Abstract

Transparent plastic device 1 for insertion into the anus, includes full thickness longitudinal slots 2 positioned 3 cm from the base of the device, longitudinal bridges 3 and circular holes 4 at the base of the device. The holes 4 allow the device to be anchored to the skin around the anus with stitches. The device enables accurate positioning of sutures for treatment of haemorrhoids. The sutures raise the mucosal lining and create folds, interrupting the blood supply to the haemorrhoids whilst maintaining supply to the rectal mucosa.

Description

I
Rectal mucosa plication anal device This invention relates to a device to be used in rectal mucosa plication.
Management of 31 and 4th degree haemorrhoids can be achieved successfully by inducing interruption of the blood supply to the haemorrhoids and removing a strip of lower rectal! upper anal mucosa, whether by excision as it is the case when using circular stapler technique (PPH) or by lifting it up as I am proposing when using the new device.
The new device will achieve treatment of haemorrhoids by allowing the application of plicating sutures to the lower rectal/upper anal mucosa. The anal device is made of transparent plastic material, so that the surgeon can clearly identify the dentate line. A full thickness longitudinal cuts are made around the device (windows), with longitudinal bridges between them. About 4 windows in total, each is 4 cm in length, and the ends of the cuts are 3 cm from the base of the device. It is optional to fit a light source to the device. A non-absorbable suture material (2/0 proline stitch, double ended needle) is used in mucosal suturing. At the end of the procedure, the device will be removed leaving the stitches behind. This will cause the haemorrhoids to be lifted up as well as interrupting the blood supply to the haemorrhoids. The bridges between the windows will prevent complete interruption of the blood supply to the rectal mucosa.
I recommend to use this technique while the patient is in jack-knife position, which will facilitate the suturing technique. I believe this technique is effective in treating 3td and 4th degree haemorrhoids. It is very cheap technique comparing to the current techniques used, which is circular stapler technique (PPH), which costs 500.00 per patient. This technique can be used in day case setting with or without general anaesthesia.
The invention will now be described solely by way of example and with reference to the accompanying drawings in which: Figure 1 Shows the anal device from out side with the longitudinal cuts and the bridges in between.
Figure 2 Shows sagital section (the device from inside) and the suturing technique.
In figure 1, A transparent plastic anal device 1, Longitudinal full thickness cuts 2, Longitudinal bridges 3, circular holes at the base of the device 4.
In figure 2, A transparent plastic anal device (Sagital section) 1, Longitudinal full thickness cuts 2, Longitudinal bridges 3, suturing technique of the rectal mucosa 5, circular holes at the base of the device 4.
