DESCRIZIONE DEL MODELLO INDUSTRIALE DI UTILITÀ’, avente per TITOLO : “ Strumento chirurgico di uso laparoscopico per enucleazione di mioma uterino (intervento di “miomectomia”) con funzione di presa tramite estremità a vite o cavatappi, caratterizzato da snodo distale a forcella e meccanismo di blocco in asse”. RIASSUNTO e STATO ANTERIORE DELLA TECNICA . NeU’ambito della chirurgia laparoscopica ginecologica la miomectomia laparoscopica è un intervento che richiede la possibilità di esercitare sul mioma adeguate trazioni al fine di isolarlo dal miometrio circostante ed enuclearlo con l’aiuto del bisturi elettrico, ovvero con strumenti a lama fredda o altro mezzo idoneo. SUMMARY and BACKGROUND OF THE ART. In the context of gynecological laparoscopic surgery, laparoscopic myomectomy is an operation that requires the possibility of exercising adequate tractions on the myoma in order to isolate it from the surrounding myometrium and enuclear it with the help of an electric scalpel, or with cold blade instruments or other suitable means .
Per tale ragione sono stati messi a punto, nel corso degli anni, vari strumenti : pinze a coccodrillo dal diametro di 10 e 5 mm ( Semm), strumento con punta a cavatappi (Semm), pinza a 2 denti tipo “pinza di Pozzi” del diametro di mm 10 (Marlow), strumento da presa con punta a vite del diametro di mm 5 e 10 (produzione J&D Medicai Salesa, Hyksville NY). Tali strumenti, come è uso generale in laparoscopia, vengono inseriti nel'addome, come pure lo strumento ottico (denominato “laparoscopio”) per la visione del campo operatorio, tramite dei tubi a tenuta di gas denominati “trocar” che vengono introdotti attraverso la parete addominale e lasciati in sede durante tutto l’intervento. La mancanza di uno snodo nella pinza da presa costringe l’operatore ad esercitare le proprie trazioni lungo l’asse dello strumento, limitando quindi le sue possibilità di trazione direzionale alla quella consentita dalle vie di accesso ( 2 o 3 trocar) della chirurgia laparoscopica, costringendolo quindi a spostare continuamente la pinza da presa da un trocar all’altro. Tale problema è ancor più sentito per via del fatto che le migliori pinze da presa in commercio ( con presa a coccodrillo ed a 2 denti “tipo Pozzi”) sono esclusivamente del diametro di mm 10, richiedendo per l’uso ovviamente un trocar di tale diametro che viene invece usualmente riservato al passaggio dell’ottica ed è poco pratico nell’uso degli altri strumenti chirurgici. Per ovviare alle suddette problematiche abbiamo ideato uno strumento composto di due parti di cui l’anteriore, della lunghezza di 5-6 cm. circa, presenta la estremità a vite o cavatappi per funzione di presa e si articola, tramite snodo a forcella con perno, con la parte prossimale, più lunga (circa 25-30 cm.), di struttura tubolare e terminante con impugnatura per l’operatore. All’interno del lume della parte prossimale scorre, coassiale, un filo metallico che, incastrandosi in una incisura della estremità posteriore del segmento anteriore dello strumento, funge da blocco della articolazione a forcella con le due parti in asse . DESCRIZIONE DEL MODELLO DESCRIPTION OF THE MODEL
Lo strumento chirurgico in oggetto ha funzione di presa su mioma uterino in corso di intervento laparoscopico, permettendone al contempo la maggiore mobilizzazione con esposizione di parti non altrimenti raggiungibili .Consta di una struttura di lunghezza variabile ( circa 30-40 cm.) in metallo, sterilizzabile. La parte anteriore dello strumento , più breve, della lunghezza di 5-6 cm. circa, presenta la estremità terminante a vite o cavatappi (punto a - tav. 1-2) e si articola tramite snodo a forcella con perno (punto b - tav. 1) con la posteriore; lunga circa 30-40 (punto c - tav. 1) . Tale snodo consente la mobilità della punta da presa intorno al perno per quasi 360°, conservando la possibilità di esercitare una trazione in varie direzioni, potendo imprimere al mioma anche spostamenti laterali e verso il basso, esponendo così segmenti dello stesso altrimenti inaccessibili e facilitandone il distacco, cosa impossibile con altri strumenti in uso formati da un unico pezzo. La presenza di uno snodo a perno consente il movimento delle due parti dello strumento unicamente intorno ad esso (1 grado di mobilità). Ciò a nostro avviso appare vantaggioso rispetto a strumenti similari con maggiore grado di mobilità (ad es. con snodo a giunto cardanico), in quanto la fissità negli altri piani consente allo strumento la necessaria rigidità per l’esercizio delle trazioni a cui è preposto. Lo strumento è inoltre congegnato per consentire la possibilità di bloccare in asse le due parti dello strumento ed ottenere alla bisogna uno strumento di tipo rigido, il che consente di avvitare e svitare la punta nello spessore del mioma. Ciò è reso possibile in quanto all’ interno del lume della parte prossimale scorre, coassiale, un filo metallico del diametro di circa 3 mm che, incastrandosi in una incisura della estremità posteriore del segmento anteriore dello strumento, funge da blocco della articolazione a forcella con le due parti in asse . Le due parti che compongono la impugnatura si solidarizzano tramite filettatura a vite. Il blocco in asse delle due parti dello strumento si otiene quindi facendo scorrere il filo (punto d - tav.l) per pochi mm. nel lume del tubo in modo da incastrare la sua estremità a punta in apposita incisura (punto e - tav.l) nella estremità prossimale della parte anteriore dello strumento (punto a - tav.1 ), tramite avvitamento nella apposita filetatura della parte posteriore della impugnatura nel cui intemo è fissato, tramite vite o altro mezzo idoneo, il filo coassiale. Il sistema di filetatura a vite tra le due parti dell’ impugnatura permette altresì di assicurare la fissità del blocco. The presence of a pin joint allows the movement of the two parts of the instrument only around it (1 degree of mobility). In our opinion, this appears to be advantageous compared to similar instruments with a greater degree of mobility (e.g. with a gimbal joint), as the fixity in the other planes allows the instrument the necessary rigidity for the exercise of the tractions it is responsible for. The instrument is also devised to allow the possibility of locking the two parts of the instrument in axis and obtaining a rigid instrument when necessary, which allows screwing and unscrewing the tip in the thickness of the myoma. This is made possible because inside the lumen of the proximal part runs, coaxially, a metal wire with a diameter of about 3 mm which, fitting into an incisor of the posterior end of the anterior segment of the instrument, acts as a block of the fork joint with the two parts aligned. The two parts that make up the handle are joined together by screw threading. The alignment of the two parts of the instrument is then obtained by sliding the wire (point d - tav.l) for a few mm. into the lumen of the tube so as to fit its pointed end into a special notch (point e - table l) in the proximal end of the front part of the instrument (point a - table 1), by screwing it into the appropriate threading of the rear part of the handle in which the coaxial wire is fixed by means of a screw or other suitable means. The screw threading system between the two parts of the handle also makes it possible to ensure the fixity of the block.
