Abstract

The prosthesis comprises a stem portion 10 having a substantially straight linear axis 11 and an integral wedge-shaped portion 13 extending medially from the stem portion 10 at or adjacent to the wider end of the stem portion. The wedge-shaped portion is surmounted by a ball head. The wedge-shaped portion has a plurality of ridges 17 or convolutions extending therearound. Each ridge 17 or convolution lies in a plane substantially perpendicular to the load bearing axis 18 through the head and wedge-shaped portion 13 when the prosthesis is in use in the body. The wedge-shaped portion 13 also has a plurality of elongate grooves extending transversely to the ridges 17 or convolutions.

Description

r. - 2341554 -1A PROSTHESIS This invention relates to a femoral prosthesis
for use in a hip replacement operation.
It is known from GB-A-2203943 to provide a femoral prosthesis comprising a tapered stem portion with a substantially straight linear axis and an integral wedgeshaped portion extending medially from the stem portion at or adjacent the wider end of the stem portion. The wedge-shaped portion is surmounted by a ball head. The wedge-shaped portion and an adjacent part of the stem portion have a plurality of ridges or convolutions extending therearound. Each ridge or convolution lies in a plane substantially perpendicular to the load bearing axis through the head and wedgeshaped portion when the prosthesis is in use in the body. Such a prosthesis has to be cemented in a cavity in a femur.
The present invention seeks to provide an improved femoral prosthesis which enables the prosthesis to be used without cement.
According to the present invention there is provided a femoral prosthesis comprising a tapered stem portion having a substantially straight linear axis and an integral wedge-shaped portion extending medially from the stem portion at or adjacent to the wider end of the stem portion, the wedge-shaped portion being surmounted by a ball head, the wedge-shaped portion having a plurality of ridges or convolutions extending therearound with each ridge or convolution lying in a plane substantially 0 -2perpendicular to the load bearing axis through the head and wedgeshaped portion when the prosthesis is in use in the body wherein the wedge-shaped portion has a Z:
plurality of elongate grooves extending transversely to the ridges or convolutions.
Preferred and/or optional features of the invention are set forth in claims 2 to 7, inclusive.
The increase in surface area provided by the grooves reduces the stresses imparted by torsional and rotational forces acting on the implant, during activities such as rising from a chair and stair climbing. Additionally, the grooves tend to concentrate the stresses imparted to the implant on the central metaphyseal section of the host bone, thus reducing the occurrence of distal stress transfer by maximising loading in the proximal femur.
The invention will now be more particularly described, by way of example, with reference to the accompanying drawings, in which:
Figure 1 is a side view of one embodiment of a femoral prosthesis according to the invention, Figure 2 is a section through the line A-A of Figure 1, Figure 3 is a section through the line D-D of Figure 1, Figure 4 is a section through the line E-E of Figure 1, Figure 5 is section through the line F-F of Figure 1, Figure 6 is a rear view of the prosthesis shown in Figure 1, and Figure 7 is an enlarged fragmentary side view of the prosthesis shown in Figure 1.
Referring to the drawings, the prosthesis shown therein is formed of an acceptable material for introduction into the human body, such as titanium alloy or a cobalt-chromium alloy. It may be all of one material or may have a ball head portion of a different material, such as a ceramic.
The prosthesis comprises a stem portion 10 which has a substantially linear axis 11. The stem portion 10 tapers towards its lower end 12, the extent of the taper depending on individual need. Spaced from the lower end 12, a somewhat flattened wedge portion 13 extends medially and proximally (i.e., in use, towards the middle of and up the body) from the stem portion 10 and is surmounted by a separate ball head (not shown). The wedge portion 13 includes a projecting end 15 which is provided for lateral support and which is provided with two apertures 16, which can receive a tool, such as that described in GB-A-2307861, for inserting the implant in the bone.
The provision of a separable ball head is used to adjust the neck length for individual needs. Thus ball heads incorporating various neck lengths can be fitted to standard stems with shortened wedge portions.
As best seen in Figure 7, the wedge portion 13 is provided with a series of ridges 17 extending therearound, each in a plane substantially perpendicular to a load bearing axis 18 of the prosthesis which passes through the ball head and wedge portion 13. Each ridge 17 defines a surface 19 facing the lower end 12 of the stem portion 10.
The wedge-shaped portion 13 also has three elongate grooves 20, 21 and 22 extending transversely to the ridges 17. The groove 20 is parallel or substantially parallel to the longitudinal extent of the stem portion 10. The groove 21 is parallel to the medial aspect of the prosthesis and is substantially perpendicular to the ridges 17. The groove 22 is between the grooves 20 and 21. The longitudinal axes of the grooves 20, 21 and 22 coincide at a point 23 sited within the confines of the stem portion 10 and spaced from the ridges 17. Typically, the point 23 is approximately 40 mm from the top of the shoulder of the prosthesis, at the mid-line of the vertical axis.
A proximal portion of the implant, typically up to a third or two fifths of the prosthesis, is coated with plasma sprayed titanium and hydroxyapatite to encourage apposition of host bone to the surface of the prosthesis and improve fixation.
To insert the prosthesis, the bone is rasped out and the prosthesis is inserted as a press fit, by using the tool previously mentioned, for example.
The increase in surface area provided by the grooves 21,22,23 reduces the stresses imparted by torsional and rotational forces acting on the implant.
Additionally, the grooves 21,22,23 tend to concentrate the stresses imparted _to the implant on the central metaphyseal section of the host bone, thus reducing the occurrence of distal stress transfer by maximising loading in the proximal femur.

Claims (7)

  1. -6CLAIMS
    A femoral prosthesis comprising a tapered stem portion having a substantially straight linear axis and an integral wedge-shaped portion extending 0 medially from the stem portion at or adjacent to the wider end of the stem portion, the wedge-shaped portion being surmounted by a ball head, the wedge- shaped portion having a plurality of ridges or convolutions extending therearound, each ridge or convolution lying in a plane substantially perpendicular to the load bearing axis through the head and wedge-shaped portion when the prosthesis is in use in the body, wherein the wedge-shaped portion has a plurality of elongate grooves extending transversely to the ridges or convolutions.
  2. 2. A femoral prosthesis as claimed in claim 1, wherein a first elongate groove is parallel or substantially parallel to the longitudinal extent of the stem portion and a second elongate groove is perpendicular to or substantially perpendicular to the ridges or convolutions.
  3. 3. A femoral prosthesis as claimed in claim 2, further comprising a third elongate groove between the first and second grooves.
  4. 4. A femoral prosthesis as claimed in any one of the preceding claims, wherein the longitudinal axes of said grooves intersect at a point spaced below the ridges or the convolutions.

  5. 5. A femoral prosthesis as claimed in any one of the preceding claims, wherein each ridge defines a substantially flat surface facing the narrower end of the stem portion.
  6. 6. A femoral prosthesis as claimed in any one of the preceding claims, wherein each ridge, commencing with the ridge nearest the ball head, lies in a plane closer to the normal of the axis of said stem portion than the corresponding plane of the preceding ridge.
  7. 7. A femoral prosthesis substantially as hereinbefore described with reference to the accompanying drawings.
    7. A femoral prosthesis as claimed in any one of the preceding claims, wherein a proximal portion of the prosthesis is coated with a dual coating of a metallic porous coating and hydroxyapatite to encourage apposition of host bone to the surface of the prosthesis and to improve fixation of said prosthesis to said host bone.
    8. A femoral prosthesis substantially as hereinbefore described with reference to the accompanying drawings.

A femoral prosthesis comprising a tapered stem portion having a substantially straight linear axis and an integral wedge-shaped portion extending medially from the stem portion at or adjacent to the wider end of the stem portion, the wedge-shaped portion being surmounted by a ball head, the wedge- shaped portion having a plurality of ridges or convolutions extending therearound, each ridge or convolution lying in a plane substantially perpendicular to the load bearing axis through the head and wedge-shaped portion when the prosthesis is in use in the body, wherein the wedge-shaped portion has a plurality of elongate grooves extending transversely to the ridges or convolutions and wherein the longitudinal axes of said grooves intersect at a point spaced below the ridges or the convolutions.
    2. A femoral prosthesis as claimed in claim 1, wherein a first elongate groove is parallel or substantially parallel to the longitudinal extent of the stem portion and a second elongate groove is perpendicular to or substantially perpendicular to the ridges or convolutions.
    3. A femoral prosthesis as claimed in claim 2, further comprising a third elongate groove between the first and second grooves.
    4. A femoral prosthesis as claimed in any one of the preceding claims, wherein each ridge defines a substantially flat surface facing the narrower end of the stem portion.

A femoral prosthesis as claimed in any one of the preceding claims, wherein a proximal portion of the prosthesis is coated with a dual coating of a metallic porous coating and hydroxyapatite to encourage apposition of host bone to the surface of the prosthesis and to improve fixation of said prosthesis to said host bone.
