Abstract

PROBLEM TO BE SOLVED: To easily insert a catheter to a thin blood vessel, to unnecessitate a special operation for fixing the catheter in the blood vessel and to attain instantaneous fixing by dividing a part of the catheter consisting of an elastic body in parallel in a longitudinal direction and arranging guide wire guides in division parts. SOLUTION: The division parts 2 divided in parallel in the longitudinal direction are arranged in a part of a catheter main body 1 and the guide wire guides 3 are disposed in them. In this case, the outer diameter of each division part 2 is made to be larger than that of the catheter main body 1 by 120-500% in a state without a guide wire 5. Then the catheter main body 1 is inserted to a dwelling part by putting through the guide wire 5 to a lumen, which is previously inserted to the blood vessel for dwelling. Therefore, it is preferable that the division parts 2 are contracted when the guide wire 5 exists in the lumen and is put-through the lumen and the shape is the tubular one. When the catheter main body 1 is the thin one, an outer sheath 4 is attached to the outside of the division part 2 to facilitate the insertion of the guide wire 5.

2. Description of the Related Art Arterial infusion chemotherapy in which a drug injection port and a catheter connected to the port are placed in the body to locally administer an anticancer drug is performed. The main target is the liver, which has a high drug uptake rate. Indwelling catheters in blood vessels can be broadly classified into two types: surgical techniques and percutaneous techniques. The surgical technique is a technique of incising a blood vessel under laparotomy, inserting and suturing a catheter, and placing a drug solution injection port in a body cavity. The percutaneous insertion method is less invasive and is superior in terms of patient burden and QOL, and the indication of this technique has been increasing in recent years. Percutaneous catheter insertion method: In the case of liver cancer, a catheter is inserted mainly from the subclavian or femoral artery, and the catheter tip is positioned and indwelled around the origin of the proper hepatic artery via the aortic arch and celiac artery It is.

In recent years, a technique has been developed in which a catheter tip is fixed to a blood vessel branching from a blood vessel to which a drug solution is to be applied using a metal coil or the like, and the drug solution is administered from a side hole formed upstream of the branching blood vessel. Compared with the conventional technique, the catheter tip does not stimulate the endothelium of the blood vessel due to the movement of the blood vessel or blood flow, so that the patency of the indwelling blood vessel is good and the displacement of the catheter hardly occurs. However, the catheter is fixed by inserting the microcatheter into the lumen of the catheter, inserting the microcatheter into the blood vessel through a side hole prepared in advance, placing the tip of the microcatheter near the tip of the indwelling catheter, It is a complicated method such as fixation to a blood vessel using a metal coil or a metal coil, and has a problem that can only be performed by a doctor who excels in technique.
In addition, when the side hole approaches the blood vessel wall, there is a risk that the blood vessel inner wall may be violated by a high concentration of an anticancer drug.

The following method is disclosed as a method for fixing a catheter in a blood vessel. 1. A method in which a balloon is provided near the distal end and the balloon is inflated and fixed in a blood vessel after a catheter is inserted. A conduit-type balloon is disclosed for fixing to a site where blood flow is required (Japanese Patent Application Laid-Open No. 7-532). The balloon type requires an indwelling catheter having a cross-sectional shape of 2 lumens or more, which makes it difficult to process into a small-diameter catheter, has a problem in the durability of the balloon, and may cause the catheter tip to be displaced. 2. A method is disclosed in which the tip of the catheter spreads like an octopus and is fixed at a target site. (Japanese Unexamined Patent Publication No. 1-68275) The tip portion is made of a shape memory resin, and is designed to open like an octopus foot due to a temperature change. In this procedure in which a guide wire is used to reach the indwelling site, the catheter tip is opened during insertion into the target site, and the present catheter is not suitable.

[0006] 3. There is a method in which the catheter has a double inner / outer structure, a slit is formed at the tip, only the tip is adhered and fixed, and the inner tube is pulled to widen the slit portion and fix the blood vessel. (Japanese Patent Application Laid-Open No. 6-34646) When this mechanism is manufactured using a small-diameter catheter, the fixing force in the blood vessel is insufficient because the slit portion is thin. If the material strength of the catheter is increased to have a fixed strength, the catheter becomes rigid, the followability of the guide wire of the catheter is deteriorated, and the inner wall of the blood vessel in the indwelling route is easily damaged. As shown above, there has been no catheter applicable to this procedure, and a complicated catheter fixing operation is required.

DESCRIPTION OF THE PREFERRED EMBODIMENTS The dividing section is provided with a guide wire guide in which a catheter body is vertically divided into at least two or more parallel to the longitudinal direction of the catheter and a guide wire is passed therethrough. The catheter follows the guide wire previously inserted into the blood vessel and is inserted to the indwelling site. When the guide wire is in the lumen, the divided portion is in a contracted state by the guide wire guide, so that it can be easily inserted into a small-diameter blood vessel. After inserting the catheter to the target indwelling site, the divided portion of the catheter is brought into a state before the insertion of the guide wire in the blood vessel in conjunction with the pull-out operation of the guide wire, and is fixed in the blood vessel. At this time, the catheter body is positioned and fixed substantially at the center of the blood vessel lumen.

Hereinafter, the present invention will be described in detail with reference to examples. FIG. 1 shows a catheter according to the invention. The catheter body (1) has a split part (2). The dividing part is divided into at least two or more parallel to the longitudinal direction of the catheter. Each of the divided portions has at least one or more guide wire guides (3). The divided portion is formed in advance and is 120 to 500% larger than the outer diameter of the main body when there is no guide wire (5) in the lumen. The catheter body and split part are polyurethane, polyester, PE
It is desirable to be made of a polymer material having appropriate elasticity such as T or nylon. Further, those made of a composite material of stainless steel, Teflon, or the like and the above-mentioned polymer, and the like, desirably increase the strength of fixing the expanded portion to the blood vessel. Materials with antithrombotic treatment on the surface are more desirable.

The catheter is inserted into a lumen through a guide wire inserted into a blood vessel to be indwelled in advance, and is inserted up to the indwelling site. For this reason, it is desirable that the dividing portion contracts when the guide wire is in the lumen and when the guide wire is passed through the lumen, and has a tubular shape. It is most preferable that the guide wire guide is formed in a ring shape and the interval is narrow so that the divided portion repeats the contracted state and the expanded state in conjunction with both the operation of inserting and extracting the guide wire. It is desirable that the clearance between the guide wire and the guide wire guide be as small as possible, so that the catheter can be easily inserted. If the catheter has a small diameter and it is difficult to prepare the catheter, an outer sheath (4) may be attached to the outside of the divided portion to facilitate insertion of the guidewire into the lumen of the catheter. The outer sheath is removed after inserting the guide wire into the division, and inserted into the blood vessel.

As apparent from the above description, the catheter can be instantly fixed in the blood vessel by returning the divided portion of the catheter to the shape before insertion in conjunction with the operation of pulling out the guide wire. To fix the catheter inside the blood vessel,
There is no need to perform complicated operations, and the operation time can be reduced.
Since the operation is simplified, damage to blood vessels and bleeding can be suppressed, and there is no need to use a bioadhesive or an expensive metal coil.
