Abstract

1461348 Surgical ligation apparatus VSES NI I ISPYTATE INST MEDITSIN TEKHNIKI 26 April 1974 [30 Oct 1973] 18495/74 Heading A5R [Also in Division B4] A surgical apparatus for ligaturing tubular organs in deep cavities comprises three elongate members extending parallel with each other and held together at their proximal ends for insertion of the distal ends thereof into a deep cavity, a magazine 4 for U-shaped staples carried on the distal end of a first one 3 of the members, a needle-shaped die 2 against which a staple is to be clinched carried on the distal end of a second one 1 of the members, the third member 5 serving as a pusher for ejecting staples from the magazine with their ends directed towards the die, the needle-shaped die having a curved hook portion for seizing a tubular organ to be ligatured, a spring- loaded mechanism for engaging and immobilizing a portion of a tubular organ seized by the die as the first and second members are relatively moved, the spring-loaded mechanism being disposed on the distal end of the first member, a movement limiter 6 for the third member 5, and means for guiding the shanks of a staple along the needle-shaped die to the curved portion thereof against which the shanks of the staple are to be clinched. The spring-loaded mechanism may comprise a fork 7 which is biased with its ends 11 away from member 1 by spring 9 to allow seizure of a tubular organ to be ligatured by the curved portion of the die 2. Member 3 is then pushed towards die 2 whereupon projections 10 on member 1 cooperate with ends 12 of fork 7 to press the fork against the organ in a position for ligaturing. A pusher lock 37 is then rotated about pivot 38 and the pusher 5 is pressed by means of bracket 34 to push a staple from the magazine down to the needle die 2 where it is clinched about the vein. The shanks of the staple are guided by grooves in the members 1 and 3, one shank in groove 14 in member 1, the other in groove 18 in member 3, these grooves being continued by grooves 16, 17 in the needle-die 2. The device may be used inside an esophagoscope in order to ligature varicose veins in the esophagus. The interior of the esophagoscope is provided with a slot which is used to remove the vein from the die 2 after it has been ligatured.
