Detailed Description of the Preferred Embodiment
An embodiment of the invention is described by referring to the drawings. Fig. 1 is a face side view showing a prepaid card of damage insurance, and Fig. 2 is a back side view of the same prepaid card.
This prepaid card 1 is a card used as both insurance application form and insurance policy of damage insurance, and the basic information of damage insurance is recorded and displayed. An applicant is supposed to fill in the columns of name, address, date of birth, telephone number and others.
This prepaid card is prepared in every type of insurance corresponding to different objects of insurance, including physical loss during shopping, school accidents, sports accidents, accidents during volunteer activities, flight accidents, journey accidents, etc.
An example of a card covering a sports injury is explained below.
This prepaid card 1 is a rectangular form made of hard 3 plastic material, and is easy to carry. The face side la has columns for insurance type 2, insured amount 3, premium payment 4, insurance period 5, number display 6, and insured person's information including name, address, date of birth and telephone number 7, and the back side 1b has columns for magnetic recording 8, insurance outline 9, barcode 10, and insurance company 12.
