The present invention relates to a method and a glove device for defining the motor status of a person suffering from a disorder or disease that exhibits motor symptoms.
Υπόβαθρο της εφεύρεσης Background of the invention
There are many factors such as diseases, drug treatments, injuries and others, which can result in a person having movement symptoms. Motor symptoms include, among others, dyskinesia, during which a person is in a hyperactive state, and bradykinesia, during which a person is in a hypoactive state.
Bradykinesia is a major disease manifestation for example in patients suffering from Parkinson's disease. Such patients are given levodopa, which may have the effect of making the patient dyskinetic for a period of time after the drug is administered. The half-life of the administered drug levodopa is approximately 90 minutes.
As Parkinson's disease progresses, the half-life of levodopa in a patient decreases, so the effective dose range also decreases. But this makes the assessment and control of the dosolonia an extremely difficult and complex matter. This is usually treated with an increase in dose frequency, several times to ten doses per day, in an attempt to control symptoms and allow the patient to have a relatively good quality of life.
This means that patients suffering from Parkinson's disease exhibit periods of bradykinesia, periods of dyskinesia and periods of relatively normal motor function. This happens more than once within a day and during the total duration of action of a single dose of levodopa. Even if a satisfactory dosage regimen is selected at a particular point in time, due to the fact that the nature of Parkinson's disease is progressive, this means that the treating neurologist should systematically monitor a patient's symptoms in order to systematically control the current dosage in the treatment of the patient. This is very difficult and in practice results in the attending physician prescribing either a higher dose which increases episodes of dyskinesia or a reduced dose which does not prevent episodes of bradykinesia. Conventional clinical treatment is based on the subjective judgment of the attending physician and has no objective measure. to determine whether a change in dose is effective in improving disease symptoms.
Also, naturally, the clinical observation is conducted for a short period of time, during which the patient is usually examined for between 10 and 20 minutes, once every two months to once every six months. However, the deviations of a patient's motor status from one day to the next and also within a day from one hour to the next make the determination of a patient's motor status a complex, difficult task. Physicians are often forced to rely on patients' descriptions and/or written diaries in order to understand and assess the patient's current motor status for the period of time between medical visits (and the corresponding subjective examination). In practice, however, patients are unable to objectively rate their condition, and the motor episodes themselves make it very difficult for a patient to record objectively and with relative accuracy the nature and timing of the motor episodes, which he manifests .
Monitoring the course of a person's Parkinson's disease and choosing when to change treatment presents the important problem of identifying the stage at which conventional treatments such as levodopa can no longer control symptoms. and need alternative treatments such as pharmacologically advanced forms or mechanically assisted therapies. In many patients, especially the elderly, the window of time in which some alternative treatments must be selected is short, and treatments may be ineffective when applied late. This means that in the case of an untimely (possibly late) application of an alternative treatment, the benefit resulting from it may also be reduced.
The reasons described above prove that it is imperative to find a "tool" and a method to identify and more accurately evaluate the motor symptoms that manifest in a person.
The present invention addresses the above-described problems of the current state of the art because it provides a device/glove ("tool") and a method that offer the ability to accurately determine and objectively evaluate the motor symptoms that manifest in a person.

The present invention relates to a device/glove (1), which is a "precision measurement instrument" of the motor state of a patient, as well as the logistical equipment and the method of supporting the device when performing the measurements and when processing the results.
The glove (1) according to the present invention consists of a combination of fabric materials with electronic components.
The glove (1) according to the present invention has sensors (1.1.1, 1.2.1) on each finger, an accelerometer (1.6) on the upper side of the palm, surface areas (3) which are good conductors of electric current , conductive threads and connections sewn to the inner side of the glove fabric. It also has a coil sewn under a plastic protection, which is internally connected to the main system element, which is covered by a case. The glove (1) according to the present invention has special sensors in order to monitor the most important movements of the hand and fingers. The five finger sensors (1.1.1) can measure the position of the fingers, adjusting their resistance.
In addition, there is a three-axis accelerometer (1.6) on the upper part of the palm, which measures the position of the wrist.
The entire palm on its underside is sewn with conductive materials. This ensures that the fingertips are able to affect any capacitive touch screen. The coil is responsible for the wireless charging of the glove (1). There are also four very small natural magnets to perform alignment on the charging base. All the sensors and the power supply are connected by very thin wires and conductive filaments to a box that contains all the necessary led indicators (1.4) switches, connectors and the memory card slot (1.7) to control all the functions.
On the upper side, there are two groups of led indicators, on the front side there are switches for controlling the device and leds that refer to them. The back of the component highlights the presence of attached storage media and the debug interface.
Current practice in relation to the follow-up of patients with mobility problems is mainly based on the patient's visits to the treating physician. During these, the doctor, through the examination and dialogue with the patient, as well as with the help of the review of the diaries kept by the patient himself every day, tries to establish and evaluate his condition as well as the effectiveness of the current treatment . The above procedure is based on subjective criteria, as during the examination, the patient's condition may not be indicative of his daily situation at home, while the data on the patient's condition at home involve a high degree of subjectivity, as they come either from him the patient (through the interview or the diary he keeps) or by friendly persons close to him.
For this reason, any strategy/method that records motor symptoms and, through their analysis, objectively assesses the patient's motor status as well as changes in his response to each treatment, offers important and objective clinical information and allows early and timely therapeutic interventions by the physician. The device-glove (1) is the "tool"/instrument for implementing such a strategy/method, since it aims at the continuous monitoring and objective evaluation of the patient's motor condition, and the optimal regulation of his medication, being equipped with a a series of sensors (1.1.1) placed above the fingers, aimed at recording the movements of the fingers, a conductive fabric, on the tips of the fingers and inside the palm, and a three-axis accelerometer (1.6) on the upper part of the palm . The basic purpose of the device-glove is for the patient to be able to use it without a doctor's supervision, and with its use an objective recording and evaluation of his motor condition is carried out. By using the glove (1) at a predetermined time during the day (e.g. before or after the dose of the drug, depending on the symptoms of each patient, in relation to meals, etc.), the doctor receives significant information regarding the patient's bradykinesia and resting tremor, which he uses to achieve the optimal regulation of his medication. This has the effect of improving the patient's quality of life, as problems are identified at birth and treated, and the patient is offered an individualized treatment based on the symptomatic expression of the disease and the side effects due to the medication. As far as the attending physician is concerned, he is able for the first time to have an objective assessment of the patient's motor status for the time intervals between his visits, improving the quality of the services he offers.
Η μέθοδος αντικειμενικού προσδιορισμού της κινητικής κατάστασης ενός ατόμου σύμφωνα προς την εφεύρεση περιλαμβάνει τρία βασικά στάδια ανάλυσης: The glove device receives a series of data (numerical values from the sensors at regular time intervals - time series), which are then processed using a new analysis method with the aim of automatically and objectively evaluating the patient's motor status. The method of objectively determining the motor state of a person according to the invention includes three basic stages of analysis:
α. την προεπεξεργασία των χρονοσειρών, με χρήση γνωστών καθαυτών τεχνικών ψηφιακής επεξεργασίας σήματος, με στόχο την βελτίωση της ποιότητάς τους και την απομάκρυνση ανεπιθύμητων χαρακτηριστικών, a. the pre-processing of the time series, using well-known digital signal processing techniques, with the aim of improving their quality and removing unwanted characteristics,
β. την επεξεργασία των χρονοσειρών, με χρήση γνωστών καθαυτών τεχνικών ψηφιακής επεξεργασίας σήματος, με στόχο την εξαγωγή χαρακτηριστικών που σχετίζονται με την κινητική κατάσταση του ασθενούς, b. the processing of the time series, using well-known digital signal processing techniques, with the aim of extracting characteristics related to the patient's motor state,
γ. την ανάλυση των εξαχθέντων χαρακτηριστικών, με χρήση γνωστών καθαυτών στατιστικών μεθόδων και ευφυών τεχνικών, με στόχο την αυτόματη αξιολόγηση και βαθμονόμηση της κινητικής κατάστασης του ασθενούς. c. the analysis of the extracted characteristics, using well-known statistical methods and intelligent techniques, with the aim of automatically evaluating and calibrating the patient's motor status.
Στην περίπτωση παραδείγματος χάριν ενός ασθενούς ο οποίος πάσχει από την νόσο του Πάρκινσον : In the case, for example, of a patient suffering from Parkinson's disease:
Ενδεικτικά αναφέρονται: during stage b: processing of the time series is carried out, using digital signal processing techniques, with the aim of extracting features related to the patient's motor state. The extracted features quantify motion characteristics. Examples include:
· Frequency of movement (e.g. frequency of fingering).
• Width of movement (eg maximum distance between index finger and thumb in each repetition of the fingering exercise).
• Hesitancy in movement (eg, delay in starting movement). • Pauses during movement.
Ενδεικτικά αναφέρονται: during stage c: an analysis of the extracted features is carried out, using statistical methods and intelligent techniques, with the aim of automatically evaluating and calibrating the patient's motor status. Various techniques are used for feature analysis, both cognitive and data-driven. Examples include:
• Cognitive model based on the Unified Parkinson's Disease Rating Scale (UPDRS). The model is a standard quantification implementation of the UPDRS global scale, and calibration is performed on the 0-4 scale. The model is based on the processing of the signals recorded by the glove with the aim of quantifying qualitative elements of the scale, such as the deceleration in the movement, the change in the amplitude of the movement, the pauses in the movement and the hesitation in the initiation of the movement. • Statistical model based on maximum likelihood, with respect to the distribution of feature values at each scale value, calculated from an initial characterized data set. • Intelligent techniques, resulting from the training of a model known from the literature (eg decision trees, random forests, neural networks, support vector machines, etc.) performed on the basis of an initial characterized data set. In addition, fuzzy logic is incorporated into the model with the aim of quantifying vague concepts, such as "mild", "moderate", "severe", "frequent". The above is implemented using fuzzy correlation functions, a fuzzy logic engine and a final output deblurring technique.
The various techniques for feature analysis, both cognitive and data-driven, are known per se and are not specifically described in this specification.
The device/glove according to the present invention due to the simple, automated and objective nature of examining the motor status of patients is particularly suitable for conducting extensive clinical experiments according to the corresponding internationally recognized protocols. At the same time it provides new possibilities which will lead to the creation of new more effective clinical or non-experimental.
It is obvious that a person skilled in the art can modify or combine in a different way, not described in the present description, the basic features of the device/glove and the method of objectively determining the kinetic state of a person while remaining within the scope of protection of the present invention which is defined by the following claims.
