Abstract

The present invention provides a surgical dressing 1 for positioning about a drain (4, fig.2). The surgical dressing comprises; an inner side and an outer side 2; a central aperture 3 through which a drain may be positioned; and first 12 and second 13 sutures extending outwardly from the outer side of the dressing, attached to the dressing at the inner end, and having a free outer end. Also disclosed is a dressing 1 comprising; an inner side and an outer side 2; a central aperture 3 through which a drain (4, fig.2) may be positioned; and first 9 and second 11 apertures adjacent a first 10 and second 22 edges of the dressing for passing a suture through; wherein, the first edge 10 of the dressing is on an opposing side of the dressing to the second edge 22. The dressing may also have third 14 and fourth 15 sutures extending outwardly from the outer side 2 of the dressing 1, attached to the dressing at an inner end, and having a free outer end.

Description

Surgical Dressing
Field of the Invention
The present invention relates to medical dressings for use about surgical drains and for securing surgical drains.
Background to the Invention
Surgical drains are tubes placed near surgical incisions in post-operative patients and are used after a variety of operations in order to drain fluid from the surgical wound or a body cavity.
113 The main indications for surgical drain use include: the need to eliminate dead space, to remove existing fluid or gas, to prevent accumulation of fluid or gas, and to monitor for post-operative complications.
Surgical drains can relieve pressure on healing tissues that may impair perfusion or cause pain, thereby reducing morbidity; they enable monitoring for potential complications by allowing easy sampling of fluid during healing; and they can be used to address complications such as a post -operative bile leak, seroma formation or leak from an anastomosis (join) Surgical drains are composed of a variety of substances, such as latex or plastic. In order to stop drains from inadvertent displacement or premature removal, they need to be secured in place The conventional method for securing a surgical drain in place is for a medical practitioner to suture the drain in place using one or two sutures that are placed through the patient's skin and then tied around the drain, most commonly using a Roman Garter technique. The drain site is then usually dressed using two or more gauze swabs and a clear adhesive dressing. As an alternative, various specialist adhesive dressings are used to secure surgical drains. Examples of these dressings include the Elist Drain Retainer developed at Solutions Matter, LLC (San Diego, CA, USA), the Niko Drain Fix' (marketed by ConvaTec) and the Opsite Visible Drain Dressing (marketed by Smith + Nephew). Despite the availability of these dressings, suturing remains the most common method of securing a surgical drain in place. This is because suturing a surgical drain in place is usually secure, the technique is relatively simple to perform, and is inexpensive.
There are disadvantages of suturing surgical drains in position. There can be significant pain from skin sutures creating a load over a small surface area, which are placed under tension as they are pulled by the drain. The drain being held by only one point, if a single suture is used, can be disadvantageous as if the suture breaks the drain can be inadvertently removed or displaced. There is also a need for bulky drain dressings when gauze swabs are used as the dressing. In addition there can be difficulty in assessing a surgical site through a dressing after suturing and there can be a risk of infection with sutures placed proximal to the surgical site.
There are also disadvantages to adhesive dressings for surgical drains. In particular if the dressing loses adhesion when the dressing gets wet and/or soiled then there is a risk of drain displacement. Further, many of the adhesive dressing are over-engineered and have prohibitive cost as compared to simple suturing.
In light of the above there is a need for an improved surgical dressings for drains that is secure, low cost and easy to apply.
Summary of the Invention
The present invention provides a first invention being a surgical dressing having an inner side and an outer side and a central aperture through which a drain may be positioned, wherein the surgical dressing further comprises: a first suture extending outwardly from the outer side of the dressing, attached to the dressing at an inner end, and having a free outer end; and a second suture extending outwardly from the outer side of the dressing, attached to the dressing at an inner end and having a free outer end.
The dressing is used in the following manner. The drain is positioned to extend through the central aperture of the dressing. The dressing is then adhered to the patient's skin about the drain site. This may be done in any manner apparent to the person skilled in the art. The inner side of the dressing may be adhesive, as discussed below. Alternatively or additionally adhesive dressings may be used to adhere the dressing in place. This may be done in any manner apparent to the person skilled in the art. Once the dressing is adhered in place about the dressing the first suture and the second suture can be used to hold the drain in place. This may be done in any manner apparent to the person skilled in the art, for example, the first suture and the second suture may be wrapped about the drain and the outer ends of the first suture and the second suture may then be tied together to secure the drain.
The present invention is advantageous in that it integrates sutures with a surgical dressing to provide a simple, cheap and straightforward dressing that can be utilised to securely hold a drain in place. The dressing can provide reduced pain by not placing a pull force on the skin at the drain site and instead distributes the load over the surface area of the dressing. Instead, as the sutures are attached to the dressing, rather than the skin, they will pull on the dressing and the pull force will be spread over the area of the dressing. The dressing can also provide a neat and self-contained dressing without the need for additional gauze swabs or other bulky dressings. As compared to prior art surgical dressings for drains the present invention is relatively simple thereby allowing it to be produced in a cost-effective manner. Further, the 113 present invention can be used to secure a drain in a manner wherein the drain is not kinked adjacent the patient, enhancing the operation of the drain. This can be seen clearly in the preferred embodiment shown in the Figures and described below.
The first suture and the second suture may be any appropriate length. Any suture of the present invention may be provided with a length that is intended to be trimmed after tying about a drain or they may be provided at the appropriate length. In embodiments of the invention the first suture may be the same length as the second suture. In alternative embodiments of the invention the first suture and the second suture may be of different lengths. The suture may be made of any suitable material including, but not limited to nylon, silk, PDS, or polyester.
The inner end of the first suture and the inner end of the second suture and the inner end of any additional sutures may be located at any appropriate position on the outer side of the dressing. Similarly, the inner end of the first suture and the inner end of the second suture and the inner end of any additional suture may be attached to the dressing in any appropriate manner. In embodiments of the invention the inner ends of the sutures may be attached to the dressing by being embedded within the dressing. In embodiments of the invention the inner end of the first suture and the inner end of the second suture are equidistant from the central aperture. This can ensure that the drain is able to be tied in an appropriate position without excessive strain on either the first suture or the second suture.
Surgical dressings according to the present invention may comprise further sutures, each extending outwardly from the outer side of the dressing, attached to the dressing at an inner end, and having a free outer end. Generally, it will be preferable that sutures are provided in pairs, such that each pair of sutures can be fixed around a drain in the manner described above and in the manner described below in relation to the preferred embodiment of the invention. However, an odd number of sutures may be provided. In embodiments of the invention one, three, four or more sutures may be tied together when the dressing is in use to hold a drain in position.
In embodiments of the invention the dressing may further comprise: a third suture extending outwardly from the outer side of the dressing, attached to the dressing at an inner end, and having a free outer end; and a fourth suture extending outwardly from the outer side of the dressing, attached to the dressing at an inner end and having a free outer end.
In such embodiments two pairs of sutures are provided: a first pair of sutures consisting of the first suture and the second suture and a second pair of sutures consisting of the third suture and the fourth suture. The two pairs of sutures can each be used to fix the drain in the manner described above. The third and the fourth suture may be the same length as each other and/or may be the same lengths as the first and second sutures respectively.
As for the first and second sutures, it may be preferable that if a third and fourth suture are provided that their inner ends are located equidistant from the aperture to ensure that stress on each suture is substantially equal when they are used to tie a drain in position.
The present invention also provides a second invention being a surgical dressing having an inner side and an outer side and a central aperture through which a drain may be positioned, wherein the surgical dressing also has.
a first suture hole adjacent a first edge of the dressing for passing a suture through; and a second suture hole adjacent a second edge of the dressing for passing a suture through; wherein, the first edge of the dressing is on an opposing side of the dressing to the second edge That is the present invention also provides a surgical dressing for a drain that has a central aperture through which a drain may be positioned and suture holes adjacent opposing edges of the dressing. The suture holes may be used to suture the dressing to a patient's skin away from the drain and surgical wound. This can act to securely hold the dressing in position in addition to or as an alternative to any other way in which the dressing is held in position, for example an adhesive inner side of the dressing and/or additional adhesive dressings used to hold the dressing in position.
A dressing according to the second invention having suture holes adjacent opposing edges may comprise any other feature of the first invention as described above or below but it is to be understood that none of those features are essential. For example, a dressing according to the present invention may additionally comprise sutures having an inner end adjacent an outer side of the dressing and a free outer end but these are not essential.
Further suture holes may be provided in a dressing according to the present invention. Three, four or more suture holes may be provided to allow a dressing to be securely held in position. Advantageously each suture hole will be positioned adjacent an edge of the dressing. Preferably the suture holes will be substantially evenly spaced around a perimeter of the dressing.
The optional features described below may be present in either the first or the second invention and are not limited to either invention or a combination thereof A surgical dressing according to the present invention may be provided with an absorbent pad on the inner side of the dressing about the central aperture. The absorbent pad may be formed in any manner apparent to the person skilled in the art. Any absorbent pad used in surgical dressing according to the prior art may be utilised in the present invention. If a dressing according to an embodiment of the present invention comprises an absorbable pad, then the pad may be removable and replaceable such that the pad can be changed if soiled without the need to replace or displace the rest of the dressing including any sutures of the dressing.
The inner side of the dressing of the present invention may be self-adhesive. All or part of the inner side of the surgical dressing may be self-adhesive. If an absorbent pad is provided on the inner side of the dressing then the inner side of the dressing may be self-adhesive on the areas of the inner side of the dressing where the absorbent pad is not located. Any adhesive suitable for self-adhesive dressings may be used in the present invention. For example, the self-adhesive may comprise an anti-microbial silicon adhesive.
If part or all of the inner side of the dressing is self-adhesive a peelable protective layer may be provided over the self-adhesive parts of the inner side of the dressing to allow easy storage and transport of the dressing without it unintentionally adhering to any other surface. This may be done in any manner apparent to the person skilled in the art.
In order to allow information relevant to the patient and the dressing to be presented on the outer side the dressing may further comprise an information label on the outer side. The information label may comprise a surface on which information can be written, for example a surface on which permanent marker or other pen may be written. The information label may 113 comprise pre-written portions relevant to the dressing, for example indicating a place at which the time and date at which the dressing was applied may be written and/or the location of the drain, and/or the name of the patient. An information label may be provided in any manner apparent to the person skilled in the art and may be positioned in any suitable position on the outer side of the dressing.
In order to allow a surgical dressing according to present invention to be positioned about a drain in a straightforward manner, a slit may be formed through the dressing from an edge of the dressing to the central aperture. A slit allows a drain to be passed through the dressing, via the slit, to be positioned in the aperture, rather than the dressing needing to be passed over an end of the dressing. A slit may be straight. A slit may be formed from a mid-point of an edge of the dressing.
A dressing according to the present invention may be any suitable shape. For example, the dressing may be rectangular, square, oval, circular, rounded or any other suitable shape.
In order to allow a drain site to be inspected wherein a central portion of a dressing about the aperture may be transparent. This may be achieved by forming the central portion of the dressing from any suitable transparent material.
Further features of the present invention will be apparent from the preferred embodiment shown in the Figures and described below. Unless otherwise defined in the claims or by context any feature of the embodiment shown in the drawings may be included in another embodiment of the invention independently from the embodiment shown in the drawings.
Drawings Figure 1 is a plan image of a surgical dressing according to the present invention in use; and Figure 3 is an isometric image of the surgical dressing of Figure 1 in use.
A surgical dressing 1 according to the present Invention is shown in use in the Figures. The outer side 2 of the surgical dressing 1 is shown in the Figure 1 whilst an isometric view of the dressing is shown in Figure 2.
The surgical dressing 1 is substantially rectangular and has a central aperture 3, through which a drain 4 is positioned when the dressing is in use. A through-thickness slit 5 is formed through the dressing 1 from the aperture 3 to a first edge 6 of the dressing. A central part 7 of the dressing 1 about the aperture 3 is formed to have a transparent outer layer and has an absorbent inner layer, for positioning against a patient's skin about the drain 4. An outer part 8 of the dressing 1 has an opaque outer layer and has a self-adhesive inner layer. The self-adhesive inner layer is formed with antimicrobial silicon adhesive for good adhesion to a user's skin.
A first suture hole 9 is formed adjacent a second edge 10 of the dressing 1 and is centrally positioned with respect to the second edge. A second suture hole 11 is formed adjacent a third edge 22 of the dressing and is centrally positioned with respect to the third edge. The first suture hole 9 is on an opposing side of the dressing 1 from the second suture hole 11.
A first suture 12, a second suture 13, a third suture 14, and a fourth suture 15 are each attached at an inner end to the dressing 1. The first suture 12 and a second suture 13 are attached to the dressing at a side of the dressing between the central aperture 3 and the second edge 10. The third suture 14 and the fourth suture 15 are attached to the dressing at a side of the dressing between the central aperture 3 and the third edge 12.
The dressing 1 is shown in position on a patient about a drain 4. First, the drain 4 is positioned through the central aperture 3 of the dressing 1 by passing the drain 4 through the slit 5. The dressing is then adhered to the patent's skin about the drain 4. The dressing 1 is sutured to the patient through the first suture hole 9 and the second suture hole 11. This is best seen in Figure 2. In this manner the dressing 1 is held secure to the patient.
The drain 4 is then sutured in position using the first suture 12, the second suture 13, the third suture 14, and the fourth suture 15. The first suture 12 and the second suture 13 are wrapped around the drain 4 and then an outer end of the first suture 12 is tied to an outer end of the second suture 13 to secure the drain 4. The third suture 14 and the fourth suture 15 are also wrapped around the drain 4 and an outer end of the third suture 14 is tied to an outer end of the fourth suture 15 to secure the drain. In this manner the drain 4 is securely held in position relative to the dressing. The sutures 12, 13, 14, and 15 are can be tied together in any manner apparent to the person skilled in the art.
The exact positioning of the sutures 12, 13, 14, and 15 and the position in which the drain is tied may vary from the embodiment shown in the Figures. Different positioning may be preferred for different drains and for different patients and dressings 1 according to the present invention may have the sutures 12, 13, 14, 15 in any appropriate position. The position of the drain 4 when sutured in position may be done according to the preference of the medical practitioner applying the dressing 1. Similarly the positioning of the suture holes 9, 11 may vary and/or additional suture holes may be provided in alternative embodiments of the invention.
A portion of the outer part 8 of the outer side 2 of the dressing 1 is formed as an information label 16 consisting of a pre-written part, indicating where the date of the dressing and the location of the dressing should be written, and a writeable part, wherein the date of the dressing and the location of the dressing can be written by a medical practitioner.

Claims (15)

  1. Claims 1 A surgical dressing having an inner side and an outer side and a central aperture through which a drain may be positioned, wherein the surgical dressing further comprises: a first suture extending outwardly from the outer side of the dressing, attached to the dressing at an inner end, and haying a free outer end; and a second suture extending outwardly from the outer side of the dressing, attached to the dressing at an inner end and haying a free outer end.
  2. 2. A surgical dressing according to claim 1, wherein the first suture is the same length as the second suture
  3. 3. A surgical dressing according to claim 1 or claim 2, wherein the inner end of the first suture and the inner end of the second suture are equidistant from the central aperture.
  4. 4 A surgical dressing according to claim 1, further comprising: a third suture extending outwardly from the outer side of the dressing, attached to the dressing at an inner end, and haying a free outer end; and a fourth suture extending outwardly from the outer side of the dressing, attached to the dressing at an inner end and haying a free outer end.
  5. A surgical dressing according to claim 4, wherein the third suture is the same length as the fourth suture
  6. 6. A surgical dressing according to claim 4 or claim 5, wherein the inner end of the third suture and the inner end of the fourth suture are equidistant from the central aperture.
  7. 7 A surgical dressing according to any preceding claim wherein the surgical dressing also has: a first aperture adjacent a first edge of the dressing for passing a suture through; and a second aperture adjacent a second edge of the dressing for passing a suture through; wherein, the first edge of the dressing is on an opposing side of the dressing to the second edge.
  8. 8 A surgical dressing having an inner side and an outer side and a central aperture through which a drain may be positioned, wherein the surgical dressing also has: a first aperture adjacent a first edge of the dressing for passing a suture through; and a second aperture adjacent a second edge of the dressing for passing a suture through; wherein, the first edge of the dressing is on an opposing side of the dressing to the second edge.
  9. 9. A surgical dressing according to any preceding claim, wherein an absorbent pad is provided on the inner side of the dressing about the central aperture.
  10. 10. A surgical dressing according to any preceding claim, wherein the inner side of the dressing is self-adhesive.
  11. 11. A surgical dressing according to any preceding claim, wherein the self-adhesive comprises an anti-microbial silicon adhesive.
  12. 12. A surgical dressing according to claim 10 or claim 11, wherein a peelable protective layer is provided over the self-adhesive inner side of the dressing.
  13. 13. A surgical dressing according to any preceding claim further comprising an information label on the outer side.
  14. 14 A surgical dressing according to any preceding claim, wherein a slit is formed through the dressing from an edge of the dressing to the central aperture
  15. 15. A surgical dressing according to any preceding claim, wherein a central portion of the dressing about the aperture is transparent.
