?SISTEMA E METODO PER RIABILITAZIONE NEUROPSICOLOGICA? ?SYSTEM AND METHOD FOR NEUROPSYCHOLOGICAL REHABILITATION?
Descrizione Description
The present invention operates in the field of neuropsychological rehabilitation, more in general all those diseases that promote a degradation of the central nervous system, favoring the alteration of the brain regions of memory, or rather severe and selective impairment of long-term memory independent of the semantic or sensory characteristics of the information. In particular, the proposed invention, which takes the name of Memento, concerns a software application for mobile devices, such as smartphones and tablets, and a hardware instrumentation, which provide the patient with amnesic deficits with a continuous rehabilitation path over time so that it can gradually regain and recover a quality? of life the most possible superimposable to that preceding the onset of the cerebral pathology.
Stato dell?arte State of art
In the current state of the art, amnesia, but brain diseases in general, find few solutions in their complexity. due to a serious lack of tools suitable for each individual problem of each patient. To date, a large part of the work of operators who deal with neuropsychological rehabilitation consists in the collection of all cognitive processes in memorization with the aim of favoring the recovery of functionality. in patients with anterograde amnesia and in facilitating the reacquisition of personal information in patients with retrograde deficits. In this path defined as ?restitutive? of the lost cognitive function, partial results are frequently obtained rather than a complete remission of the deficit and therefore requires a transition to a different phase called the ?compensatory? path? in which the patient is guided to acquire the ability? to use tools that allow him to manage deficits by vicariating the altered cognitive component. Currently in both phases of the rehabilitation process paper tools are used such as agendas, notebooks, diaries, post-its, or any other stratagem useful for the patient to create an ?external memory? consultable, or to digital tools such as apps or digital diaries. The limit of paper tools? given by the excessive size of the material produced over time, by non-interactivity? of the instrument and the excessive length of recovery times that require space and organization for cataloging. These limits can now be overcome by technological advances in terms of effectiveness of software and hardware size allowing the possibility? to have on small devices quantity? of enormous information.
However, the current digital tools available to patients and people involved in rehabilitation are generic digital agendas born largely to support the needs of rehabilitation. of people with normal abilities? cognitive. Does the very structure of digital diaries or applications for tablets and smartphones have non-functional characteristics for people with reduced cognitive efficiency with negative consequences on functional use and adequate compensation efficiency for disabilities? ecologically. The main limitations of the tools currently available, as also highlighted in the patent EP1360576A1 aimed again at the development of personal digital assistance, are of various kinds:
? Ergonomics? cognitive: excessively rich graphics of elements that subject the user to an excessive cognitive load with consequent difficulty? in visual research and memorization of structure and functionality.
? Lack of specific sections that allow you to retrieve information relating to the identity? of the person.
? Absence of tools that allow you to have a history associated with the attendance of places.
? Assenza di strumenti che integrino in modo efficace funzioni differenti quali contatti, luoghi, eventi. ? Absence of tools that effectively integrate different functions such as contacts, places, events.
? Impossibility? to automatically integrate information in order to generate an organization of memories, i.e. recorded information. ? Absence of tools that combine psychophysiological detection tools with software for recording personal life events.
? Absence of tools useful for the interaction between patient and therapist that allows a continuous and constant monitoring for the modulation of the activities? rehabilitation and the support of adaptation strategies in daily life.
The limitations resulting from the use of hybrid tools in the rehabilitation of cognitive memory deficits and from the use of paper tools are many with high costs for the rehabilitation and a reduced effectiveness in the functional recovery of the sick person. Therefore, the purpose of this patent is that of providing an innovative mobile application that integrates hardware and software tools to support all those people suffering from neurological pathologies, acting as a guide and support tool for the activation of cognitive processes that are little or no more? efficient in the person with deficit, able to enhance the recovery phase of the disability? cognitive skills and to favor an effective compensation of the residual deficits at the end of the rehabilitation.
Descrizione dell?invenzione Description of the invention
According to the present invention, an electronic system and method is created, called Memento, which solves the problems mentioned above, allowing the use of common portable electronic devices, such as smartphones and tablets, in order to promote a new type of psychotherapeutic and rehabilitative path for all those people affected by amnesic deficits, which can occur following diseases of the central nervous system such as cerebrovascular diseases, skull-brain trauma, degenerative diseases of the central nervous system, among these primarily neurodegenerative diseases, which involving brain regions involved in memory circuits alter the effectiveness of the cognitive processes involved in the generation and retrieval of memories (that is, information).
The decision to use mobile devices, of limited size and weight, has the advantage of being able to use this tool anywhere and an essential requirement for the modality? of safety operation.
I remember ? made up of three distinct and integrated components which aim to support the rehabilitation process and functional recovery of the neurologically impaired patient, consisting of three fundamental components:
- App per smartphone e tablet, - Apps for smartphones and tablets,
- Dispositivi weareable di rivelazione fisiologica, - Wearable physiological detection devices,
- Piattaforma software. - Software platform.
The application for smartphones and tablets turns out to be the core of the invention, the central element for revealing and recording data, presenting itself as a tool for personal recording and automatic recording of information, entered by the user in the first case and starting by physiological indices related to the cognitive and emotional processing of the stimuli and events of the person's life, in the second case. So, at the same time, it stands as a guide and support tool for the autonomous activation of cognitive processes that are little or no more? efficient in the person with deficit. The App, in fact, allows the patient with amnesic deficits to record what? what happens to him through a tool that guides organization and coding, favoring and facilitating their recovery through the same coding criteria used in the registration phase, thus providing the patient with continuous and constant support for carrying out activities? cognitive.
The application ? characterized by a user interface, designed to be simple to use and places the emphasis on immediacy and accessibility, advantageously constructed from a home page, in which there are keys for accessing the various sections and for event logging and search. The phase of entering and registering an event ? the crucial phase of the functioning of the application: every time the user carries out an activity? significant, records it on the App by automatically activating the temporal and spatial coding (location) and voluntarily activating the coding of the participants in the event and of the area of interest to which that event belongs (semantic coding). Also, each event is logged with an event name to which it can multimedia content such as text, images, video, audio, etc. be added. This mode? of coding is activated in the same way both when logging past events and when logging activity? to be performed in the future.
The functioning of the application fully follows the structure of human memory: the previous sections follow the functioning of episodic memory, or the capacity? to record the events of life in a rich way full of meaningful details on the perceptive and emotional level, and prospective memory, or the ability to to memorize activities? to be carried out in the future by associating them with contexts and inserting them within the sequence of activities? that the person expects to perform. This type of coding allows the person to retrieve a large number of information relating to individual events, i.e. to remember when carried out an activity, where, with whom, in what personal interest. This information can be reviewed every time the person opens the page relating to that event, thus favoring the generation of a historical memory of the person. This coding allows you to use the same criteria for the search phase: by pressing the ?Search? you can search for events using all the parameters used in the coding phase: ? It is possible to search by inserting a word that is present in the title of the event or in the text, inserting the participants in the event, the place of the event, the sphere of interest of the event.
In addition to these sections, which we could define as dynamics of the Application, from the home page you can access sections that we could define as static even if improperly. These sections have the aim of displaying information or events in an orderly manner with respect to the different coding criteria. So we have a section called ?TimeLife? in which the events are all present and ordered chronologically. We have a section called Diary in which there are different sections set by the person that refer to different areas of interest or activity? of the single person: for example, can you? have a coding for ?cognitive rehabilitation? but also ?family reunions?, etc. ? present a section called ?People? in which the user enters his contacts by encoding them in a personalized way in order to facilitate their recovery: each person can? provide numerous categories for your contacts (for example family, friends, colleagues, therapists, etc.?) and within each category your contacts are recorded. For each contact, personal information is recorded such as address, telephone number, email, but also information relating to the user's personal history with that person with an indication of the circle of people with whom that specific contact is made. in relation together with the user of the application. The profile, therefore, is built by the individual user, while ?Timelife? with that? user it is structured automatically according to the activities? conducted with that contact. Therefore, the section ?People? not only represents the contact list, but allows the user to quickly retrieve information relating to the identity of the person and to one's relationship with him and a circle of people. This ? further aided by the presence of a contact representation in a family tree. Inside the page of each contact ? There is a timeline in which all the events that the user has recorded are found, associating them with that specific contact: in this way, by entering the page of each contact, the user can view your history with that single contact, or see everything? what yes? done together.
In addition, from the home screen you can access the ?Routes? that ? divided in turn into two sections:
? Eventi ? Events
? Punti di interesse ? Points of interest
Both sections are linked to a geolocation map and have two functionalities: distinct, in particular, ?Events? includes the? list of places in which ? was an activity carried out? and for which? an event has been recorded, for each single place a timeline is automatically generated with all the events associated with that place, thus creating a topographical memory and, moreover, in this way the user can? remember if and when ? ever been in a place, with whom and what activities? has done. Instead, in the ?Points of interest? section, the user can? register the places he frequents most frequently and for which he needs rapid recovery, in order to access the information necessary to reach the places he frequents by registering them with a personalized name that facilitates recognition. Moreover, ? It is also possible to associate elements of geolocation that tell us if that? user ? been or not been in a place or even drive it to that place until it hooks with the ?beacon?.
On the home screen ? present the ?Agenda? which allows access to the section aimed at planning the activities? to be performed in the future. In this section are displayed all the appointments, all the activities? that the? user is ? set to complete. The events are displayed in this section whenever the start date is greater than the entry date by at least five minutes (arbitrary criterion for defining a future event). For each future event, a system of notifications and pre-alarms is activated, functional to the effective implementation of the activity. by the user. Furthermore, once the time for carrying out the activity has passed, the Does the application send a notification requesting confirmation or disconfirmation of the successful execution of the activity? prefix. This system helps the user to monitor his own effectiveness in completing tasks and activities, but at the same time it also allows you to precisely generate the timelife of individual events, i.e. all activities? fixed for the future for which the user provides confirmation of successful realization become part of the timeline, those for which the user provides a negative response of realization, do not become part of the timelife. Every single event can be stored and encapsulated within the ?Report? section, which can be easily accessed by clicking a button on the initial screen of the App.
Another section present within the application is also very useful for planning activities. future ? the ?To do list?, which can be accessed with a key on the home page. This section? useful for planning activities? in bullets for which it is not foreseen to define a time duration. Once entered, they appear in the agenda section. Even for these activities? ? a confirmation of successful execution is expected and they will become part of the Report. Finally, within this App ? There is a further section, called ?Privacy?, which can be accessed by clicking a button on the initial screen of the app, in this section, through a very complex and secure system, you can access all the private documents of the subject (identity cards, credit, health system, etc.).
All the information that is recorded within the App is in turn saved in a cloud in order not to lose any data in the event of loss or breakage of the devices.
In addition to all that already? said, the software of this App sar? also able to automatically detect any discrepancies between the current position in which the user is located and the position set in the diary through a frequent comparison between these positions. The presence of discrepancies greater than 500 m are signaled to the user with a notification and at the same time an SMS message is sent to all in pre-set telephone numbers. This function has the aim of providing users and family members with a security tool with respect to the possibility of that users get lost and put themselves in danger.
What's more, everything is structured in such a way that it is possible to configure all the necessary parameters cut directly on the patient (warning time of the various notifications, usable sections) and also the ?Remember to use? setting, a reminder that helps, with times also set in this case by the user, the subject to be conditioned to use the tool.
Mostly the? App will be? advantageously multiplatform (?cross-platform?), downloadable and installable on pi? popular mobile devices and compatible with different operating systems, such as Android, iOS, Windows Mobile, etc., and it will result? be the element that will allow? to put patient and therapist in communication, favoring a real new type of psychotherapeutic sessions.
Additionally, this App will communicate? with an external device, which will represent? the system of this invention. That is, will you put? available to each patient a tool for revealing physiological parameters useful for detecting the presence of relevant events on an emotional level, but also useful for detecting physiological parameters useful for monitoring the person's state of health. The tool used by the program ? a mechanism (wearable and configurable instrument such as a ring, t-shirt, elastic band or any other instrument that can be made or available to be adapted on the market) for detecting the heartbeat, perspiration for detecting skin conductance and body temperature, blood pressure, rhythm sleep, wakefulness and all other useful vital parameters. These parameters are recognized by the scientific literature as indicators significantly correlated to variations in the person's emotional state. The instrument ? in connection with the? app through Bluetooth or other connection systems: whenever the? ring detects a significant change in one or more? of these parameters activates the recording (audio and/or video through an ad hoc lens ?worn? by the user or directly from the smartphone) of an event and invites the person to complete the recording of the event by sending a notification giving it a title, although however recording the event independently. The registration system can therefore be defined as automatic since? registration takes place automatically.
A second feature? in the use of psychophysiological detection hardware tools? given by? activation of security systems with respect to health and all? integrity? of the person: the system provides the possibility? of analysis of the physiological parameters at regular intervals allowing the sending of notifications to the user and SMS to predefined numbers in case of serious alteration of the parameters. Additional tools used indoors are small proximity devices? called Beacons that work with BLE (Bluetooth Low Energy) technology and are hardware that allow you to activate the app every time the user approaches. The beacons can be used both to record the removal and to warn the person when they approach a beacon by preparing specific notifications for each device whenever they approach it.
Finally, the third and final part on which Memento is based ? consisting of a software platform that allows the user?s application to communicate with a tool for displaying and processing what the user?s done on the App. This tool allows rehabilitation therapists, doctors and psychologists to observe the behavior of patients, monitor their progress and modulate the rehabilitation process. Therefore, the immediacy and speed? provided by the interaction with the software platform, allows healthcare professionals to implement the effectiveness of the rehabilitation paths and to modulate more? quickly the adaptations to the various paths. However, this platform will allow also to view the reports and to access the titles of the events, the places and the participants, but it will not be? It is possible for rehabilitators and caregivers to access multimedia content such as audio photos, videos, text messages and geolocation in order to maintain user privacy.
With the use of this invention we want to promote the birth of a new concept and approach to the rehabilitation and functional compensation of disabilities? consequent to cognitive deficits of memory and executive functions thus favoring the return to a more active daily life. close to the disease state. Therefore, the invention just mentioned adapts to situations in which the deficits have different levels of severity, providing the patient with uses and functionalities adaptable and customizable.

With reference to FIGS. 2 and 3 will come detailed below the mode? of operation in practice of the App 100. The Memento application allows the patient to record what? which happens through a tool that guides their organization and coding, favoring and facilitating their recovery through the same coding criteria used during registration. I remember ? characterized by an initial user interface in which there are keys for accessing the various sections and keys for recording and searching for events. Among the various sections on the home page, ? It is possible to differentiate between dynamic and static keys.
Among the dynamic ones, only a single key belongs, Search 102, through which ? It is possible to search by inserting a word that is present in the title of the event or in the text, inserting the participants in the event, the place of the event, the sphere of interest of the event. While the rest of the keys are part of the static section, i.e. these sections have the aim of displaying the information or events in an orderly manner with respect to the various coding criteria, we therefore have:
- Timelife 103: within this section there is? a list 601 of all the events programmed by the patient and mostly? also sorted chronologically. Furthermore, next to every single event sar? there is a green or red tick 602, which color represents the successful execution or non-execution of the activity.
- Diary 106: within this there are different sections set by the person, for example can you? have a coding for ?cognitive rehabilitation? but also ?family reunions?, etc.
- People 104: in this section the user enters his contacts by encoding them in a personalized way in order to facilitate their retrieval. Each person can have several categories for your contacts, such as family, friends, colleagues and therapists, and within each category your contacts are recorded. For each contact, personal information is recorded such as: telephone number 501, address 502, e-mail 503, information relating to the user?s personal history with that person 504 with indication of the circle of people with whom that particular contact? in relation together with the user of the application. Moreover, ? present a further section called ?Timelife with..? 505, which has an automatic completion and the entered events associated with that person at the time of entry appear. Therefore, the People section is not only the contact list, but allows the user to quickly retrieve information relating to the identity of the contact. of the person and to one's relationship with him and a circle of people. This ? further aided by the presence of a contact representation in a family tree. Inside the page of each contact ? There is a timeline in which all the events that the user has recorded are found, associating them with that specific contact: in this way, by entering the page of each contact, the user can view your history with that single contact, or see everything? what yes? done together.
- Routes 107: this in turn ? divided into two subsections 401 events and 402 points of interest. The first section 401 includes the list of 403 places where ? was an activity carried out? and for which? an event was logged. For each single place, a timeline is automatically generated with all the events associated with that place, thus creating a topographical memory. The user in this way can? remember if and when ? ever been in a place, with whom and what activities? you have done. In the second 402 section, the user will be? able to register all those places frequented more frequently or for which rapid recovery is required 404; for both sections will be? provision of a 405 geolocation map to help the user reach a certain place.
- Agenda 105: this key allows access to the section aimed at planning the activities? to be performed in the future. Inside this, all the appointments and all the activities are displayed. that the? user is ? set to complete. Events will be shown within this section whenever the start date is greater than the entry date by five minutes. Therefore, for each future event, a system of notifications and pre-alarms functional to the effective implementation of the activity is activated. by the user. Furthermore, once the time for carrying out the activity has passed, the Does the application send a notification requesting confirmation or disconfirmation of the successful execution of the activity? prefix. This system helps the user monitor their effectiveness in completing tasks and activities. This system also allows you to accurately generate the timelife of individual events, i.e. all activities? fixed for the future for which the user provides confirmation of successful realization become part of the timeline, those for which the user provides a negative response of realization, do not become part of the timelife.
- Report 108: does this last section include every single event previously programmed in the Agenda 105 section and for each day? there is an 801 histogram which displays the completed events in green and the events that the user has not completed in red. Each event corresponds to a small square that contributes to forming the histogram, on which ? It is possible to click and open the page relating to the event, in order to be able to review which events have taken place and which have not been completed.
In addition, always on the home screen in the upper left ? there is a figure in the shape of gears 802, which leads to two other very important sections:
? To do list 803: which ? a useful section to plan activities? in bullets for which it is not foreseen to define a time duration. Once entered, they appear in the agenda section. Even for these activities? ? a confirmation of execution is expected and they will become part of the report.
All the information that is recorded by the App is always saved in a special cloud in order not to lose any data in the event of loss or breakage of the device.
Also, the software behind the Memento app? able to automatically detect any discrepancies between the current position in which the user is located and the position set in the agenda through a frequent comparison between these positions. The presence of discrepancies greater than 500 m are signaled to the user with a notification and at the same time an SMS message is sent to all in pre-set telephone numbers. This function has the aim of providing users and family members with a security tool with respect to the possibility of that users get lost and put themselves in danger. In order to set up the notice system in the best possible way, ? It is also possible to structure the app in such a way as to characterize every single parameter on the patient (warning time of the various notifications, usable sections) and also the ?Remember to use Memento? setting, or rather a reminder that helps, with times also in this case set by? user, the subject to be conditioned to? use of the tool so that? deprive yourself of it as little as possible.
Nello specifico lo strumento utilizzato dal programma Memento ? caratterizzato da: To support all the features? of?App 100, how can? see from FIG.4, sar? given to the patient a tool for revealing physiological parameters useful for detecting the presence of relevant events on an emotional level, but also useful for detecting physiological parameters useful for monitoring the person's state of health. Specifically, the tool used by the Memento ? characterized by:
- easy configuration and wearable, such as 901 ring, 902 shirt, 903 elastic band or any other tool that can be made or available to adapt on the market.
- supported by a set of sensors capable of measuring and monitoring heart rate detection, perspiration for detection of skin conductance and body temperature, blood pressure, sleep-wake rhythm and all other useful vital parameters.
- a Bluetooth connection capable of interfacing the instrument with the App 100.
A second feature? in the use of psychophysiological detection hardware tools? given by? activation of security systems with respect to health and all? integrity? of the person: the system provides the possibility? of analysis of the physiological parameters at regular intervals allowing the sending of notifications to the user and SMS to predefined numbers in case of serious alteration of the parameters, such as relatives and close friends. Furthermore, whenever the instrument detects a significant variation in one or more? of these parameters activates the recording (audio and/or video directly from the smartphone) of an event and invites the person to complete the registration of the event by sending a notification giving it a title, while still recording the event independently, so that? these may then be of study to therapists and those working in patient rehabilitation. Further tools, as an advantageous addition to the invention, used by Memento in closed environments are small proximity devices? called Beacon 904 that work with BLE (Bluetooth Low Energy) technology and are hardware that allow you to activate the app every time the user approaches. The beacons can be used both to record the removal and to warn the person when he approaches a beacon by preparing specific notifications for each device every time he approaches it.
Finally, a third party included in the system of this invention, ? made up of an ad hoc software platform that allows the user?s application to communicate with a tool for displaying and processing what the user?s done on the App. This additional tool can allow all rehabilitation therapists, doctors and psychologists to remotely observe the behavior of patients, monitor their progress and modulate the rehabilitation process. The immediacy and speed? provided by the interaction of memento with the software platform allows healthcare professionals to implement the effectiveness of rehabilitation pathways and to modulate more? quickly the adaptations to the various routes. The platform allows you to view the Reports (108) and access the titles of the events, places and participants, but it will not be? It is possible for rehabilitators and caregivers to access multimedia content such as audio photos, videos, text messages and geolocation in order to maintain user privacy.
In conclusion, with this invention we want to promote a new support to all patients with severe amnesic deficits that allows to follow up on the retrieval of information in the same way as and the same paths that human memory uses spontaneously, thus favoring the return to a more intimate daily life. close to the morbid condition, and at the same time also a new type of psychotherapeutic and rehabilitative approach to the functional compensation of disabilities? resulting from cognitive deficits in memory and executive functions.
? finally, it is clear that modifications, additions or variations that are obvious to a person skilled in the art can be made to the invention described up to now, without thereby departing from the scope of protection which ? provided by the appended claims.
