ABSTRACT:

GII Authors present an innovative technique for the intraoral surface EMG recording of the lateral pterygoid muscles, responsible for the most important mandibular movements (protrusive and lateral disclusion) and investigated, so far, only through the use of a needle for skin or transgingival puncture.

This technique is based on the use of two recording electrodes designed by us and positioned inside the oral cavity.

The EMG thus detects the increase in the motor units affected by the maximum contraction, evaluates the amplitude of discharge and records the group function of the muscles and their symmetry.

It is an instrumental method for analyzing the dynamic functionality of the jaw, representing an effective complement to the clinical examination and, above all, an important moment of therapeutic control.

The EMG survey of masticators provides information on their static or functional activity. A fundamental complement to this routine investigation was found in the experimentation of a new surface analysis technique of the lateral pterygoid muscle that allows us to verify the activity of the condyle and meniscus in protrusive and lateral movements

The surface EMG examination of masticatory mm, up to now routinely conducted in specialized dental centers for TMD, shows their activity in both the static and functional phases (window and parafunction). An examination conducted in this way presents limits for the diagnosis and for the evaluation of therapeutic results, which are represented by: 1) A small period of registration in a laboratory and often without a quantitative analysis of the interferential trace; 2) Difficulty of repeatability of the recordings and therefore of the standardization of the mechanisms that regulate the mandibular posture; 3) Lack of any information relating to mandibular movements related to the most important activities such as disclusion in protrusive and laterality.

The incompleteness of analysis, after EMG examination, was therefore filled by an innovative surface EMG recording technique, adopted by the Aa on an experimental basis and the results of which are illustrated in the present work. The registration of lateral pterygoid mm, carried out up to now only through the use of a needle for percutaneous or transmucosal puncture, has therefore been overcome by the introduction of registration electrodes designed by the Aa.

The recording electrodes consist of a spherical Ag or Cu fusion with a diameter of 5mm spaced 1cm apart and an electrically insulated connection extension about ten cm long, from whose terminal the recording wire comes out. Emg.

The electrodes are introduced into the oral cavity in the upper gingival fornix after the last molar.

The positioning of the spherical plates is identified in the highest and distal point of the fornix of the zygomatic arch, after digital exploration.

The terminals are held in this correct position and spread outwards by 30 ° on each side.

To obtain an effective muscular contraction and therefore a clear registration, the spontaneous movement will be opposed by a hand placed on the mandible, while the patient, previously instructed, will perform the mandibular disclusion movements.

The check of the correct positioning of the plates is detected by the EMG scan which records the interferential trace of the activated muscle. A further verification of the exact positioning of the plates was obtained with a control CT scan conducted on a subject who had been inserted an upper plate whose terminals were positioned electrodes that confirmed the proximity between the detector and the pterygoid muscle itself.

Finally, a comparative EMG examination was performed to exclude interference in the recording of the buccinator muscle activity.

All registrations in the research standardization phase were performed in the neurophysiopathology service, with the collaboration of Dr. L. LISPI neurologist at San Camillo Hospital in Rome.

Subsequently, the Aa needed to carry out E.M.G. for the continuation of observations. Therefore, being portable electromyographs available on the market, they availed themselves of the collaboration offered by BIOTRONIC, which adapted the registration plates, connecting them to their portable electromyographs.

Purpose of the study:

• obtain a quantitative analysis of the dynamic activity of the clinically most important muscle of the mandible, because it is also responsible for the functional activity of the meniscus;

• long-term monitoring of chewing activity;

• detection, not only of the increase in the number of motor units affected by the maximum contraction and of the amplitude of discharge, which increases in the interferential trace, but also of the group function of mm. and their symmetry;

• obtain, with the evaluation of the condyle-meniscal relationships in the glenoid fossa, the diagnostic and therapeutic indications.
