精神障害の診断及び治療をモニターするための方法及びキット 発明の分野
本発明は、精神障害(psychiatric disorders)の生化学的鑑別診断に係わり、
特に種々の精神障害の正確な診断において、そしてそのような障害の治療の経過
を正確に追跡するために、特定のタンパク質の機能及びレベルの測定を使用する
ことに関する。発明の背景
現在使用されている精神障害の分類のための鑑別診断法は主として現象学的な
ものである。診断は特定の徴候の観察と障害の経過に基づく。現在使用されてい
る精神障害の診断及び統計マニュアル(DSM IIIR)は、経験に基づいて判断され
ており、主として現象学的な運用基準を使用するものである。
精神障害の生物学的基盤を解読しようとする試みが多くなされてきた。精神障
害の病因論及びそのような障害の撲滅に使用される様々な生物学的治療のメカニ
ズムにおける、種々の神経伝達物質とそれらの受容体の関与について種々の仮説
が提唱されてきた。しかし、精神医学的診断を行うことができる生物学的または
生化学的な日常使用できる実験室的交差診断試験はいまだに利用できるものとな
っていない。精神医学における鑑別診断に役立つ生化学的性質の実験室的試験に
対する必要性には大きなものがあり、十分に認識されている。
精神障害の鑑別診断のための科学的に確立された生化学的アッセイに対する必
要性は広く認められており、それが可能になることは大きな利点を有する。その
ようなアッセイは、入院患者、外来患者、及び一般開業医により治療される患者
の鑑別診断の助けとなる。特に重要なのは、2つの主要な精神病、精神分裂症と
躁鬱病の間の最初の精神病症状の発現後の鑑別診断である。
例えば、現在公知の現象学的な診断法を使用すると、精神分裂症と明確に診断
する疾病特徴的徴候はない。従って、精神分裂症との最終的判断に到るために
は、最初の診断の後、少なくとも6カ月間の経過観察診断が必要である。
主要な精神病を鑑別することは、重要な予後及び治療的意義を有し、さらに多
くの非常に望ましい社会的な結果をもたらす。重度鬱病のような精神障害につい
ての生化学的アッセイの重要な点は、重度鬱病患者の半分を治療している一般の
開業医、及び精神衛生の専門家が、薬理学的な抗鬱薬治療の妥当性を決定するこ
とを可能にするということである。
通常一般の開業医によって最初に治療され、身体的障害を除外するためには一
連の理学的検査と費用のかかる一群の実験室的試験を通常必要とする別の障害と
して恐慌性障害がある。この障害について生化学的アッセイが確立されれば、現
在利用できる有効な薬理学的治療を使用してこれらの患者の早期治療に役立つ。
種々の特異的な療法が情緒障害に適用されている。リチウムのような情緒安定
剤及び電気痙攣療法(ECT)は、躁病及び鬱病の治療、及び両者の疾患状態の予
防に有効である。抗鬱薬は鬱病の治療と予防に使用される。これらの治療はいず
れも直ちにその治療効果を示すというわけではなく、3週間〜1カ月を要する。
患者の約30%は、特定の治療には応答しないが、別の種類の治療に応答する可能
性がある。従って、精神医学的治療に対する患者の応答を生化学的に調べること
を可能とする生化学的試験が存在することは特別な重要性を有する。
また、精神医学的患者において診断アッセイにより測定される生化学的パラメ
ータの変化が疾病の疾患状態の指標となり、有効な精神医学的治療の後に正常と
なるならば、そのようなアッセイは開業医がそのような治療の有効性を生化学的
に追跡し、測定するのに役立つ可能性がある。
精神障害における、シグナル伝達、特に受容体共役Gタンパク質のファミリー
のメンバーの関与の可能性が先行技術において指摘されている。例えば、白血球
膜調製物におけるβ-アドレナリン受容体の密度の減少が鬱病患者において認め
られ、減少したβ-アドレナリン受容体の応答性が、β-アドレナリン受容体によ
り剌激されたcAMP生産の測定により、鬱病に罹患する患者の白血球において
認められている。β-アドレナリン受容体の密度及び応答性のこれらの変化の程
度は、鬱病の重篤度と相関していることが判った(これについてはMazzola-Pomi
ettoら、1994を参照)。
β-アドレナリン受容体並びにその他の受容体を越えたシグナル伝達において
最も早期に認められる事象は、活性化された受容体のGタンパク質との共役を含
む。Gタンパク質のファミリーは、現在12〜15の公知の個々のタンパク質を含む
。G-タンパク質は、3つのサブユニット、α、β及びγからなる。α-サブユニ
ットはグアニンヌクレオチドの結合部位を含み、GTPアーゼ活性を有する。ま
たα-サブユニットは、細菌毒素により触媒されるニコチンアミドアデニンヌク
レオチド(NAD)依存性ADPリボシル化のための部位を含む。α-サブユニット
の不均質性により、G-タンパク質は主要なクラス(Gs、Gi、Gq)に分類され
る。
剌激ホルモンの受容体、すなわちβ-アドレナリン受容体は、アデニル酸シク
ラーゼを活性化するG-タンパク質、Gsと相互作用し、阻害リガンドのもの、例
えばM2-ムスカリン受容体はアデニル酸シクラーゼ阻害調節因子Giと相互作用
する。Gqは、ホスホリパーゼCによるホスファチジルイノシトール-4,5-ビホス
フェートの分解に到る受容体活性化に関与し得る。
アゴニストによる刺激作用は、グアニンヌクレオチドのGタンパク質への結合
を増加させ、その活性化を導く。この特徴に基づき、グアニンヌクレオチド結合
アッセイはGタンパク質機能についての確立された試験として使用されている。
受容体共役G-タンパク質の機能は、リチウム(Avissarら、1988)、その他の抗
双極性治療(antibipolar treatment)、及び抗鬱薬(Avissar及びSchreiber1992a,
b)により様々に弱まることが見いだされた。さらに、β-アドレナリン共役及び
ムスカリン共役G-タンパク質の機能亢進が躁病患者のMNLにおいて検出され(
Schreiberら、1991)、Gs-タンパク質のα-サブユニット(Gαs)の上昇したレ
ベルが双極性の患者の死後の大脳皮質において見出された(Youngら、1991)。
これらの刊行物はいずれも精神障害の診断のためにこれらの所見を使用する可
能性については言及しておらず、そしてこれらを合わせても、主要な精神障害の
鑑別診断の可能性についての示唆がないことは確かである。
本発明においては、受容体共役G-タンパク質の機能とレベルの測定を、精神
障害患者の鑑別診断のために使用し、さらに患者の精神医学的治療に対する応答
性の評価のために使用する。発明の概要
本発明によれば、(a)少くとも1種の受容体共役Gタンパク質の機能及び/ま
たはレベルを測定し、(b)(a)における測定に基づいて精神障害を診断するか、ま
たは患者に対する治療の効果を測定することを含む、精神障害を診断するための
、または精神病患者に対する治療の効果を測定するための方法を提供する。
本発明の好ましい態様によれば、受容体共役G-タンパク質の機能を、患者の
単核白血球(MNL)中のグアニンヌクレオチド結合能のアゴニスト誘発性増加
として測定する。
本発明の別の好ましい態様によれば、受容体共役Gタンパク質のレベルを、G
αsまたはGαiサブユニットに対する抗体を使用して定量する。
また本発明は、本発明の方法を実施するためのキットを提供する。

本発明は、以下のような主要な精神障害患者からのMNLにおける受容体共役
G-タンパク質機能及び免疫反応性レベルの異なるパターンを発見したことに基
づく。
躁病では、正常(対照)個体と比較してβ-アドレナリン及びムスカリン受容
体共役G-タンパク質(β-GP及びM-GP)機能が亢進し、Gαs及びGαiの免
疫反応性レベルが増加する。
鬱病では、正常(対照)個体と比較してβ-GP及びM-GP機能が低下し、Gαs
及びGαiの免疫反応性レベルが減少する。
精神分裂症では、正常(対照)個体と比較してドーパミン受容体共役G-タン
パク質(D-GP)機能が亢進し、β-GP及びM-GP機能のレベルは対照と同様で
あり、Gαs及びGαiの免疫反応性レベルは対照と同様である。
恐慌性障害では、正常(対照)個体と比較してβ-GP機能が宜進し、M-GP機
能は低下し、Gαs及びGαiの免疫反応性レベルは対照と同様である。
本発明はさらに、MNLにおける受容体共役Gタンパク質機能及びレベルの変
化が疾病の疾患状態の生化学的指標であり、治療された無症候性の患者は正常値
を示すという発見に基づく。
本発明は、G-タンパク質の機能的及び/または定量的な測定に基づいて精神
障害を区別することができる、生化学的交差診断アッセイを提供する。本発明は
また、精神医学的治療に対する患者の反応性を評価するための生化学的アッセイ
を提供する。
上記の「G-タンパク質の機能」は、アゴニストによって誘発されたGタンパ
ク質の活性化を意昧する。本発明の好ましい態様によれば、Gタンパク質機能は
グアニンヌクレオチド結合能のアゴニストによって誘発された増加として測定さ
れる。
本発明の方法を実施するための好ましいアゴニストは、イソプロテレノール、
カルバミルコリン及びドーパミンであり、これらはそれぞれβ-アドレナリン、
ムスカリン、及びドーパミン受容体を活性化する。但し、同様な種類の受容体を
剌激するその他のβ-アドレナリン、M2-ムスカリン、及びD1/5-ドーパミンア
ゴニストも適するものであり、それらの使用は本発明の範囲内にあるものである
。
本発明の別の好ましい態様によれば、Gタンパク質の上記の「定量的な測定」
あるいは「免疫反応性レベル」は、そのα-サブユニットの免疫反応性レベルと
して測定される。
本発明の別の好ましい態様によれば、本発明の方法によって診断される精神障
害は、躁病、鬱病、恐慌性障害及び精神分裂症である。
本発明の好ましい方法によれば、受容体共役G-タンパク質機能及びレベルは
患者からのMNLにおいて測定するが、本発明の方法を実施するためにはその他
の細胞の種類も適している可能性がある。例えば、その他の末梢血細胞または成
分(例えば血小板)あるいは皮膚細胞が診断に適し得る。しかし異なる細胞は受
容体の異なるレベル及び種類を示し得ることに留意する必要があり、従って、調
査は最初にすでに診断された患者の集団及び健康な被検者の集団において行い、
これらの目的について細胞の各種類の適合性を調べ、これらの異なる細胞を使用
する場合について方法を調整しなければならない。
本発明においては、1、2または3種の受容体共役Gタンパク質の機能の測定
を、精神障害の診断のために、あるいは精神医学的治療の効果を測定するために
適当かつ効果的に使用することができる。実際に、鬱病はβ-GPの機能低下に基
づいて診断することができ、躁病は高い値の特異性及び感度を示すM-GPの機能
亢進に基づいて診断することができる。しかしほとんどの場合においては、正確
な診断を可能にするために1種より多い受容体共役Gタンパク質の機能を測定す
べきである。このようなものとしては、躁病、鬱病及び恐慌性障害に対してはβ
-GP及びM-GP機能、精神分裂症の診断には、D-GP機能とβ-GPまたはM-GP
機能あるいはその両方が挙げられる。従って、患者の鑑別診断のためには、3種
すべての(β-GP、M-GP及びD-GP)の機能を測定することが好ましい。特定
の障害を診断するための試験の特異性は、最小限の要件と閾値を決定することに
よりコントロールすることができる。
しかし、精神医学的治療の効果をモニターするためには、通常は1種の受容体
共役Gタンパク質の機能、好ましくは疾病の急性の状態における機能が正常のレ
ベルから最も乖離するものの機能を測定することで十分である。評価は、治療の
前と治療の間に得られた値の比較に基づき、正常値へシフトすれば治療に対して
反応性であると言える。
本発明の好ましい態様によれば、受容体共役G-タンパク質機能をグアニンヌ
クレオチド結合アッセイによって測定する。しかし本発明の方法を実施するため
に、Gタンパク質のアゴニストによって誘発された活性化の測定を可能にするそ
の他の任意のアッセイを使用することもできる。
本発明の別の好ましい態様によれば、アッセイにおいて使用するグアニンヌク
レオチドはGpp(NH)pである。その他のグアニンヌクレオチドあるいは類似体が適
している可能性があり、それらの使用は本発明の範囲内にあるものである。その
ようなものとしては、例えばGTPγS(Wieland T.及びJakobs K.H.,1994)あるい
はGTP-アジドアニリド(UV光に曝されるとGタンパク質と不可逆的に結合する
光不安定性化合物、Laugwitz K.L.ら、1994)が挙げられる。
本発明のさらに別の態様によれば、アッセイにおいて使用するグアニンヌクレ
オチドを標識する。グアニンヌクレオチドの標識は、放射性標識、蛍光性標識等
のような、適当な検出手段により検出できる任意の種類の標識とすることができ
る。また、標識する代わりに、標識プローブを使用して結合グアニンヌクレオチ
ドを検出してもよく、この場合標識プローブは、非結合物質を分離しただけの後
にサンプルと接触させて、複合体Gタンパク質-グアニンヌクレオチドに特異的
に結合できるもの、あるいはそのグアニンヌクレオチド自体に特異的に結合でき
るものである。本発明の方法を実施するためには、Gタンパク質に対するグアニ
ンヌクレオチドの結合に比例する検出可能な変化を生じる任意の方法を利用する
ことができる。
アゴニストによって誘発された結合能の増加として測定されるG-タンパク質
の機能は、一般的手順及び実施例1に記載したように多点測定により計算するこ
とができる。しかし、グアニンヌクレオチドの単一の濃度での単点測定を使用し
てアッセイを単純化してもよい。Gpp(NH)pについては、5mMの濃度において、
多点測定と単点測定に基づいて同様な値が得られた。
また本発明による精神障害の診断あるいは精神医学的治療の効果の追跡は、G
α-サブユニットのレベルの測定に基づいてもよい。Gαs及びGαiの免疫反応
性レベルの測定は、最初にSDS-PAGEで分離したMNL膜タンパク質のイ
ムノブロット分析によって行った。しかし、日常行うアッセイではサンプルを先
に分離することなくGα-サブユニットの免疫反応性を測定することが好ましい
。Gα-サブユニットの異なる種類の定量的なイムノアッセイを膜の粗調製物に
ついて行うことができ(例えばLesch K.P.,Manjii H.K.,1992を参照)、それ
らの使用は本発明の範囲にあるものである。
また、本発明の方法のためのGαs及びGαiの免疫反応性レベルの測定はEL
ISA試験によって行ってもよい。競合ELISAに基づく方法を使用してラッ
トの脳からの膜におけるGα-サブユニットの種々の種類が定量されている(Les
ch K.P.及びManjii H.K.,1992)。しかし、ELISA試験のその他の形態
あるいはその他のイムノアッセイも使用することができ、それらの使用は本発明
の範囲内にあるものである。
また、本発明の方法を実施するために、Gタンパク質機能の測定には多くの種
類のものが可能である。例えば、結合反応の最後に、固定化抗体または磁性粒子
に結合させた抗体とサンプルを接触させることによって、グアニンヌクレオチド
-Gタンパク質複合体を捕捉することができる。これにより非結合物質の洗浄が
可能となり、サンプルを濾過する必要性が回避できる。種々の方法によるGタン
パク質-グアニンヌクレオチド複合体の免疫分離が開示されている(FriedmanE.ら
、1993)。その後結合グアニンヌクレオチドを、直接(それが標識されている場
合)または間接的に(Gタンパク質-グアニンヌクレオチド複合体に特異的に結
合できる標識プローブにより)検出することができる。
本発明の別の形態は、本発明の方法を実施するためのキットに関する。キット
は、Gタンパク質機能の測定、またはGαs及び/またはGαiの免疫反応性レベ
ルの測定に基づくものである。キットは、精神障害の診断または精神医学的治療
の効果の追跡のために使用することができる。
本発明の方法によるGタンパク質機能測定に基づく例示的なキットは、以下を
含む:
(a) (i)β-アドレナリン受容体
(ii)ムスカリン受容体
(iii)ドーパミン受容体
を含む群から選択される1、2または3種の受容体のアゴニスト、
(b)標識グアニンヌクレオチド
(c)キットを使用するための製造業者の説明書。
また、主要な精神障害の各々の診断のための基準を、製造業者の説明書に含め
てもよい。
本発明の方法によるGαs及び/またはGαi測定に基づく例示的なキットは、
以下を含む:
(a)抗Gαs及び抗Gαiを含む群から選択される1種または2種のモノクロー
ナルまたはポリクローナル抗体、
(b)(a)の抗体に特異的に結合できる検出可能なプローブ、
(c)標準サンプル、
(d)キットを使用するための製造業者の説明書。
標準サンプルは、結果を標準化し、正常な被検者の既知の平均値との比較する
ことを可能とするために含まれていなければならない。標準化のための詳細な説
明は、診断の基準とともに製造業者の説明書に含まれていなければならない。
本発明を以下の例示的で非限定的な実施例によりさらに説明する。一般的手順 患者:精神障害患者のサンプル及び健康な志願者の対照群
全ての患者は、DSM-IIIR基準に従って少くとも二名の経験のある精神科医によ
り診断された。理学的検査、心電図、及び腎臓、肝臓、血液及び甲状腺機能につ
いての実験室的試験において正常な結果を示すことを採用基準とした。鬱病のBe
ck調査(Beck Inventory of Depression)をさらに使用して患者及び健康な志願
者の評価を行った場合は明記した。患者は実験のために60mlの献血に同意したも
のである。患者は実験に紹介される前1か月の間に精神医学的治療を受けていな
かった。デポー剤抗精神病薬による治療歴を有する場合(精神分裂症患者のうち
の3人)は、紹介の前少くとも2か月はそのような薬物投与を受けていないこと
を確認した。
陽性の徴候を示す精神分裂症患者の群は13人の男性及び10人の女性の入院被験
者からなり、平均年齢は31.4歳(19-59)であり、急性、急性再燃を示す亜慢性
、または急性再燃を示す慢性と分類された経過を有する妄想型または解体型の精
神分裂症と診断された患者である。この群の患者は、陽性及び陰性症候群スケー
ル(Positive and Negative Syndrome Scale,PANSS)により調べて、主に陽性の
症候学的特徴を示した(合計PANSSスコア=92.8+5.5、陽性スケール=26.7±1.5)
。患者のうちの8人は、以前にドーパミンアンタゴニストによって治療されたこ
とがなく、最初の精神病の症状発現の間に調べられ、少くとも1年の臨床経過観
察の後に精神分裂症と診断されている。
躁病患者の群は13人の男性及び7人の女性の入院被験者からなり、平均年齢は
34.8歳(20-57)で、急性の躁病患者であった。
重度鬱病(単極性及び双極性)の未治療患者の群は、15人の女性と13人の男性
被験者からなり、平均年齢は42.7歳(20-68)であり、18人は外来患者で、
10人は入院患者であった。
恐慌性障害の外来患者の群は、広場恐怖症を有する患者を含み、6人の男性及
び7人の女性被験者からなり、平均年齢は37.5歳(23-55)であった。
健康な志願者の群は、17人の男性と13人の女性からなり、平均年齢は38.9歳(
20-77)であった。MNL単離
MNLは、Boyum(Boyum A.,1968)に従い、Ficoll-Paque勾配を使用して成人の
ドナーの60mlのヘパリン添加新鮮血液から単離した。細胞は、25mMトリス-HCl,
pH 7.4及び1mMジチオスレイトール(DTT)中でホモジナイズした。ホモジネー
トをチーズクロスの2つの層を通過させて破片を除去し、18,000gで10分間さら
に遠心分離することにより膜を回収した。その後、膜は機能的結合測定に新鮮な
まま使用するか、1mM ATP、1mM EGTA、2mMMg2+及び30%スクロースw/vを含む
ホモジナイゼーションバッファー中に懸濁し、定量的な測定によりアッセイする
まで-70℃で凍結させた。ブラッドフォードの標準アッセイを使用したタンパク
質濃度測定のためにアリコートを採取した。グアニンヌクレオチド結合アッセイ
グアニンヌクレオチド結合アッセイは、Avissar S.ら(AvissarS.ら、1988)に
従って行った。グアノシンβ、γ、イミドトリホスフェート(Gpp(NH)p)、すな
わちG-タンパク質により高いアフィニティを示すGTPの加水分解できない類
似体をグアニンヌクレオチド結合アッセイに使用した。
結合反応は3H-Gpp(NH)pの種々の濃度(0.05-5μM)で行った。膜タンパク質
の50μgのアリコートを、反応バッファー(25mMトリス-HCl、pH 7.4、1mM ATP
、1mM Mg2+、1mM EGTA及び1mM DTT)中に3H-Gpp(NH)pを種々の濃度で含む一
連のチューブに加え、200μlの最終容量として全結合を測定した。チューブを10
分間室温(18℃〜25℃)でインキュベートし、5mlの氷冷バッファー(10mMト
リス-HCl,pH 7.4、100mM NaCl)で反応を停止させた。サンプルをGF/Cワットマ
ンフィルターで濾過した。フィルターを2回、
3mlの冷却バッフアーで洗浄し、その放射能を測定した。各3H-Gpp(NH)p濃度に
おいて、100μM非標識GTPγSの存在下で非特異的結合を平行して測定した
。結合反応は3回ずつ行い、非特異的結合を全結合から減ずることによって特異
的結合を算出した。アゴニスト誘発結合
3種のアゴニスト、β-アドレナリンアゴニストのイソプロテレノール、ムス
カリンアゴニストのカルバミルコリン、及びドーパミンアゴニストのドーパミン
を使用した。イソプロテレノール(25μM)、カルバミルコリン(50μM)また
はドーパミン(50μM)を反応混合物に加えることによって3H-Gpp(NH)p結合に
対するアゴニストの効果を調べた。これらは、アゴニストの最大効果を示す最小
濃度を表す。アゴニスト誘発結合に対するアンタゴニストの効果 3H-Gpp(NH)p結合能のアゴニスト誘発増加に対するアンタゴニストの効果を、
アンタゴニストを以下に示す最終濃度になるよう反応混合物に加えることによっ
て調べた。1μMプロプラノロール(β-アドレナリンアンタゴニスト)、10μM
ADFX116及び10μMピレンゼピン(M2及びM1ムスカリンアンタゴニスト)、1
μM SCH23390及び1μMスルピリド(それぞれD1/5及びD2ドーパミン受容体ア
ンタゴニスト)。アゴニスト誘発結合に対するコレラ及び百日咳毒素の効果
MNL膜(3-4mg)を、25mMトリス-HCl、pH 7.4、10mM NAD、1mMATP、10mM
チミジン及び100μM GTPを含む1mlのバッファー中に懸濁した。20mM DTTにより
10分間37℃で予備活性化したコレラ毒素(20μg/ml)または20mM DTTにより10分
間30℃で予備活性化した百日咳毒素(10μg/ml)を加えることにより、ADPリボ
シル化を15分間30℃で行った。25mlの氷冷した25mMトリス-HCl、pH 7.4を添加す
ることにより反応を停止し、直ちに18,000gで10分間遠心分離した。その後、上
記したように3H-
Gpp(NH)p結合を行った。イムノブロット分析
膜を解凍し、SDS-(10%)ポリアクリルアミドゲル電気泳動でのタンパク質分離
のために10μgの膜のアリコートを採取した。電気ブロッティング装置によリタ
ンパク質をニトロセルロース紙へ移した。ブロットを3%Tween-20を含むトリス-
緩衝食塩水(TBS)で洗浄し、0.1%Tween-20を含むTBS(TTBS)中で5%BSAと1
時間インキュベートすることによりブロックした。TTBS中での2回の洗浄の後、
ブロットを、以下の抗血清(NEN-DuPont)、αsに対して特異的なもの(希釈度
1:2,500)、αI,1,2に対して特異的なもの(希釈度1:5,000)のそれぞれと
一晩インキュベートし、その後西洋ワサビペルオキシダーゼ標識ヤギ抗ウサギIg
Gとインキュベートした。免疫反応性をEnhancedChemiluminescence Western Blo
t Detection System(Amersham)で検出し、その後KodakX-Omatフィルムに露光し
た。免疫反応性バンドのピーク高さは画像分析システムで測定した。免疫反応性
バンドの光学密度は、標準値として各ブロットにおいて行った10μgラット皮質
膜に対して標準化した。健康な志願者からの2.5〜15μg膜タンパク質の範囲にお
いて、タンパク質濃度に関するアッセイの直線性が見られた。統計的分析
Dunn検定を使用して、患者MNLにおける受容体共役G-タンパク質機能及び免疫
反応量の、非同一サンプルサイズの単一コントロール群に対する非パラメーター
多重比較を行い、検定統計値をQ'の臨界値の表と比較した(Zar J.H.,1984)。実施例1:健康な志願者における受容体共役G-タンパク質機能
健康な志願者からのMNL膜への3H-Gpp(NH)pの結合を、種々の3H-Gpp(NH)p濃度
で、アゴニストなしに、あるいはイソプロテレノール、カルバミルコリンもしく
はドーパミンの存在下で測定した。特異的結合(全結合から非
特異的結合を減ずることにより算出)は5分以内に平衡に達し、少くとも30分間
定常状態のままであった。
アゴニストがない場合(白丸)、イソプロテレノールの存在下(白三角)、カル
バミルコリンの存在下(黒四角)及びドーパミンの存在下(黒丸)での健康な志
願者からのMNL膜に対する3H-Gpp(NH)pの特異的な結合の代表的な飽和曲線、
及び平行実験の非特異的な結合曲線(黒三角)を図1に示す。3種全てのアゴニ
ストによって誘発されたGpp(NH)p結合能の増加は結合のアフィニティに実質的に
影響を及ぼさない。
図2aは、健康な志願者からのMNL膜における基底及びアゴニスト誘発結合曲
線の代表的なスキャッチャードプロットを示す。特異的結合Gpp(NH)pの遊離Gpp(
NH)pに対する割合を、特異的結合Gpp(NH)pに対してプロットしたものである。最
大の結合能Bmaxは、外挿された線形回帰直線の水平の軸での切片から推定するこ
とができる。
健康な志願者被検者の全ての集団についての平均値を調べた。Bmax-
アゴニストが存在しない場合のBmax基底値は、40.3±1.0pmole/mgタンパク
質である。β-Bmax-
β-アドレナリンアゴニストイソプロテレノールの存在下におけるBmax
値は、50.9±1.9pmole/mgタンパク質(Q'=3.2p<0.01)である。M-Bmax
-ムスカリンアゴニストカルバミルコリンの存在下におけるBmax値は、52
.5±1.6pmole/mgタンパク質(Q'=3.57p<0.01)である。D-Bmax-
損Lドーパミンの存在下におけるBmax値は、54.9±2.0pmole/mgタンパク
質(Q'=4.34,p<0.01)である。
3種全てのアゴニストによって誘発されたGpp(NH)p結合能の増加は統計学的に
非常に有意である。
Bmax値は、年齢によっても(L.Barki-Harringtonら、1996、印刷中)、あるいは
身体的運動の後にも有意な変化を示さなかった。
Gpp(NH)p結合能のアゴニストにより誘発される増加は、
([アゴニストの存在下でのBmax]/[Bmax-基底レベル]-1)×100
で計算される。
健康な志願者群(黒丸)についてのイソプロテレノール(β)、カルバミルコ
リン(M)及びドーパミン(D)により誘発されたGpp(NH)p結合能の増加の散乱
プロットを図3dに示す。
Gpp(NH)p結合能のアゴニスト誘発増加の平均及び平均の標準誤差を計算したと
ころ、イソプロテレノールについては26.4%±l.3(白棒)、カルバミルコリンに
ついては28.3%±1.7(斜線を付した棒)、ドーパミンについては26.8%±1.9(黒
棒)であった(図4)。
表Iは、β-アドレナリン、ムスカリン及びドーパミンアンタゴニストの効果
、並びにコレラ及び百日咳毒素のGpp(NH)p結合のアゴニスト誘発増加に対する効
果を表す。コレラ毒素は、αサブユニットのリボシル化によりGsタンパク質を
阻害することが知られている。表1に示したように、ドーパミン及びイソプロテ
レノールによって誘発されたGpp(NH)p結合能の増加はコレラ毒素によリ消失し、
Gsタンパク質機能に対するドーパミン及びイソプロテレノールの効果を示唆し
ている。ヒトMNLは、Gsタンパク質を介してアデニリルシクラーゼ機能を活
性化する表面β-アドレナリン受容体を有することが知られている。グアニンヌ
クレオチド結合のドーパミンによって誘発された増加は、選択的なD1及びD5ア
ンタゴニストであるSCH-23390によって特異的にブロックされ、選択的なD2アン
タゴニストであるスルピリドによっては影響されなかった。D1及びD5受容体選
択性アゴニストSKF-383931μMはGpp(NH)p結合能の濃度依存的な増加を誘発し
、これはドーパミンによって誘発された増加(表示せず)と同様なものであつた。
D1及びD5は、Gsを介してアデニリルシクラーゼを活性化することが現在確認
されている唯一のドーパミン受容体である。しかし、D3及びD5受容体はヒト末
梢血リンパ球においてのみ発現されることがこれまでに示されている。従って、
ドーパミンはおそらくD5受容体を介してMNLにおいてGsタンパク質機能を生
じるのであろう。
表1に示したように、カルバミルコリンのGpp(NH)p結合に対する効果は百日咳
毒素前処理によって阻害されたが、コレラ毒素には影響されず、M2アンタゴニ
ストADFX116によって特異的にブロックされ、一方M1アンタゴニストピ
レンゼピンには影響されなかった。M2受容体はGi-タンパク質に共役する。Gi
-タンパク質は、そのαサブユニットのリボシル化を介して百日咳毒素により阻
害されることが知られている。従ってこれらの所見は、カルバミルコリン効果が
、アデニリルシクラーゼを阻害するGiに結合するM2受容体を介して発揮される
ことを示唆している。a3H-Gpp(NH)p結合能の統計学的に有意な増加は検出されなかった。実施例2:精神病患者における受容体共役Gタンパク質機能
また、受容体誘発G-タンパク質機能を種々の精神障害患者においてアッセイ
した。図2(a-e)は、健康な志願者(a)と比較して、躁病(b)、鬱病(c)、
精神分裂症(d)、及び恐慌性障害(e)の個々の患者におけるGpp(NH)p結合の代
表的な例を示す。結合は、基底条件下(白丸)、あるいはアゴニストであるイソ
プロテレノール(白三角)、カルバミルコリン(黒四角)もしくはドーパミン(
黒丸)((a)及び(d))のみ)の存在下でアッセイした。5人の個体間において基底
結合能の有意な変化は認められなかった。しかし各個の患者は、結合能のアゴ
ニスト誘発増加の特異的で異なるパターンを示した。
図3(a-d)及び図4は、種々の精神障害の患者の群における結合能のアゴニス
ト誘発増加の結果を要約したものである。健康な志願者(黒丸)と比較して、躁
病(3a)、鬱病(3b)、精神分裂症(3c)及び恐慌性障害(3d)を有する患者(
白丸)の散乱プロットを図3に示す。
イソプロテレノール(白棒)、カルバミルコリン(斜線を付けた棒)、及びド
ーパミン(黒棒)で誘発されたGpp(NH)p結合能の増加について平均及び平均の標
準誤差を計算したものを、躁病患者(C)、鬱病患者(D)、恐慌性障害患者(
E)、精神分裂症患者(F)、及び安静時(A)あるいは運動した後(B)の健康
な志願者について図4に示す。
患者の各グループは、受容体共役Gタンパク質機能の異なるパターンにより特
徴付けられる。対照被検者と比較すると、躁病の患者のMNLは、高いβ-アド
レナリン受容体共役Gタンパク質(β-GP)機能、及び高いムスカリン受容体共
役Gタンパク質(M-GP)機能を特徴とし、鬱病患者のMNLにおいては、β-
GP及びM-GP機能はいずれも低下していることが検出された。恐慌性障害の患
者のMNLは、高いβ-GP機能と低下したM-GP機能を特徴とした。上記に挙げ
た患者の群とは異なり、精神分裂症と対照被検者との間ではβ-GP及びM-GP機
能の相違は検出されなかった。これらの患者においては、正常な被検者と比較し
て、高いドーパミン受容体共役Gタンパク質(D-GP)機能が検出された。
神経弛緩性先天性精神分裂症の患者(23人の患者のうちの8人)におけるGpp(
NH)p結合能のアゴニスト誘発増加のパターンは、他の精神分裂症の患者のものと
同様であった。
強い身体的運動の後の正常の被検者は対照被検者と異なるGタンパク質機能を
示さなかったことから、患者におけるG-タンパク質機能の変化は単に運動活動
過剰あるいは活動低下だけによるものではない。
結果を表IIに要約する。 5mMでの特異的な結合(基底及びアゴニスト誘発)を、Gpp(NH)p結合能のアゴ
ニスト誘発増加を計算するための単点測定として使用した場合、同等の結果が得
られた。
経験的な知見を精神病患者の鑑別診断のために使用できる可能性がある。
G-タンパク質機能測定に基づく、可能性のある診断アッセイの感度及び特異
性は、あらかじめ決められた任意の閾値、及びあらかじめ決められた試験基準に
ついて有効性試験を使用して評価することができる。
例えば、Gタンパク質機能亢進と機能低下をそれぞれ反映する閾値として、Gp
p(NH)p結合能のアゴニスト誘発増加の≧45%及び15%≦の値を測定することによ
り以下の結果が得られた。
β-GP及びM-GPの両方の機能亢進を必要とする躁病の診断については、感度
は0.8、特異性は0.93であることが判った。
D-GPの機能亢進と正常なβ-GP及びM-GPを必要とする精神分裂症の診断に
ついては、感度は0.87、特異性は0.97であることが判った。
β-GP及びGPの機能低下を必要とする鬱病の診断については、感度が0.89、
特異性が0.90であることが判った。
β-GPの機能亢進と正常または機能低下したM-GPを必要とする恐慌性障害の
診断については、感度が0.75、特異性が0.93であることが判った。
結果の分析は、1(2つの場合においてのみ)、2または3種のアゴニストに
より得られた値に基づくことができ、患者と対照被検者において見られた値の分
布に従い、異なる閾値をアゴニストのそれぞれに使用することができる。アッセ
イの結果のその他の処理も可能であり、信頼できる診断をもたらし得る。例えば
、評価を、各パラメータに別々に基づくものではなく、β-GP/M-GPの比に基
づくものとすると、恐慌性障害の診断の感度及び特異性が上昇し得る。実施例3:精神病患者におけるGαs及びGαiレベル
MNLにおけるGs及びGiタンパク質のレベルを、Gαs及びGαiに対するポ
リクローナル抗体を使用したイムノブロット分析により測定した。躁病患者(A)
、鬱病患者(C)及び年齢と性が一致する健康な志願者(B)からのMNL膜タ
ンパク質による代表的なイムノブロットを図5に示す。抗Gαsにより認識され
るGαsの2つの種類(45kDa及び52kDa)のうち、45kDa種GαsのみがMNLに
おいて検出された。
標準化したデータを散乱プロットにより示す(図6)。Gαsの45kDa種のレベル
(6a)とGαiのレベル(6b)は、対照被検者(白丸)と比較して、躁病患者(
黒四角)においては有意に上昇し、鬱病患者(黒丸)では減少した。精神分裂症
患者(黒三角)あるいは恐慌性障害患者(図示せず)からのMNLにおいては有
意な変化は検出されなかった。
対照群の平均値と比較して、Gαs及びGαi免疫反応性レベルの≧15%上昇ま
たは減少の値を閾値とし、試験の特異性と選択性を評価することができる。少く
とも1種類のGα-サブユニット(Gαs,またはGαi)の免疫反応性レベルにお
ける≧15%の増加を必要とする躁病の診断については、感度は0.73で、選択性は
0.81である。少くとも1種類のGα-サブユニット(GαsまたはGαi)の免疫
反応性レベルにおける≧15%の減少を必要とする鬱病の診断については、感度は
0.73で、選択性は0.90である。実施例4:生化学的パラメータと鬱病の精神医学的評価との間の相関関係
正常な被検者、軽度の鬱病の外来患者、中程度及び重篤な鬱病の患者を、鬱病
のBeck調査とβ-GP及びM-GP機能を使用して評価し、同時にGαs及びGαiレ
ベルを分析した。
図7に示すように、Gpp(NH)p結合能のイソプロテレノール(7a)及びカルバミ
ルコリン(7b)誘発増加が、統計学的に非常に有意な形態でBeck調査のスコアと
相関していることが判った(Spearman相関係数=それぞれ-0.781p<0.001及び-0.
749p<0.001)。また、Gαs及びGαi免疫反応性レベルとBeckスコアとの間で
も有意な相関関係が得られた(それぞれ-0.63p<0.001及び−0.693p<0.001)
(図示せず)。実施例5: G-タンパク質の機能及びレベルに対する精神医学的治療の影響
リチウムで治療された気分正常の双極性患者におけるβ-GP及びM-GP機能は
対照被検者のものと同様であることが見いだされている(Schreiberら、1991,Avi
ssar及びSchreiber 1992a)。
図8は、抗鬱薬による治療前(黒丸)及び治療後(白丸)の鬱病患者における
β-GP機能を示す。調べた欝病患者の殆ど(8人のうち7人)においてβ-GP機
能の変化は、抗鬱薬による治療の後に正常になった。また、唯一の例外は精神医
学的パラメータに基づいた治療に対しても非応答性であることが判った。
ECT治療の間、鬱病患者からのMNL膜におけるβ-GP機能(白丸)及びM-GP
機能(白四角)を一定間隔で測定した。
平行して、鬱病のBeck調査(黒丸)、またはHamilton及びBriefの精神医学的
レーティングスケール(BPRS)スコア(図示せず)を使用して精神病の状態を評
価した。
図9aは、電気痙攣療法[ECT]での4人の鬱病患者における、β-GP機能(白丸
)、M-GP機能(白四角)、及びBeckスコア(黒丸)の変化の動態の代表的な例
を示す。
図9bは、電気痙攣治療[ECT]での4人の鬱病患者における、Gαs(白丸)、G

上記に挙げた図から、4種全ての生化学的パラメータの正常化が臨床的改善に
先立って見られることは明らかである。精神医学的基準によれば治療に応答した
別の10人の患者において同様な結果が得られた。精神医学的基準によればETCに
応答しなかった患者は、機能低下及び減少した免疫反応性の回復を示さなかった
(図示せず)。
このように、G-タンパク質の機能的及び定量的な測定は、種々の精神医学的
治療に対する精神病患者の応答性の予測となり得るものである。参考文献
