Abstract

A shoulder joint prosthesis comprises a part-spherical member adapted to be secured to the scapula, a cup-shaped member adapted to be secured to the humerus (said members being made of metal), and a high- density polyethylene meniscus, spacer or plastic joint tensor adapted to be positioned between said members. The members are preferably bonded to the respective bones e.g. and by acrylic bone cement, and the arrangement is such that they are capable of captive relative articulation. <IMAGE>

Description

SPECIFICATION The Oni-MacKenny total shoulder replacement This invention relates to the artificial replacement of painful shoulder joints that have sustained irreparable severe mechanical damage.
Shoulder joint replacement has been undertaken by a number of workers since the pioneering work of Pean (1894) and Neer (1950s) but while all claim pain relief, most accept that functional performance is a hit or miss affair. Our experience with a number of the available prostheses have been most disappointing indeed. We have now designed a new gleno-humeral prosthesis to satisfy the anatomical, biomechanical and physiological requirements unique to the shoulder joint because of its prehensile function.
The Oni-MacKenny prosthesis is a reverse balland-socket type utilizing the stability of a stack-ofbowls. It consists of a stainless steel or alloy ball or "nipple" for the glenoid (scapula) captively articulating with a stainless steel or alloy cup or "runner" for the humerus; between these two components is interposed a "meniscus" or spacer made of high density polyethylene. The metal components are bonded to the respective bones with acrylic bone cement.
Fig. 1 shows the Oni-MacKenny Total Shoulder prosthesis in place with each component clearly marked; Fig. 2 shows the special glenoid reamer; and Fig. 3 shows the special humeral reamer.
The arthroplasty operation is performed through the standard anterior approach splitting the deltoid muscle and dividing the subscapularis. After dislocating the joint, a portion of the head of the humerus is excised and the rest prepared with the special reamer (Fig. 3); next the glenoid is prepared using its own reamer (Fig. 2) and the appropriate sized meniscal spacer inserted to place the surrounding soft-tissue in tension. Post-operatively, the shoulder is immobilised and active mobilisation is commensed three weeks later.

Claims (6)

1. The Oni-MacKenny prosthesis is functionally a gliding joint with some element of rolling in the relative motion similar to that described for the normal shoulder ny Wallace.
2. The prosthesis dislays functional congruence as in the normal shoulder and the forces reach a maximum at 900 as described for the normal shoulder by Inman et al.
3. The anatomical centre of articulation is preserved as well as the instant centre of movement to within 6 mm. of the centre of the humeral head (Popper & Walker).
4. The greater tuberosity does not impinge upon the acromion during abduction as in most of the prostheses currently available.
5. The fulcrum provided is dynamic and assumes the role of the rotator cuff to allow normal deltoid action when the cuff is absent.
6. A shoulder joint prosthesis according to Claim 1 and substantially as hereinbefore described with reference to, and as illustrated in, Figure 1 of the accompanying diagrammatic drawings.
6. The relationships between the prosthetic components are such that a functional nonmechanical check to subluxation and dislocation is provided.
7. The physiological soft tissue stretchproprioceptor mechanism which normally maintains proper muscle action (Davis) is preserved.
8. Internal rotation vital to the performance of everyday tasks (Lucas, McCornaill) is preserved.
9. The components are made of materials of suitable biological and wear characteristics and the fixation points are not subjected to undue stresses and strains to lead to loosening; and
10. The relevant procedures are technically simple and salvage procedures, if ever necessary, are easy to undertake.
Amendments to the claims have been filed, and have the following effect:~ (a) Claims 1 to 10 above have been deleted.
(b) New claims have been filed as follows:~ CLAIMS
1. A shoulder joint prosthesis comprising a partspherical member adapted to be secured to the scapula, a cup-shaped member adapted to be secured to the humerus, and a polyethylene meniscus, spacer or plastic joint tensor adapted to be positioned between, and for movement independently of, said members, the arrangement being such that said members are capable of captive relative articulation.
2. A prosthesis according to Claim 1, wherein the meniscus is a high density polyethylene meniscus.
3. A prosthesis according to Claim 1 or Claim 2, wherein the surface of the meniscus which is adapted to be positioned adjacent to the partspherical member is concave and the surface of the meniscus which is adapted to be positioned adjacent to the cup-shaped member is convex.
4. A prosthesis according to any one of the preceding claims, wherein said members are made of metal.
5. A prosthesis according to any one of the preceding claims, wherein said members are adapted to be bonded respectively to the scapula and the humerus with acrylic bone cement.
