La promozione e l'accelerazione dei processi di cicatrizzazione è da sempre un obiettivo terapeutico di preminente interesse nel trattamento di lesioni cutanee di vario tipo come ulcere varicose ed arteriose, piaghe da decubito, ustioni, ferite traumatiche e chirurgiche. Gravi e multiformi sono i problemi che si debbono affrontare nella terapia di queste patologie e delle complicazioni successive ad interventi chirurgici che comportano necrosi del tessuto cellulare e/o processi riparativi di seconda intenzione. The promotion and acceleration of healing processes has always been a therapeutic objective of paramount interest in the treatment of various types of skin lesions such as varicose and arterial ulcers, pressure sores, burns, traumatic and surgical wounds. Serious and multifaceted are the problems that must be faced in the therapy of these pathologies and of the complications following surgical interventions that involve necrosis of the cellular tissue and / or reparative processes of secondary intention.

Esso trae orìgine dalla tradizione popolare, ma più recentemente una serie di studi ne hanno confermata l'utilità e l'efficacia terapeutica facendo luce sulla struttura dei prìncipi attivi e sul loro meccanismo d'azione. The use of Centella Asiatica, or of Centella Asiatica extracts, has long been known to favor the healing processes. It originates from popular tradition, but more recently a series of studies have confirmed its usefulness and therapeutic efficacy, shedding light on the structure of the active principles and their mechanism of action.
Da lungo tempo gli acidi asiatico, madecassico ed il glucoside asiaticoside sono stati indicati come i principali componenti ed i prìncipi attivi dell'estratto. For a long time, the Asiatic, Madecassic acids and the Asiaticoside glycoside have been indicated as the main components and active principles of the extract.
Comune meccanismo d'azione di questi terpenoidi della Centella Asiatica sarebbe la stimolazione dei fibroblasti a promuovere la sintesi di collagene [Maquart et al., Connect. Notoriamente, la causa del successo non ottimale di questi tipi di trattamento farmacologico, che ne rappresenta l'elemento limitante, è posta nell'inadeguata biodisponibilità dell'insieme dei principi attivi costitutivi dell'estratto. Un problema così vero e di generale avvertimento che, anche recentemente, si è tentato di porvi rimedio con l'impiego di miscele di fosfolipidi sintetici e naturali per elaborare nuove composizioni farmaceutiche a base di seracosidi [E. Bombardelti et al., E.P. Appi. 283,713 (28.09.1988)]. J. De Souza et ai., E.P. Appi. 383171 (22.08.1990)] o come coadiuvante in quello delle ulcere gastriche [Chatterjee et al., Indian J. Exp. Biol., 30,889, (1992)]. Infine, recentemente è stato proposto l'uso dell'asiaticoside come tool farmacologico, poiché è un inibitore selettivo e dose dipendente delle contrazioni dell'ileo di cavia indotte da serotonina ma non di quelle da bradikinina. Per quanto, meglio che per il passato, le più moderne tecniche analitiche consentano la valutazione qualitativa e quantitativa dei componenti di estratti vegetali, la loro composizione variabile (in funzione della sottospecie vegetale, dei tempo di raccolta, delie diverse condizioni climatiche, dello stoccaggio e delle diverse manipolazioni e purificazioni alle quali un estratto può essere sottoposto) è generalmente riconosciuta uno dei principali aspetti che limitano la riproducibilità di precisi effetti farmacologici. Un aspetto non sempre positivo è l'uso di miscele eterogenee di principi attivi in particolare quando alcuni dei principali componenti non sono di per sè attivi (come i glucosidi asiaticoside e madecassoside nel caso della Centella Asiatica) ma possono divenire tali solo dopo conversione metabolica nei loro agliconi (Rush e Grimaldi, prima citati). Un aspetto questo limitante e poco predittivo sotto il profilo terapeutico per l'individualità della risposte cliniche che saranno da attendersi, non solo funzione della bio-disponibilità dei singoli componenti e dello stato di salute, età e sesso del paziente ma anche e soprattutto della attitudine metabolica di quest'ultimo, cioè della capacità individuale di elaborazione metabolica dei singoli glucosidi. Although, better than in the past, the most modern analytical techniques allow the qualitative and quantitative evaluation of the components of plant extracts, their variable composition (depending on the plant subspecies, the harvest time, the different climatic conditions, storage and of the various manipulations and purifications to which an extract can be subjected) is generally recognized as one of the main aspects that limit the reproducibility of precise pharmacological effects. A not always positive aspect is the use of heterogeneous mixtures of active ingredients in particular when some of the main components are not active in themselves (such as the glycosides asiaticoside and madecassoside in the case of Centella Asiatica) but can become such only after metabolic conversion in the their aglycones (Rush and Grimaldi, cited above). This is an aspect that is limiting and not very predictive from a therapeutic point of view for the individuality of the clinical responses to be expected, not only a function of the bio-availability of the individual components and of the state of health, age and sex of the patient but also and above all of the aptitude metabolic rate of the latter, i.e. of the individual metabolic processing capacity of the individual glucosides.

Essi non sembrano attribuibili all'uso dell'estratto grezzo come tale ma, più probabilmente, all'associazione con eccipienti e veicoli diversi usati nella preparazione delle formulazioni farmaceutiche fin qui disponibili. In fact, better than the extracts, the single active principles and their mixtures, the use of these salts allows the preparation of pharmaceutical formulations which are dispersible in water and which in any case contain the active principles or allow their administration in a highly dispersed form. Among the salts of the invention of formula (1), particularly preferred and most suitable compounds for the preparation of said pharmaceutical compositions, useful for oral administration, are the salts of L-lysine and L-arginine of formula 1, to be administer to the adult in a dose interval between 10 and 150 mg / day. The use of L-lysine and L-arginine salts, even in the case of water-dispersible formulations for oral use, is particularly advantageous due to the peculiarities of these salts such as high tolerability accompanied, moreover, by a more adequate and better bioavailability.
