Description
Descrizione dell’invenzione industriale Description of the industrial invention
The present invention relates to pharmaceutical tablets with high and rapid availability of the active principle conveyed therein.
One of the main requirements that a pharmaceutical form that carries an active ingredient with pain-relieving, antiaginosa and analgesic activity in general must have is that of rapidly determining effective plasma levels and therefore recovery. in a short time, de! desired therapeutic effect.
Now, in the most striking and serious cases of painful manifestations, it is necessary to resort, on the recommendation and on medical prescription. to the administration of drugs and / or active substances, preferably parenterally and preferably intravenously. This obviously entails a series of problems linked both to the availability of the pharmaceutical form, which must be compulsorily prescribed by the doctor, and to the need to have recourse, for administration, to specialized medical or paramedical personnel.
However, there is a whole series of pathological pictures and painful manifestations of lesser importance (rheumatic pain, menstrual pain, headaches, etc.) which, generally, occur with cyclic relapses, for which it is preferable to resort to pharmacological treatments with simpler methods of administration. Particularly useful is the oral administration of antalgic, painkiller and antianginal drugs, that is, overcoming many of the difficulties related to parenteral administration.
However, if, on the one hand, the dosage schedule is simpler, oral administration involves, in most cases, a considerable delay in the onset of the therapeutic effect compared to parenteral administration. The solid pharmaceutical form (be it capsule or tablet) must in fact undergo processes of disaggregation and subsequently of dissolution of the active principle conveyed in it. Only the dissolved (solubilized) active substance can be absorbed and therefore determine effective plasma levels and exert the desired therapeutic effect.
This series of processes can take place in profoundly different times and ways and therefore the therapeutic efficacy can manifest itself even after an excessively long period of time from administration as the whole series of processes, generally indicated, from the biopharmaceutical point of view, as LADME (Liberation Absorption, Distribution, Metabolism, Elimination) and above all the time interval for the occurrence of the first two factors (liberation and absorption), can nullify or drastically reduce the therapeutic efficacy of a preparation intended for the treatment of acute painful manifestations. .
This delay, in the onset of the therapeutic effect of an active substance administered orally, can also be determined by the poor solubility of the drug in aqueous medium or in gastric or intestinal fluids, which causes a slowdown in the phases of release and absorption of the active principle.
To overcome this drawback, which is found for example with many non-steroidal anti-inflammatory drugs (NSAIDs), there have been many proposals and advanced solutions such as, for example, the preparation of complexes with cyclodextrins or the use of derivatives (salts, ethers , foreign or others). These complexes and derivatives, which generally have better characteristics of solubility and bioavailability, are however characterized by the fact that they themselves constitute new molecular entities for which, correctly, the health authorities require, for any therapeutic use, a whole series of chemical, chemical-physical, technological and above all pharmaco-toxicological and clinical documentation capable of demonstrating the specific activities of the new molecular entity.
A second and not negligible aspect, in the preparation of complexes with cyclodextrins, is determined by the fact that in the formation of the complex, equiniolecular ratios between active pyrnipium and cyclodextrin are generally used.
Since the molecular weight of cyclodextrin (in its beta form, most frequently used) is about 1135 and the molecular weight of an anti-inflammatory such as ibuprofen is about 206, it follows that the molecular complex (1: 1) Ibuprofen / Cyclodextrin has a molecular weight of about 1341, ie to administer 200 mg of ibuprofen it is necessary to use about 1300 mg of the aforementioned complex alone, an absolutely excessive amount for the preparation of a solid form such as tablet or capsule. Furthermore, such pharmaceutical forms require, as is well known to those skilled in the art, the use of other excipients and technological adjuvants which obviously cause a further increase in the weight of the pharmaceutical form which is practically impossible to administer.
Other attempts have been directed more specifically to pharmaceutical technology and auxiliary components that usually accompany the active ingredient.
As regards the first aspect, an attempt was made to greatly reduce the particle size of the active principle by resorting to the so-called micronization.
The modest advantages obtained with this technological approach do not justify the higher production costs and the problems it entails (for example the great difficulty in handling impalpable powders):
As regards the additives and auxiliaries, an attempt was made to intervene on the breaking and crushing characteristics of the tablets (as this is the preliminary condition for making the active ingredient available) using substances that take the name of disintegrants and super-disintegrants and which are added to the pharmaceutical formulation to an extent that is usually around 1-2% by weight, with maximum peaks of less than 10%.
Tra questi superdisgreganti di possono citare i seguenti composti polivinilpirrolidone reticolato, sodio carbossimetilcellulosa reticolata, carbossimetilamido, sodiocarbossimetilamido, copolimero potassio metacrilatodivinilbenzene, polivinilalcoli, amidi derivati degli amidi, cellulosa microcristallina e derivati cellulosici, beta ciclodestrina e derivati dellle destrine in genere. Among these superdisegregants the following compounds can be mentioned cross-linked polyvinylpyrrolidone, cross-linked sodium carboxymethylcellulose, carboxymethyl starch, sodium carboxy methyl starch, copolymer potassium methacrylatedivinylbenzene, polyvinyl alcohol, starch derivatives of starches, cellulose microcrystalline derivatives and derivatives of the genus cellulose.
Another approach from the point of view of additives and auxiliaries consisted in adding a substance with a surfactant effect to the formulation, in order to promote the solubilization of the active ingredient following the increase in the wettability of the solid substances forming the tablet or capsule.
Naturally, additives of this kind were selected from pharmaceutically acceptable and compatible ones, as non-ionic surfactants selected from polysorbates, sorbitan derivatives (monolaurate and nionopalmitate) and anionic surfactants such as sodium lauryl sulfate and sodium tetradecylsoiphate.
However, none of these approaches has led to a satisfactory solution to the problem for which there is still the need to create a solid pharmaceutical form that can be administered orally, capable of releasing the active principle immediately after administration in order to bring the piasmatic level of the principle. active at desired values within a limited time interval thus determining the desired therapeutic activity.
This and other purposes are achieved with the present invention which provides a pharmaceutical form for the oral administration of at least one active principle, in the form of a tablet possibly coated with a gastro-resistant material and comprising said at least one active principle and adjuvant substances, characterized from the fact that said at least one active ingredient is in micronized form with a particle size not exceeding at least 20 microns and said adjuvant substances comprise at least 10% of a compound belonging to the class of disintegrants or super-disintegrants and at least 0.01% of a pharmaceutically compatible tension.
In the preferred embodiment of the tablet according to the present invention, said disintegrating and / or super-dissolving agents and said tenase active are selected from those previously mentioned and the micronization of the active principle is such that the size distribution is less than 10 microns.
In a particular embodiment of the tablet according to the invention, the adjuvant belonging to the class of disintegrating and / or super-dissolving agents is used in a particular way, since a part of it is mixed with the active principle and with the surfactant, while the remaining part it is used in the preparation of the excipient phase of the tablet.
In this way, after administration, the first coarser disintegration of the tablet into coarse fragments takes place, after which the disintegrant mixed intimately with the active principle causes a second disintegration that releases the micronized particles of the active principle, particles which thanks to the presence of the surfactant quickly pass into solution.
Basically, in the tablet object of the present invention particular expedients are used in the formulation of an active ingredient which is not very soluble in water (or with a limited solubility) and new and specific excipients and technological adjuvants are used for which it is possible to obtain a solid pharmaceutical form ( tablet) characterized by a rapid and complete disintegration and by an equally rapid dissolution process as will be highlighted by the results of appropriate in vitro tests. The rapid disintegration and dissolution also make it possible to drastically reduce the release phase of the active ingredient from the pharmaceutical form and to promote absorption, thus causing a rapid and high appearance of plasma drug levels and the appearance of the desired therapeutic effect.
The new pharmaceutical form is mainly intended for the administration of one or more drugs which must act immediately after administration in order to determine a piasmatic level and therefore a therapeutic activity within a very limited time interval; preferably, but not limitedly, the invention can be used in the formulation of non-steroidal anti-inflammatory drugs and in general for antalgic, anti-asthma drugs, characterized by poor or limited solubility in water. The new embodiment, object of the present patent, therefore allows using consolidated production technologies and using particular excipients and known per se in combination with new formulation criteria, to obtain pharmaceutical forms, generally usable for oral administration, capable of yielding in a range of in a very short time the active principle is conveyed in them, in a complete and rapid way, such as to allow a high bioavailability, as will be better highlighted in the examples accompanying the present application.
The tablets according to the invention are intended, in general, for the oral administration of an active ingredient which can consist of 1- a single drug that is immediately sold
2- a combination of two or more drugs that are both sold immediately after administration.
In the pharmaceutical form described, active ingredients such as: non-steroidal anti-inflammatory drugs (NSAID) or steroids or sleep-inducing substances and tranquilizers such as diazepam, nitrazepam, clorazepam, or antihistamine drugs and: or anti-asthmatics such as terfenadine, chlorpheniramine can be conveyed.
In the case of active ingredients that are not very soluble in water (such as Ibuprofen), the use of particular adjuvants is also used, as will be better highlighted in the examples accompanying the present invention.
Natural and / or synthetic and / or semi-synthetic polymers belonging to the class of so-called super-disintegrating agents and / or disintegrants such as: cross-linked polyvinylpyrrolidone, cross-linked sodium carboxymethylcellulose can be used as adjuvant substances in the preparation of the rapid-release tablet of the active ingredient. , carboxymethyl starch, sodium carboxymethyl starch, copolymer potassium methacrylate divinylbenzene, polyvinyl alcohols, starch derivatives of starches, microcrystalline cellulose and cellulosic derivatives, beta cyclodextrin and derivatives of dextrins in general with a disintegrating function.
Said polymeric substances constitute at least 10% of the weight of the tablet itself, and preferably from 15 to 70% and can be conveyed in different ways. A part of said substances can be used in admixture with the active principle so as to be contained in the granules (ie as an intragranular disintegrant) and a part can be added to the granulate thus constituting the so-called extragranular disintegrant.
Another increase in the dissolution rate is achieved by using the active substance in the form of a micronized powder which must have a particle size distribution within a defined size range.
In the case of ibuprofen, the size distribution must be less than 20 microns and preferably less than 10 microns.
A further improvement in the speed of disintegration of the tablet and dissolution of the active ingredient conveyed is obtained with the use of said substances with surfactant activity and favoring the wettability of the compact and the solubilization of the active ingredient. These substances must be pharmaceutically acceptable and are preferably non-ionic surfactants with different HLB (hydrophiiic / lipophilic balance) values, such as polysorbates, sorbitan derivatives such as sorbitanmonolaurate, sorbitan-monopalmitate or anionic surfactants such as sodium tetradecyl sulfate.
The substance with surfactant activity and favoring the wettability and solubilization of said active principle is contained in a percentage between 0.01% and 25% with respect to the active principle. In order to favor a more homogeneous distribution, it can be solubilized and / or dispersed in the solution or suspension used as a binder for the preparation of the granulate.
They can also be used in the formulation, Elitre adjuvant substances constituted by the so-called effervescent mixtures, that is capable of producing the rapid disintegration of the tablet when it comes into contact with aqueous liquids and, preferably with gastric juice. These substances can include sodium carbonates and bicarbonates and other alkaline or alkaline earth metals, sodium carbonate glycol and other pharmaceutically acceptable salts capable of producing effervescence in an acid environment.
Finally, excipients of normal use in the pharmaceutical technique such as mannitol, lactose, magnesium stearate, colloidal silica and others such as glycers and monostearate, hydrogenated castor oil, waxes, mono-bi and trisubstituted glycerides can be used.
On said finished tablets, a film of gastro-resistant and enterosoluble polymeric material can be further applied. in order to allow the system to be activated only after the tablet has reached the duodenal-intestinal tract.
