Description
The present invention relates to a lower limb prosthesis and, in particular, to a limb prosthesis of the exoskeletal type.
When as a result of accidents or incidents or due to specific vascular or metabolic pathologies (diabetes, peripheral arteriopathies) foot amputation or transtibial amputation is necessary and therefore the ankle joint is missing, the autonomous walking of the patient is restored by applying exoskeletal prostheses. Such prostheses comprise an attachment or socket portion and an artificial foot connected to the attachment portion itself. The socket defines a seat for housing a rigid terminal portion or distal stump of the amputated limb and has a shape substantially complementary to that of the stump itself. The foot is chosen according to the characteristics of the patient and the stresses transmitted by the patient himself and from the outside during walking. In most applications, artificial feet of the type comprising a heel, a forefoot and a non-articulated, i.e. rigid ankle joint are associated with the socket.
Although universally used, as it is able to allow the patient to walk independently, walking is nevertheless uncomfortable due to the lack of relative freedom of rotation originally allowed by the ankle joint.
In order to obviate the above problem, attempts are known to use artificial feet articulated at the ankle level, which are provided with elastomeric elements which deform during walking. However, this type of artificial feet is not without disadvantages and difficulties that in practice inhibit its use especially when, in tibio-calcaneal arthrodeses, the stump is long and therefore the space between the apex and the walking surface is insufficient. when fitting a standard articulation. The foregoing is essentially a consequence of the fact that, on the one hand, these artificial feet have a weight and a manufacturing complexity and therefore extremely higher costs than feet with non-articulated ankle joint and, on the other hand, due to their conformation they are not however able to totally restore the original amputated joint. In fact, the hinge axis around which the artificial foot rotates with respect to the socket is arranged near a free end of the abutment and therefore in a lowered position with respect to the natural hinge axis defined by the joint of the amputated ankle. This aspect prevents the resumption of natural walking and therefore generates a situation of compromise and discomfort, from which the patient cannot escape.
The object of the present invention is to provide a lower limb prosthesis, which allows the above problems to be solved in a simple and economical way and at the same time has an invariant functional efficiency and reliability and low costs.
According to the present invention, a lower limb prosthesis is provided comprising an attachment portion defining a housing seat for a rigid abutment of the patient following a transtibial amputation or tibio-calcaneal arthrodesis of the lower limb, an artificial foot and interposed hinge means. between the attachment portion and the foot to allow rotation of the foot with respect to the attachment portion around a hinge axis, characterized in that said hinge axis is arranged at a distance from a walking surface of the prosthesis substantially equal to the distance of the physiological axis of rotation defined by the joint of the amputated ankle, and by the fact that reaction means are interposed between said attachment portion and said foot to exert an antagonistic action to the actions transmitted to said hinge means.
Preferably, in the prosthesis defined above, said hinge axis extends through a distal portion of said attachment portion while, conveniently, the reaction means are arranged between a distal end portion of said attachment portion and a heel of said foot.
The invention will now be described with reference to the attached drawings, which illustrate a non-limiting example of embodiment, in which:
la figura 1 illustra, in elevazione laterale, una preferita forma di attuazione di una protesi d'arto inferiore realizzata secondo i dettami della presente invenzione e disposta all'interno di una calzatura indicata con linea tratteggiata; Figure 1 is a side elevation view of a preferred embodiment of a lower limb prosthesis made according to the dictates of the present invention and arranged inside a shoe indicated by a dashed line;
la figura 2 illustra, in scala ingrandita, la protesi di figura 1 con parti asportate per chiarezza; e Figure 2 illustrates, on an enlarged scale, the prosthesis of Figure 1 with parts removed for clarity; And
Figure 3 is a section along the line III-III of Figure 2.
In Figures 1 and 2, 1 indicates, as a whole, a lower limb prosthesis inserted in a shoe 2 (Figure 1) and coupled to a rigid monolithic abutment 3 formed, for example, following transtibial amputation or tibiocalcaneal arthrodesis of a patient's lower limb.
The portesi 1 comprises a rigid attachment body 4, commonly known as a socket, and an artificial foot 5.
The attachment body 4 defines a seat 7 for housing the abutment 3 and conveniently comprises two half-shells 8 and 9 facing each other and hinged to each other by means of a hinge 10 to move between an opening position for application. / the removal of the prosthesis 1 and a functional closing position, in which the half-shells 8 and 9 define the housing seat 7, on the bottom of which the free end of the abutment 3 rests.
Still with reference to Figures 1 and 2, the foot 5 comprises a heel 12, a forefoot 13 consisting of two portions 14 and 15 overlapping each other and of which a lower one is integral with the heel 12 and an upper removable one, and is coupled to the body 4 for attachment by means of a hinge or articulation device 16 (Fig. 3). The joint 16 allows the foot 5 to rotate with respect to the body 4 around a hinge or fulcrum axis 18, which crosses a distal or lower end portion 4a of the body 4 and, in use, also the lower terminal portion of the abutment 3 which rests on the bottom of the seat 7. The hinge axis 18 extends in a position substantially parallel to a foot tread plane P (figure 2) and at a distance from the tread plane P itself equal to or comparable with the distance of the natural axis of the hinge defined by the articulation of the ankle amputated from the same floor P.
In the particular embodiment described, the joint 16 comprises a fork element 20, which extends upwards from the heel 12 of the foot and has two arms 21 arranged on opposite sides of the distal portion 4a of the body 4 and hinged to the portion distal 4a itself by means of a pair of pins 22. The pins 22 extend coaxially to the hinge axis 18 and, in the particular example described, are integrally connected to the wall of the distal portion 4a and engage respective through holes 23 obtained through the arms 21. Each pin 22 has an external diameter which approximates by default the diameter of the relative hole 23 so as to engage the relative hole 23 itself with a determined radial clearance.
The ends of the arms 21 opposite to those hinged to the body 4 are arranged on opposite sides of a reaction body 25, which forms part of the joint 16 and is interposed between the distal portion 4a and the heel 12.
The reaction body 25 is, in turn, stably connected to the heel 12 and comprises a base or attachment portion 26 and an intermediate support or reaction portion 27, which rises upwards with respect to the base portion 26 to cooperate in sliding with the distal portion 4a and to support in use, completely or at least in part, the loads transmitted to the pins 22 during the walking of the patient. For this purpose, the portion 27 is delimited, from the part facing the distal portion 4a, by a cylindrical concave surface 29 coaxial to the hinge axis 18, while the distal portion 4a itself is delimited by a surface 30 with a rectilinear sliding contact of the surface 29 itself to define a coupling with a joint or cylindrical joint.
Alternatively, according to a variant not shown, the surfaces 29 and 30 have respective generating lines that are not straight but provided with respective curved portions to define at least a circular guide coaxial to the axis 18 and a slide sliding inside said guide to inhibit the arising of bending torques on the pins 22 and on the arms 21.
Conveniently, the body 25 is a monolithic body made of plastic or elastomeric material resistant to wear by rubbing or, again, of composite material. According to a variant not shown, the portions 26 and 27 of the body 25 are distinct from each other and connected stably with each other or releasably. In both cases, one or both of the portions 26 or 27 can be chosen from a plurality of portions having different mechanical characteristics. In this way, it is possible to adapt the prosthesis 1 to the specific needs or requirements of the patient and, in particular, to create in an extremely simple and immediate way a range of prostheses with different stiffness, resistance and / or controlled shock damping.
Still with reference to Figures 1 and 2, the prosthesis 1 finally comprises two springs 32 and 33 arranged on opposite sides of the fork element 20 to maintain and return the attachment body 4 and the foot 5 to their relative relative position. rest or reference. In the particular example described, the spring 33 is a tension spring while the spring 32 is a compression spring. According to a different embodiment, both springs 32 and 33 are compression springs.
According to a further variant not shown, the springs 32 and 33 are replaced by elastomeric elements or by leaf or leaf springs performing the same function.
From the foregoing it is evident that the described prosthesis 1 allows patients with forefoot amputations to resume natural walking in a short time and without particular efforts or adaptations. What has just been explained is essentially a consequence of the fact that the described prosthesis 1, unlike the known solutions, practically creates substantially the same walking conditions originally allowed by the natural ankle joint since the attachment body 4 and the foot 5 are coupled together. by a hinge, the axis of which crosses a terminal portion of the body 4 itself and is arranged practically at the same height from the ground in which, before the amputation, the natural axis of rotation defined by the ankle joint was located. Not only. In the described prosthesis 1, the joint 16 has a high, but above all, an invariant reliability and functional efficiency. This advantage derives essentially from the fact that the fork-pin-socket contact areas are in use "discharged" by the reaction body 25, on which both the actions attributable to the patient's body weight and the actions transmitted from the outside act. . In other words, the presence of a cylindrical joint or surfaces suitable, under load, to slide against each other, reduce or completely avoid the formation of localized tensions in the area of the hinge pins which, if present, would reduce in time the efficiency and functionality of the joint are particularly short. The radial play present between the hinge pins and the relative holes, on the one hand, does not affect the relative rotation of the foot 5 with respect to the attachment body 4 and is not felt by the patient, and on the other hand, it allows the distal portion 4a of the attack body to lean on the reaction body which, through the heel, discharges the transmitted actions to the ground.
From the foregoing it is then evident how the manufacturing characteristics of the described prosthesis 1 make the prosthesis itself extremely efficient and reliable, configurable according to the various needs and, therefore, versatile and with particularly low manufacturing costs.
Finally, from the foregoing it is evident that modifications and variations can be made to the described prosthesis 1 without thereby departing from the protective scope defined by the claims. In particular, the reaction body 25 described could be made in a different way and with materials different from those indicated by way of example, but always in such a way as to reduce or cancel the loads acting on the prosthesis in the areas of the hinge pins.
