In modo più specifico, tale dispositivo è del tipo che comprende un sacchetto dove vengono riposti i pezzi anzidetti ed un applicatore a tubo dove il sacchetto viene alloggiato per essere introdotto in una cavità addominale da operare; l’applicatore è altresì dotato di mezzi per fare fuoriuscire il sacchetto nella cavità di cui sopra. A tale riguardo si segnala che per il modello di dispositivo noto appena illustrato, è anche prevista la possibilità di mantenere il sacchetto aperto nella cavità addominale dopo aver rimosso l’anello di pretensionamento: in tal caso bisogna però eseguire la separazione di quest’ultimo dal sacchetto diversamente da quanto visto prima, cioè non mediante la chiusura dell’imboccatura del sacchetto a seguito della trazione sulla sua sutura in poliammide, ma bensì agendo nella cavità addominale per praticare un taglio parziale del sacchetto in corrispondenza dell’anello. Da quanto sopra esposto è possibile comprendere che sebbene questo modello di dispositivo abbia il pregio di una apertura automatica e rapida del sacchetto grazie all’anello elastico di pretensionamento, esso risulta altresì caratterizzato da una scarsa maneggevolezza per via delle dimensioni dell’anello una volta che questo si apre nella cavità da operare, e da una certa laboriosità nel funzionamento; soprattutto è da sottolineare il fatto che in questo modello noto di dispositivo, una volta che il sacchetto viene chiuso non è più possibile riaprirlo: in altri termini, con un simile dispositivo non si possono eseguire cicli di apertura e successiva chiusura del sacchetto che sono molto utili in taluni interventi, ad esempio quelli in cui devono essere prelevate più parti anatomiche (asportazioni di piccoli nodi linfatici, biopsie multiple, etc.) in successione all’interno della cavità addominale, senza pericolo di disperderle. Simili difficoltà si possono inoltre verificare anche nel caso delle eventuali successive aperture del sacchetto, così da limitare le reali prestazioni di questo dispositivo. For the use of the device of this invention, starting from its layout in fig. 4, the distal end 2 la of the applicator tube is introduced into the abdominal cavity to be operated through a laparotomy, in the desired position; pushing the stem 23 into the tube 21, the piston 24 integral with it advances towards the distal end 21a above and acts on the bag contained therein making it come out of the tube itself.

Analogamente, per la riapertura del sacchetto a partire da questa condizione è sufficiente agire in modo inverso facendo scorrere l’anello 10 rispetto all’elemento flessibile 7, così da riportare i due lobi 7a e 7b alle dimensioni iniziali che in questo caso corrispondono con quanto mostrato nella fig. 2; occorre poi sottolineare come l’elemento flessibile 7 contribuisce anch’esso a riportare il sacchetto nella condizione aperta in virtù della sua elasticità, e questo aspetto rende quindi possibile eseguire ripetuti cicli di chiusura ed apertura del sacchetto senza alcuna difficoltà. Infatti si è visto che per quanto concerne la prima apertura del sacchetto nella cavità addominale, essa avviene in modo rapido e senza la necessità di uno specifico intervento da parte del chirurgo per svolgere il sacchetto, similmente a quanto accade nel primo modello noto di dispositivo preso in esame precedentemente; contestualmente poi, il dispositivo in accordo con il presente trovato permette di aprire e chiudere più volte il sacchetto al fine di riporvi pezzi anatomici prelevati in successione, grazie al comportamento elastico dell’elemento flessibile messo in luce nell’esempio realizzàtivo qui riportato. In altre parole, si può affermare che nel dispositivo dell’invenzione l’elemento flessibile 7 offre le medesime prestazioni del già citato anello di pretensionamento, previsto nel relativo modello noto, per ciò che riguarda la prima apertura del sacchetto: grazie però alla sua flessibilità ed elasticità, tale elemento può venire deformato ripetutamente al fine di consentire la chiusura ed apertura ciclica del sacchetto. Un ulteriore effetto rilevante connesso con il presente trovato, risiede nel fatto che anche da un punto di vista medico esso non presenta di per sé delle controindicazioni; più in particolare, da quanto sopra esposto è possibile apprezzare che l’impiego dell’elemento flessibile anzidetto non comporta rischi di lesioni nella cavità addominale cui viene applicato il dispositivo: infatti, anche se tale elemento durante l’apertura del sacchetto urta la parete della cavità stessa o di qualche organo ivi contenuto, la sua flessibilità consente di non danneggiare i tessuti con i quali entra in contatto prevenendo il pericolo di ferite. E’ peraltro evidente che un simile vantaggio si riflette positivamente sulla eventualità di ottenere le necessarie autorizzazioni alla messa in commercio di questo dispositivo per la rimozione di pezzi anatomici. It is also clear that such an advantage has a positive effect on the possibility of obtaining the necessary authorizations for the marketing of this device for the removal of anatomical pieces.

Occorre poi sottolineare come i risultati dell’invenzione che sono stati qui spiegati, non sono ottenibili dal di spositivo del secondo modello conosciuto e di cui si è fatta prima la disamina; il nastro in materiale plastico di cui esso è dotato, infatti, è solamente pieghevole e non fornisce una azione elastica minimamente paragonabile a quella dell’elemento flessibile di questa invenzione: conseguentemente in tale dispositivo noto la prima apertura del sacchetto presenta i problemi già discussi sopra ed in aggiunta, anche le successive aperture e chiusure sono condizionate dai difetti connessi con le caratteristiche di funzionamento intrinseche più sopra evidenziate. Claims

CLAIMS 1. Device for the removal of anatomical pieces in laparoscopic surgical procedures a, which includes a pouch (3), a tube applicator (21) where the pouch (3) is housed to be introduced into an abdominal cavity, which applicator is equipped with means (22-26) for making the bag (3) come out into the aforementioned cavity, characterized in that the bag (3) is associated along the edge of its mouth (5) with a flexible element ( 7) closed and having characteristics of elasticity such as to bring it back to a substantially slot configuration when it is not subjected to deformation.

2. Device according to claim 1, in which the flexible element (7) is a helical spring.

Device according to claims 1 or 2, in which the flexible element (7) is coupled in a sliding manner with a substantially annular clip (10), defining two lobes (7a, 7b) substantially configured as a slot and of variable dimensions a following the reciprocal sliding between the flexible element and the clip.

4. Device according to claim 3, in which the flexible element is bent at "8" and the clip (10) is located astride the junction of the lobes (7a, 7b) forming this "8".

Device according to any one of the preceding claims, in which the means (22-26) of the tube applicator (21) for making the bag (3) come out comprise a cursor (24) sliding back and forth inside the tube (21) itself, moved by means of a stem (23) passing through the proximal end (21b) of the tube (21), said applicator being equipped with means (25) for sealing the stem.

6. Device according to claim 5, wherein the means for effecting the seal on the stem (23) comprises a membrane (25).

7. Device according to any of the preceding claims, in which the hose applicator (21) is equipped with means (24, 24a) for hooking the flexible element (7) associated with the bag (3).

8. Device according to claim 7 when it depends on claim 5, wherein the means (24, 24a) for hooking aforesaid comprise a hook part (24a) of the slider (24).

9. Device according to any of the preceding claims, in which the bag (3) is housed in the tube (21) of the relative applicator in a rolled-up condition around the flexible element (7) or a part (7a) of it.

10. Device according to any one of the preceding claims, in which the bag is made of a material chosen from polyurethane, Kevlar and polypropylene,
