988,130. Surgical instruments. GOSUDARSTVENNY ELEKTROTEKHNICHESKY ZAVOD VEF. Nov. 11, 1963, No. 44444/63. Heading A5R. [Also in Division H1] An instrument producing an electric discharge for removing concretions from the urinary system, comprises a stem for insertion into the system, electrical leads in the stem and electrodes at the distal end of the stem such that when the high-voltage pulse generator to which the leads are connected is switched on an electric discharge occurs between the electrodes to break up the concretion. In Figs. 1 and 3 the instrument comprises stem 1 of a cystoscope inserted in which is a flexible probe 2 comprising hollow needle 5 with head 6 which acts as an electrode, and surrounded by insulation 4, wire braided sheath 7 and lacquer coating 8 and also by cupshaped electrode 3<SP>1</SP>. The needle 5 and sheath 7 act as leads are connected by wires to a highvoltage pulse generator capable of producing steep-fronted pulses. Needle 5 may be connected to a syringe 9 for introducing fluid into the urinary system before and after the concretion has been broken up. The needle need not be hollow. The probe 2 of Fig. 1 may be replaced by a probe in which the cup electrode 3<SP>1</SP> has a disc-like radial portion 28 with a central aperture in which the other electrode 29 lies (Fig. 9) or by a probe consisting of a rod 30 with leads 31 mounted therein (Fig. 10). In Fig. 7 the stem comprises two tubes rotatable relative to each other by handles 20 and having arms 18 and 19 at the distal end for grasping the concretion. The electrodes 23 and 24 are connected to the arm 18. The leads pass along a groove inside the outer of the two tubes.
