The attachment 22 has an annular portion 24 mountable on a rim 16 of the acetabular cup 10, and at least one fixing member (not shown) for attaching the annular portion 24 to the patient's acetabulum. The attachment preferably has at least one projection on its underside for location in a performed aperture in the rim of an acetabular cup.
Description
IMPROVEMENTS IN OR RELATING TO THE ATTACHMENT
OF AN ACETABULAR CUP TO A DYSPLASTIC ACETABULUM
The present invention relates to a dysplasia attachment for preventing movement of an acetabular cup attachable to a patient's dysplastic acetabulum, a dysplastic cup, and a method of preventing movement of an acetabular cup attachable to a patient's dysplastic acetabulum.
Hip prostheses commonly consist of an acetabular cup and a femoral stem having a ball joint designed to be held inside a socket in the acetabular cup. Prior to the replacement operation, a surgeon will often not exactly know the specifics of the hip prosthesis a patient will require, and will thus rely on a stock of"off-the-shelf" components. Occasionally, a surgeon will find that a malformation of a patient's acetabulum exists. In this case, a dysplastic cup may be required in place of a standard acetabular cup. However, this often necessitates that a stock of all sizes of acetabular cup and all sizes of dysplastic cup be available.
A known dysplastic cup has projecting therefrom an apertured flange which is able to receive a fixing member that securely attaches the cup to a dysplastic acetabulum. Movement of the cup relative to the patient's acetabulum is thus prevented, but an undesirable moment is imparted to the cup due to the single point attachment.
The present invention seeks to provide a dysplasia attachment by which a known acetabular cup can be transformed into a dysplastic cup.
According to a first aspect of the invention there is provided a dysplasia attachment for preventing movement of an acetabular cup in a patient's dysplastic acetabulum, the attachment having an annular portion mountable on a rim of the acetabular cup, and at least one fixing member for attaching the annular portion to the patient's acetabulum.
Thus, a surgeon can interoperatively determine whether to use the mountable annular portion on the acetabular cup to create a dysplastic cup without having to rely on an availability of a dysplastic cup of correct size.
Preferably, the inner radius of the annular portion is greater than the inner radius of the rim of the acetabular cup. In this case, the articulating inner surface of the acetabular cup is not extended by the addition of the annular portion.
The dysplasia attachment may comprise at least one projection securely locatable in an associated aperture in the acetabular cup to prevent rotation of the cup relative to the attachment. Advantageously, the at least one projection may be formed on the underside of the annular portion of the attachment, and the associated aperture may be a preformed aperture in the rim of the acetabular cup primarily used to engage an introducing device for introducing and locating the acetabular cup into the patient's acetabulum.
Preferably, the annular portion has at least two flanges integrally projecting from the annular portion. In this case, the at least two flanges are preferably diametrically opposed. Each flange may have an opening through which a fixing member is receivable.
In use, a resultant moment imparted to the acetabular cup through the fixing members of the dysplasia attachment is negligible due to the advantageous diametric positioning of the flanges.
Preferably, each flange is angled relative to the annular portion. In this case, the flange angle is a declination of 20 or substantially 20 from the underside of the annular portion.
According to a second aspect of the invention, there is a dysplastic cup comprising an acetabular cup and a dysplasia attachment according to the first aspect of the invention, wherein the attachment is attachable to the acetabular cup and a patient's movement of the cup in use relative to the patient's acetabulum.
Preferably, the acetabular cup has a polyethylene liner.
According to a third aspect of the invention, there is a method of preventing movement of an acetabular cup attachable to a patient's dysplastic acetabulum, the method comprising the steps of:
i) inserting an acetabular cup in the patient's acetabulum,
ii) keying a mountable dysplasia attachment to the acetabular cup,
and
iii) fixing the keyed dysplasia attachment to the dysplastic
acetabulum, whereby movement of the cup relative to the
acetabulum is prevented.
The invention will now be more particularly described, by way of example, with reference to the accompanying drawings, in which:
Figure 1 is a plan view of a known acetabular cup,
Figure 2 is a side sectional view of the known acetabular cup,
Figure 3 is a plan view of an embodiment of a dysplasia attachment in
accordance with the first aspect of the present invention, and
Figure 4 is a side view of the dysplasia attachment in accordance with the first
aspect of the present invention.
Referring firstly to Figures 1 and 2, an acetabular cup 10, which is known
from GB-A-9802499.5, has a part-circular outer surface 12 of an appropriate diameter
to fit into the acetabulum of a patient. The acetabular cup 10 also has a part circular,
typically hemispherical or substantially hemispherical, articulating surface 14 having a diameter appropriate for receiving a femoral head (not shown). The diameter of the articulating surface 14 is within the range 40 to 60 millimetres. The acetabular cup 10 is formed of metal, preferably high carbon cobalt chrome-alloy, although it could be formed of a ceramic material, such as zirconia and alumina. The acetabular cup may also be capable of receiving a polyethylene liner, such as an Ultra High
Molecular Weight Polyethylene liner.
Extending between the outer surface 12 and the articulating surface 14 is an annular rim 16. The surface of the rim 16 is planar. The rim 16 is provided with three equi-angularly spaced introducer recesses 18. The recesses 18 open out only into the rim 16 and are spaced from the outer surface 12 and the inner surface 14.
Prongs of an introducing device, known from GB-A-9802499.5, are releasably locatable in the recesses 18. The acetabular cup 10 is thus introducible and locatable in a patient's acetabulum via the introducing device.
To improve the fixation of the acetabular cup 10 in the acetabulum of the patient, the outer surface of the cup may be textured and/or comprise a plurality of splines 20. These splines 20 are located at approximately 45 to the position of one of the recesses 18. To further aid fixation, the acetabular cup 10 is shaped such that the equatorial diameter de of the outer surface 12 is greater than the polar diameter dp of the outer surface 12. The acetabulum is then reamed to the same dimension as the polar diameter dp. The cup 10 can then be jammed into the acetabulum due to the larger equatorial diameter of the cup 10. This is particularly advantageous in the case of introducing metal cups for metal-on-metal articulation since the larger frictional torque can cause the cup to tear out of the acetabulum before it can become fixed by bone growth.
Metal cups 10 are either formed by forging or investment casting. The investment casting is subjected to hot isostatic pressing (HIP) and solution heat treatment processes. The inner surface 14 of the cup is then highly polished.
The outer surface 12 of the cup 10 may be coated with artificial bone substitute material, such as Hydroxyapatite, or the surface may have a metal porous coated or plasma coated surface to enhance fixation of the cup 10 within the acetabulum of the patient. The outer surface 12 may, for example, be coated with
Hydroxyapatite and a porous coating over a majority of the area of the outer surface 12.
Referring now to Figures 3 and 4, the dysplasia attachment 22 shown therein comprises an annular portion, which is typically a ring 24, flanges 26, a plurality of acetabular cup engaging projections 28, and fixing members (not shown).
The ring 24 of the dysplasia attachment 20 is formed in the same manner and of the same material as that described above with respect to the acetabular cup 10.
The radius of the inner circumferential edge of the ring 24 is slightly greater than the radius of the inner edge of the rim 16 of the cup 10. The outer edge of the ring 24 may have a radius equal to or greater than the radius of the outer edge of the rim 16.
In order that the dysplasia attachment 22 can be surely mounted on the cup 10, advantage is taken of the recesses 18 in the rim 16 of the cup 10. Each of the acetabular cup engaging projections 28 is locatable in a corresponding recess 18, so that, when in engagement, the cup 10 is unable to rotate relative to the ring 24.
The flanges 26 integrally project from the ring 24 and are typically diametrically opposed to limit any in use resultant moment imparted to the cup 10.
Apertures 30, through which the fixing members are receivable, are provided in the flanges 26. The flanges 26 are also angled, typically with a declination of 20 , relative to the underside surface of the ring 24. This allows a higher clamping force to be imparted by the ring 24 when acting on the acetabular cup 10.
During a hip prosthesis implantation, a surgeon may interoperatively decide that a standard acetabular cup is inappropriate and that a dysplastic cup is required.
According to the invention, the acetabular cup 10 may be mounted in the patient's dysplastic acetabulum as originally intended, and the dysplasia attachment 22 is then mounted on the rim 16 of the cup 10 with the projections 28 engaging the recesses 18. The fixing members, inserted through the apertures 30, fix to the patient's dysplastic acetabulum, whereby the acetabular cup 10 is securely held in place by the dysplasia attachment.
The acetabular cup 10 in combination with the dysplasia attachment 22 enable, interoperatively, a commonly available acetabular cup to be fixed to a patient's dysplastic acetabulum without necessitating a specialised dysplastic cup being available. The provision of diametrically opposed flanges and fixing members enables an increase in stability of fixture over previously known dysplastic cups.
The embodiment described above is given by way of example only and various modifications will be apparent to persons skilled in the art without departing from the scope of the invention.
Claims (11)
- CLAIMS 1. A dysplasia attachment for preventing movement of an acetabular cup in a patient's dysplastic acetabulum, the attachment having an annular portion mountable on a rim of the acetabular cup, and at least one fixing member for attaching the annular portion to the patient's acetabulum.
- 2. A dysplasia attachment according to claim 1, wherein the inner radius of the annular portion is greater than the inner radius of the rim of the acetabular cup.
- 3. A dysplasia attachment according to claim 1 or claim 2, wherein the attachment further comprises at least one projection securely locatable in an associated aperture in the acetabular cup to prevent rotation of the cup relative to the attachment.
- 4. A dysplasia attachment according to claim 3, wherein the at least one projection is formed on the underside of the annular portion of the attachment, and the associated aperture is a preformed aperture in the rim of the acetabular cup primarily used to engage an introducing device for introducing and locating the acetabular cup into the patient's acetabulum.
- 5. A dysplasia attachment according to any one of the preceding claims, wherein the annular portion has at least two flanges integrally projecting from the annular portion.
- 6. A dysplasia attachment according to claim 5, wherein the at least two flanges are diametrically opposed.
- 7. A dysplasia attachment according to claim 5 or claim 6, wherein each flange has an opening through which a fixing member is receivable.
- 8. A dysplasia attachment according to any one of claims 5 to 7, wherein each flange is angled relative to the annular portion.
- 9. A dysplasia attachment according to claim 8, wherein the flange angle is a declination of 20 or substantially 20 from the underside of the annular portion.
- 10. A dysplastic cup comprising an acetabular cup and a dysplasia attachment as claimed in any one of the preceding claims, wherein the attachment is attachable to the acetabular cup and a patient's dysplastic acetabulum to prevent movement of the cup in use relative to the patient's acetabulum.
- 11. A dysplasia attachment substantially as hereinbefore described with reference to the accompanying drawings.11. A dysplastic cup according to claim 10, wherein the acetabular cup has a polyethylene liner.12. A method of preventing movement of an acetabular cup attachable to a patient's dysplastic acetabulum, the method comprising the steps of: i) inserting an acetabular cup in the patient's acetabulum, ii) keying a mountable dysplasia attachment to the acetabular cup, and iii) fixing the keyed dysplasia attachment to the dysplastic acetabulum, whereby movement of the cup relative to the acetabulum is prevented.13. A dysplasia attachment substantially as hereinbefore described with reference to the accompanying drawings.Amendments to the claims have been filed as follows CLAIMS 1. A dysplasia attachment for preventing movement of an acetabular cup in a patient's dysplastic acetabulum, the attachment having an annular portion mountable on a rim of the acetabular cup, at least one fixing member for attaching the annular portion to the patient's acetabulum, and at least one projection securely locatable in an associated aperture in the acetabular cup to prevent rotation of the cup relative to the attachment.2. A dysplasia attachment according to claim 1, wherein the inner radius of the annular portion is greater than the inner radius of the rim of the acetabular cup.3. A dysplasia attachment according to claim 1 or claim 2, wherein the at least one projection is formed on the underside of the annular portion of the attachment, and the associated aperture is a preformed aperture in the rim of the acetabular cup primarily used to engage an introducing device for introducing and locating the acetabular cup into the patient's acetabulum.4. A dysplasia attachment according to any one of the preceding claims, wherein the annular portion has at least two flanges integrally projecting from the annular portion.5. A dysplasia attachment according to claim 4, wherein the at least two flanges are diametrically opposed.6. A dysplasia attachment according to claim 4 or claim 5, wherein each flange has an opening through which a fixing member is receivable.7. A dysplasia attachment according to any one of claims 4 to 6, wherein each flange is angled relative to the annular portion.8. A dysplasia attachment according to claim 7, wherein the flange angle is a declination of 20 or substantially 20 from the underside of the annular portion.9. A dysplastic cup comprising an acetabular cup and a dysplasia attachment as claimed in any one of the preceding claims, wherein the attachment is attachable to the acetabular cup and a patient's dysplastic acetabulum to prevent movement of the cup in use relative to the patient's acetabulum.10. A method of preventing movement of an acetabular cup attachable to a patient's dysplastic acetabulum, the method comprising the steps of: i) inserting an acetabular cup in the patient's acetabulum, ii) then keying a mountable dysplasia attachment to the acetabular cup, and iii) then fixing the keyed dysplasia attachment to the dysplastic acetabulum, whereby movement of the cup relative to the acetabulum is prevented.
