Abstract

La diversità dei quadri anatomo-angiografici nel trattamento delle lesioni atero-sclerotiche coronariche e periferiche richiede una continua ricerca di nuove endoprotesi atte ad essere utilizzate nelle situazioni che poco si prestano all'uso di endoprotesi già esistenti, i cui risultati clinici, a breve e lungo termine, continuano a presentare alcuni inconvenienti. QUI VENGONO PROPOSTI TRE NUOVI TIPI DI ENDOPROTESI (STENT):A) ENDOPROTESI A SPIRALE CONTRAPPOSTA: si tratta di uno stent monofilo, espandibile con il palloncino. Il filo, smussato alle due estremità, assume un andamento spiraliforme particolare; ogni due giri e mezzo di spirale la direzione della stessa viene invertita mediante un ripiegamento ad "U" stretta del filo e si procede in questo modo fino alla lunghezza desiderata dello stent. Quando viene espanso lo stent, la spirale può perdere fino ad un giro e mezzo del suo periodo potendo in tal modo aumentare il suo diametro fino a due volte e mezzo (vedi figure e descrizione dettagliata);B) ENDOPROTESI AD ANDAMENTO SINUSALE: si tratta ancora di una endoprotesi monofilo in cui il filo, procedendo con andamento sinusale, realizza, mediante periodiche inversioni di direzione, una sequenza di file di onde giustapposte (vedi figure e descrizione dettagliata) che vengono parzialmente saldate tra loro e ravvicinate in modo da ottenere una rete formata da maglie di forma simile ad un "8"; questo permette di partire con un diametro ancor più piccolo e di aumentarlo ancor di più in fase di espansione.C) ENDOPROTESI PER LA BIFORCAZIONE DEI VASI FORMATE DA MONOFILO: si tratta di uno stent monofilo, biforcato ad una estremità, che può essere realizzato secondo due diverse modalità: nel primo caso l'andamento del filo ricalca quanto già esposto al punto B e anche i due rami della biforcazione presentano lo stesso andamento sinusale; nel secondo caso il corpo dello stent presenta l'andamento sinusale, mentre i due rami della biforcazione procedono con l'andamento spiraliforme descritto al punto A (vedi figure e descrizione).Tutte le tre endoprotesi descritte (stent), essendo formate da un filo unico (di materiale senza memoria elastica) presentano vantaggi in termini di flessibilità che le rendono applicabili anche in condizioni anatomiche difficili, con procedure semplificate. Vengono allegate descrizioni dettagliate e figure.The diversity of anatomical-angiographic pictures in the treatment of coronary and peripheral athero-sclerotic lesions requires a continuous search for new endoprostheses suitable for use in situations that do not lend themselves to the use of already existing endoprostheses, whose clinical results, in the short and long term, continue to present some drawbacks. THREE NEW TYPES OF ENDOPROTESIS (STENT) ARE PROPOSED HERE: A) SPIRAL ENDOPROTESIS WITH OPPOSITE: it is a monofilament stent, expandable with the balloon. The wire, bevelled at both ends, takes on a particular spiral shape; every two and a half turns of the spiral, the direction of the same is reversed by means of a narrow "U" folding of the wire and this is done up to the desired length of the stent. When the stent is expanded, the spiral can lose up to a turn and a half of its period, thus being able to increase its diameter up to two and a half times (see figures and detailed description); B) SINUSED ENDOPROTESIS: it is again of a monofilament endoprosthesis in which the wire, proceeding with sinus course, realizes, through periodic direction reversals, a sequence of rows of juxtaposed waves (see figures and detailed description) which are partially welded together and close together so as to obtain a net formed by meshes similar in shape to an "8"; this allows you to start with an even smaller diameter and to increase it even more during the expansion phase. C) ENDOPROTESIS FOR BIFURCATION OF VESSELS FORMED BY SINGLE WIRE: it is a monofilament stent, bifurcated at one end, which can be made according to two different ways: in the first case, the course of the wire follows what has already been explained in point B and also the two branches of the bifurcation have the same sinus trend; in the second case the body of the stent presents the sinus trend, while the two branches of the bifurcation proceed with the spiral-like course described in point A (see figures and description) .All the three described endoprostheses (stents), being formed by a wire unique (of material without elastic memory) they have advantages in terms of flexibility that make them applicable even in difficult anatomical conditions, with simplified procedures.
