FIXATION DES PROTHESES MAMMAIRES PAR DES LANGUETTES LINTRODUCTION La présente invention est relative à la pose de prothèses mammaires, quelles qu'en soit la forme (anatomiques, rondes ou autres), et quelles qu'en soit la matière (silicone, 10 sérum physiologique, polyuréthane ou autres), utilisées pour des raisons purement esthétique ou dans le cadre de chirurgie reconstructrice. 2.PROBLEMATIQUE 15 En pratique, nous pouvons constater deux sortes de problèmes dans la pose de prothèses mammaires aboutissant à un résultat inesthétique, pouvant obliger le chirurgien à réopérer la patiente au bloc opératoire sous anesthésie générale : -Les prothèses mammaires dites anatomiques, (comme pour toutes prothèses qui 20 pourraient répondre à une forme spécifiquement adaptée au sein), seraient largement utilisées en chirurgie esthétique en raison de leur forme fine vers le haut et large vers le bas, se rapprochant ainsi davantage au visuel d'une poitrine naturelle. Cependant, beaucoup des chirurgiens hésiteraient à les utiliser par crainte que les prothèses ne tournent après leurs poses, déformant ainsi le sein. -Un autre problème aurait été mis en évidence dans la pose des prothèses mammaires : qu'elles soient anatomiques, rondes ou autres, les prothèses auraient tendance à remonter et à se positionner hors de la loge prévue initialement par les chirurgiens lors de l'intervention, créant de la même façon, une déformation du sein. 3/SOLUTION 10 Le but de mon invention serait de créer une ou plusieurs languettes qui permettraient au chirurgien de fixer la prothèse mammaire lors de l'intervention et éviter ces complications. Les languettes (rectangulaires, carrées, rondes ou autres) pourraient être de la même composition que la matière utilisée pour créer l'enveloppe de la prothèse ( la capsule) 15 (silicone, polyuréthane ou autres), ou d'une matière différente de l'enveloppe tant que celle-ci est inoffensive pour le corps, en prolongation de la prothèse. En pratique, idéalement, trois languettes pourraient s'avéraient utiles (même si on peut en créer plus, ou moins), positionnées sur la prothèse de la façon suivante (cf figure 1,2) : une languette sur la face postéro-inférieure 180°( 2-4) , et 2 languettes latérales aux angles 20 90° et 2700(1-3) , de manière à se trouver accessibles par toutes les voies d'abord envisagées par le chirurgien. Elles dépasseraient de 2 à 5 centimètres (ou plus, ou moins) du bord libre de la prothèse, ce qui permettrait de les fixer avec des points de suture au niveau thoracique, musculaire ou glandulaire. 25 Les languettes pourraient donc être percées pour faciliter le passage du fil de fixation ou pré-percées ou non percées mais avec une finesse permettant la perforation de la languette.The present invention relates to the laying of breast prostheses, whatever their shape (anatomical, round or other), and whatever the material (silicone, physiological saline, polyurethane or other), used for purely aesthetic reasons or in the context of reconstructive surgery. 2.PROBLEMATIQUE In practice, we can see two kinds of problems in the installation of breast prostheses resulting in an unsightly result, which may require the surgeon to reoperate the patient in the operating room under general anesthesia: -The anatomical breast prostheses, (as for all prostheses that could respond to a form specifically adapted to breast), would be widely used in cosmetic surgery because of their fine shape upward and broad downward, thus getting closer to the visual of a natural breast. However, many surgeons would hesitate to use them for fear that the prostheses will turn after their poses, thus deforming the breast. -Another problem would have been highlighted in the placement of breast prostheses: whether anatomical, round or otherwise, prostheses would tend to rise and position themselves outside the box initially provided by the surgeons during the intervention , creating in the same way, a deformation of the breast. 3 / SOLUTION 10 The purpose of my invention would be to create one or more tabs that would allow the surgeon to fix the breast prosthesis during surgery and avoid these complications. The tabs (rectangular, square, round or other) could be of the same composition as the material used to create the envelope of the prosthesis (the capsule) 15 (silicone, polyurethane or other), or a material different from the envelope as long as it is harmless for the body, in prolongation of the prosthesis. In practice, ideally, three tabs could prove useful (even if we can create more, or less), positioned on the prosthesis as follows (see Figure 1, 2): a tab on the posterior-inferior surface 180 ° (2-4), and 2 side tongues at the angles 90 ° and 2700 (1-3), so as to be accessible by all the approaches proposed by the surgeon. They would protrude 2 to 5 centimeters (or more, or less) from the free edge of the prosthesis, allowing them to be fixed with sutures at the thoracic, muscular or glandular level. The tongues could thus be drilled to facilitate the passage of the fastening wire or pre-drilled or not pierced but with a fineness allowing the perforation of the tongue.
Donc mon invention se résume à une prothèse mammaire comprenant , une enveloppe de matière souple, très mince et très résistante, formée d'une face avant et d'une face arrière thermo-soudées le long de leurs bords, remplie par une masse de gel bio-médical pour avoir la résistance, l'ensemble étant moulé et figé élastiquement à la forme voulue, cette prothèse est caractérisée en ce que les languettes soit de forme rondes, rectangulaires, carrées, triangulaires ou autres, et les languettes peuvent être trouées ou non, et que les languettes dépassent de la prothèse au minimum de 1cm, et qu'elles soient composées de la même matière que celle de la capsule de la prothèse ou d'une autre matière bio compatible. Les languettes peuvent être placées sur n'importe quelle zone pour les prothèses mammaires rondes et de préférence sur les zones latérales et basses des prothèses anatomiques ou asymètriques. Ces languettes peuvent être fabriquées soit avec le même moulage que celui de la prothèse ( en mono-bloc ), ou avec un moulage séparé pour être ensuite collées à la prothèse par une colle biologique. 4/TECHNIQUES CHIRURGICALES Deux techniques chirurgicales seraient alors proposées selon les voies d'abord envisagées: a/ Dans le cas des voies aréolaires ou sous-mammaires: Pour ces deux voies d'abord, seule la languette inférieure serait utile (2,4), les 2 autres (1,3) pouvant être coupées avec des ciseaux ou un bistouri avant la mise en place de la prothèse. Un point de suture pourra alors être réalisé : -pour une pose en sous-musculaire : entre la languette et le périoste des côtes ou entre la languette et le muscle grand pectoral, avec un fil non résorbable en mono-filament. -pour une pose sous-glandulaire: entre la languette et la glande ou entre la languette et le muscle grand pectoral avec un fil non résorbable en mono-filament. b/Dans le cas des voies axillaires Par cette voie, seule la languette latérale (languette latérale droite pour la prothèse positionnée à droite et languette latérale gauche pour la prothèse positionnée à gauche) serait utile, les 2 autres pouvant être coupées avec des ciseaux ou un bistouri, avant la mise en place de la prothèse. Un point de fixation pourrait être réalisé entre la languette et le muscle pectoral avec un fil mono-filament non résorbable. c/Bilan La création de trois languettes laisserait donc un libre choix au chirurgien de l'endroit de fixation selon la voie d'abord choisie, en coupant celles inutiles. Cependant la possibilité de créer moins de languettes n'est pas exclut. Cela impliquerait, de créer des prothèses avec des languettes spécifiques à la voie d'abord envisagée ; voir même de spécifier, dans le cas de pose de prothèses anatomiques par voie axillaire, quelle prothèse serait destinée à être posée à droite ou à gauche, la languette de fixation devant être accessible. De la même façon, davantage de languettes pourraient être créées pour laisser 20 davantage de choix de fixation au chirurgien ou en vue de techniques de poses novatrices. 25 6/CONCLUSION Mon invention a pour but de répondre à deux problèmatiques pouvant survenir lors de la pose de prothèses mammaires, créant alors, un résultat inesthétique pouvant obliger 5 le chirurgien à reprendre la patiente au bloc opératoire sous anesthésie générale : -Lors de la pose de prothèses mammaires dites anatomiques ou toutes autres prothèses pouvant être crées avec une forme spécifiquement adaptée à celle du sein, ce type de prothèse peut se retourner et déformer ainsi le sein. -Lors de la pose de toutes prothèses mammaires (rondes, anatomiques ou autres), on 10 peut constater que les prothèses peuvent remonter, sortant ainsi de leur loge prévue par le chirurgien. Ce qui est également responsable de déformations du sein. Afin d'éviter ces complications, je propose donc de créer des languettes de fixation aux prothèses qui permettraient aux chirurgiens de les fixer avec un point de suture au niveau thoracique ou musculaire ou glandulaire lors de l'intervention. 15 7/FIGURES FIGURE 1: Vue de face et vue de profil d'un prototype de prothèse anatomique avec languettes de fixation.So my invention comes down to a breast prosthesis comprising, a flexible envelope, very thin and very resistant, formed of a front face and a back side thermo-welded along their edges, filled with a mass of gel bio-medical to have the resistance, the assembly being molded and elastically fixed to the desired shape, this prosthesis is characterized in that the tabs are round, rectangular, square, triangular or other, and the tongues can be perforated or no, and that the tabs protrude from the prosthesis at least 1cm, and that they are composed of the same material as that of the capsule of the prosthesis or another bio compatible material. A stitch can then be made: -for a sub-muscular pose: between the tongue and the periosteum of the ribs or between the tongue and the pectoralis major muscle, with a non-absorbable monofilament thread. -for a sub-glandular pose: between the tongue and the gland or between the tongue and the pectoralis major muscle with a non-absorbable monofilament thread. b / In the case of axillary pathways In this way, only the lateral tongue (right lateral tab for the prosthesis positioned on the right and left lateral tab for the prosthesis positioned on the left) would be useful, the other 2 can be cut with scissors or a lancet, before the prosthesis is put in place. A fixation point could be made between the tongue and the pectoral muscle with a non-resorbable monofilament thread. c / Bilan The creation of three tabs would leave a free choice to the surgeon of the location of fixation according to the chosen approach, cutting unnecessary ones. However the possibility of creating fewer tabs is not excluded. This would imply, to create prostheses with tabs specific to the intended approach; even to specify, in the case of laying of anatomical prostheses by the axillary way, which prosthesis would be intended to be posed on the right or on the left, the tongue of fixing to be accessible. In the same way, more tabs could be created to allow more choice of fixation to the surgeon or for innovative posture techniques. 6 / CONCLUSION My invention aims to answer two problems that may arise during the installation of breast prostheses, thereby creating an unsightly result that may require the surgeon to return the patient to the operating room under general anesthesia: -When placing anatomical breast prostheses or any other prosthesis that can be created with a shape specifically adapted to that of the breast, this type of prosthesis can turn over and thus deform the breast. During the placement of all breast prostheses (round, anatomical or other), it can be seen that the prostheses can go up, thus leaving their box provided by the surgeon. Which is also responsible for breast deformities.
