Abstract
A bipolar screw microprosthesis consisting of an outer screw which is secured into the Femur head and reaches the hip joint space. Inside the outer screw at the Femur head end is 50mm of screw thread 2 to position a inner screw carrying at the joint end a spherical metallic head 4 covered by a ceramic cup 5. By turning the inner screw with a screwdriver on ridge 8, articular space can be maintained or created in the hip joint.
Description
2346327 BIPOLAR SCREW MICROPROSTHESIS OF THE HIP This invention relates to
the use of a bipolar screw microprosthesis as a alternative to standard hip replacement technology and methods.
Hip replacements have remained unchanged in the last 30 years, namely the replacement of the Femur head with alloy irnp!lants which allow 'Ball & Socket' motion of the hip joint. The surgery involved is invasive, 'requires a great deal of physical work and so requires a great deal of training which mak s it a costly procedure to perform.
There are several hip implants in the market vhich require the removal of a large amount of bone mass to be replaced by a stiff metallic' implant which produces complications which are very well documented in medical fltrature. The removal of a large amount of bone mass to replace very thin articular damape (a couple of millimetres) is like removing a tooth because of partial decay as I)as been practised in the past.
Hip replacements are set to be one of the most common surgical procedures as life expectancy in the developed and developing orld increases so will demand for a new and cost effective method and technology.
According the existing methods and technology it is proposed to carry an endoscopy through a tunnel (18-20 mm) drilled through thl neck of the Femur, in the same way as in pin & plate methods. The tunnel will reach t6. the tip of the joint creating access to the joint space. By endoscopy we can remove forelgn bodies and re-shape the damaged articular surfaces, removing ostheophites with rotor or a special articulated and rotating burr. To implant cultivated cartilage or bone mcirphonic proteins (BMP) by using reabsorbaple staples again using endoscopy. o insert the bi - polar screw microprosthesis thought the tunnel drilled in the. Femur head to the joint space to preserve or create joint space and protect the implanted cartilage or BMP.
Once the articular surface has been re-surfaceq or repaired, the screw microprosthesis can be removed entirely to be replaced by a thrpaded Dowel type of surgibone with a cultivated cartilage at its tip.
A specific embodiment of the invention will nowrbe described by way of example with reference to the accompanying drawings in whiqh:- Figure 1 shows the construction of the inner screw Figure 2 shows the construction of the outer screw.
Figure 3 shows the position of the inner screw in relation to the outer screw once in position.
Figure 4 shows an external view of the screw miproprosthesis. This diagram shows the position of the ceramic cup and end views of the device.
Figure 5 shows a view of the outer screw in position in the Femur with the ceramic cup in position.
Figure 6 shows a cross section of the Femur and a cross section of the screw microprosthesis in position NOTE DIAGRAMS ARE NOT TO SCALE.
Referring to the drawings the bipolar screw microprosthesis will be manufactured of titanium or any other type of suitable metal, alloy or synthetic material.
It consists of an outer screw as shown in Figure 2 of 18 to 20mm in diameter, which grasps the neck of the Femur and at least 1/2 of the Femur head as shown in figure 5. The outer screw is inserted by positioning a key, which will fit into the ridges seen in figure 4 (12). In its interior at the Femur head end there are internal screw ridges as in Figure 3 (11). Into these internal ridges is screwed:
A inner screw 1 Omm shorter than the outer screw shown in figure 1. At the Femur head end is a spherical metallic head (13) covered by a ceramic cup 14 of the same circumference as the tunnel provided by the outer screw as figure 3 (15). Following from the sphere end is a series of ridges 15 which fit into the inner screw ridges of the outer screw in Figure3(1 1). The whole inner screw is inserted by positioning a screw driver into the ridge shown in figure 4(16). By turning this inner screw you can maintain or create articular space in the joint.
Claims (4)
1 Through the use of drilling (as in 'Pin Plate' method) and the use of endoscoPY, a bi-polar screw microprosthesis of th hip can be inserted. This comprises of an outer screw which is secured firmly to I the Femur and a inner screw containing a guide screw to position a spherical metallic head with a ceramic cup at the hip joint. This will maintain or create artiqular space in the joint while at the same time allowing for articular movement, The movement and positioning of both screws are controlled by a key which will fit into ridges for the outer screw and a screwdriver which will fit into the ridge Jor the inner screw.
2. A bipolar screw microprosthesis as in Iaim 1 wherein the ceramic head of the inner screw could by made of other suitable material.
3. A bipolar screw microprosthesis as in claim 1 wherein the length of the internal guide screw ridges could vary in length'
4. A bipolar screw microprosthesis as in claim 1wherein the length of the outer screw ridged length could vary.
