Abstract
A USB device is shaped and sized like a credit card or similar card. The card can carry personal details and a photograph of the holder as well as a logo. The device can store medical forms and medical information such as relate to diabetes, blood groups, allergies, donor preferences, immunisations, doctors details etc.. The card is intended for personal use and would not necessarily share data with wider data storage systems. The data on the card could be password protected.

Description:
1. My Medical eCard is a USB device shaped and sized like a credit card and fits nicely into a wallet.
2. My Medical eCard has an inscription of My Medical eCard' and logo on the card and the facility for the photo of the user, name and a reference identifier to be printed on to the front of the medical card.
3. My Medical eCard has a disclaimer printed on the back of the medical card with respect to falsifying medical information and discretionary use by Medical Professionals.
4. My Medical eCard comprises of simple, fillable, save-able, medical forms designed for the use of the lay person.
5. The medical forms are made up of basic demographic information, a photo of the user, notes on allergies, blood groups, donor information) pertinent medical information like diabetic on insulin) In Case of Emergency (ICE) contact details) doctor's details, healthcare providers) health insurance providers, medical history (current, past) family, social), medications, immunisations, infectious diseases, medical devices) a log of hospitalisations, doctors', dental, optical, therapy visits, investigations and results, a blood pressure and blood sugar log), billing and medico-legal directives.
6. Documents, such as tests and results, x-rays, hospital letters, blood and organ donation documents, Medico-Legal Directives) Wills, Revocable Living and other Trusts) Power of Attorney, Advanced Directives, etc., can be scanned and attached to My Medical eCard.
7. Additional modules for ante-natal, renal dialysis log and other specialist healthcare, are available as separate items.
8. My Medical eCard is a stand-alone device.
9. My Medical eCard is strictly for personal use.
10. The information on My Medical eCard is not stored or transmitted to any servers or to the internet.
11. The user of My Medical eCard is fully and completely in control of how the card and the information therein is used.
12. Use of the contents of the My Medical eCard is at the discretion of the Medical Professional.
13. Parts of the contents of My Medical eCard can be password protected by choice by the user.

- Claims: 1. My Medical eCard is a USB device, shaped like a credit card with My Medical eCard' and logo printed on the card and the facility for the photo of the user, name and a reference identifier to be printed on to the front of the medical card.
- 2. My Medical eCard comprises of purpose designed blank medical forms, suitable for a lay person to complete with their basic personal medical information, maintain and update as at if and when required.
- 3. My Medical eCard is strictly for personal use. Use of the information contained within My Medical eCard is at the discretion of the Medical Professional.
- 4. My Medical eCard is a standalone device. The information supplied within the medical forms on the device is retained on the device.
- 5. The information on My Medical eCard is not stored or transmitted to any servers or connected to the internet.
- 6. The user may choose to save or transmit the contents of the My Medical eCard, but only at the user's prerogative.
- 7. other medical information, such as tests and results, x-rays, billing information, medico-legal directives etc., can be scanned, attached and stored on My Medical eCard.
