Abstract

A video introducer has electrically conducting wires 13 embedded within the wall thickness of a hollow 20 plastics rod 10 or bourgie between an image sensor camera and illumination unit (11, Fig 1) mounted at one end and a low profile electrical connector (14) mounted at the opposite end. The rod 10 and wires 13 are cut away to form two conical or frusto-conical surfaces 33 and 34 on which enlarged elliptical contact surfaces at the ends of each wire are located. The image sensor 11 and connector 14 have matching conical or frusto-conical recesses 22 fitted on opposite ends of the rod 10 and are connected by conductive adhesive between electrical contact pads 23 in its recess 22 and on the ends of the wires 13.

MEDICO-SURGICAL ASSEMBLIES AND METHODS
This invention relates to medico-surgical assemblies and methods of making such assemblies.
The invention is more particularly concerned with medico-surgical assemblies for use' in placing apparatus within a patient such as inserting an endotracheal tube into the trachea. Traditional introducers or bougies take the form of a simple rod that can be bent to an approximate desired shape and can flex to accommodate the shape of the anatomy during insertion. The introducer may be made with an angled, Coude tip to facilitate introduction. The introducer can be inserted more easily than the tube itself because it has a smaller diameter and can be bent and flex to the ideal shape for insertion. The small diameter also gives the clinician a better view of the trachea around the outside of the introducer. These may be used with or without the aid of a laryngoscope. When the bougie has been correctly inserted, a tube can be slid along the outside of the bougie to the correct location, after which the bougie is pulled out of the tube, which is left in position. Bougies are available from Smiths Medical. GB2312378 describes an introducer or bougie moulded of an aliphatic polyurethane material and also describes an earlier bougie made from a braided polyester filament repeatedly coated in layers of resin. The characteristic flexural and recovery properties of these bougies are highly valued by clinicians.
More recently it has been proposed to use fibre optics or a CCD camera with an introducer to provide the clinician with a view of the trachea as the introducer is inserted. W02007/089491 describes an arrangement with an introducer and a separate camera assembly clipped onto the outside of the introducer, which is removed before an endotracheal tube can be slid along the introducer. Alternative arrangements have a handle at the machine end of the introducer so that the apparatus has to be inserted with the tube already loaded on the introducer from its patient end. Such arrangements are more difficult to use because of the presence of the tube during insertion. W02010/136748 describes an introducer arrangement with a camera at one end of an introducer and connected via a cable to a display screen at the opposite end. The introducer is disconnected from the display when correctly positioned so that an endotracheal tube can be slid along the introducer into
position, following which the introducer is removed by pulling rearwardly from the endotracheal tube.
It is particularly important that these video introducers or bougies retain the flexural and recovery properties of conventional bougies, even after sterilisation and prolonged storage. If the diameter of the bore along the bougie has to be enlarged to receive a cable or other components this can adversely affect the handling properties of the bougie. Another problem is that it can be difficult to connect the cable at the rear end of the bougie with an electrical connector. If the cable length is extended beyond the bougie to allow access for connection to a connector there can be surplus cable but no space within the bougie to receive this.
It is an object of the present invention to provide alternative medico-surgical assemblies and methods of manufacture.
According to one aspect of the present invention there is provided a medico-surgical assembly including a rod, an imaging sensor at a patient end of the assembly, an electrical connector at a machine end of the assembly, electrical conductors extending along the length of the rod connected with an imaging sensor at the patient of the assembly and with the electrical connector at the machine end of the assembly, wherein the electrical conductors are embedded in the rod and are exposed at least at one end for connection with the imaging sensor or the electrical connector by removal of material of the rod.
The conductors may be exposed at the one end on a plane of the rod inclined at an angle away from the normal to the axis of the conductors. The inclined plane is preferably on a conical or frusto-conical surface. The conductors are preferably exposed by removing both material of the rod and the conductors. The conductors may be exposed by cutting away both material of the rod and the conductors. The conductors may be exposed at both ends of the rod by removal of material of the rod. The image sensor is preferably provided with a plurality of contacts on a surface of the sensor and arranged to make electrical contact with exposed ends of respective conductors. The contacts are preferably provided on a surface of the sensor, of conical or frusto-conical shape. The electrical connector at the machine end of

the assembly may be provided with a plurality of contacts on a surface of the connector and arranged to make electrical contact with exposed ends of respective conductors.
According to another aspect of the present invention there is provided a method of making a video introducer including the step of providing a rod of a bendable plastics material having electrical wires embedded in the material and extending along the length of the rod, removing at least some of the plastics material at one end of the rod to expose the wires, attaching to the one end either an imaging sensor or a connector having contacts arranged to engage the exposed wires.
The plastics material is preferably removed by cutting. The plastics material may be removed to form a conical or frusto-conical surface coaxial of the rod.
According to a further aspect of the present invention there is provided a video introducer made by a method according to the above other aspect of the present invention.
An endotracheal tube placement assembly and its method of manufacture, according to the present invention will now be described, by way of example, with reference to the accompanying drawing, in which:
Figure 1 shows an assembly of viewing means in the form of a video introducer and an endotracheal tube;

is a perspective view showing one end of the introducer rod at a preliminary step in the manufacturing process; and
Figure 3
is a perspective view showing the end of the introducer rod at a subsequent step in the manufacture process.
With reference first to Figure 1, the assembly consists of viewing means or a video introducer 1 in the form of a flexible, bendable plastics rod or bougie 10 . with a small camera and illumination unit 11 mounted at its forward patient end 12. The camera and illumination
unit 11 are connected by six wires 13 (although any other number of wires could be used) to a low profile electrical connector 14, which is small enough to allow an endotracheal tube 3 to be slid over the connector. The connector 14 mates removably with a connector 15 attached at one end of a second cable 16 extending to a video display monitor 18 on which is shown a display representation of the illuminated field of view of the camera unit 11.
As shown in Figure 2, the wires 13 are embedded in the plastics material of the rod 10 being evenly spaced from one another around a circle extending midway across the wall of the rod, between its outer surface and an inner bore or passage 20. The rod 10 with the embedded wires 13 may be made by extrusion, with the wires being drawn through the extrusion die with the fluid plastics material. After cutting the extruded stock rod material to the desired length, the opposite ends 31 and 32 of the rod are processed to remove plastic material in such as way as better to expose the ends of the wires 13. More particularly, the two ends are cut or milled, in the same manner as a pencil sharpener, to form conical or fhistoconical surfaces 33 and 34 concentric with the axis of the rod 10, as shown in Figure 3. The angle of these surfaces 33 and 34 is typically about 45° to the normal to the axis of the wires so that the cut end of each wire presents an elliptical shape instead of a circular shape and hence has an increased area.
The camera and illumination unit 11 is of generally cylindrical shape having the same external diameter as that of the rod 10. Around its rear end 21 the unit 11 is fomied with a frusto-conical recess 22 the surface of which supports six electrical contact pads 23, which are electrically connected within the unit 11 to the image sensor and illuminating LEDs. The angle of the recess 22 is chosen to be the same as that of the end surface 33.
The camera unit 11 is attached with the rod 10 by orienting it so that its contact pads 23 align with the exposed ends of the wires 13 on the surface 33. An electrically-conductive adhesive or the like is applied to the ends of the wires 13 or the contacts 23 so that, when the camera unit 11 is brought up to the end surface 33 of the rod 10 the contacts make electrical connection with respective ones of the wires and the adhesive firmly bonds the unit in place.

The electrical connector 14 at the opposite end of the rod 10 is similarly formed with a conical or frusto-conical recess having contact pads (not shown) disposed around it to align with the exposed ends of the wires 13 at the rear end of the rod. The connector 14 is similarly attached with the surface 34 by a conductive adhesive.
The method of the present invention enables electrical connection to be made to one or both ends of a video introducer without the need to connect to loose wires. The technique enables a small profile connector to be connected at the rear end of the rod so that a medical tube can be slid over the connector.
Although the invention can be used to make connection at both ends of the rod, it could be used just to make connection at one or other of the ends.

Claims (1)

  1. A medico-surgical assembly including a rod, an imaging sensor at a patient end of the assembly, an electrical connector at a machine end of the assembly, electrical conductors extending along the length of the rod connected with an imaging sensor at the patient of the assembly and with the electrical connector at the machine end of the assembly, wherein the electrical conductors are embedded in the rod and are exposed at least at one end for connection with the imaging sensor or the electrical connector by removal of material of the rod.
    An assembly according to Claim 1, wherein the conductors are exposed at the one end on a plane of the rod inclined at an angle away from the normal to the axis of the conductors.
    An assembly according to Claim 2, wherein the inclined plane is on a conical or frusto-conical surface.
    An assembly according to any one of the preceding claims, wherein the conductors are exposed by removing both material of the rod and the conductors.
    An assembly according to Claim 4, wherein the conductors are exposed by cutting away both material of the rod and the conductors.
    An assembly according to any one of the preceding claims, wherein the conductors are exposed at both ends of the rod by removal of material of the rod.
    An assembly according to any one of the preceding claims, wherein the image sensor is provided with a plurality of contacts on a surface of the sensor and arranged to make electrical contact with exposed ends of respective conductors.
    An assembly according to Claim 7, wherein the contacts are provided on a surface of the sensor of conical or frusto-conical shape.

    9. An assembly according to any one of the preceding claims, wherein the electrical connector at the machine end of the assembly is provided with a plurality of contacts on a surface of the connector and arranged to make electrical contact with exposed ends of respective conductors.
    10. An assembly substantially as hereinbefore described with reference to the accompanying drawing.
    11. A method of making a video introducer including the step of providing a rod of a bendable plastics material having electrical wires embedded in the material and extending along the length of the rod, removing at least some of the plastics material at one end of the rod to expose the wires, attaching to the one end either an imaging sensor or a connector having contacts arranged to engage the exposed wires.
    12. A method according to Claim 11, wherein the plastics material is removed by cutting.
    13. A method according to Claim 11 or 12, wherein the plastics material is removed to form a conical or frusto-conical surface coaxial of the rod.
    14. A method substantially as hereinbefore described with reference to the accompanying drawing.
    15. A video introducer made by a method according to any one of Claims 11 to 14.
    16. Any novel and inventive feature or combination of features as hereinbefore described.
