Abstract
Description
LesThe invention relates to a femoral stem for hip prosthesis. The
Hip prostheses generally include a femoral stem lodged in the medullary canal of the femur at the upper end thereof. The femoral stem is equipped with a prosthetic femoral head cooperating, via an insert, with a cup located in the acetabular cavity of the iliac bone so as to form a ball joint reproducing the natural movement of the hip of the hip. patient. Prior to the installation of the stem, the femur is cut at its upper part so as to define a planar upper surface inclined relative to the axis of the femur. In addition, the medullary canal is generally enlarged using rasps so as to allow the insertion of the rod. In order to guarantee the proper functioning of the prosthesis and to prevent premature wear thereof, it is necessary that the prosthesis is correctly positioned and, in particular, that the positioning of the femoral stem in the medullary canal can be precisely controlled by the surgeon during the pose. For this purpose, certain femoral stems are equipped with a flange forming a positioning stop. Thus, during installation, the rod is inserted into the medullary canal until the collar comes to bear on the aforementioned upper face of the femur. In this case, the femoral stem may not be satisfactorily fixed if the collar reaches the stop before complete insertion of the stem into the medullary canal. Other femoral stems are equipped with laser positioning markings. However, it has been shown that such marking strongly damages the femoral stem so that it presents risks of rupture in the marked areas. The invention aims to overcome these disadvantages by providing a femoral stem whose insertion and positioning relative to the femur can be controlled easily, without risking damaging the stem. To this end, the invention relates to a femoral stem for a hip prosthesis comprising a metaphyseal proximal portion and a diaphyseal distal portion of elongate shape, intended to be introduced into the medullary cavity of a femur, the metaphyseal and diaphyseal parts being connected by A substantially flat interface area corresponding to the femur section, characterized in that the femoral stem has, in its diaphyseal portion, at least one groove or rib at least a portion of which is parallel to the interface and adjacent to this one. The groove or rib thus forms a visual cue allowing the surgeon to easily assess the positioning and insertion of the rod in the medullary canal. In particular, the diaphyseal portion is inserted into the medullary canal until the portion of the groove or rib that is parallel to the interface coincides with the plane defined by the cutting face of the femur. The position of the groove or rib is adapted to ensure sufficient depression of the rod in the medullary canal. In addition, the space between the ribs or grooves can serve as drainage or storage volume for the excess cement, in the case where the rod is attached to the femur by means of cement. It will be recalled that the femoral stem can be fixed in the medullary canal, either by means of cement or by forced insertion without the use of cement. Advantageously, the plane of the interface forms an angle of between 40 and 50, preferably of the order of 45, with the axis of the diaphyseal part. Preferably, the groove or rib parallel to the interface is located at a distance of between 0.5 mm and 3 mm from the latter. This ensures that the groove or rib is completely covered by the femur. According to a first embodiment of the invention, the groove 25 or rib forms a corrugation oriented perpendicularly to the axis of the diaphyseal portion. Such waving makes it possible to favor fixation of the rod when it is forcibly inserted into the medullary canal. According to a possibility of the invention, the rod comprises a plurality of grooves or ribs extending parallel to each other and offset along the axis of the diaphyseal portion. The presence of several grooves or rib ensures increased fixation of the stem in the medullary canal. Advantageously, the rod comprises at least one groove, the bottom of which consists of a succession of alternately inclined planes relative to each other.
According to a second embodiment of the invention, the rod comprises a plurality of grooves or ribs each having two branches joined by a loop-shaped portion having a section parallel to the interface.
In any case, the invention will be better understood with the aid of the description which follows, with reference to the appended schematic drawing showing, by way of nonlimiting examples, several embodiments of this femoral stem. Figure 1 is a front view of a rod according to a first embodiment of the invention; Figure 2 is a view corresponding to Figure 1, a second embodiment of the invention. FIG. 1 represents a femoral stem 1 for a hip prosthesis comprising a metaphyseal proximal portion 2 and a diaphyseal distal portion 3 of elongate shape and defining an axis A. The diaphyseal portion 3 is intended to be introduced into the medullary cavity of an femur (not shown), as is known per se. The metaphyseal 2 and diaphyseal portions 3 are connected by a substantially planar interface area 4 corresponding to the cutting zone of the femur once the stem 1 is in position, as described hereinafter. The plane of the interface 4 forms an angle α of between 40 and 50, preferably of the order of 45, with the axis A of the diaphyseal portion 3. The femoral stem 1 comprises, in its diaphyseal portion 3, a plurality grooves 5 in the form of waves or zig-zag, extending along an axis perpendicular to the axis A of the diaphyseal portion 3. The grooves 5 extend parallel to each other and are offset according to the Axis A of the diaphyseal portion 3. The grooves 5 thus consist of a succession of sections 6 ', 6 ", inclined alternately with respect to each other, a section 6' out of two being parallel to the plane of the As can be seen more particularly in FIG. 2, the sections 6 'parallel to and adjacent to the interface 4 are arranged at a distance d of between 0.5 mm and 3 mm from the latter, preferably of the order of 1 mm The position of the sections 6 'is adapted so as to guarantee insertion 35 sufficient rod 1 in the medullary canal of the femur.
The grooves 5 in the rod 1 comprise a bottom consisting of a succession of alternately inclined planes relative to each other. During the insertion of the rod 1 into the medullary canal, the surgeon can easily appreciate the insertion distance of the rod since, when the sections 6 'disposed near the interface are located in the cutting plane of the femur , the stem 1 is sufficiently pushed into the medullary canal to provide a satisfactory fixation. Once reached this sufficient insertion point, the surgeon can continue inserting the rod a short distance, for example 1 mm, so as to ensure that no grooved area protrudes from the bone. After complete insertion of the rod into the medullary canal, the cutting plane of the femur corresponds substantially to the plane of the interface 4 of the rod 1.
This first embodiment is particularly suitable for laying without cement in the femur, the fixing being carried out by insertion of the rod 1 through the grooves 5, formed on the rod 1, promote fixation. Another embodiment of the invention is shown in FIG. 3. In this variant embodiment, the rod differs in that it comprises a plurality of grooves 5 each comprising two branches 7 joined by a loop-shaped part having a parallel section 9 at the interface 4.
As can be seen more particularly in FIG. 4, the parallel sections 9 are adjacent to the interface 4 and arranged at a distance d of the order of 2.5 mm with respect thereto. Each groove has a rounded section whose radius of curvature r is of the order of 1 mm. As before, the sections 9 parallel to the interface 4 form elements for visualizing the position of the rod 1 in the medullary canal. This second embodiment is particularly suitable for laying with use of cement. In this case, the specific shape of the grooves 5 promotes drainage of the excess cement.
As is obvious, the invention is not limited to the embodiments of this system, described above by way of example, but embraces all variants. It is thus particularly that the rod could be equipped with ribs, or that the shape of the ribs or grooves could be adapted according to the use.
