Abstract

A prosthesis for implantation in a bone particularly the femoral component of an artificial hip, comprises a proximal part 1 shaped to fit within the medullary canal of the metaphysis of the bone and to project from the end of the bone and a distal part 2 shaped to fit within the medullary canal of the diaphysis of the bone. The proximal and distal parts are separately formed and arranged to fit together selectively in either one of first and second different manners. The first manner permits movement between the two parts in a direction which, in use, corresponds to the length of the bone and the second manner locks the two parts together to form a rigid construction. The locking means is a collar 12 which is clamped between an abutment 11 (on a stem 8 of the one part) and the other part.

Description

A PROSTHESIS This invention relates to a prosthesis for implantation in a bone, and more particularly but not exclusively, to the femoral component of an artificial hip joint.
GB-A-2222776 describes a prosthesis for implantation in a bone, and more particularly the femoral component of an artificial hip joint, which comprises a proximal part shaped to fit within the medullary canal of the metaphysis of the bone and to project from the end of the bone and a distal part shaped to fit within the medullary canal of the diaphysis of the bone, the proximal and distal parts being separately formed and arranged to fit together in a manner permitting movement between the two parts in a direction which, in use, corresponds to the length of the bone. This enables the two separate parts to be independently selected to provide an optimum fit within the bone and, following implantation, the two parts are able to move relative to each other so that the proximal part can settle within the bone without transferring load to the distal part. As described in this prior British specification, this movement is desirable since it is known that (particularly with un-cemented prostheses) there is some movement between the prosthesis and the bone, especially during the first six months following implantation. By allowing the two parts to move relative to one another over a few millimetres the prosthesis is able to"bed down"or settle into the metaphysis and constantly re-adjust its press fit with the metaphysis to ensure that proximal loading is ideally achieved. However, there are times, when it is desirable to modify a prosthesis so that such movement is not permitted. One such occasion of this is when the device is used in revision hip surgery, where the patient has fractured the bony femur below the tip of a previously implanted prosthesis.
According to the present invention, there is provided a prosthesis for implantation in a bone, comprising a proximal part shaped to fit within the medullary canal of the metaphysis of the bone and to project from the end of the bone and a distal part shaped to fit within the medullary canal of the diaphysis of the bone, the proximal and distal parts being separately formed and arranged to fit together selectively in either one of first and second different manners, the first manner permitting movement between the two parts in a direction which, in use, corresponds to the length of the bone and the second manner locking the two parts together to form a rigid construction.
Preferred and optional features of the invention are set forth in claims 2 to 8 inclusive.
The invention will now be more particularly described with reference to the accompanying drawing which is an exploded side view of one embodiment of a prosthesis according to the present invention.
Referring now to the drawing, the prosthesis shown therein comprises a proximal part 1 which is shaped to fit within the medullary canal of the metaphysis and a distal part 2 which is shaped to fit within the medullary canal of the diaphysis. The proximal part 1 may be shaped to be a wedge fit within the metaphysis or a slightly smaller size may be selected to enable it to be cemented in place. The distal part 2 is selected to fit within the diaphysis but not to be jammed or wedged therein so that it acts as a keel or"anti-toggle"device.
The proximal and distal parts 1 and 2 are arranged to fit together so parts of different sizes can be interchangeably fitted to one another to obtain the optimum fit within the bone. In particular, it is possible to fit a small proximal part 1 to a large distal part 2 or a large proximal part 1 to a small distal part 2.
The proximal part 1 has a wedge portion 3 which has a tapered form to facilitate its fixation within the metaphysis by means of a wedge fit and a trunnion neck 4 to which a head, such as the ball of a ball and socket joint, may be attached, for example by a screw thread. The effective length of the neck 4 may be adjusted in a known manner by providing a range of heads having different length holes within which the neck 4 is fitted. A through-hole 7 is provided in the wedge portion 3 to enable it to be fitted to the distal part 2.
The distal part 2 has an elongate form, and comprises a cylindrical stem 8 for fitting in the hole 7 of a proximal part 1 and a tapered end portion 9 which fits within the medullary canal of the diaphysis. As shown, the distal part 1 also has a cylindrical central portion 10 between the stem 8 and end portion 9 with a shoulder 11 between the shaft 8 and the central portion 10.
The through-holes 7 and stems 8 of different sized parts 1 and 2 are all of the same size so that different size proximal and distal parts can be interchangeably fitted to one another.
The prosthesis also comprises a collar 12 which can be clamped between the shoulder 11 and the wedge portion 3 of the proximal part 1 to selectively lock the proximal and distal parts 1 and 2 against movement in a direction which, in use, corresponds to the length of the bone. The stem 8 has a threaded portion 13 which is threadably engaged by a nut 14 to clamp the collar 12 between the shoulder 11 and the wedge portion 3 of the proximal part 1. A key 15 is also provided. The key 15 is insertable in a key way 16 between the proximal and distal parts 1 and 2 to selectively lock the two parts against relative angular movement.
It is thus possible to fit the proximal and distal parts together selectively in either one of two different manners. The first manner permits movement between the two parts 1 and 2 in a direction which, in use, corresponds to the length of the bone and in this case the collar 12 and the key 15 are omitted. The second manner locks the two parts 1 and 2 together to form a rigid construction by placing the collar over the stem 8 between the shoulder 11 and the wedge shaped portion of the proximal part 1, placing the key 15 in the key way 16 and threadably engaging the threaded portion 13 with the nut 14.
The invention is applicable to prostheses made of a wide variety of materials although the majority are these days made of titanium or an alloy thereof. The various surface treatments used in this field may also be applied to the prosthesis and in view of the two part construction it is a simple matter to form the proximal and distal parts 1 and 2 with different surface treatments should this be desired.

Claims (10)

  1. CLAIMS 1. A prosthesis for implantation in a bone, comprising a proximal part shaped to fit within the medullary canal of the metaphysis of the bone and to project from the end of the bone and a distal part shaped to fit within the medullary canal of the diaphysis of the bone, the proximal and distal parts being separately formed and arranged to fit together selectively in either one of first and second different manners, the first manner permitting movement between the two parts in a direction which, in use, corresponds to the length of the bone and the second manner locking the two parts together to form a rigid construction.
  2. 2. A prosthesis as claimed in claim 1, wherein the proximal and distal parts are arranged to fit together by means of a stem on one part which fits within a hole in the other part.
  3. 3. A prosthesis as claimed in claim 2, further comprising a collar which is clamped between an abutment on the stem of the one part and the other part to selectively lock the two parts against movement in a direction which, in use, corresponds to the length of the bone.
  4. 4. A prosthesis as claimed in claim 3, the stem has a threaded portion which is threadably engageable by a threaded element to clamp the collar between the abutment on the stem of the one part and the other part.
  5. 5. A prosthesis as claimed in any one of claims 2 to 4, further comprising a key insertable in a key way between the two parts to selectively lock the two parts against relative angular movement.
  6. 6. A prosthesis as claimed in any one of the preceding claims, wherein the proximal part includes a neck portion and means for attaching a head, e. g. the ball of a ball and socket joint, thereto.
  7. 7. A prosthesis as claimed in any one of the preceding claims, wherein up to 5 mm of movement is permitted between the proximal and distal parts when the first and second parts are fitted together in said first manner.
  8. 8. A prosthesis as claimed in any one of the preceding claims, which forms the femoral component of an artificial hip joint.
  9. 9. A plurality of proximal and distal parts of different sizes which may be interchangeably fitted together to provide a prosthesis as claimed in any preceding claim.
  10. 10. A prosthesis substantially as hereinbefore described with reference to the accompanying drawing.
