Description
“Catetere bimodale per uso urologico†⠀ œBimodal catheter for urological useâ €
TESTO DELLA DESCRIZIONE TEXT OF THE DESCRIPTION
The present invention relates to bladder catheters, used to evacuate clots in the presence of macro hematuria. The bladder end of the catheter is provided with holes, which connect the lumen of the catheter with the internal cavity of the bladder to allow urine and clots to be conveyed outwards. To carry out this operation more effectively, a special syringe called â € œschizzettoneâ € is inserted in the external end of the catheter, to allow adequate suction power.
The catheter is chosen on the basis of the external urethral meatus, the urethra and the volume of clots.
Often in patients with macro hematuria various problems are encountered: an occlusion of the lumen of the catheter may occur, during insertion or after insertion, due to the presence of clots in the urine. In cases of severe hematuria, it is often necessary to remove the clogged catheter and insert others with a larger diameter and with larger holes suitable for evacuating clots, which causes severe pain, increased risk of urethral lesions and infection.
This maneuver is carried out by specialized medical personnel who, however, are often forced to operate in an emergency and therefore at the risk of coming into contact with sometimes infected organic material.
The invention intends to solve the technical problems described above, with the solution set forth in the attached claim 1) and in the subsequent dependent claims, which exploits the following solution idea.
The invention proposes a new catheter thus formed: two catheters placed one inside the other, tightly fixed together with the proximal and external ends. The external catheter guarantees effective drainage even in the presence of severe hematuria and voluminous clots, while the internal catheter has a lower caliber and drainage holes which together have a passage opening lower than the overall opening of the external catheter holes. In addition, the two catheters are connected to a sealed and removable connection, to which the â € œschizzettoneâ € can easily be connected and to avoid contamination with organic material.
In case of partial or total clogging of the catheter, just remove the internal catheter from the external one that remains in place; its larger caliber and more numerous holes ensure better suction and allow the rapid flow of urine and clots. This operation is faster, painless and at a lower risk of infections. Furthermore, the use of multiple catheters is avoided, guaranteeing economic savings.
More characteristics of the invention and the advantages deriving from it, will become clearer from the following description of some preferred embodiments of the same, illustrated purely by way of non-limiting example, in the figures of the two attached drawing tables, in which:

- La fig. 4 illustra con parti in sezione l’estremità prossimale del catetere esterno, dopo lo sfilamento del catetere interno ed a ripristino avvenuto della funzionalità di collegamento ad apparati esterni; - Fig. 4 shows with parts in section the proximal end of the external catheter, after the extraction of the internal catheter and once the connection functionality to external devices has been restored;
- Fig. 5 illustrates with parts in section, as in Figure 3, an executive variant of the proximal end of the internal catheter, to facilitate grasping and extraction of the same internal catheter.
From figure 1 it can be seen that the catheter C according to the invention consists of an external catheter 1 having such a shape and size that it can be introduced into the patient's urethra U, until it flows into the bladder V with a portion of the its own distal end 101, equipped with holes 2 in such a number, distributed in such a way and with a diameter such as to ensure effective drainage of urine even in the presence of significant hematuria and to allow, if necessary, the evacuation of any clots by aspiration and therapeutic treatments for washing the bladder. The catheter 1 remains protruding outside the urethra U with its proximal portion 201 which is connected tightly to a fitting 3 designed for sealing, removable, rapid and simplified fixing, for example with an attachment 7 of the type screwing or bayonet fitting, to a complementary bushing 4 with a conical nozzle 104 to which the â € œschizzettoneâ € can be removably and sealed.
Inside the external catheter 1 there is at least a second catheter 5, hereinafter referred to as â € œinternal catheterâ €, which can intimately marry the external catheter to assume a coaxial position in this, or which can be housed there with sufficient clearance radial, so that between the two catheters there is a small gap. The caliber of the internal catheter 5 is necessarily lower than that of the external catheter 1 and the same internal catheter 5 is in turn equipped with holes 6 having a diameter equal to or less than those 2 of the external catheter 1, which communicate with the external catheter 1. ™ inside of the bladder V through the same holes 2, some of which can be closed by the wall of the same internal catheter 5. The holes 6 of the internal catheter 5 are of such characteristics and in such quantity as to ensure a good drainage of the bladder V, with a limited light of connection with the outside.
The proximal end of the internal catheter 5 protrudes with its own truncated and diverging section 205 outside the fitting 3, sealing itself in the bushing 4 and in the connection with the relative mouth 104. If necessary, specific references can be provided on the fitting 3 and / or on the proximal ends 201, 205 of the two catheters, to ensure a correct reciprocal angular position of the same catheters, useful for collimating the distal holes 6 of the internal catheter 5 with the predetermined ones of the distal holes 2 of the external catheter 1.
The materials for forming the catheters 1 and 5 may be of any type suitable for the purposes in question, bearing in mind that in this new combination, the composite catheter C will inevitably be more rigid than only one of the two catheters and that this condition may become an advantage in the implantation phase of the catheter itself C.
After implantation in the patient, catheter C is in the condition of figure 1. Its distal portion 101, 105 is placed inside the bladder V and communicates with the outside through only the holes 6 of the catheter internal 1, through the lumen of this same internal catheter and through the mouth 104.
In the event of clogging or a significant loss of flow of catheter C, it is sufficient to unlock the attachment 7, remove the bushing 4 from the connector 3, then grasp the proximal and protruding portion 205 of the internal catheter 5 and, holding the same connection 3, as indicated by the arrows R in figure 3, remove the internal catheter 5 from the external one 1, as indicated by the arrow F. If the internal catheter 5 is made with a sufficiently elastic material, its extraction can be facilitated by the fact that when this catheter is subjected to longitudinal traction according to arrow F, the same catheter tends to lengthen, to decrease in diameter and therefore to flow better in the external catheter 1 which remains stably in the bladder V and in the urethra U. the withdrawal phase according to arrow F, a slight torsional movement can also be imparted to the internal catheter 5 through its proximal and pren sile 205.
Once the internal catheter 5 has been removed, the operator reconnects the bushing 4 to the connector 3 with the connection 7, as shown in figure 4. If the connection 7 is of the bayonet type and does not ensure a tight fastening between parts 3 and 4, the recovery by cutting of the proximal and funnel-shaped extremity 205 of the internal catheter and first extracted 5 can be envisaged, and this small recovery funnel 205 can be reinserted in connection 3 and at the ™ inside of the bushing 4, exactly as in the solution illustrated in figure 1, to ensure a tight connection between the same fitting 3 and the conical nozzle 104. This solution has not been illustrated in figure 4 as it can be guessed and easily realized on the basis as stated above.
After the extraction of the internal catheter 5 (figs. 3, 4), the patient's bladder V communicates with the mouth 104 through the wide lumen of the external catheter 1 and through the wide opening of its holes 2, and through this new wide channeling, clots that had previously clogged the composite catheter C can be easily aspirated and drained and any therapeutic operations to wash the bladder can be carried out.
With reference to figure 5, it can be seen that to facilitate the extraction of the internal catheter 5, its proximal end 205â € ™ can be fixed in any useful way to the bushing 4â € ™, for example by welding, gluing and / or with the mechanical fixing of the same end 205â € ™ between at least two pieces making up the same bushing 4â € ™, all in a way that can be understood and easily made by technicians in the field. With this variant the operator will be able to extract the internal catheter 5 by gripping not the short and slippery distal end 205 of the same catheter, as in the previous solution, but by grasping the wide bushing 4â € ™ which is rigid and whose external surface can be equipped with grooves or roughenings 204 which improve its grip, with the obvious advantages that this entails. Once the internal catheter 5 has been extracted, it is possible to use a new bushing 4 which can be supplied with the composite catheter in question, to be fixed to said connector 3 in order to connect the residual external catheter 1 to the bag or to other external systems .
From these considerations it is highlighted that the use of the bimodal catheter C allows to operate in greater safety for the patient and for the staff, ensuring less pain, less exposure to infectious risks and urethral lesions.
It is understood that the description refers to some preferred embodiments of the invention, to which numerous variants, modifications and constructive equivalences can be made, all without abandoning the guiding principle of the invention, as described, illustrated and as claimed below.
