Tra le più comuni patologie tricologiche sono da annoverare l’alopecia androgenetica, il telogen effluvium acuto o cronico e le forme di defluivo cronico. Among the most common trichological diseases are androgenetic alopecia, acute or chronic telogen effluvium and forms of chronic discharge.
L’alopecia androgenetica à ̈ caratterizzata da una maggior caduta e da un assottigliamento diffuso dei capelli dell’area del vertice, con tendenza ad un progressivo diradamento, sicuramente riconducibile ad un’alterata recettività dei follicoli allo stimolo negativo esercitato dagli androgeni . Il telogen effluvium à ̈ una caduta massiccia ma limitata nel tempo, con tendenza ad auto-risolversi con restituito ad integrum, dovuta ad un brusco e massiccio passaggio in fase di caduta (telogen) di un elevato numero di follicoli: le cause non sono ben note ma ruoli scatenanti possono essere rappresentati da pregresse situazioni di stress fisico e psichico, marcate carenze di ferro, diete drastiche , brusche modifiche ormonali (per esempio post-partum). I fattori di crescita rappresentano una via di trasferimento dei segnali di regolazione cellulare a livello della papilla dermica e delle altre strutture del bulbo pilifero, segnali determinanti per la sua trasformazione ciclica attraverso le varie fasi, determinando la rapida proliferazione dei cheratinociti follicolari (cellule che producono la cheratina del capello), dell’allungamento e dello spessore del fusto. L’attenzione si à ̈ focalizzata sull’importanza dei fattori biologici all’interno della papilla dermica del bulbo del capello che à ̈ la zona di scambio grazie alla presenza dei capillari delle anse papillari ed in particolare sul ruolo svolto dal fattore di crescita vascolare (VEGF) e del fattore di crescita fibroblastico (FGF). I dati mostrano una modificazione di miglioramento di tutti i parametri valutati nel gruppo attivo, significativa rispetto al gruppo placebo (vedi grafici 1-3). Se al terreno di coltura si aggiunge un principio attivo in grado di mantenere in vita il bulbo (fattori di crescita, antiapoptosi, ecc.) la fase anagen può prolungarsi per 28 – 30 giorni. Per poter eseguire un’analisi statistica dei dati sono stati selezionati i casi in cui erano presenti sia le riprese fotografiche al tempo T0 sia le misure ai tempi dal giorno 10 al giorno 35 (con non margine di tolleranza di due riprese mancanti al massimo per ciascun follicolo). In particolare si evidenzia una differenza altamente significativa sia della durata della fase anagen che dell’allungamento del fusto dei follicoli trattati con la composizione dell’invenzione rispetto a quelli trattati con i singoli principi attivi (p > 0,001). E’ interessante notare come l’attività dei singoli principi sia significativa rispetto al gruppo di bulbi coltivato nel solo terreno di coltura, con una efficacia non significativa dal punto di vista statistico tra un gruppo e l’altro. Ma, in particolare, si à ̈ evidenziato un aumento di efficacia in vitro della composizione secondo la presente invenzione rispetto ai singoli principi attivi.
