Abstract

1514479 Prosthetic joints CHAS F THACKRAY Ltd and R A ELSON 29 Sept 1975 [1 Oct 1974] 42621/74 Heading A5R A prosthetic joint, especially a knee joint, comprises a tibial member including a metal stem I rotatably mounted in a plastics sleeve 5 fixed in the tibia 3, and a femoral member forming a hinge-type connection with an upper extension 4 of stem 1, whereby relative rotation is permitted between the bones of the joint about the longitudinal axis of the tibia. The sleeve 5 has an integral tapered peg 8, sleeve 5 and peg 8 having annular grooves 9 to assist in keying bone cement 11 thereto. In a modification, the stem is made of plastics and the sleeve of metal. The femoral member may be as described in Specification 1349587.

Description

This invention relates to prosthetic joints, particularly replacement hinge-type joints in the body.
According to the present invention there is provided a prosthetic joint for location between two bones and com5 prising a first component for location within one bone and a second component for location in the other bone and forming a hinge-type connection with the first component, said first component including a sleeve member for fixing in said one bone and a stem for rotational mounting within said sleeve member to allow relative rotation between the bones about the longitudinal axis of said one bone.
The invention has particular application as a prosthetic knee joint in which case the joint comprises a tibial member and a femoral member, either of which may be in the form of a sleeve member and rotatable stem.
In one embodiment in accordance with the present invention a prosthetic knee joint comprises a tibial member having a stem for location within a sleeve mounted in the tibia and a spindle extending transversely to the stem for loc2q ation within some mechanism affording attachment to the femur and providing rotation resembling flexion and extension of the knee.
The present invention provides a means whereby an artificial hinge-type joint may reduce torsional stresses which would otherwise prejudice the strength of the components of the joint or the strength of the bonds to the bones, by incorporating a rotational freedom into the longitudinal axis of the system.
An embodiment of the invention will now be described by way of example only and with reference to the accompanying drawings in which;Figure 1 is a cross section through part of the human leg showing an artificial joint of the invention in position therein; and Figure 2 is a section on line X-Y of Figure 1.
Referring to the drawings the prosthesis comprises a tibial member including a stem 1 made of metal, the stem including a lower portion 2 located within the tibia 3 and an upper extension 4, the axis of the extension making an obtuse angle β with the axis of the lower portion 2.
The femoral portion of the prosthesis is not shown in the drawings and it may, for instance be similar to that described in British Patent Specification No. 1,349,587.
In this case a spindle, which extends transversely to the stem of the tibial member, is provided for location within the femoral condyles, said spindle engaging with e.g. by passing through a bore in, the free end of the extension. The spindle projects on each side of the extension and the projecting ends are each pivotally supported by a bearing bush which is designed for anchorage in a femoral condyle. However, it will be understood that the femoral portion of the prosthesis may be different, both in design and principle, from that described in the above-mentioned 3,7 3 S Patent Specification, although still forming a hinge joint vzith the tibial member.
Lower portion 2 of stem 1 is cylindrical in from and is located for free rotation in the sleeve portion 5 of tibial member 6 made of plastics material. Stem 1 further includes an annular shoulder 7 which engages the Upper annular surface of sleeve portion 5.
Member 6 includes an integral tapered peg portion 8, which when the said member is in position in the tibia 3, extends downwardly therein along an extension of the longitudinal axis of the sleeve portionl. Peg portion 8 and the sleeve portion 5 of the member 6 are provided with transverse annular grooves 9.
Member 6 is fitted into a reamed bore 10 in the tibia and is held in place by a conventional bone cement 11. The grooves 9 assist in keying the cement to the member 6.
In the above described embodiment, rotation of the stem 1 within sleeve portion 5 is possible thereby relieving torsional stresses which would otherwise have been imparted to the components of the device during the functioning of the lower limb.
It should be appreciated that the materials used for the member 6 and the stem may be changed. Thus, for instance, it is possible that the member 6 may be made of metal and the stem of plastics material.
