Abstract

A form arrangement 10 having a bag 11, in which a sample of blood, urine or other substance may be placed, a form 12, on which information relating to the sample may be written, and sealing means 13 for sealing the bag, has a detachable label 14 with an identification marking on it, assembled with the bag, form and sealing means. One or more labels, with common identification in the form of numerals or bar code, may be adhesively secured to a backing sheet 17. The backing sheet may be attached to the form by mutually abutting edges or adhesively to the face of the form into which the labels have been cut or onto which the labels have been tabbed. The bag may be attached directly to the form or to the backing sheet. The sealing means of the disclosed embodiments is a strip of adhesive with a detachable cover strip over it so that when the cover strip is removed the form or bag may be folded to close the opening on the bag. The arrangement reduces the risk of the form and sample becoming separated, or of mis-matching samples and information. <IMAGE>

Description

This invention relates to form arrangements, and in particular to form arrangements which comprise an assembly of a bag for containing a sample and a form on which information may be written relating to the sample in the bag.
Such assemblies are known, particularly for use in connection with the testing of blood, urine or other samples in hospitals, clinics, laboratories or the like. In such cases the sample, generally within a bottle or like container, is placed in the bag which is then sealed.
Information relating to the source of the sample, e.g. the patients name, address and/or other details, is then written on the form and the form assembly is sent to the testing facility, often together with several other form assemblies containing other samples from other patients. This type of form arrangement reduces the risk of a form becoming separated from its respective sample, and the possibility of miss-matching of samples and information.
In order to perform the testing of the sample it is necessary to remove the sample from the bag. To identify the sample after it has become separate from its form, a label on which some identification marking is provided is affixed to the sample bottle, and the same identification marking is recorded on the original form. This may be by applying a second label to the form, with the second label having the same identification marking as the first label. The form may be of the multi-copy, non-carbon duplicating type so that a copy may be retained by the testing facility, a copy returned to the originating person or organisation, and possibly copies sent to other relevant parties. Furthermore it may be necessary to split the sample into several parts so that differing tests can be done on the various parts.In either case an equivalent number of extra labels with the same identification marking on them will be required. Such a procedure can lead to error with possibly serious consequences if test results and the initial information are miss-matched. There is also the inconvenience for the testing facility of writing the identification number or other data on each of several labels, or obtaining supplies of appropriately pre-printed labels.
It is an object of the present invention to provide a form arrangement by means of which the abovementioned disadvantages are avoided or minimised.
The invention provides a form arrangement comprising a bag having an opening whereby a sample may be received therein, sealing means adapted to seal the bag opening, a form on which information relating to the sample may be written, and at least one detachable label having an identification marking thereon, wherein the bag, the sealing means, the form and the detachable label are secured to each other in a single assembly.
The label may be adhesive and may be mounted on a backing sheet. A plurality of labels may be provided, in which case a common identification marking may be provided on each label. The identification marking may be in the form of numerals or may be in the form of a bar code. The labels may be adhesively mounted on a common backing sheet. The backing sheet may be a silicone paper sheet, and may be secured to the form so as to detachably secure the label thereto.
The bag may be secured to the form at a first location, and the label or labels may be secured to the form at a second location spaced from the first location. The form arrangement may be a twin webbed product in which the backing sheet, on which at least one label is mounted, is secured by one edge to an edge of the form. Alternatively, the backing sheet may be secured to the surface of the form by means of a reusable adhesive and the label or labels provided by cutting the form sheet in the region covered by the backing sheet, whereby the cut label portion or portions may then be removed from the backing sheet and the remainder of the form. As a further alternative, the backing sheet may be adhesively bonded onto the surface of the form whereby the label or labels mounted on the backing sheet are tabbed on to the form.In this case the label or labels may be of the print through type whereby the identification marking may be pressure printed thereon so as to provide that a corresponding identification marking is formed on the backing sheet beneath the label or labels. As yet a further alternative, the label or labels may be mounted on a backing sheet which is secured to the form and the bag may be secured to the backing sheet.
The sealing means may be provided by a strip of adhesive having a cover strip applied thereto. The strip of adhesive may be provided on an extension portion of the bag adjacent the opening, on an end part of the form, an end part of the backing sheet, or on the former and also on either of the other locations, so that that part of the form arrangement on which the strip of adhesive is provided may be folded over, once the cover strip is removed therefrom, so as to overlie and adhere to the bag adjacent the opening therein to seal such opening. Where appropriate a fold line on the form may be provided. In the former case the bag may be secured to the form by adhesive bonding thereto of the extension portion. In the second case the bag may be secured to the form so as to overlie that part of the form other than the end part with the opening of the bag adjacent the end part.Alternatively, the sealing means may be provided by an adhesive label which may be removed from the backing sheet and secured to the bag and the form so as to overlie and seal the opening in the bag.
The form may be of paper, and may be of the multi-leaf, carbonless reproducing type. The bag may be of a substantially transparent plastics material and may be of polythene.
Embodiments of the invention will now be described with reference to the accompanying drawings in which: Fig 1. is a front view of a first embodiment, Fig 2. is a rear view of the embodiment of Fig 1, Fig 3. is a front view of a second embodiment, Fig 4. is a front view of a third embodiment, and Fig 5. is a front view of a fourth embodiment.
Referring now to Figs 1 and 2, there is illustrated a form arrangement 10 comprising a bag 11, a form 12, a sealing means 13, and a label attachment 14. The bag 11 should be leak proof, and is preferably of a substantially transparent plastics material such as polythene. The form 12 is preferably of stiff paper or thin card and may be either single ply or multi-ply. In that case the form 12 may be of the carbonless reproducing type so that the information written on the top sheet will be reproduced on the other sheet or sheets. The sealing means 1 3 comprises a strip of adhesive 15 and a cover strip 1 6 (shown partly removed in Fig 1).
The label attachment 14 comprises a backing sheet 1 7 and any desired number of adhesive labels 18 mounted on the front surface thereof, six such labels 1 8 being shown in this case.
The backing sheet 1 7 is a silicone paper sheet, which in this case is translucent and the labels 18 are visible from the rear in Fig 2., although this is not necessary. The form assembly 10 is a twin webbed product in that the backing sheet 1 7 is bonded to the form 12 in the region 19 of their mutually overlapping edges. The bag 11 has an opening 20 and an extension portion 21 which is adhesively bonded to the form 12 at a first location spaced from the second location at which the label attachment 14 is attached to the form 12. The adhesive strip 15 of the sealing means 1 3 extends over the extension portion 21 of the bag 11 and an end portion 23 of the form 12. The form 12 is partly scored adjacent the opening 20 to provide a fold line 22 located between the bag opening 20 and the adhesive strip 1 5.
With the form assembly 10, a specimen (not shown), usually in a bottle, may be placed through the opening 20 into the bag 1 1 The cover strip 1 6 is then removed and the form 12 folded along the fold line 22 together with the adhesive covered part of the bag extension 21 so that the adhesive strip 1 5 overlies the opening 20 and seals the bag 11. Since the bag 11 is then leak proof, any liquid sample will be contained therein in the event that the bottle containing the sample is broken, thereby minimising the risk of infection of anyone handling the sample containing form assembly 1 0.
Information relating to the sample may be written on the form 12, which is pre-printed for that purpose as is visible at 24 through the bag 11 as shown in Fig 1. Such information may be the name and address of the person supplying the sample, the type of sample, the doctor's or requesting organisation's name, the test or tests required or the like. At the same time that the printing of the form 12 is performed, the printing of an identification marking on each label 1 8 may be performed. Such identification marking may be in the form of numerals 25 and/or a bar code 26. The same identification marking may be printed on the form 12 or one of the labels 1 8 may be removed from the backing sheet 1 7 and affixed to the form 1 2 in an appropriately designated "box".A line of perforations 27 (Fig 2) is provided in the form 12 so that that part of the form 12 on which the information is written may be detached from the bag 11 and the end part 23 of the form 12 to which the bag 11 is secured. Other labels 1 8 may be affixed to the bag 11 or the end part 23 of the form 1 2, and any other copies of the form 12 if the latter is of the multi-ply type. The sample/bag/form part may be hung up if desired using the hole 29 provided in the end part 23 of the form 12. If the sample is subsequently split into two or more part samples for differing tests, labels 1 8 may be attached to each part sample with again minimum risk of mis-matching sample part and information relating to the sample concerned.When the required test has or tests have been carried out the test results may be recorded on the rear face of the form 12, which has been pre-printed at 28 for that purpose. The form 12 may then be returned to the person or organisation requesting the test or tests.
Referring now to Fig 3, there is shown an alternative embodiment of form arrangement 30, which is used in the same manner as the previously described embodiment and like parts are identified by the same reference numerals. In this case however, the form 12 is larger than that of the previous embodiment and incorporates the labels 18. These are formed by making cuts 31 in the extended form 12 in the region 32 remote from the bag opening 20 and end part 23. The backing sheet 1 7 (not shown in Fig 3) overlies the rear face of the region 32 and is bonded thereto by means of a re-usable adhesive. The cuts 31 may extend to the edges of the form 12 as shown at 33, in which case the cuts 33 may be intermittent, i.e. be lines of perforations. With this embodiment the labels 18 are removed from both the backing sheet 1 7 and the form 12 simultaneously.
In an alternative embodiment of form arrangement 40 as shown in Fig 4, the backing sheet 1 7 is bonded to the front face of the form 1 2. In this case the labels 18 are tabbed onto the form 1 2 by being adhesively mounted on the backing sheet 1 7. If the labels 18 are of the carbonless reproducing type, the identification markings 25 and/or 26 will be formed on the backing sheet 1 7 also, provided that the means of printing used involves sufficient pressure.
In yet a further embodiment of form arrangement 50, as shown in Fig 5, the backing sheet 1 7 having labels 1 8 adhesively mounted thereon forms a twin webbed product by being bonded to the form 1 2 in the region 1 9 of their overlapping edges. However this embodiment differs from that shown in Figs 1 and 2 in that in this case the bag 11 is adhesively bonded by an extension portion 21 to the backing sheet 1 7 at the opposite edge thereof from that to which the form 1 2 is attached. The opening 20 lies adjacent the overlapping region of the backing sheet 1 7 and the extension portion 21, and the adhesive strip 15 extends over the extension portion 21 and part of the backing sheet on which labels 18 are not mounted.
In any of the above described embodiments the sealing means 13 may be provided by an appropriately sized label 18 which can be removed from the backing sheet 1 7 and affixed over the opening 20 to seal it.
In all cases, the inclusion of adhesive labels in the form arrangement avoids the need for the testing facility staff to obtain labels in the correct numbers and with the appropriate identification markings on them when required. Furthermore, since the identification markings 25 and/or 26 may be printed at the same time as the form 12, the risk of mismatching the samples and the relevant forms 12 is greafly reduced, and the risk associated with mis-copying the same identification number on each label by hand is avoided.

Claims (7)

1. A form arrangement comprising a bag having an opening whereby a sample may be received therein, sealing means adapted to seal the bag opening, a form on which information relating to the sample may be written, and at least one detachable label having an identification marking thereon, wherein the bag, the sealing means, the form and the detachable label are secured to each other in a single assembly.
2. A form arrangement according to claim 1, wherein the label is adhesive.
3. A form arrangement according to claim 2, wherein the label is mounted on a backing sheet.
4. A form arrangement according to any one of claims 1 to 3, wherein a plurality of labels is provided, and wherein a common identification marking is provided on each label.
5. A form arrangement according to claim 4, wherein the identification marking is in the form of numerals.
6. A form arrangement according to claim 4, wherein the identification marking is in the form of a bar code.
7. A form arrangement according to claim 4 when dependent on claim 3, wherein the labels are adhesively mounted on a common backing sheet.
8, A form arrangement according to claim 7, wherein the backing sheet is a silicone paper sheet.
9, A form arrangement according to claim 3, wherein the backing sheet is secured to the form so as to detachably secure the label thereto.
10, A form arrangement according to any one of claims 1 to 9, wherein the bag is secured to the form at a first location, and the or each label is secured to the form at a second location spaced from the first location.
11, A form arrangement according to claim 7 or any claim dependent thereon, wherein the form arrangement is a twin webbed product.
12, A form arrangement according to claim 11, wherein the backing sheet, on which at least one label is mounted, is secured by one edge to an edge of the form.
13, A form arrangement according to claim 11, wherein the backing sheet is secured to the surface of the form by means of a re-usable adhesive.
14, A form arrangement according to claim 13, wherein the or each label is provided by cutting the form sheet in the region covered by the backing sheet, whereby the cut label portion or portions may then be removed from the backing sheet and the remainder of the form.
15, A form arrangement according to claim 11, wherein the backing sheet is adhesively bonded onto the surface of the form whereby the label or labels mounted on the backing sheet are tabbed on to the form.
16, A form arrangement according to claim 15, wherein the or each label is of the print through type whereby the identification marking may be pressure printed thereon so as to provide that a corresponding identification marking is formed on the backing sheet beneath the label or labels.
17, A form arrangement according to claim 11, wherein the or each label is mounted on a backing sheet which is secured to the form and the bag is secured to the backing sheet.
18, A form arrangement according to any one of claims 1 to 1 7, wherein the sealing means is provided by a strip of adhesive having a cover strip applied thereto.
19, A form arrangement according to claim 1 8, wherein the strip of adhesive is provided on an extension portion of the bag adjacent the opening.
20, A form arrangement according to claim 18, wherein the strip of adhesive is provided on an end part of the form, 21, A form arrangement according to claim 18, wherein the strip of adhesive is provided on an end part of the backing sheet.
22, A form arrangement according to claim 19, wherein the strip of adhesive is provided also on either of an end part of the form or an end part of the backing sheet, so that that part of the form arrangement on which the strip of adhesive is provided may be folded over, once the cover strip is removed therefrom, so as to overlie and adhere to the bag adjacent the opening therein to seal such opening.
23, A form arrangement according to claim 20 or claim 22, wherein a fold line is provided on the form.
24, A form arrangement according to claim 19, wherein the bag is secured to the form by the adhesive bonding thereto of the extension portion.
25, A form arrangement according to claim 20, wherein the bag is secured to the form so as to overlie that part of the form other than the end part, with the opening of the bag adjacent the end part.
26, A form arrangement according to any one of claims 1 to 1 7, wherein the sealing means is provided by an adhesive label which may be removed from the backing sheet and secured to the bag and the form so as to overlie and seal the opening in the bag.
27, A form arrangement according to any one of claims 1 to 26, wherein the form is of paper.
