Τρισδιάστατος Σύμμορφος Εφαρμοστής για Διεγχειριτική Ακτινοθεραπεία 3D Conformal Applicator for Intraoperative Radiotherapy
Η παρούσα εφεύρεση αφορά σε ένα τρισδιάστατο σύμμορφο εφαρμοστή για εφαρμογές Διεγχειριτικής Ακτινοθεραπείας ζώων συντροφιάς που πάσχουν από καρκίνο. The present invention relates to a three-dimensional conformal applicator for Intraoperative Radiotherapy applications of companion animals suffering from cancer.
Διεθνείς μελέτες έχουν δείξει ότι το 30% των ζώων συντροφιάς πεθαίνει από καρκίνο. Η ογκολογική αντιμετώπιση των μικρών ζώων είναι ανάλογη με αυτή των ανθρώπων. Οι κύριοι άξονες είναι: η Χειρουργική, η Ακτινοθεραπεία και η χημειοθεραπεία. Η χειρουργική αποτελεί σήμερα την κύρια αντιμετώπιση των ογκολογικών περιπτώσεων σε μικρά ζώα. Οι περισσότερες των περιπτώσεων εντοπισμένων βλαβών χρήζουν ακτινοθεραπείας ως: (α) κύριας θεραπείας σε περίπτωση που δεν είναι δυνατή η χειρουργική εξαίρεση της βλάβης, είτε λόγω θέσης είτε λόγω μεγέθους, (β) επικουρικής θεραπείας, μετά την χειρουργική επέμβαση, είτε για την αποστείρωση του χειρουργικού πεδίου από καρκινικά κύτταρα είτε για την ακτινοβόληση εναπομείναντα όγκου που δεν ήταν δυνατόν να εξαιρεθεί και (γ) ανακουφιστικής θεραπείας σε μη-εγχειρήσιμους όγκους για την ανακούφιση πόνου και άλλων κλινικών συμπτωμάτων. Η Κτηνιατρική Ακτινοθεραπεία δεν είναι νέα ιδέα. Οι πρώτες εφαρμογές αναφέρονται από το 1972 όπου μονάδες Κοβαλτίου-60 και Καισίου-137 χρησιμοποιήθηκαν και την εξωτερική ακτινοβόληση καρκινικών όγκων σε μικρά ζώα. Σήμερα υπάρχουν πολλές κτηνιατρικές μονάδες, κυρίως στην Δυτική Ευρώπη και στην Βόρεια Αμερική, οι οποίες προσφέρουν υπηρεσίες ακτινοθεραπείας σε σκύλους και γάτες. Στις περισσότερες των περιπτώσεων, η τεχνική ακτινοβόλησης που χρησιμοποιείται είναι εξωτερική ακτινοθεραπεία με δέσμες φωτονίων ή ηλεκτρονίων υψηλής ενέργειας προερχόμενες από γραμμικό επιταχυντή. International studies have shown that 30% of companion animals die from cancer. The oncological management of small animals is similar to that of humans. The main axes are: Surgery, Radiotherapy and chemotherapy. Surgery is currently the main treatment for oncological cases in small animals. Most of the cases of localized lesions require radiation therapy as: (a) primary treatment in cases where surgical excision of the lesion is not possible, either due to location or size, (b) adjunctive treatment, after surgery, or to sterilize the surgical field from cancer cells either to irradiate residual tumor that could not be excised and (c) palliative treatment in inoperable tumors to relieve pain and other clinical symptoms. Veterinary Radiotherapy is not a new concept. The first applications are reported from 1972 where units of Cobalt-60 and Cesium-137 were used and the external irradiation of cancerous tumors in small animals. Today there are many veterinary facilities, mainly in Western Europe and North America, that offer radiation therapy services to dogs and cats. In most cases, the radiation technique used is external beam radiation therapy with high-energy beams of photons or electrons from a linear accelerator.
Μία καινοτόμος εφαρμογή ακτινοθεραπευτικής αντιμετώπισης, τόσο σε ανθρώπους όσο και σε ζώα είναι η Διεγχειρητική Ακτινοθεραπεία (ΔΑ). Διεγχειρητική Ακτινοθεραπεία (IntraOperative RadioTherapy - 10RT) είναι ένας τρόπος εφαρμογής θεραπείας ιοντιζουσών ακτινοβολιών απευθείας στη περιοχή του καρκίνου κατά τη διάρκεια της χειρουργικής επέμβασης. Η ΔΑ δίνει μία συγκεντρωμένη και εστιασμένη δόση ακτινοθεραπείας στην κοίτη του όγκου αμέσως μετά την αφαίρεση του βοηθώντας να καταστραφούν τα καρκινικά κύτταρα που δύναται να έχουν απομείνει. Η κοίτη του όγκου (tumor cavity) είναι κατεξοχήν στο υψηλότερο ποσοστό κινδύνου για τυχών υποτροπή. Γ ια την εφαρμογή της ΔΑ έχουν αναπτυχθεί διάφορα συστήματα ακτινοβόλησης που κάνουν χρήση είτε υψηλής ενέργειας δέσμες ηλεκτρονίων είτε χαμηλής ενέργειας δέσμες φωτονίων. Ειδικά για τα συστήματα με δέσμες φωτονίων, έχει αναπτυχθεί σύστημα ακτινοβόλησης ενέργειας έως 50kV το οποίο αποτελείται από μικρό γραμμικό επιταχυντή που επιταχύνει ηλεκτρόνια σε καθοδικό σωλήνα μήκους 10cm και η παραγωγή των φωτονίων επιτυγχάνεται με την παρεμβολή στόχου χρυσού στο τέλος της διαδρομής δημιουργώντας μία σημειακή πηγή φωτονίων φασματικής ενέργειας 0-50kV. Για την κλινική εφαρμογή γίνεται χρήση ειδικών σφαιρικών εφαρμοστών (spherical applicator) διαφόρων διαμέτρων (2.0 - 5.0cm σε 0.5cm διαβάθμιση) ανάλογα με την διάμετρο της κοίτης του όγκου που δημιουργείται κατά την αφαίρεση του. Η παρουσία των εφαρμοστών είναι απαραίτητη ώστε να δημιουργηθεί η δέουσα απόσταση μεταξύ της πηγής και του ιστού ώστε να επιτευχθεί η ομοιόμορφη ακτινοβόληση στο επιθυμητό βάθος. Οι σφαιρικοί εφαρμοστές που χρησιμοποιούνται σήμερα είναι κατασκευασμένοι από πολυμερές βιοσυμβατό υλικό ULTEM και έχουν 8 μεγέθη σταθερής διαμέτρου από 1.5cm έως 5.0cm με βήμα 0.5cm. Το μεγάλο μειονέκτημα αυτών των εφαρμοστών είναι ότι το σταθερό σφαιρικό τους σχήμα το οποίο αδυνατεί να προσαρμοστεί στην χειρουργική κοίτη του όγκου η οποία στην πλειοψηφία των περιπτώσεων έχει ακανόνιστο σχήμα. Η μησυμμόρφωση με το σχήμα της κοίτης δημιουργεί κενά τα οποία καταλήγουν σε μη ομοιόμορφη ακτινοβόληση του παρακείμενου ιστού που είναι απαίτηση για την πλήρη αποστείρωση από καρκινικά κύτταρα. An innovative application of radiotherapy treatment, both in humans and animals, is Intraoperative Radiotherapy (IA). IntraOperative RadioTherapy (10RT) is a way of applying ionizing radiation therapy directly to the cancer area during surgery. DA delivers a concentrated and focused dose of radiation therapy to the tumor bed immediately after it is removed, helping to destroy any remaining cancer cells. The tumor cavity is particularly at the highest risk of recurrence. For the application of DA, various irradiation systems have been developed that use either high-energy electron beams or low-energy photon beams. Especially for systems with photon beams, an energy irradiation system up to 50kV has been developed which consists of a small linear accelerator that accelerates electrons in a 10cm long cathode tube and the production of photons is achieved by interposing a gold target at the end of the path creating a point source of photons spectral energy 0-50kV. For clinical application, special spherical applicators of various diameters (2.0 - 5.0cm in 0.5cm increments) are used depending on the diameter of the tumor bed created during its removal. The presence of the applicators is necessary to create the proper distance between the source and the tissue to achieve uniform irradiation to the desired depth. The spherical applicators in use today are made of polymer biocompatible material ULTEM and have 8 sizes of constant diameter from 1.5cm to 5.0cm with a step of 0.5cm. The big disadvantage of these applicators is that their fixed spherical shape cannot be adapted to the surgical bed of the tumor, which in the majority of cases has an irregular shape. Non-conformity with the shape of the bed creates gaps which result in non-uniform irradiation of the adjacent tissue which is a requirement for complete sterilization from cancer cells.

Ο τρισδιάστατος σύμμορφος εφαρμοστής κατασκευάζεται από θερμοπλαστικό υλικό το οποίο, με την κατάλληλη επεξεργασία, διαμορφώνεται στο ακριβές σχήμα της χειρουργικής κοίτης του καρκινικού όγκου δημιουργώντας εσωτερική κοίλη επιφάνεια στην οποία εισέρχεται η πηγή της ακτινοβολίας. Με αυτό τον τρόπο επιτυγχάνεται η απαραίτητη απόσταση μεταξύ πηγής και ιστού χωρίς κενά ώστε η ακτινοβόληση να είναι πραγματικά ομοιόμορφη στο επιθυμητό βάθος. In this way, the necessary gap-free source-to-tissue distance is achieved so that the irradiation is truly uniform at the desired depth.
Το θερμοπλαστικό υλικό από το οποίο κατασκευάζεται ο τρισδιάστατος σύμμορφος εφαρμοστής είναι βιοσυμβατό σύμφωνα με το ISO 10993 ώστε να δύναται να έρθει σε άμεση επαφή με ιστούς και σωματικά υγρά χωρίς κίνδυνο επιμόλυνσης. Το θερμοπλαστικό υλικό αποτελείται από μίξη συμπολυμερές στυρολίου ακρυλονιτριλίου και πολυκαπρολακτόνης, ή άλλου αντίστοιχου πολυμερούς και το οποίο έχει τις εξής ιδιότητες: (α) ελαστικοποιείται στους 60°-70° C, παίρνει το επιθυμητό σχήμα και στην συνέχεια σταθεροποιείται σε 2-3 λεπτά σε θερμοκρασία περιβάλλοντος, (β) έχει πάχος 1.5-1.6 mm και (γ) έχει φυσική πυκνότητα 1.10-1.15 g/cm<3>. Υλικά με τις ανωτέρω ιδιότητες υπάρχουν διαθέσιμα και χρησιμοποιούνται για την κατασκευή ακινητοποιητικών μασκών στην ακτινοθεραπεία. Materials with the above properties are available and are used for the manufacture of immobilizing masks in radiotherapy.
Ο τρισδιάστατος σύμμορφος εφαρμοστής κατασκευάζεται προσωποποιημένα μέσα στην χειρουργική αίθουσα. Μετά την χειρουργική αφαίρεση του όγκου, γίνεται οπτική επισκόπηση της κοίτης. Φύλλο θερμοπλαστικού υλικού θερμαίνεται σε μπάνιο νερού σε θερμοκρασία 60°-70° C για 3-5 λεπτά μέχρις ότου να γίνει ελαστικό. Το ελαστικό φύλλο τοποθετείται στην κοίτη του όγκου και χειροκίνητα προσαρμόζεται προσεκτικά σε όλες τις πτυχές αφήνοντας κοίλη περιοχή στο κέντρο διαμέτρου 2cm-5cm ανάλογα με το μέγεθος της κοίτης. Στο εξωτερικό μέρος διατηρείται κατάλληλη οπή ώστε να εισέλθει η πηγή ακτινοβολίας. Μετά την πάροδο 2-3 λεπτών, ο τρισδιάστατος σύμμορφος εφαρμοστής έχει σταθεροποιηθεί σε σχήμα και δύναται να γίνει η ακτινοβόληση. Ο εν λόγω εφαρμοστής είναι αναλώσιμος και δύναται να χρησιμοποιηθεί μόνο μία φορά. The 3D conformal applicator is custom made in the operating room. This applicator is disposable and can only be used once.
