“TUTORE PER SPALLA”. “SHOULDER BRACE”.
TESTO DELLA DESCRIZIONE TEXT OF THE DESCRIPTION
La presente domanda di brevetto per invenzione industriale ha per oggetto un tutore per spalla per curare le ortesi. The present patent application for industrial invention relates to a shoulder brace for treating orthoses.
La parola “orthosis” (deriva dal greco orthos, che significa correggere o raddrizzare) viene utilizzata nel gergo del settore per indicare genericamente tutta la gamma completa dei dispositivi attualmente prodotti per la cura delle ortesi. The word “orthosis” (derives from the Greek orthos, which means to correct or straighten) is used in the jargon of the sector to generally indicate the complete range of devices currently produced for the care of orthoses.
L’ortesi è un qualsiasi dispositivo applicato esternamente usato per modificare le caratteristiche strutturali e funzionali del sistema scheletrico neuromuscolare. The orthosis is any externally applied device used to modify the structural and functional characteristics of the neuromuscular skeletal system.
Le ortesi servono per : Orthoses are used to:
- assistere la funzione lesa; - assist the injured function;
- ridurre le sollecitazioni meccaniche; - reduce mechanical stresses;
- diminuire il dolore; - decrease pain;
- controllare il movimento; - control the movement;
- correggere una deformità o rallentarne il peggioramento. - correct a deformity or slow down its worsening.
Nel soggetto emiplegico si ha una modificazione dei normali rapporti articolari tra scapola ed omero per assenza/riduzione dell’attività muscolare, con nella maggior parte dei casi: In the hemiplegic subject there is a modification of the normal joint relationships between the scapula and humerus due to the absence / reduction of muscle activity, with in most cases:
- instabilità scapolare; - scapular instability;
- anteposizione; - anteposition;
- intrarotazione e discesa verso il basso della testa omerale ( sublussazione) (come mostrato in figura 1). - internal rotation and downward descent of the humeral head (subluxation) (as shown in figure 1).
Attualmente esistono i seguenti modelli per curare questa patologia: Currently there are the following models to treat this pathology:
1) Ortesi di spalla con supporto spalla gomito polso. Questo tipo di tutore è’ costituito da una fascia che sostiene braccio, avambraccio e polso mantenendoli a contatto con il tronco. 1) Shoulder orthosis with shoulder elbow wrist support. This type of brace consists of a band that supports the arm, forearm and wrist, keeping them in contact with the trunk.
Esistono dei modelli completamente chiusi oppure modelli con presa al braccio e al polso, come mostrato nelle figure allegate 2 e 3. There are completely closed models or models with arm and wrist grip, as shown in the attached figures 2 and 3.
Questi modelli svolgono una funzione di supporto favorendo la riduzione della sublussazione, ma non correggono l’intrarotazione della spalla e la retrazione dei muscoli adduttori e intrarotatori. Va detto inoltre che, anche se facili da indossare, costringono il paziente alla posizione flessa del gomito non permettendo l’utilizzo dell’arto plegico; inoltre alterano le funzioni di equilibrio in deambulazione. Questa terza tipologia, mostrata in fig. 5 e 6, sostiene l'arto superiore colpito da emiplegia con un bendaggio rigido a livello dell’omero ed un bendaggio ad otto posteriore. Questo quarto tipo di tutore, mostrato in figura 7, consiste in un bendaggio elastico con presa sulla spalla controlaterale e al tronco per il trattamento funzionale precoce dell'articolazione scapolo-omerale.(Più utilizzato nella lussazioni recidivanti”) This fourth type of brace, shown in figure 7, consists of an elastic bandage with grip on the contralateral shoulder and trunk for the early functional treatment of the scapulohumeral joint. Questa sesta tipologia di tutore, mostrato in fig. 9, è stata creata con presa sull’omero e controlaterale al tronco in neoprene e silicone; inoltre c’e’ la possibilità di aggiungere il supporto dell’avambraccio. Tutte le tipologie di ortesi sopra descritte: All the types of orthoses described above:

- devono essere ben posizionati affinché siano efficaci. - they must be well positioned for them to be effective.
In alternativa alle ortesi vengono poi utilizzati i bendaggi. Bandages are then used as an alternative to orthoses.
L’utilizzo di bende per ottenere un tutore idoneo a curare una simile patologia: The use of bandages to obtain a brace suitable for treating such a disease:
- permette un buon allineamento dell’omero nelle sua sede evitando lo scivolamento; - allows a good alignment of the humerus in its seat avoiding slipping;
- é difficile da applicare e comunque serve un professionista per l’applicazione; - it is difficult to apply and in any case a professional is needed for the application;
- non può essere rimosso e quindi una volta fatto viene portato per circa tre giorni anche la notte. - it cannot be removed and therefore once done it is worn for about three days even at night.
Scopo della presente invenzione consiste nell’ideare un nuovo tipo di tutore che riduca i dolori del paziente, ripristini i normali rapporti articolari ed infine possa garantire una maggiore stabilità a livello prossimale. Ulteriore scopo della presente invenzione è quella di ideare un tutore che possa essere utilizzato anche per prevenire i traumi della spalla dovuti a manovre scorrette, fornendo al paziente che se ne avvale una stimolazione percettiva e continuativa nel tempo. Dato che l’unico tutore che veramente funziona per il posizionamento dell’omero sulla glena è il bendaggio, si è pensato di ideare un tutore che mantenga gli stessi vantaggi di un bendaggio, ma ne riduca gli svantaggi. Per realizzare il nuovo tutore è previsto di : To make the new brace it is planned to:

- misurare la circonferenza dell’omero distale a circa tre centimetri dal cavo ascellare del paziente (Fig. 11); - measure the circumference of the distal humerus about three centimeters from the patient's axillary cavity (Fig. 11);
- misurare la lunghezza dell’omero dalla linea del cavo a ascellare presa esternamente alla “piega” del gomito del paziente (fig.12); - measure the length of the humerus from the line of the cable to the axillary taken externally to the "crease" of the patient's elbow (Fig. 12);
- misurare la lunghezza del bendaggio ad otto sul paziente (fig. 13); - measure the length of the figure-of-eight bandage on the patient (fig. 13);
- riportare tutte le misure prese su un foglio di gel di stirene con una “forma standardizzata”del tutore (fig. 14); - report all the measurements taken on a sheet of styrene gel with a “standardized shape” of the brace (fig. 14);
- preparare il tutore, le fibbie di ancoraggio, il velcro per la chiusura sull’omero ed il bendaggio ad otto di sostegno con la “stringa di cotone” rivestita dal gel di stirene tranne che nel tratto ascellare dove è presente solo il gel di stirene (fig.15). - prepare the brace, the anchoring buckles, the velcro for the closure on the humerus and the eight support bandage with the "cotton string" covered with styrene gel except in the axillary section where only the styrene gel is present (fig. 15).
Una volta ultimato tutte le parti che compongono il tutore secondo il trovato quest’ultimo può essere montato e testato sul paziente, per verificare le circonferenze e le lunghezze prese anticipatamente (fig. 16). Se tutto va bene le parti: Hopefully the parts:

Un tutore siffatto risulta quindi: Such a guardian is therefore:

- estaticamente non ingombrante in quanto posizionato a contatto con la pelle (non vincola l’abbigliamento dell’utente); - ecstatically not bulky as it is positioned in contact with the skin (does not bind the user's clothing);
- semplice da utilizzare, sia nel posizionamento che nella sua igiene. - easy to use, both in positioning and in its hygiene.
Le figure da 18 a 24 mostrano il tutore secondo il trovato e le modalità della sua messa in opera. Figures 18 to 24 show the brace according to the invention and the methods of its implementation.
Con particolare riferimento alla fig. 18 è possibile vedere che il tutore secondo il trovato comprende un tirante posteriore, un tirante anteriore, un ancoraggio e una cintura. Per posizionarlo si deve: To position it you must:
