La presente invenzione concerne composizioni antiossidanti da impiegarsi nella prevenzione e nel trattamento delle patologie ascrivibili al decadimento mitocondriale. Sono noti gli effetti dannosi prodotti dalle specie reattive dell’ossigeno (ROS) che si formano nelle cellule, principalmente a livello mitocondriale, nel normale metabolismo energetico. Le cellule hanno sviluppato dei meccanismi di difesa per debellare la produzione di tali forme radicaliche, cioà ̈ di metaboliti che mostrano effetti tossici alterando la permeabilità delle membrane cellulari e l'integrità dì compartimenti, come quelli mitocondriali. In questo modo, i ROS interferiscono con i normali processi biochimici e biomolecolari scatenando spesso cascate di segnali che si concludono con la senescenza o con l’apoptosi cellulare. Un significativo incremento dei ROS, associato ad una diminuzione dell’attività degli enzimi antiossidanti, si manifesta, ad esempio, nelle fasi associate all'invecchiamento di un organismo. In particolare, estremamente rilevante risulta il decadimento mitocondriale a carico delle cellule neuronali e cardiache, le cellule maggiormente colpite durante la fase di invecchiamento. È stato osservato, infatti, che il danno ossidativo che si rileva nei mitocondri risulta sensibilmente più alto di quello di altri organelli. Questo fenomeno à ̈ dovuto principalmente al fatto che i mitocondri rappresentano dei compartimenti cellulari in cui si sviluppano delle significative funzioni metaboliche che regolano il fabbisogno energetico della cellula. L’invecchiamento porta ad una ridotta capacità catalitica di vari enzimi mitocondriali. Tra questi, particolarmente danneggiati risultano quelli dei complessi respiratori III e IV. Alcuni degli enzimi di questi due complessi riducono la loro affinità per l’ubichinolo e il citocromo C ed innescano cascate di reazioni che possono portare aH’apoptosi cellulare. La perdita di funzione di questi enzimi, scatenata dallo stress ossidativo, riduce l’efficienza dei sistemi di difesa intracellulare. Il danno ossidativo prodotto dai ROS nei mitocondri colpisce principalmente il DNA/RNA mitocondriale, provocando la sintesi di enzimi mutati con ridotta affinità per i loro substrati o con ridotta attività catalitica. Come facilmente immaginabile, una situazione di questo tipo innesca un ciclo vizioso che causa un continuo aumento del danno mitocondriale compromettendo le funzioni cellulari riducendo la capacità della cellula di produrre ATP. Le cellule maggiormente danneggiate perciò da quello che viene definito “decadimento mitocondriale", inteso come diminuzione dell’integrità strutturale e funzionale del mitocondrio, sono quelle in cui viene richiesto un elevato consumo di ATP. In particolare, come detto, tale composizione à ̈ impiegata nella prevenzione e nel trattamento delle patologie ascrivibili al decadimento mitocondriale tipico dell'invecchiamento cellulare e che si manifesta in situazioni associate ad uno stress fisico prolungato oltre che a neuropatie, patologie neurodegenerative, cardiomiopatie, sindromi canicolari, radiculopatie, vasculopatie e disturbi dell’invecchiamento cronologico. In particolare, tali patologie comprendono Morbo di Alzheimer, Morbo di Parkinson, Ictus cerebrale, neuropatie diabetiche, sindromi del tunnel carpale/tarsale, lombosciatalgie, cervicobracalgie, ernia discale, neuropatie diabetiche, neuropatie periferiche, neuropatie post herpetiche e neuropatie ottiche. Nell'organismo viene sintetizzato nel mitocondrio a partire dall’acido octanoico. L’acido lipoico viene anche assunto dall’esterno attraverso la dieta e poiché quello endogeno non à ̈ spesso sufficiente a svolgere un’efficiente azione antiossidante, la sua assunzione à ̈ accompagnata da una maggiore efficienza dei sistemi biologici nei quali à ̈ coinvolto. Va però evidenziato l’acido lipoico, contenuto in diversi alimenti (soprattutto nella carne rossa, patate, broccoli spinaci) à ̈ scarsamente assorbito in quanto si presenta legato alla lipolisina. Inoltre, come evidenziato da diversi studi di farmacocinetica, il rapido assorbimento à ̈ comunque associato ad una bassa concentrazione piasmatica di acido lipoico, in quanto, l’acido lipoico da una parte viene rapidamente trasportato ed accumulato in vari tessuti (soprattutto fegato, cuore e muscolo scheletrico) e dall’altra viene rapidamente metabolizzato ed escreto, principalmente attraverso la via urinaria. Nell’organismo, α-LA viene facilmente ridotto ad acido diidrolipoico (DHLA). Data la sua particolare struttura chimica, DHLA funge da trasportatore di elettroni e da antiossidante e, diversamente da altri antiossidanti, svolge la sua azione sia in ambiente idrofilo (citoplasma, matrice extracellulare) che in ambiente lipofilo (membrane piasmatiche). Considerando che mostra una rapida assorbibilità ed à ̈ trasportato facilmente attraverso le membrane, l’acido lipoico esplica la sua attività in diversi compartimenti cellulari. Il basso potenziale di ossidoriduzione del sistema LA/DHLA giustifica la capacità dell’acido lipoico di essere coinvolto nelle reazioni di neutralizzazione delle specie reattive dell’ossigeno (ROS) e nel processo di riduzione delle forme ossidate di altri antiossidanti. Sia la forma ossidata dell’acido α-lipoico, che quella ridotta contribuiscono a determinare la sua complessiva azione antiossidante, ma il contributo di DHLA, per la sua azione riducente diretta, à ̈ sensibilmente maggiore. L'azione antiossidante dell’acido lipoico à ̈ stata ampiamente dimostrata “in vitro†e “in vivo†. I meccanismi attraverso i quali si estrinseca l’azione antiossidante sono molteplici ed alcuni di essi non sono ancora del tutto chiari. Il sistema α-LA/DHLA rafforza e completa la rete difensiva messa a punto da altre molecole antiossidanti. Nella forma ridotta, DHLA, l’acido lipoico à ̈ in grado di donare i suoi elettroni alle forme ossidate del glutatione (GSSG) e della vitamina C, rigenerandole a glutatione ridotto (GSH) e ad acido ascorbico. L’acido lipoico ossidato viene rapidamente ridotto ad opera delia lipoammide deidrogenasi usando come agente riducente NADH. Per questa caratteristica, e cioà ̈ quella di riciclare altre fonti antiossidanti, l'acido lipoico viene spesso definito “antiossidante di antiossidanti". Nel miocardio e nel fegato, con l’invecchiamento, si ha una drastica riduzione della quantità e di conseguenza dell'attività del glutatione. Poiché non à ̈ facilmente assorbibile a livello intestinale, in queste situazioni risulta poco efficiente un’integrazione per via orale del glutatione. In quest’ottica, formulazioni a base di acido lipoico possono risultare di grande utilità per incrementare i livelli di glutatione nel nostro organismo. Un trattamento per 2 ore con H202(1,5 mM) determinava una significativa produzione di ROS (f.i.: 19,2). Questa produzione era ridotta in seguito ad una somministrazione di R-a-acido lipoico (f.i.: 9.3), ma solo debolmente con acetil-L-carnitina (f.i.: 14.6) o con L-selenometionina (f.i.: 17.9) quando usate singolarmente (Rif. 1, Rif. 2 e Rif. 3 rispettivamente), rilevata a 24h da tale somministrazione. La combinazione di Rif. 4, ossia R-a-acido lipoico - acetil-L-carnitina, mostrava invece una attività anti-ROS decisamente debole e quindi del tutto ìnsoddìsfacente (f.i. 17.1). Come detto, uno degli enzimi maggiormente interessati a questo processo à ̈ la glutatione perossidasi (GPx) che riduce il perossido d’idrogeno a spese del glutatione. Tale enzima à ̈ una selenoproteina che nel suo sito attivo necessita di Se, incorporato sottoforma di selenocisteina. Un apporto esogeno di L-selenometionina si traduce con una maggiore disponibilità di selenio per la sintesi di cisteina modificata da utilizzare per la sintesi di GPx. Come descritto in Figura 3, R-a-acido lipoico determinava un’induzione di GCLC in entrambi i tipi cellulari, indicando che alla concentrazione di studio (concentrazione testata anche nello studio della produzione dei ROS, Esempio 3) era sufficiente ad attivare il complesso meccanismo per la trascrizione di questo gene. Per rilevare il glutatione si usava un kit colorimetrico (ApoGSH Glutathion, Valter Occhiena) che sfruttava il sistema di rigenerazione del glutatione (GSH/GSSG) in presenza del substrato chimico DTNB e dell’enzima glutatione riduttasi. L’assorbanza rilevata a 405 nm era proporzionale al contenuto di glutatione in forma ridotta presente nelle cellule. I valori ottenuti (ng di GSH) erano normalizzati con le quantità di proteine estratte nei singoli campioni e determinate mediante il saggio di Bradford. A colorimetric kit (ApoGSH Glutathion, Valter Occhiena) was used to detect glutathione, which exploited the glutathione regeneration system (GSH / GSSG) in the presence of the chemical substrate DTNB and the enzyme glutathione reductase. Anche in questo caso, la combinazione di Rif. 4, ossia R-a-acido lipoico - acetil-L-carnitina, mostrava un risultato peggiore del solo R-a-acido lipoico. Again, the combination of Ref. 4, ie R-a-lipoic acid - acetyl-L-carnitine, showed a worse result than R-a-lipoic acid alone.

In particolare, tale composizione si à ̈ mostrata sorprendentemente e vantaggiosamente in grado di esaltare ed incrementare la funzione di regolazione della trascrizione genica e la capacità antiossidante attribuibile all’acido R-alipoico, grazie alla opportuna combinazione con la acetil-L-carnitina ed il selenio, sorprendentemente ottenendo la riattivazione delle funzioni mitocondriali compromesse in caso di stress ossidativo e l’aumento dell’attività antiossidante endogena. In questo contesto, l’acetil-L-carnitina ed il selenio, per motivi differenti, mostrano infatti un’azione sinergica con l’acido R-a-lipoico potenziandone la capacità di debellare le specie reattive dell’ossigeno e quindi vantaggiosamente preservando la funzionalità cellulare.
