Descrizione di Brevetto di Invenzione Industriale avente per titolo:
“PROCESSO PER ACCOPPIARE UN COMPONENTE POLIMERICO AD UN
COMPONENTE METALLICO COSTITUENTI PARTE DI O UNA PROTESI MEDICALE”
CAMPO TECNICO DELL’INVENZIONE
L’invenzione riguarda un processo per accoppiare un componente polimerico ad un componente metallico costituenti parte di o una protesi medicale, e la protesi così ottenuta.
STATO DELLA TECNICA ANTERIORE
La chirurgia ortopedica ha a disposizione varie endoprotesi studiate per sostituire diverse componenti scheletriche o porzioni di esse (ginocchio, anca, spalla, dita, gomiti, colonna vertebrale, ecc...), al fine di restituirne la funzionalità articolare e rispecchiando il più possibile la morfologia della parte che andranno a sostituire.
Molte di tali endoprotesi sono costituite da due componenti che articolano fra loro, ognuno di tali componenti aventi una parte articolante ed una parte ancorante: la parte articolante è a contatto con la corrispondente parte articolante del componente protesico opposto, mentre la parte ancorante è a contatto con l’osso.
Spesso, una delle parti articolanti è realizzata in materiale polimerico ed è contrapposta alla parte articolante in materiale metallico o ceramico del secondo componente protesico. Il materiale polimerico solitamente utilizzato è il polietilene ad altissimo peso molecolare (UHMWPE: Ultra High Molecular Weight Polyethylene), che offre un bassissimo coefficiente di attrito ed una elevata resistenza all’usura. Ultimamente altri polimeri vengono proposti sul mercato, quali PEEK (polietereterchetone), PEEK rinforzato con fibre di carbonio, policarbonati e poliuretani. Il UHMWPE resta comunque il materiale polimerico di maggior utilizzo.
Per via della sua bassa capacità osteointegrativa e della sua bassa rigidità, il polietilene UHMWPE viene accoppiato ad uno strato metallico (detto anche “metal back”), cosicché la parte articolante e la parte ancorante del componente protesico sono costituite da due materiali diversi.
Ad esempio, molti piatti tibiali, come quello in Fig.1, sono costituiti da una parte articolante 10 in polietilene accoppiata ad un supporto metallico 12 a contatto con l’osso 14; coppe acetabolari, come quella in Fig. 2, presentano un guscio ancorante 20 metallico contenente un inserto 22 in polietilene.
Un problema che nasce da questo tipo di accoppiamento sono i micromovimenti fra componente in polietilene e quello in metallo di supporto. Dato che non si utilizzano adesivi, i due componenti assemblati possono spostarsi leggermente uno rispetto all’altro e staccarsi localmente, creando un potenziale rischio di instabilità della protesi. Altro problema sono i fenomeni di usura tra l’inserto polimerico e la base metallica che porta alla abrasione della superficie polimerica, problema conosciuto come “backside wear” soprattutto nei componenti tibiali.
Per evitare tali problematiche, sarebbe ideale poter rivestire direttamente l’elemento in polietilene con uno strato metallico, ottenendo una interfaccia stabile.
EP0761242 descrive il metodo per rivestire componenti polimerici con coating in Titanio mediante thermal spray, ma ciò è possibile solamente con polimeri ad alto punto di fusione come il poliarilchetone ivi descritto, ma non con polietilene: le temperature raggiunte durante il thermal spray farebbero infatti fondere il polietilene, deformandolo e degradandone le proprietà meccaniche.
EP1082074 descrive l’accoppiamento di una maglia metallica su coppe acetabolari in polietilene, ma ciò non garantisce la rigidità del componente protesico che, vista la sottigliezza e la facile deformabilità di una maglia metallica, rimane bassa.
EP1800700 descrive la realizzazione di strutture metalliche mediante laser sintering. Tali strutture sono costituite da strati superficiali porosi e un corpo interno non poroso, quindi con almeno 3 strati a porosità variabile. Mettendo a contatto un componente metallico siffatto con un componente polimerico in una forma o cavità, ed applicando dall’esterno calore e pressione, si può secondo EP1800700 far compenetrare polimero e metallo. Il polimero può essere in polvere, oppure essere un componente finito. Nel casi in cui si abbia a disposizione un componente finito, il processo non tiene però conto del fatto che riscaldando il componente polimerico oltre la sua temperatura di fusione e applicando una pressione esterna, si deforma l’intero componente polimerico, si generano degli stress interni dovuti alla dilatazione termica e si aumenta il rischio di ossidazione termica del polimero stesso, soprattutto quando il polimero utilizzato è UHMWPE, che è caratterizzato da una elevata viscosità e che perciò richiede delle alte pressioni per ottenere la compenetrazione nel componente metallico.
SCOPI DELL’INVENZIONE
Scopo principale dell’invenzione è realizzare un metodo che migliori lo stato dell’arte.
Altro scopo è produrre protesi medicali più durature e di maggior qualità.
Questi scopi sono raggiunti con un processo per accoppiare un componente polimerico ad un componente metallico costituenti parte di o una protesi medicale, comprendente le fasi di
(i) mettere a contatto il componente polimerico col componente metallico,
(ii) riscaldare il solo componente metallico ad una temperatura di processo maggiore o uguale della temperatura di fusione del componente polimerico in modo da ottenere una fusione o rammollimento locale del componente polimerico alla superficie di contatto tra i due detti componenti.
Con il processo dell’invenzione si riesce ad accoppiare il componente polimerico, ad es. in polietilene, e il componente metallico. Si aumenta la superficie di contatto reciproca creando una compenetrazione tra i due materiali all’interfaccia, sfruttando le capacità viscose/adesive del polimero allo stato fuso e la capacità conduttiva termica del metallo. Si riscalda solamente il componente metallico, così da evitare la deformazione di tutto il componente polimerico, poiché si fonde solo la zona all’interfaccia.
Come componente polimerico ben si prestano ad es. quelli in UHMWPE o in PEEK o in PEEK rinforzato con fibre di carbonio, in policarbonato o in poliuretano.
Come componente metallico, tutti i metalli attualmente utilizzati in ortopedia sono adeguati allo scopo, preferibilmente Titanio, leghe di Titanio, acciaio inossidabile, leghe di Cobalto-Cromo, Tantalio, leghe di Tantalio, Niobio, leghe di Niobio.
Preferibilmente nella fase (ii) si attua la fase ulteriore di (iii) di premere tra loro i due detti componenti per favorirne la compenetrazione lungo la superficie di contatto.
Si noti che grazie al riscaldamento del solo componente metallico si può applicare la pressione, che migliora l’adesione, ma si evita la deformazione di tutto il componente polimerico.
Il componente metallico può essere un volume di materiale in polvere da sinterizzare o un componente finito.
Per facilitare l’adattamento tra componente polimerico e metallico, il componente metallico è un corpo avente una superficie con la stessa geometria/curvatura della superficie del componente polimerico da rivestire. Si realizza in sostanza un accoppiamento di forma. Preferibilmente, per migliorare la tenacità dell’interfaccia, il componente metallico su detta superficie con stessa geometria/curvatura presenta rugosità o porosità o elementi ritentivi o sottosquadri in modo da incrementare la superficiale di contatto. Si veda ad es. il contenuto di PCT/IB2008/002261 per una tecnica con cui aumentare la superficie di contatto.
Può accadere che non si ottenga una unione ottimale ad es. quando il polietilene degli impianti è l’UHMWPE che, anche quando allo stato fuso, ha una viscosità molto elevata. Per far compenetrare le polveri o il componente metallico sarebbe necessaria allora una pressione molto elevata, che deformerebbe tutto il componente.
Per risolvere questo problema di una possibile bassa adesione tra il componente metallico e polimerico, prima della fase (i) si riveste o copre il componente polimerico e/o il componente metallico con un film o delle polveri di un polimero con funzione sigillante avente una fluidità maggiore del componente polimerico allo stato fuso. Nella fase (ii) si riscalda poi il componente metallico ad una temperatura di processo maggiore od uguale della temperatura di fusione di detto polimero sigillante, in modo da ottenere alla superficie di contatto tra i due detti componenti una fusione o un rammollimento locale di detto polimero sigillante.
Il polimero sigillante funge da strato intermedio legante. Avendo maggiore fluidità del componente polimerico, migliora il contatto e la connessione tra componente polimerico e metallico. Inoltre richiedendo di minore pressione per fluire rispetto al componente polimerico, evita la necessità di utilizzare pressioni troppo elevate che potrebbero alterare le dimensioni del componente polimerico
In particolare, il polimero sigillante dovrebbe essere scelto in base al materiale polimerico del componente di partenza: la temperatura di fusione del polimero sigillante Ts non dovrebbe essere maggiore di Tp+50°C, dove Tp è la temperatura di fusione del materiale polimerico di cui è composto il componente iniziale. Molto preferibilmente Ts dovrebbe essere minore od uguale a Tp.
Il film o le polveri del polimero sigillante sono scelti preferibilmente nel gruppo costituito da poliolefine, poliesteri, polisulfoni, polichetoni, poli-immidi, polimetacrilati, policarbonati, poliuretani o loro copolimeri.
Il processo si può comunque estendere anche ad altri materiali polimerici impiegati in ortopedia.
In particolare, dovendo unire un componente in UHMWPE ad un componente metallico, il film o le polveri del polimero sigillante sono scelti preferibilmente nel gruppo costituito da polietilene ad alta densità (HDPE), polietilene a bassa densità (LDPE), polietilene a bassissima densità (LLDPE), polietilene lineare (LPE), polietilene ad alto peso molecolare (HMWPE), polietilene ad altissimo peso molecolare (UHMWPE) o da una miscela di tali polimeri.
La fase (ii) del processo descritto è realizzabile ad esempio inducendo una circolazione di corrente all’ interno del componente metallico: viste le elevate conducibilità elettrica e termica del metallo e viste le basse conducibilità elettrica e termica del polimero, il calore si sviluppa all’interno del componente metallico per effetto joule fino all’interfaccia metallo/polimero; per irraggiamento o per contatto viene riscaldata anche la superficie del componente polimerico, mentre il resto del componente polimerico rimane a temperature molto minori. La pressione descritta nella fase (iii) può venire applicata ai due componenti tramite ad esempio una pressa, in modo da indurre la compenetrazione dei due componenti e/o la fusione e la fluidificazione del polimero sigillante.
Qualsiasi congegno, apparecchio o insieme di apparecchi che inducono tali fenomeni sono adatti all’unione dei due componenti.
Come variante preferita le fasi (ii) e (iii) sono realizzate via SPS (Spark Plasma Sintering), data la facilità con cui è possibile integrare le fasi (ii) e (iii). In effetti, nel processo SPS la pressione è applicata naturalmente allo stampo, e le correnti circolanti nello stampo stesso e nel componente metallico riscaldano per effetto Joule le parti metalliche ed anche l’interfaccia tra componente metallico e polimerico. L’SPS ha il vantaggio ulteriore di offrire un riscaldamento veloce (variabile tra 20-500 °C/min) e proprio dove serve, cioè all’interfaccia metallo-polimero.
Il processo SPS può apportare un ulteriore vantaggio nel caso in cui sia il componente metallico che il polimero sigillante siano inizialmente sotto forma di polveri: il passaggio di corrente nelle polveri metalliche genera dei plasmi tra particelle metalliche che innalzano le temperature delle particelle di polimero sigillante attigue, portandole a fusione e favorendo l’inglobamento delle particelle metalliche. Laddove inoltre vi sia contatto diretto fra particelle metalliche, la generazione di tali plasmi abbinata alla pressione esterna può portare alla saldatura di tali particelle metalliche, aumentando in tal modo la coesione dello strato metallico finale.
L’invenzione riguarda anche la protesi ottenuta secondo il processo suesposto.
Il processo dell’invenzione si presta benissimo a realizzare (ma non solo) come componente risultante una coppa acetabolare, il piatto tibiale di una protesi di ginocchio, la componente rotulea di una protesi di ginocchio, il componente gleonide di una protesi di spalla, il componente omerale di una protesi di spalla inversa, una protesi di disco invertebrale, il componente radiale di una protesi di gomito, il componente di una protesi di polso o di caviglia, le protesi articolari falangee della mano e del piede.
BREVE DESCRIZIONE DEI DISEGNI
Ulteriori caratteristiche e vantaggi dell’invenzione risulteranno maggiormente evidenti dalla descrizione esemplificativa di un processo, insieme agli allegati disegni in cui:
La figura 1 mostra un noto componente tibiale di una protesi di ginocchio;
La figura 2 mostra la sezione di una nota coppa acetabolare di una protesi d’anca;
La figura 3 mostra in sezione uno dei possibili schemi di stampo usati in un processo secondo l’invenzione.
Fig. 3 mostra uno stampo formato da una matrice 32 e un punzone 30 in materiale conduttivo (ad esempio grafite) di una macchina di sinterizzazione via SPS. Al suo interno sono disposti sovrapposti un componente in UHMWPE 40, uno strato di HDPE 42 e un componente metallico 44.
Lo strato 42 ha una fluidità maggiore del componente 40 allo stato fuso.
Durante il processo viene applicata dall’esterno una pressione assiale sulla matrice 32 e sul punzone 30; una corrente pulsata CR viene generata in modo noto ed inviata alla matrice ed al punzone. Le frecce indicano un verso di corrente possibile.
Le correnti CR riscaldano per effetto Joule in modo rapido ed uniforme unicamente il componente 44, il quale trasferisce calore allo strato 42 fondendolo.
Lo strato 42, oltre a schermare termicamente il componente 44, fluidifica molto più di esso e fonde totalmente. La pressione applicata allo stampo tra il punzone 30 e la matrice 32 perfeziona l’adesione e l’uniformità dello strato 42 tra i componenti 40, 44.
In breve si ottiene un componente finito formato dai tre elementi 40, 42, 44, perfettamente saldati tra loro.
Claims (13)
- RIVENDICAZIONI 1. Processo per accoppiare un componente polimerico (44) ad un componente metallico (40) costituenti parte di o una protesi medicale, comprendente le fasi di (i) mettere a contatto il componente polimerico col componente metallico, (ii) riscaldare il solo componente metallico ad una temperatura di processo maggiore o uguale della temperatura di fusione del componente polimerico in modo da ottenere una fusione o rammollimento locale del componente polimerico alla superficie di contatto tra i due detti componenti.
- 2. Processo secondo la rivendicazione 1, in cui nella fase (ii) si attua la fase di (iii) premere tra loro i due detti componenti per favorirne la compenetrazione lungo la superficie di contatto.
- 3. Processo secondo la rivendicazione 1 o 2, in cui il componente metallico è un componente finito.
- 4. Processo secondo la rivendicazione 3, in cui il componente metallico è un corpo avente una superficie con la stessa geometria/curvatura della superficie del componente polimerico da rivestire.
- 5. Processo secondo la rivendicazione 4, in cui il componente metallico su detta superficie con stessa geometria/curvatura presenta rugosità o porosità o elementi ritentivi o sottosquadri in modo da incrementare la superficiale di contatto.
- 6. Processo secondo una delle rivendicazioni precedenti, in cui il componente metallico è un volume di materiale in polvere.
- 7. Processo secondo una qualsiasi delle rivendicazioni precedenti, in cui prima della fase (i) si riveste o copre il componente polimerico e/o il componente metallico con un film o delle polveri di un polimero (42) con funzione sigillante avente una fluidità maggiore del componente polimerico allo stato fuso, e nella fase (ii) si riscalda il componente metallico ad una temperatura di processo maggiore od uguale della temperatura di fusione di detto polimero sigillante, in modo da ottenere alla superficie di contatto tra i due detti componenti una fusione o un rammollimento locale di detto polimero sigillante.
- 8. Processo secondo la rivendicazione 7, in cui il film o le polveri del polimero sigillante sono scelti nel gruppo costituito da poliolefine, poliesteri, polisulfoni, polichetoni, poliimmidi, polimetacrilati, policarbonati, poliuretani o loro copolimeri.
- 9. Processo secondo la rivendicazione 7, in cui il film o le polveri del polimero sigillante sono scelti nel gruppo costituito da polietilene ad alta densità (HDPE), polietilene a bassa densità (LDPE), polietilene a bassissima densità (LLDPE), polietilene lineare (LPE), polietilene ad alto peso molecolare (HMWPE), polietilene ad altissimo peso molecolare (UHMWPE) o da una miscela di tali polimeri.
- 10. Processo secondo una qualsiasi delle rivendicazioni precedenti da 2 a 9, in cui le fasi (ii) e (iii) sono realizzate via Spark Plasma Sintering.
- 11. Processo secondo una qualsiasi delle rivendicazioni precedenti, in cui il componente metallico è selezionato dal gruppo comprendente Titanio, leghe di Titanio, acciaio inossidabile, leghe di Cobalto-Cromo, Tantalio, leghe di Tantalio, Niobio, leghe di Niobio.
- 12. Processo secondo una qualsiasi delle rivendicazioni precedenti, in cui il componente risultante del processo è un una coppa acetabolare, il piatto tibiale di una protesi di ginocchio, la componente rotulea di una protesi di ginocchio, il componente gleonide di una protesi di spalla, il componente omerale di una protesi di spalla inversa, una protesi di disco invertebrale, il componente radiale di una protesi di gomito, il componente di una protesi di polso o di caviglia, le protesi articolari falangee della mano e del piede.
- 13. Protesi ottenuta secondo il processo di una o più delle precedenti rivendicazioni.
