Tale tipi di scarpe sono composte da una suola di tre parti (FIGURA 1.2), una esterna di cuoio, o in materiale plastico, o in gomma, che tocca il terreno, una suola intermedia, S.2, figura 1.2, provvista di un sistema di raffreddamento o riscaldamento come si voglia adoperarla (FIGURA 1), e una parte interna alla scarpa formata da una soletta in gel, o in sughero, anche ortopedica, non fa differenza, sulla quale poggia il piede (suola S.1, figura 1.2). Sul lato della suola intermedia sono praticati dei fori cilindrici (1, 2, 3, 4, 5, 6, 7 della figura 1), in numero qualsiasi, all'interno dei quali vengono inseriti dei piccoli cilindretti, A, B, C, della figura 1, di materiale metallico, o plastico, o composito, riempiti nel loro interno con liquido refrigerante, o acqua, etc.. . Tali cilindretti, prima di essere inseriti nella suola intermedia, saranno posti in frigorifero fino a quando non avranno raggiunto una temperatura molto fredda fra i meno 15 gradi Celsius, e i meno 25 gradi Celsius, e successivamente inseriti nella suola intermedia S.2, figura 1.2. Dopo pochi secondi il freddi, si diffonderà alla suoletta superiore in gel (S.1, figura 1.2), uniformemente, e all'interno di tutta la scarpa. La temperatura di raffreddamento interna della scarpa può essere regolata inserendo un numero a scelta di cilindretti: solo tre cilindretti A, B, C, figura 1, oppure 4, o 5, anziché sette nei sette fori 1, 2, 3, 4, 5, 6, 7, figura 1, oppure tenendo in frigo i cilindretti per minor tempo in modo che raggiungano una temperatura inferiore fra i meno 5 gradi Celsius, e i meno 8 gradi Celsius, facendo raffreddare di meno e per meno tempo l'interno della scarpa. Per il riscaldamento, invece, scegliamo dei cilindretti metallici che porremo all'interno di un forno e li faremo riscaldare fino ad una temperatura a nostra scelta di 45 gradi Celsius, o di 40 gradi Celsius, e successivamente li inseriremo all'interno della suola intermedia S.2, figura 1.2, così da riscaldare la scarpa per un tempo e una temperatura variabili con il numero di cilindretti che useremo, e con la temperatura alla quale avremo riscaldato i cilindretti.
