Description
“Dispositivo elettrochirurgico per perforazioni ovariche” "Electrosurgical device for ovarian perforations"
TESTO DELLA DESCRIZIONE TEXT OF THE DESCRIPTION
Campo dell’invenzione Field of the invention
The present invention refers in general to the field of reproductive medicine with particular reference to artificial fertilization.
Some women of childbearing age and desirous of offspring suffer from chronic lack of ovulation due to an ovarian disease called “polycystic ovary” (PCO). This pathology often involves the total absence of the menstrual cycle. In cases of PCO there is an increased ovary volume and the presence of a whitish, thickened and resistant ovarian capsule.
PCO therapy is represented by ovulation-inducing drugs, which, however, are effective only in a part of the subjects. In fact, some patients do not respond to drugs and remain anovular despite therapy.
Stato della tecnica anteriore State of the prior art
The medical literature shows that it is possible to improve the response to drugs, and sometimes restore ovulation even without administering drugs, if holes are made in the capsule of the polycystic ovaries. This procedure is internationally known as "ovarian drilling" and involves the use of a coagulator device equipped with a tip capable of conducting electricity and piercing the ovarian capsule by direct contact.
The monopolar or bipolar coagulator is used during an endoscopic surgery called “laparoscopy”, which is carried out in hospital and under general anesthesia, lasting about 40 minutes. During laparoscopy, a fiber optic device is inserted through the abdominal wall to view the viscera on a screen and two other hollow tubes within which it is possible to pass the surgical instruments to perform the surgery. The coagulator is introduced into one of them and is brought into contact by the ovarian capsule to perform electrosurgical drilling.
In the course of in vitro fertilization procedures for infertile couples, it is common practice to collect oocytes from the ovaries by transvaginal ultrasound-guided ovarian puncture. This operation takes a few minutes, is carried out on an outpatient basis (without hospitalization) and under local anesthesia.
A needle guide (disposable or sterilizable) is mounted on the transvaginal ultrasound probe, inside which a needle with an ecoreflective tip is passed and connected to a suction pump. By repeatedly pricking the ovary, the oocytes are aspirated, checking their actions on the ultrasound screen. A dotted line (guide) appears on the latter which acts as a "pointer" to orient the angle of the puncture so as to puncture the ovaries in the most suitable point.
Sommario dell’invenzione Summary of the invention
The object of the present invention is to provide an electrosurgical device for "ovarian drilling" which is free from the drawbacks of conventional coagulators for laparoscopy mentioned above, and which instead allows the use of the same technique of ovarian puncture via the transvaginal route.
According to the invention, this object is achieved by means of an electrosurgical device for ovarian perforations characterized in that it consists of a coagulating needle equipped with an electrically conductive and echo-reflective perforation tip which can be connected to an electrical power source, the needle being inserted into a needle guide applied to a transvaginal ultrasound probe.
Descrizione dettagliata dell’invenzione Detailed description of the invention
In a preferred embodiment, the ultrasound-guided transvaginal electrosurgical device for ovarian drilling consists of a metal needle substantially similar to the needles already commercially available for oocyte retrieval.
This needle is typically about 35 cm long, has a diameter of 16-17 gauges. It is equipped at one end with a plastic handle and at the opposite end with a blunt flute-beak tip, formed with external knurls that make it echo-reflective and clearly visible to a transvaginal ultrasound.
The tip is electrically conductive for about 3-5 mm, while the remaining part of the needle body is shielded by an insulating jacket.
The needle is hollow and is crossed by conductors designed to connect the tip with an electrical power source in a similar way to the needles currently used for electrosurgery and for laparoscopic drilling.
The size and shape of the needle are, as mentioned, similar to those of the needles for egg retrieval: in this way it can be inserted into a needle guide, disposable or sterilizable, of the type normally applied to transvaginal ultrasound probes.
In use, the needle inserted through the guide of the ultrasound probe is introduced in an ultrasound-guided way transvaginally, with simple local anesthesia. By checking the operation on the ultrasound screen, the surgeon orients the needle by making it penetrate into the ovarian capsule to perform the perforations. The shielding of the needle body prevents the electric current from damaging anatomical structures adjacent to the ovaries, such as intestinal loops. The unshielded ecoreflective tip of the needle allows the surface of the ovary to be electrocoagulated at the points of perforation, producing a hemostatic effect on the tissue and preventing the occurrence of unwanted bleeding.
Rispetto al drilling ovarico laparoscopico il dispositivo secondo l’invenzione permette di conseguire i seguenti vantaggi: Compared to laparoscopic ovarian drilling, the device according to the invention allows the following advantages to be achieved:
- nessuna necessità di ricovero per la paziente, - no need for hospitalization for the patient,
- significativa riduzione del rischio anestesiologico, grazie alla possibilità di sottoporre la paziente a semplice anestesia locale e non generale, - significant reduction of the anesthetic risk, thanks to the possibility of subjecting the patient to simple local and not general anesthesia,
- significativa riduzione del rischio chirurgico, grazie al fatto che l’intervento è sensibilmente meno invasivo e più veloce, - significant reduction in surgical risk, thanks to the fact that the surgery is significantly less invasive and faster,
- maggiore accettabilità per le pazienti, - greater acceptability for patients,
- maggiore ripetibilità della procedura, - greater repeatability of the procedure,
- saving of resources for the hospital and the community.
Of course, the embodiments of the device can be widely varied with respect to what is described above, without thereby departing from the scope of the present invention as defined in the following claims.
