Description
The present invention relates to a tibial component for a prosthetic reconstruction of the knee joint. In particular, the present invention relates to a monocompartmental prosthesis of the type "cementless" (without cement).
Prostheses for the reconstruction of the knee joint include a tibial component and a corresponding femoral component. These two components include an inner surface for engaging relative bone to be reconstructed and a hinge surface formed to anatomically replace the original joint.
Therefore, in particular, a known tibial component comprises a plate member having an internal tibial anchorage surface and an external coupling surface with a corresponding articular surface associated with the femur and anchoring means for engages the tibial component itself with the tibia.
Such tibial components are now divisible into two groups, that is, those using adhesive / cementitious grip elements on the tibia, or those using mechanical attachment means. These, just to differentiate themselves from the first, are known as "cementless" prostheses (without cement).
In the case where the reconstruction of the knee joint concerns only one of the medial and lateral compartments, the prosthesis is defined as being monocompartmental. Nowadays, cementless prostheses, whether monocompartmental or not, use as common fastening means 25 threaded elements, for example, real screws or its own screws, which engage in the seat, the plate of the tibial component. Such a trick offers, on the one hand, better guarantees in terms of the stable duration of the coupling compared with the use of setting additives such as adhesive / cement additives, which in the long run may indeed lose their effect. and cause detachments and misalignments between the components. However, the use of simple threaded couplings, distributed today commercially, does not allow small adjustments necessary for optimal engagement of the prosthesis on the shin, nor any freedom from the point of view of the positioning of the screws by relative to the relative seat axis. From this known technique, the object of the present invention is to provide a tibial component for a knee joint reconstruction prosthesis which is an alternative to those which are known.
In particular, the object of the present invention is to provide a tibial component which: guarantees better main stability immediately after the intervention and secondary in time, promoting bone growth; reduces the risk of loosening the implant; allows to make alignment adjustments between the screws and the component plate or at least increases the freedom of positioning of the screw relative to the axis of the relative seat. According to the general aspect of the invention, these advantages are achieved in an innovative manner, by proposing: a spherical type coupling between the through-hole 15 and the head 17 of the threaded element 14 in order to allow a first reciprocal snap coupling at a plurality of inclinations a of the threaded element 14 with respect to the axis A of the through-hole 15; means for locking 18 of the threaded element 14 according to a chosen inclination. Other features of the invention provide that: said head of said threaded element has a spherical coupling profile with a corresponding spherical seat arranged in said through hole. the snap coupling between said head of said threaded member and said spherical seat is effected by means of an annular projection (20) arranged above said spherical seat. said locking means of said threaded member at an inclination (a) selected from said axis (A) of said through hole comprises an expansion member of said head of said threaded member against the walls of said spherical seat. said locking means comprise an associated locking screw in a seat arranged in said head of said threaded member and configured so that during clamping it acts in progressive pressure against the inner walls of said head of said threaded member. said locking screw and said inner walls of said head of said threaded member (14) comprise wedge-coupled profiles for transforming the axial movement of said radial expansion locking screw of said head of said threaded member. The features and advantages of a tibial component for a knee joint reconstruction prosthesis according to the present invention will emerge more clearly from the following description, given by way of example and not limitation, with reference to the accompanying drawings, in which: FIG. an exploded perspective view of a tibial component according to the present invention; Figure 2 is a sectional view of an assembled tibial component according to the present invention; Figures 3 to 5 are views of the threaded member proposed by the present invention; and Figures 6 to 8 are views of the threaded member associated with the seat relative to different stages of mounting the prosthesis. With reference to the figures, reference numeral 10 indicates a tibial component for a knee joint reconstruction prosthesis according to the present invention. In general, the present invention finds its optimum application in monocompartmental prostheses, but can also be used for prostheses which concern both the medial and lateral compartments. Such a tibial component 10 comprises: a plate member 11 having an inner tibia anchoring surface 12 and an outer coupling surface 13 with a corresponding surface for the knee prosthesis joint associated with the femur ; anchoring means 14 for engaging the tibial component 10 with the tibia. In a manner known per se, such anchoring means 14 comprise at least not only a threaded element 14 but also a plate element 11 at least 25 having a through hole 15 having an axis A and configured to allow the passage of the body 16 of the threaded element 14 and to block the head 17. In a completely innovative manner, according to the present invention, the through hole 15 and the head 17 of the threaded element 14 are configured so as to spherical coupling to allow a first snap-in mutual coupling 30 at a plurality of inclinations a of the threaded member 14 with respect to the axis A of the through-hole 15. In FIG. B axis 14 of the screw 14 with respect to the axis A of the hole 15. Such an axial coupling makes it possible, on the one hand, to make small alignment corrections and, on the other hand, to increase the freedom of positioning of the screw relative to the axis of the si relative age.
Means 18 are then provided for locking the threaded element 14 in an inclination (a) chosen with respect to the axis A of the through hole 15. In this way, the axial displacements are therefore impossible between the plate and the screw , which instead take place with implants fixed with simple screws.
These axial displacements can cause in time the setting in movement of the base and increase the risk of detachments of the implant. In particular, the snap coupling between the head 17 of the threaded element 14 and the spherical seat 19 is achieved by means of an annular projection 20 arranged above the spherical seat 19.
In this way, once the annular protrusion 20 has passed, the head 17 is locked in the seat 19. The means for locking 18 of the threaded element 14 at a chosen inclination comprise an expansion element of the head 17 of FIG. the threaded element 14 against the walls of the spherical seat 19.
En pratique, non seulement les matériaux utilisés mais également leurs dimensions, peuvent être d'un type quelconque selon les exigences techniques.10In particular, such locking means comprise a locking screw 18 associated in a threaded seat 21 arranged in the head 17 of the threaded element 14 and configured so that during clamping, it acts in progressive pressure against the internal walls of the head 17. In turn, the head 17 thus comes into forced contact against the walls of the seat 19, locking by friction. In particular, the inner walls of the head 17 of the threaded element 14 and the outer walls of the screw 18 comprise wedge-coupled profiles 22, 23 for transforming the axial movement of the locking screw 18 in radial expansion of the head. 17 of the threaded element 14. Further details complete the invention, not only as to provide the spherical head 17 made with edges to obtain a better elasticity, but also the choice of the materials used as the alloy. cobalt chromium for the plate 11 and the titanium alloy either for the main screw 14 or for the locking screw. It has thus been seen that a tibial component for a knee joint reconstruction prosthesis according to the present invention achieves the previously recessed goals. Indeed, the tibial component of the present invention offers an alternative solution to the components used today, and at the same time offers many advantages, such as: the guarantee of a better main stability immediately after the intervention and secondary in time, promoting bone growth; reducing the risk of loosening the implant; the possibility of making alignment adjustments between the screws and the component plate or at least increase the freedom from the point of view of the positioning of the screw relative to the axis of the relative seat. The tibial component for a knee joint reconstruction prosthesis of the present invention so conceived is susceptible to many modifications and variations, all in the same innovative concept; in addition, all details may be replaced by technically equivalent elements. In practice, not only the materials used but also their dimensions, can be of any type according to the technical requirements.
