Abstract
1,130,656. Thermostat and ligating device. AMP Inc. 24 Oct., 1966 [8 Nov., 1965], No. 47568/66. Heading A5R. The invention relates to a haemostat which is arranged to be loaded with a surgical ligating device in which a ligature extends through openings in a locking member to form a running noose which can be closed and locked about a severed blood vessel by pulling one end of the ligature, for example a ligating device described and claimed in Specification 1086979. The haemostat comprises a pair of arms connected by a pivot, the arms having gripping jaws which can be closed about the blood vessel by relative movement of the arms about the pivot, and a pin removably carried by the haemostat one of the jaws having a passageway through which the ligature of the ligating device can be passed to extend between the locking member of the ligating device and the pin to which one end of the ligature can be secured, the pin projecting from the haemostat to form a post for anchoring to the haemostat the noose of the ligature. As illustrated in Figure 1, a haemostat 10 made for example of stainless steel or a synthetic plastics material, for example glass-filled polystyrene, comprises a pair of arms which are pivotable about a hollow pivot pin. Arms 12 and 14 have respective gripping jaws 16 and 18 at one end, the jaws having respective serrated gripping surfaces 20 and 22, and digit-receiving handles 24 and 26 at the other end. The arms have respective interengaging serrated locking projections 28 and 30 intermediate their ends thereby to lock the jaws in their closed position. As illustrated in Figure 3, one jaw is formed with a through passageway 44 opening at one end into the free end surface of the jaw and at the other end into a lateral surface of the jaw; positioned within the passageway is a hollow metallic insert 46 terminating in a cutting edge at the end of the passageway remote from the tip of the jaw. Removably carried by the hollow pivot pin is a further hollow pin 34 which is formed with oppositely disposed axial slots 40 and with an enlarged head 38 having a slot 42 through which can be passed end 51 of ligature 50 which may be of catgut or cotton, the end being enlarged, for example knotted, to prevent withdrawal of the ligature from the slot. Enlarged, for example knotted, end 5 of the ligature is secured to a locking member in the form of a disc 52 which is arranged to abut one of the jaws. The disc has three holes 1, 2 and 3 through which the 'ligature is passed to form a noose 4, which embraces both arms of the haemostat and extends about the further hollow pin to be tensioned between the pin and the disc, and a loop 6 which presses the noose against the disc after the disc has been positioned against the blood vessel 54, the further hollow pin has been removed from the hollow pivot pin and moved away from the haemostat to tighten the noose, and the excess length of ligature has been severed by the cutting edge. Figure 6 (not shown) illustrates a modified haemostat which has a shorter passageway through the jaw so that the cutting edge is nearer the tip of the jaw and which is in combination with a locking member in the form of a disc having four holes and carrying a ligature of nylon.
