ELEMENTO Di FISSAGGIO BIODEGRADABILE IN PARTICOLARE UTI-LIZZABILE IN CAMPO ORTOPEDICO E MAXILLOFACCIALE. BIODEGRADABLE FIXING ELEMENT, ESPECIALLY USED IN THE ORTHOPEDIC AND MAXILLOFACIAL FIELD.
R I A S S U N T O SUMMARY
(FIG. 2) The fastening element in question consists of a pin (2) completely made of bioabsorbable and biocompatible material able to be grafted into a corresponding first hole (3) made in the thickness of a fractured bone structure (1), which has a its end (2a) provided with one or more longitudinal notches (4) defining at least one pair of yielding fins (5a, 5b) arranged bilaterally to the longitudinal axis (X) of the pin (2); the latter axially has a second through hole (6) where bioabsorbable and biodegradable divarication means (7) are housed, acting on the fins (Sa, 5b), so as to allow constant adherence of the same pin with the walls of the first hole (3). (FIG. 2)
The present invention relates to a biodegradable fastening element which can be used in particular in the orthopedic and maxillofacial field.
In the field of orthopedic surgery, and in particular in the maxillofacial surgery (i.e. of the bony parts of the face), it is known the use of particular means of joining or synthesizing the fractured bones with which it is possible to keep the two bone segments correctly united. injured.
In particular in the field of facial traumatology, if bone fractures occur at the level of the mandible and upper jaw, it is necessary to rigidly fix the two bone segments to allow the complete repair of the bone structure as a whole and the "restitutio ad integrum" of the itself. To do this, various types of equipment are used: one of the most used is represented by the application of metal plates (for example titanium alloys, but there are also biodegradable synthetic material), arranged astride the bone fracture line, and joined to the latter by means of self-tapping screws (also metal ones) introduced into the thickness of the bone structure, previously drilled where necessary.
As regards the solidarization screws, screws of the degradable and resorbable type are also known (but little used), which however have the considerable disadvantage of requiring, in addition to the aforementioned drilling in the bone, also the tapping of the same holes, since the screw, being made of polymeric material (for example PLLA, PGA, copolymers, etc.), is not very resistant in its threading and obviously cannot thread the previously made hole by itself.
However, the first type of orthopedic equipment described is disadvantageous, since it always requires two rather long and complex surgical interventions, one for the installation and one for the removal of the equipment with a stabilized bone structure. Obviously, the removal procedure is necessary to prevent the patient from arising, over time, any biocompatibility problems between the equipment itself and the surrounding tissue.
In the second case, i.e. the use of plates and screws in degradable and absorbable synthetic material, the time required for the installation of all the equipment is very long (a consequence of the need to tap the holes) and the tightness of the screws. inside the threaded holes does not provide sufficient guarantees for a correct and safe stabilization of the plate, and therefore of the injured bone structure, especially in the postoperative transient period, where the required seal must be as reliable as possible.
The purpose of the present invention is therefore to eliminate the aforementioned drawbacks by making a plug in biodegradable and biocompatible material, for fastening joining plates, which are quick and easy to install and have a secure mechanical seal both for the same and for the plate. .
The invention, as it is characterized by the claims, solves the problem of providing a fastening element consisting of a pin completely made of bioabsorbable and biocompatible material able to be grafted into a corresponding first hole made in the thickness of a fractured bone structure, which it has its own end provided with one or more longitudinal notches defining at least one pair of yielding fins arranged bilaterally to the longitudinal axis of the pin; the latter axially presents a second through hole where bioabsorbable and biodegradable divarication means are housed, acting on the fins so as to allow constant adherence of the same pin with the walls of the first hole.
The advantages and characteristics of the present invention will become more evident in the detailed description that follows, made with reference to the attached drawings, which represent a purely exemplary and non-limiting embodiment thereof, in which:

In accordance with the attached figures, the biodegradable fixing element in question can be used, in particular, in the orthopedic and maxillofacial field for joining and stabilizing fractured bone structures, which in the cases illustrated in figures 1 and 3 are defined by a maxilla and respectively from an ankle area both indicated as a whole with 1.
This element consists of a pin 2 completely made of bioabsorbable and biocompatible material and can be grafted into a corresponding first hole 3 (see in particular Figs. 2 and 3) made in the thickness or between the two fractured segments of the injured bone structure 1. Said pin 2 has its own end, indicated with 2a, provided with one or more longitudinal notches 4 (in the cases illustrated there is only one of these), which define one or more pairs of yielding fins 5a and 5b arranged bilaterally to the axis longitudinal X of the pin 2 (the number of these depends on the diameter and length of the pin 2 and on the type of fastening to be performed); in addition to this longitudinal notch 4, the pin 2 also has, axially, a second through hole 6 within which divarication means, indicated with 7, can be housed and also bioabsorbable and biocompatible.
These spreading means 7 are provided acting on the fins 5a and 5b so as to allow constant adherence of the pin 2 to the walls of the first hole 3 where the latter is engaged.
More in detail (see Fig. 4) the spreading means 7 consist of a second pin 9 engaged in the second hole 8, which has several sectors, along its longitudinal extension, of different diameters from each other; one of these sectors, the one indicated with 9a, makes one end of the second pin 9, and has its diameter greater than the diameter D presented by the innermost end (with respect to the direction of engagement in the first hole 3) of the second hole 6 of the first pin 2: in this way it is possible to have the wings 5a and 5b spread apart after the pin 2 is inserted into hole 3.
In an alternative solution, the spreading means 7 (see Fig. 5) consist of a spherical element 10 always housed in the innermost end of the second hole 6, which is joined to a wire 11 also made of biodegradable material and biocompatible, developing for a length such as to protrude from the opposite end of the same second hole and with which it is possible, in correspondence with the insertion of the first pin 2 in the first hole 3, a longitudinal sliding (see arrow F), towards the external, of the spherical element 10 inside the second hole 6 such as to originate the aforementioned divarication of the fins 5a and 5b. The pin 2 is also equipped, on its opposite end to the one having the longitudinal notch, with a countersunk locking head 12 with which it is possible to stabilize the pin on the bone structure.
This pin 2 can obviously be used both singularly and directly on the fracture (as illustrated in fig. 3) and for joining stabilization plates 8 (also made of biocompatible and biodegradable material) with the aforementioned fractured bone structures 1 ( see fig. 1); these plates 8 are placed opposite and in contact with the same bone structure and straddling the fracture to be repaired: in this case the pins 2 described above are in such a number as to be able to fix and support, also by means of the countersunk head 12, the plate 8 on the bone structure 1.
The installation of this pin 2, comparable to a dowel, is already widely deducible from the description made so far, since it is necessary to make on the bone structure 1 the holes 3 indispensable for the insertion of the pin 2, which once inserted is locked inside the hole 3 by inserting the second pin 9 or by sliding the spherical element 10 thanks to the wire 11 protruding from the second through hole 6.
The stabilization is made even greater thanks to the particular synthetic material with which the pin is made: the constant adherence to the walls of the hole is in fact made even greater by the fact that thanks to the progressive imbibition of liquids of the human body by the synthetic material, in particular in the pre-resorption phase, it tends to increase the thickness of the post itself.
The element thus created also allows various uses in addition to the purely specific one in the maxillofacial field: in the orthopedic field, for example, it is possible to perform reconstructions of parcel fractures or detachments of small bone fragments, which require surgical reduction as the same site of the lesion requires it (see for example malleolar and / or plurimalleolar fractures, tibial plateau fractures, etc.), and this in a single and quick surgical operation.
The invention thus conceived is susceptible of numerous modifications and variations, such as for example the synthetic materials that can be used and having the characteristics of biocompatibility and biodegradability such as PLLA, PGA, polyhydroxybuttirate, polycaprolactone, etc., all of which are within the scope of the inventive concept . Furthermore, all the details can be replaced by technically equivalent elements.
