Abstract
An output for an ultrasonic surgical tool vibratable in torsional mode comprises a generally cylindrical waveguide 1 provided with a radially extending protrusion 3 having two grooves 6 which converge to define at their intersection a retro-facing cutting edge 7. Each groove is part cylindrical, and the two grooves may be aligned to converge at an angle, with respect to a longitudinal axis of the waveguide, of 7 to 15 degrees. The grooves may be aligned to diverge in a proximal direction from the longitudinal axis of the waveguide by 11.5 to 20 degrees. The radial extremity 4 of the tool may be used to subject tissue to high amplitude compression waves for welding and/or ablation.

1 2425480 IMPROVED ULTRASONIC SURGiCAL TOOL The present invention relates
to an improved waveguide output for torsionally vibratable ultrasonic surgical tools, particularly but not exclusively, such as are known from British :.:. Patent number 2333709B. * S **** *.SS

Surgical tools having outputs purely for cutting purposes have been described in our British Patent No. 2365775B. However, these are not always easy to use and are not adapted to carry out welding and/or ablation procedures.
It has now been found that further improvements in the configurations of the output of such tools may be made.
It is an object of the present invention to provide an improved waveguide output for such a tool.
According to a first aspect of the present invention there is provided an output means for use with an ultrasonic surgical tool vibratable in torsional mode, said output comprising a generally cylindrical waveguide provided at a distal end with a radially extending protrusion having two groove means so converging in a proximal direction as to define at their intersection a retro- facing cutting edge.

Ideally, the two groove means may each be aligned to converge at an angle, with respect to a longitudinal axis of the waveguide, of less than 10 degrees.
In a preferred embodiment, the two groove means may each be aligned to converge at an angle, with respect to a longitudinal axis of the waveguide, in the region of 7 degrees.
In any event, both said groove means may diverge in a proximal direction from the longitudinal axis of the waveguide by an angle less than 20 degrees.
Preferably, both said groove means may diverge in a proximal direction from the longitudinal axis of the waveguide by an angle less than 15 degrees.
Optionally, both said groove means may diverge in a proximal direction from the longitudinal axis of the waveguide by an angle in the region of 11.5 degrees.
According to a second aspect of the present invention, there is provided a surgical tool comprising means to generate torsional mode ultrasonic vibrations, and a waveguide output as described above operatively connected thereto.

A torsional mode vibration provides concentric motion about the axis of the waveguide.
* Sections of the blade non-parallel with the motion provide vibrating faces capable of imparting energy directly into tissue brought to bear on such a face rather than cause friction as in conventional, parallel motion devices.
The greater the distance of a point from the longitudinal axis, the greater will be the amplitude of the vibration.
Referring now to the drawings, Figure 1 is a side elevation of a "hooktype" output at a distal end of a waveguide 1, torsionally vibratable about its longitudinal axis 2.
A single asymmetric extension 3 projects radially outwardly at a distal end from this generally cylindrical waveguide 1 and is able to generate high angular vibrational displacements at its radial extremity 4. The waveguide and the radial extension 3 are machined at their distal end to have a smoothly rounded nose 5 which continues around to the radial extremity 4 of the extension 3.
Two part cylindrical grooves 6 are milled or otherwise formed in the extension 3, and extend along lines which converge in a proximal direction, each at an angle in the region of 7 from the longitudinal axis 2 of the waveguide 1 so as to form a cutting edge 7 at their convergence.

Figure 2 shows an end elevation of the hook shaped output from the distal end, illustrating the angled grooves 6 extending beyond the curved surface 5 of the tool. These features are illustrated thore fully in Figure 3, the view from below.
Use of the output offers several critical features.
Firstly there is a high rotational displacement at the extremity 4 radially distant from the axis of vibration about axis 2, giving vibrations of increased amplitude. * 5
* Secondly, a sharp blade 7 is formed which can be used by retro-action of the waveguide for cutting tissue by pulling the hook backwardly having engaged the tissue in close proximity with the cutting edge 7.
The thickness of the web 8 formed between the hook extremity 4 and main axial body I of the waveguide represents a region of maximum displacement due to its cross section variation, having a minimum thickness along thc cutting edge 7, and greater thicknesses towards the generally cylindrical waveguide 1 and towards the radial extremity 4.

Claims (9)
1. An output means for use with an ultrasonic surgical tool vibratable in torsional mode, said output comprising a generally cylindrical waveguide provided at a distal end with a radially extending protrusion having two groove means so converging in a proximal direction as to define at their intersection a retro- facing cutting edge.
2. An output as claimed in claim 1, wherein each said groove means is part cylindrical.
3. An output as claimed in either claim I or claim 2, wherein two groove means may each be aligned to converge at an angle, with respect to a longitudinal axis of the waveguide, of less than 15 degrees.
4. An output as claimed in claim 3, wherein the two groove means one each aligned to converge at an angle, with respect to a longitudinal axis of the waveguide, of less than degrees.
5. An output as claimed in claim 4, wherein the two groove means are each aligned to converge at an angle, with respect to a longitudinal axis of the waveguide, in the region of 7 degrees.
6. An output as claimed in either claim I or claim 2, wherein said groove means diverge in a proximal direction from the longitudinal axis of the waveguide by an angle less than 20 degrees.
7. An output as claimed in claim 6, wherein said groove means diverge in a proximal direction from the longitudinal axis of the waveguide by an angle less than 15 degrees.
8. An output as claimed in claim 7, wherein said groove means diverge in a proximal direction from the longitudinal axis of the waveguide by an angle in the region of 11.5 degrees.
9. An output for a surgical tool comprising means to generate torsional mode ultrasonic vibrations, and a waveguide output and as claimed in any one of the preceding claims operatively connected thereto.
