Il presente studio è un “trial” clinico controllato, della durata di 24 settimane, prospettico, randomizzato, in doppio cieco con placebo. Lo studio venne progettato per valutare l'efficacia, la sicurezza e la tollerabilità del cerotto transdermico di Rotigotina al dosaggio di 4 mg rispetto al placebo, in terapia aggiuntiva (add-on) al trattamento standard con inibitori dell’acetilcolinesterasi (AChEI) (rivastigmina 9.6 mg cerotto transdermico) in pazienti con AD di grado lieve secondo i criteri diagnostici con punteggio MMSE di ≥18 e ≤24 allo screening. La diagnosi di malattia di Alzheimer venne eseguita sulla base di una valutazione neuropsicologica (Carlesimo et al., Eur Neurol. 1996;36(6):378-84) e dei biomarker � ��������proteina tau totale e p-tau tramite esame del liquido cerebrospinale (CSF) (Vanderstichele et al., Alzheimers Dement. 2012 Jan;8(1):65-73). Un gruppo di pazienti venne randomizzato al trattamento con rotigotina (RTG) e l'altro al placebo (PLC) come add on per la terapia AChEI (Rivastigmina). Le misurazioni cliniche e neurofisiologiche vennero raccolte prima e dopo la somministrazione del farmaco. This study is a 24-week, prospective, randomized, double-blind placebo controlled clinical trial. The study was designed to evaluate the efficacy, safety and tolerability of the 4 mg rotigotine transdermal patch compared to placebo as an add-on to standard acetylcholinesterase inhibitor (AChEI) treatment (rivastigmine). 9.6 mg transdermal patch) in patients with mild AD by diagnostic criteria with MMSE score of ≥18 and ≤24 at screening. The diagnosis of Alzheimer's disease was performed on the basis of a neuropsychological assessment (Carlesimo et al., Eur Neurol. 1996; 36 (6): 378-84) and the biomarkers � �������� total tau protein and p-tau by cerebrospinal fluid (CSF) examination (Vanderstichele et al., Alzheimers Dement. 2012 Jan; 8 (1): 65-73). One group of patients was randomized to rotigotine (RTG) and the other to placebo (PLC) as an add on to AChEI (Rivastigmine) therapy. Clinical and neurophysiological measurements were collected before and after drug administration.
STRUMENTI DI VALUTAZIONE E PROCEDURE EVALUATION TOOLS AND PROCEDURES
Prima e dopo le 24 settimane di trattamento si eseguirono le seguenti scale cognitive: Before and after 24 weeks of treatment, the following cognitive scales were performed:
1) ADAS-Cog (Alzheimer's Disease Assessment Scale-Cognitive subscale), per valutare l’impatto globale sulle funzioni cognitive. L'ADAS-Cog richiede un tempo di somministrazione di 30-40 minuti. È costituito da due scale una cognitiva e una non cognitiva che possono essere somministrate insieme o separatamente a seconda delle necessità. L'approfondita valutazione sia degli aspetti cognitivi che comportamentali della demenza la rendono uno degli strumenti più utilizzati sia a scopi clinici che di ricerca. La parte cognitiva dell'ADAS, preceduta da una breve conversazione con il paziente su argomenti neutrali come il tempo, la colazione del paziente ecc, consiste di 12 test atti a valutare la memoria a breve e medio termine (rievocazione di parole; riconoscimento di parole; apprendimento delle istruzioni di un test); l'orientamento temporo-spaziale; il linguaggio (abilità verbale, difficoltà di denominazione nel linguaggio spontaneo, comprensione del linguaggio parlato, denominazione di oggetti e dita, esecuzione di comandi); la prassia; l'attenzione e la concentrazione. Il punteggio della maggior parte dei test cognitivi venne assegnato sulla base di stime (ratings) cliniche effettuate dall'esaminatore nel corso della conversazione e della sessione testistica. I punteggi della parte cognitiva dell'ADAS vanno da zero, che equivale ad assenza di errore ovvero di deficit, a un massimo di 75, che indica invece un deficit grave in tutti i test. La parte non cognitiva dell'ADAS comprende la valutazione della presenza o assenza di depressione, pianto, deliri, allucinazioni, deambulazione compulsiva, aumento dell’attività motoria e grado di cooperazione durante i test. Il punteggio a ciascuna di queste aree comportamentali (così come per i ratings della parte cognitiva) venne assegnato in un range da 0 (nessun deficit ad un test o assenza di uno specifico comportamento) a 5 (deficit di massima gravità ad un test o comparsa molto frequente di un comportamento). Il punteggio per la parte non cognitiva, risultante dalla somma ai punteggi assegnati nelle sette sezioni comporta mentali, può andare da 0 a 35. Il punteggio totale (cognitivo più comportamentale) varia da 0 a 110. In pratica alla parte cognitiva poche persone, persino fra i non dementi ottengono un punteggio di zero (assenza di deficit) poiché i test di memoria sono sufficientemente difficili da far commettere alcuni errori anche alle persone normali. Studi longitudinali su pazienti affetti da malattia di Alzheimer hanno mostrato che i punteggi della parte cognitiva dell’ADAS aumentano in media di 9 punti l’anno. Il tasso di cambiamento è più lento nei pazienti molto lievi e in quelli con una demenza grave rispetto ai pazienti con forme moderate di deterioramento (Stern et al. 1994), mentre i punteggi della parte non cognitiva dell’ADAS generalmente non mostrano un incremento con la progressione della demenza come invece avviene con i punteggi della parte cognitiva (Zec et al. 1992). 1) ADAS-Cog (Alzheimer's Disease Assessment Scale-Cognitive subscale), to assess the global impact on cognitive functions. ADAS-Cog requires an administration time of 30-40 minutes. It consists of two scales, one cognitive and one non-cognitive that can be administered together or separately as needed. The in-depth evaluation of both the cognitive and behavioral aspects of dementia make it one of the most used tools for both clinical and research purposes. The cognitive part of ADAS, preceded by a short conversation with the patient on neutral topics such as time, the patient's breakfast, etc., consists of 12 tests designed to evaluate short and medium-term memory (recall of words; recognition of words ; learning the instructions of a test); temporal-spatial orientation; language (verbal ability, naming difficulties in spontaneous language, understanding of spoken language, naming of objects and fingers, execution of commands); praxia; attention and concentration. Most of the cognitive tests were scored on the basis of clinical ratings made by the examiner during the conversation and test session. The scores of the cognitive part of the ADAS range from zero, which is equivalent to no error or deficit, to a maximum of 75, which instead indicates a serious deficit in all tests. The non-cognitive part of ADAS includes the assessment of the presence or absence of depression, crying, delusions, hallucinations, compulsive walking, increased motor activity and degree of cooperation during the tests. The score to each of these behavioral areas (as well as for the ratings of the cognitive part) was assigned in a range from 0 (no deficit to a test or absence of a specific behavior) to 5 (deficit of maximum severity to a test or appearance very frequent behavior). The score for the non-cognitive part, resulting from the sum of the scores assigned in the seven mental behavior sections, can range from 0 to 35. The total score (cognitive plus behavioral) varies from 0 to 110. In practice, the cognitive part has few people, even among the non-demented they get a score of zero (no deficit) because memory tests are difficult enough for even normal people to make some mistakes. Longitudinal studies on patients with Alzheimer's disease have shown that scores on the cognitive part of ADAS increase by an average of 9 points per year. The rate of change is slower in very mild patients and in those with severe dementia than in patients with moderate forms of deterioration (Stern et al. 1994), while scores on the non-cognitive portion of ADAS generally do not show an increase with the progression of dementia as instead occurs with the scores of the cognitive part (Zec et al. 1992).
2) ADCS-ADL (Alzheimer's Disease Cooperative Study-Activities of Daily Living), per valutare la funzionalità globale delle attività di vita quotidiana. Le ADL sono le attività autonome che una persona deve eseguire ogni giorno (p. es., mangiare, vestirsi, lavarsi, spostarsi dal letto alla sedia, utilizzare il bagno, controllare lo sfintere vescicale e anale). I pazienti incapaci di eseguire queste attività e di nutrirsi adeguatamente, di solito, richiedono un supporto per 12-24 h al giorno. 2) ADCS-ADL (Alzheimer's Disease Cooperative Study-Activities of Daily Living), to evaluate the overall functionality of daily living activities. ADLs are the self-contained activities a person must perform each day (eg, eating, dressing, bathing, moving from bed to chair, using the toilet, checking bladder and anal sphincters). Patients unable to perform these activities and to feed themselves adequately usually require support for 12-24 hours per day.
3) FAB (Frontal Assement Battery) per indagare le funzioni esecutive frontali. 3) FAB (Frontal Assement Battery) to investigate frontal executive functions.
Valuta estensivamente le funzioni frontali quali: concettualizzazione, flessibilità mentale, programmazione, sensibilità all’interferenza, controllo inibitorio, autonomia dall’ambiente. E’ caratterizzata da una buona validità di costrutto e discriminativa. Ha inoltre una buona reliability /consistenza interna. E’ considerata sensibile alle disfunzioni del circuito prefrontalemediale ma meno sensibile alle disfunzioni prefrontali ventrolaterali e dorso laterali. Extensively evaluates frontal functions such as: conceptualization, mental flexibility, programming, sensitivity to interference, inhibitory control, autonomy from the environment. It is characterized by good construct validity and discrimination. It also has good internal reliability / consistency. It is considered sensitive to dysfunctions of the prefrontal medial circuit but less sensitive to ventrolateral and dorsal lateral prefrontal dysfunctions.

L'obiettivo primario di questo studio fu quello di verificare la tollerabilità e la efficacia clinica di rotigotina 4 mg rispetto al placebo sulla base del cambiamento dal punteggio basale utilizzando la scala ADAS-Cog. The primary objective of this study was to test the tolerability and clinical efficacy of 4 mg rotigotine versus placebo based on the change from baseline score using the ADAS-Cog scale.

RISULTATI RESULTS
Dati preliminari, relativi ad un primo gruppo di 38 pazienti arruolati nello studio clinico (19 pazienti trattati con Rotigotina e 19 con placebo), dimostrano gli agonisti dopaminergici sono agenti utili nella prevenzione o per il rallentamento della progressione dei sintomi del declino cognitivo, dei sintomi del declino funzionale, e dei sintomi del decadimento dello stato funzionale in pazienti affetti da malattia di Alzheimer allo stadio precoce trattati da almeno tre mesi con i farmaci tradizionali senza che quest’ultimi abbiano rallentato in modo significativo il progredire della patologia. Preliminary data, relative to a first group of 38 patients enrolled in the clinical study (19 patients treated with Rotigotine and 19 with placebo), demonstrate dopaminergic agonists are useful agents in the prevention or slowing of the progression of symptoms of cognitive decline, symptoms functional decline, and symptoms of functional status decay in patients with early stage Alzheimer's disease treated for at least three months with traditional drugs without the latter having significantly slowed the progression of the disease.

Nella scala ADCS-ADL i pazienti AD trattati con Rotigotina non hanno presentato un significativo declino delle funzioni di vita quotidiana (p=0.35), al contrario de gruppo di pazienti trattai con placebo (p=0.03). Nella scala FAB non vi sono differenze significative nei due gruppi di pazienti rispetto alla valutazione basale (Rotigotina: p=0.46, Placebo: p = 0.18) I risultati ottenuti, sopra riportati, mostrano che i pazienti trattati con gli agonisti dopaminergici secondo l’invenzione mostrarono un rallentamento statisticamente significativo della progressione dei sintomi del declino cognitivo, funzionale e dei sintomi del decadimento dello stato funzionale rispetto al gruppo del placebo. È degno di nota sottolineare che detti pazienti trattati ed il placebo erano contemporaneamente in trattamento, da almeno tre mesi, con farmaci inibitori dell’acetilcolinesterasi usati nel settore. In the FAB scale there are no significant differences in the two groups of patients with respect to the baseline assessment (Rotigotine: p = 0.46, Placebo: p = 0.18) The results obtained, reported above, show that patients treated with dopaminergic agonists according to the invention showed a statistically significant slowing of the progression of symptoms of cognitive, functional decline and symptoms of functional status decay compared with the placebo group. It is worth noting that these treated patients and the placebo were simultaneously being treated, for at least three months, with acetylcholinesterase inhibitor drugs used in the industry.
