造影剤は、好ましくはトリヨードフェニル化
合物、特に好ましくは非イオン性化合物である。モノマー類(例えば、イオヘキ
ソール)およびダイマー類(例えば、イオジキサノール)の両方を使用すること
ができる。好適なX線造影剤の例としては、WO-A-96/09282、WO-A-96/092
85に記載の化合物、ならびにイオジキサノール、イオペントール、イオヘキソー
ル、メトリザミド、メトリゾエート、イオベルソル、イオキシラン、イオキサグ
ラート、イオパミドール、イオトロラン、イオグリケート(ioglicate)、イオ
メプロール(iomeprol)、ヨーフェンジラート、イオプロミド、イオジミド(io
simide)、イオタースル(iotasul)、ヨータラム酸塩、イオトロキセート(iot
roxate)およびイオキシタラム酸等の化合物があげられる。しかし、イオヘキソ
ール、イオパミドールおよびイオペントール等の非イオン性モノマー類およびイ
オジキサノールおよびイオトロラン等の非イオン性ダイマー類がもっとも好まし
い。
カプセル化されたヨウ素を肝臓に輸送するための小胞として本発明の組成物中
で使用される小胞は、小胞膜形成物質、好ましくはリン脂質を用いて従来の方法
によって製造することができる。好適な膜形成物質の例がWO95/26205に挙げられ
ているが、ここにその開示を参考のために記載する。つまり、前記膜は、中性リ
ン脂質および荷電リン脂質の両方を含むことが好ましい。さらに、両方のリン脂
質がほぼ飽和していることが好ましい。
中性リン脂質は、少なくとも1つのほぼ飽和した脂肪酸残基を含むことが好ま
しい。そのような脂肪酸残基中の炭素数は、好ましくは15以上、より好ましくは
16以上である。そのような脂肪酸残基中の炭素数が14未満の場合、リポソームの
内部水相に対する保持能は低く、投与後の血中リポソームの安定性は低い。一方
、脂肪酸残基の炭素数が28以上の場合、生体適合性が低くなり、また、リポソー
ム製造中、高温を保つことが必要となる。
上で使用されている「ほぼ飽和した」という表現は、中性もしくは荷電リン脂
質の脂肪酸残基が完全に飽和していること(すなわち、炭素−炭素の二重結合を
含まない)、または、例えば、ヨウ素価が20以下、好ましくは10以下、もっとも
好ましくは5以下であることからわかるように、それらの不飽和度が非常に低い
ことを意味する。不飽和度が高すぎるとリポソームは容易に酸化する。
前記小胞膜は、荷電リン脂質、例えば、少なくとも1つのほぼ飽和した脂肪酸
残基を有するものを含むことが好ましい。そのような脂肪酸残基中の炭素数は、
通常14以上、好ましくは15以上、より好ましくは16以上である。理由は上に述べ
た通りである。
本発明の組成物中の小胞のゼータ電位の絶対値は、−50mV未満、とくに−58未
満、さらに−60未満、例えば、−50〜−70mVであることが好ましい。
本発明に有用な中性リン脂質としては、例えば、中性グリセロホスホリピド、
例えば、部分的もしくは完全に水素化された天然発生性(例えば、大豆由来もし
くは卵黄由来)または合成ホスファチジルコリン、半合成または合成ジパルミト
イルホスファチジルコリン(DPPC)、またはジステアロイルホスファチジル
コリン(DSPC)が挙げられる。
本発明に有用な荷電リン脂質としては、例えば、正電荷もしくは負電荷を帯び
たグリセロホスホリピド類が挙げられる。負電荷を帯びたリン脂質類としては、
例えばホスファチジルセリン、例えば部分的もしくは完全に水素化された天然発
生性(例えば大豆由来もしくは卵黄由来)または半合成ホスファチジルセリン、
特に半合成または合成ジパルミトイルホスファチジルセリン(DPPS)、また
はジステアロイルホスファチジルセリン(DSPS);ホスファチジルグリセロ
ール、例えば、部分的もしくは完全に水素化された天然発生性(例えば大豆由来
もしくは卵黄由来)または半合成ホスファチジルグリセロール、特に半合成また
は合成ジパルミトイルホスファチジルグリセロール(DPPG)、またはジステ
アロイルホスファチジルグリセロール(DSPG);ホスファチジルイノシトー
ル、例えば部分的もしくは完全に水素化された天然発生性(例えば大豆由来もし
くは卵黄由来)または半合成ホスファチジルイノシトール、特に半合成または合
成ジパルミトイルホスファチジルイノシトール(DPPI)、またはジステアロ
イルホスファチジルイノシトール(DSPI);ホスファチジン酸、例えば部分
的もしくは完全に水素化された天然発生性(例えば大豆由来もしくは卵黄由来)
または半合成ホスファチジン酸、特に半合成または合成ジパルミトイルホスファ
チジン酸(DPPA)、またはジステアロイルホスファチジン酸(DSPA)が
挙げられる。通常、そのようは荷電リン脂質は単独で使用するが、複数の荷電リ
ン脂質を使用してもよい。複数の荷電リン脂質を使用する場合、凝集を避けるた
めに、両方の荷電リン脂質を正に荷電することが好ましく、または両方の荷電リ
ン脂質を負に荷電することが好ましい。
正の電荷を帯びた脂質としては、例えば、アミノアルコールを含むホスファチ
ジン酸のエステル、例えば、ジパルミトイルホスファチジン酸またはヒドロキシ
エチレンジアミンを含むジステアロイルホスファチジン酸のエステルが挙げられ
る。
本発明によれば、中性リン脂質と荷電リン脂質の比率は、重量比で通常200:1
〜3:1、好ましくは60:1〜4:1、より好ましくは40:1〜5:1、例えば、約10:
1である。
小胞の平均サイズは、50〜4000nm、特に100〜3000nm、さらに150〜2000nmであ
ることが好ましい。そのサイズの上限は、毛細管のサイズの制約に部分的に影響
されるが、特に柔軟な膜を有する小胞の場合は、その範囲を超えてもよい。その
サイズの下限は、本発明の望ましい局面によれば、診断効果のある用量のヨウ素
を肝臓およびRESの他の部分に送達するのに必要とされる数の小胞を最小限に
するために決定される。小胞のサイズ選別は、適切な孔径のフィルターを通して
、より大きな小胞を抽出することによって(バイオ ケミバイオ フィジアクタ、
Biochem Biophys Acta 557:9(1979))、または重力もしくは遠心分離技術に
よって達成できる。
本発明の造影剤組成物は、膜生成物質およびX線造影剤の他に様々な任意の成
分を有してもよい。例えば、抗酸化剤としてビタミンE(α−トコフェロール)
および/またはビタミンE酢酸エステルを、脂質総量に対して0.01〜2モル%、
好ましくは0.1〜1モル%の量で含んでもよい。同様に、ソルビトールを張度(ton
icity)調整剤として含むことが好ましい(特に、非イオン性ダイマーをX線造影
剤として使用する場合重要である)。
上記リン脂質類を含有する小胞を含む診断用組成物の脂質の総量に対する濃度
は、前記小胞中の造影剤のカプセル化を高めるために、通常10mg/ml〜100mg/ml
、好ましくは15mg/ml〜90mg/ml、そしてより好ましくは20mg/ml〜80mg/mlである
。
そのような造影剤は、投与後、小胞が体内で安定に保持されるように、適切な
媒体中で、小胞中に等張液もしくは懸濁液の形状(体内での生理学的な浸透圧に
関係する)でカプセル化されることが好ましい。媒体として、水、TRIS−H
CL緩衝液、リン酸緩衝液、クエン酸緩衝液等の緩衝液等を使用することができ
る。
室温でのpH範囲は、4.5〜8.5であることが好ましい。6.8〜7.8であることが
より好ましい。造影剤が、多数の水酸基を有する非イオン性X線造影剤、例えば
、イオヘキソール、イオジキサノールもしくはイオパミドールである場合、緩衝
液は、US-A-4278654に記載のように負の温度係数を持つものであることが好まし
い。アミン緩衝液は、必要とされる特性、特にTRISを有する。この型の緩衝
液は、高圧滅菌温度でのpHがより低く、高圧滅菌中のX線造影剤の安定性を増
加させ、一方、室温で生理学的により良好なpHに戻る。
等張液または懸濁液を得るために、前記造影剤を、等張液を提供する濃度での
媒体に溶解もしくは懸濁させる。例えば、造影剤の溶解度が低いなどの理由で、
造影剤が単独で等張液を生成できない場合、等張液を生成するために他の非毒性
水溶性物質、例えば、塩化ナトリウム等の塩類や、マンニトール、グルコース、
スクロース、ソルビトール等の糖類を媒体中に添加してもよい。
WO95/26205にも開示されているが、本発明の組成物の1つの特に有利な点は、
高圧滅菌に耐えられることである。本発明の組成物は、カプセル化能力が高く、
また、それらが脂質組成物であることからカプセル化率が高い。
前記小胞は、マルチラメラリポソームの製造に使用される従来の方法によって
製造することができる。この方法としては、バンガム(Bangham)法(ジャーナ
ル オフ モル ダイアル(J.Mol.Dial.)13,238-252,1965)、多価アルコール
法(日本特許公報(公告)4-36734号)、脂質溶液法(日本特許公報(公告)4-3
6735号)および機械化学法(日本特許公報(公告)4-28412号)が挙げられる。
通常、望ましいマルチラメラ小胞は、前記リン脂質を、クロロフォルム、メタ
ノール、ジクロロメタン、エタノール等の揮発性有機溶媒、または、前記有機溶
媒と水との混合溶媒に溶解し、前記溶媒を除去し、得られた残留物と造影剤を含
有する水相とを混合し、その混合物を攪拌することによって調製することができ
る。
前記方法における溶媒除去工程として、バンガム法では蒸着を利用しているが
、噴霧乾燥もしくは凍結乾燥も利用できる。
前記小胞調製工程において、脂質に対する溶媒の使用量は重要ではなく、脂質
を溶解させ得る量であればよい。得られた脂質と溶媒の混合物からの溶媒の除去
は、従来の方法、例えば、減圧下、もしくは、必要であれば不活性ガスの存在下
で蒸着することによって行うことができる。実際、前記揮発性有機溶媒を、望ま
しくは、前記有機溶媒(体積10)と水(体積1まで)との混合物中で使用するこ
ともできる。
凍結乾燥によって溶媒除去を行うために、約0.005〜0.1Torrの減圧下で、その
溶媒の凍結温度より低い温度、通常−30℃〜−50℃で除去することができる溶媒
を選ぶ。必要であれば、トレハロースもしくはスクロース等の凍結防止剤を添加
してもよい。噴霧乾燥によって溶媒の除去を行う場合、通常空気圧を1.0kg/cm2
、空気流速を0.35cm2/分に調節する。吸気口の温度は、使用する溶媒の沸点より
高い温度に調節する。例えば前記溶媒にクロロホルムを用い、温度を60〜90℃に
調節することができる。尚噴霧乾燥は、従来の方法によって行うことができる。
このようにして得た脂質残基を、造影剤を含有する水溶液と、使用する脂質の
相転移温度(Tc)と等しいか、もしくはそれより高い温度のもとで混合し、続い
てその混合物を激しく、もしくはより穏やかに振とうし、または、前記Tcと等し
いか、もしくはそれより高い温度で攪拌し、造影剤を含有する水溶液中に懸濁し
た望ましい小胞を生成する。造影剤を含有する水溶液中の電解質イオン濃度は、
望ましくは、カプセル化の効率に悪影響をもたらさない範囲でできるだけ低くす
べきであり、通常、造影剤を離れる陽イオンと陰イオンの濃度の合計は、40mM以
下であることが望ましく、20mM以下であることが望ましい。
このようにして調製した小胞は、水性媒体(外液)中に懸濁物として存在し、
通常それ自身で診断用組成物として使用される。小胞の製造中、小胞内でカプセ
ル化されなかった造影剤の溶液が外液として存在する。また、その外液は、他の
液体に変えることが可能である。もっとも、造影剤の濃度は、外液と内液で同じ
であることが望ましい。いずれにしても、その外液(分散媒体)は、小胞の内側
の液体相と等しい浸透圧であることが好ましい。このようにして調製された懸濁
液の電解質イオン濃度は、できるだけ低いことが望ましく、通常、造影剤から離
れた陽イオンと陰イオンの合計濃度は、加熱殺菌および長期保存の際の小胞の安
定性を高めるために、約40mM以下であることが望ましく、約20mM以下であること
が好ましい。
前記小胞のカプセル化能力は、通常、5ml/g(脂質)以上であるが、6ml/g以上
であることが好ましい。本発明の方法によってヨウ化X線造影剤をカプセル化す
る際、水溶液原液中の造影剤の濃度は高いことが望ましい。例えば、200〜460mg
I/mL、さらに250〜400mg I/mLであることが好ましい。
本発明のもっとも好ましいX線造影剤組成物は、中性リン脂質(例えば、水素
化されたフォスファチジルコリン)および荷電リン脂質(例えば、水素化された
フォスファチジルセリン)を重量比約10:1で含有するリポソームを含む。その
ような組成物は、カプセル化能7ml/g以上をもち得る。そのような組成物を使用
したもっとも好ましいX線造影剤は、イオジキサノールおよびイオヘキソールで
ある。リポソームの内側と外側の水性媒体は、好ましくは約300〜400mg I(イオ
ジキサノール)/ml、ならびにソルビトール等の張度調整剤、EDTANa2Ca
等の安定剤およびTRIS緩衝剤(pH約7.4)を含むことが好ましい。
本発明では、小胞組成物に関して述べてきたが、非経口投与(特に、経静脈投
与及び経動脈投与)用の他の粒子含有水性組成物でもよい。ここで、その粒子は
X線造影剤、例えば、ヨウ化化合物であるか、またはそれを含有する。そのよう
な粒子は、懸濁液、例えば、水溶性液体X線造影剤の小滴であってもよく、また
は、それらは水溶性固体X線造影剤の固体粒状物であってもよい。そのような粒
子は、8×1012以下(好ましくは6×1012未満、特に好ましくは4×1012未満、さ
らに特に好ましくは2×1012未満、さらに1012未満、例えば、0.3〜4.5×1012小
胞)の粒子中に100mg Iの用量が含有されるようなヨウ素含有量とすべきである
。また、そのような、組成物は、望ましくは、水相に溶解したヨウ化X線造影剤
を、例えば、ヨウ素用量の合計を最高60%とするように含むことが好ましい。

以下の実施例および添付の図面によって本発明をさらに説明する。
図1は、本発明の調製物をラットに経静脈投与した4時間後の粒径(Dz)に
対する肝臓:脾臓分布率のプロットを示す。
実施例1の懸濁液は、実施例9〜12の懸濁液と同一とする。実施例2の懸濁液は
実施例13および17の懸濁液と同一とする。実施例5の懸濁液は、実施例10および1
4の懸濁液と同一とする。実施例11の懸濁液は、実施例15および19の懸濁液と同
一とする。実施例1(比較例)〜4
イオジキサノール溶液をビーカーに準備し、攪拌しながら95℃まで加熱し、透
明溶液を得た。もっとも濃度の高いイオジキサノール溶液を、多めの水に溶解し
、過剰分を後に蒸発させた。その溶液を暗室で一晩4℃で保存した。その後、張
度を調節するために、必要に応じてソルビトールを添加した。前記イオジキサノ
ール溶液を、80℃の水槽に連結した加熱ジャケットの付いたガラス製リアクター
に入れた。前記リン脂質混合物(水素化卵ホスファチジルコリンと水素化卵ホス
ファチジルセリンを重量比11:1で混合)を前記溶液に攪拌しながら添加した。
その分散液を攪拌しながら65〜76℃に加熱し、さらに20分以上攪拌した。その分
散液を高速ローターステーター(イーストラル(Ystral))を用いて13000rpmで
ホモジナイズし、その後、80℃の加熱ジャケットの付いた圧力タンクに移した。
その分散液を、7つの1.0μmポリカーボネートフィルターに通して3〜6Barで抽
出した。そのリポソーム懸濁液を50℃以下に冷却した後、TRIS/EDTA溶
液を添加し、その調製物を高圧滅菌した。
その組成の詳細を下記の表1および2に示す。
実施例2
1μmではなく3〜5μmの大きな孔径に通して抽出した以外は、実施例1と同様
にリポソームを作製した。粒径範囲の小さい粒子を遠心分離またはクロスフロー
濾過によって除去し、脂質比率3.6、平均体積径(mean volume diameter)0.95μ
