Abstract

Description

PERFECTIONNEMENT A UNE CAGE INTERVERTEBRALE La présente invention concerne un implant dit "cage intersomatique" ou "cage intervertébrale", destiné à être implanté, généralement par voie postérieure, entre les corps vertébraux de deux vertèbres lombaires consécutives, à la place du disque déficient initialement présent entre les deux vertèbres. Une telle cage permet la fusion des corps vertébraux après discectomie, tout en conservant la distance intervertébrale. Il est déjà connu de telles cages, comme celles qui sont décrites et 10 illustrées dans le FR-A-2 715 293 et le FR-A-2 866 229. Une telle cage selon l'art antérieur est par exemple illustrée sur la figure 1 en annexe de la présente description. Elle comprend un corps en forme de U comportant de ce fait deux branches et une pièce de liaison reliant deux premières extrémités des branches, les faces respectivement 15 des deux branches situées aux secondes extrémités des deux branches et se faisant face l'une l'autre comportant chacune une surface d'appui délimitant un dièdre comprenant une petite base et une grande base, une vis comportant une tête en appui contre la pièce de liaison et une tige de vis définissant un axe longitudinal traversant librement la pièce de liaison et la 20 petite base et passant entre les deux branches, et une pièce d'écartement mobile en translation vissée sur la tige de vis, cette pièce d'écartement étant en contact glissant sur les deux surfaces d'appui. La structure de la cage selon le premier document antérieur cité ci-dessus est illustrée sur la figure 1, par la combinaison des traits continus et 25 interrompus, tandis que la cage selon le second document antérieur cité est illustrée sur cette figure 1 uniquement par la représentation en traits continus. L'homme du métier pourra facilement interpréter cette figure 1 en se reportant d'une part aux deux documents cités ci-dessus et d'autre part à la description donnée ci-après en regard de la figure 2. 30 Ces cages antérieures donnent satisfaction sur le plan médical, mais peuvent présenter un inconvénient, notamment lors d'une manoeuvre inadaptée au cours de leur mise en place. L'invention a donc pour but de réaliser une cage lombaire perfectionnée par rapport à celles qui sont décrites dans les deux 5 documents référencés ci-dessus, pour tenter de limiter autant que possible cet inconvénient. Plus précisément, la présente invention concerne une cage intervertébrale comprenant un corps en forme de U comportant deux branches et une base reliant deux premières extrémités des branches, les 10 faces respectivement des deux branches situées aux secondes extrémités des deux dites branches et se faisant face l'une l'autre comportant chacune une surface d'appui, ces deux surfaces d'appui délimitant un dièdre comprenant une petite base et une grande base, une vis comportant une tête en appui contre la portion de base et une tige de vis définissant un axe 15 longitudinal traversant librement la base et passant entre les deux branches, une pièce d'écartement mobile en translation vissée sur la tige de vis, cette dite pièce d'écartement étant en contact glissant sur les deux surfaces d'appui, ledit dièdre ayant sa petite base tournée vers les premières extrémités des deux branches et sa grande base tournée vers les secondes 20 extrémités de ces deux branches, chacun des deux bords du dièdre correspondant à la grande base étant conformé en cavité concave dont la concavité est tournée vers les premières extrémités des deux branches, la surface concave de chacune des deux dites cavités ayant une forme sensiblement congruente de celle de la surface extérieure de ladite pièce 25 d'écartement, la distance séparant les deux bords libres parallèles des deux cavités étant inférieure à la largeur de la pièce d'écartement prise selon une droite perpendiculaire à l'axe de la tige de vis et au plan des deux branches, les deux couples de portions des deux surfaces d'appui et de la pièce d'écartement qui sont aptes à glisser les unes sur les autres sont soumises 30 à un traitement anti-glissement.
