Abstract
Description
De façonThe present invention relates to a set of hip prostheses. In a way
In conventional practice, a hip prosthesis comprises a rod intended to be introduced into the femur bone, which is extended by a collar, itself terminated by a head. This prosthesis is adapted to cooperate with an acetabulum portion intended to be fixed on the pelvic bone, which comprises a metal cup in the shape of a hemisphere. The latter ensures the housing of an insert, for example plastic or ceramic, in which is articulated the aforementioned head. During the installation of such a prosthesis, the surgeon implants on the one hand the stem inside a cavity hollowed longitudinally in the bone of the femur. Moreover, the cotyloid part mentioned above is received in a globally hemispherical cavity, hollowed out in the pelvic bone. That being said, the invention aims to provide a set of prostheses ensuring optimal adaptation to different morphologies of patients. For this purpose, it relates to a set of hip prostheses, wherein each prosthesis comprises a stem, a neck and a head, characterized in that, in a reference frame, a first axis corresponds to the main axis of the rod and of which a second axis corresponds to the axis perpendicular to said first axis passing through the cortical bearing zone of the prosthesis, the absolute value of the coordinates of the center of said head increases with the value of the thickness of the rod, taken along said second axis. According to other characteristics of the invention: - three values of the ordinate for the center of the head, as a function of the value of the thickness, as well as three values of the abscissa of said center, for each value of the ordinate. If the thickness is less than 26 mm, the coordinates of the center of the head are (45.95 mm, 28 mm) if the thickness is between 26 and 31 mm, the coordinates of the center of the head are (49, 50 mm, 29.8 mm) and if the thickness is greater than 31 mm, the coordinates of the center of the head are (53.50 mm, 31.60 mm). - If the thickness is less than 26 mm, the coordinates of the center of the head are (46.55 mm, 28 mm) if the thickness is between 26 and 31 mm, the coordinates of the center of the head are (50 , 25 mm, 29.8 mm) and if the thickness is greater than 31 mm, the coordinates of the center of the head are (54.20 mm, 31.60 mm). If the thickness is less than 26 mm, the coordinates of the center of the head are (47.15 mm, 28 mm) if the thickness is between 26 and 31 mm, the coordinates of the center of the head are (50, 90 mm, 29.8 mm) and - if the thickness is greater than 31 mm, the coordinates of the center of the head are (55.00 mm, 31.60 mm). Each prosthesis is provided with a collar, and the main dimension of said collar increases with the value of the thickness of the stem; if the thickness is less than 26 mm, the main dimension of the flange is equal to 7.2 mm, - if the thickness is between 26 and 31 mm, the main dimension of the flange is equal to 8.2 mm ; and if the thickness is greater than 31 mm, the main dimension of the flange is equal to 9.2 mm.
The invention will be better understood on reading the description which follows, given solely by way of example and with reference to the appended drawings, in which: FIG. 1 is a perspective view illustrating a hip prosthesis belonging to to a set of prostheses according to the invention; FIG. 2 is a front view illustrating this hip prosthesis and an orthogonal reference mark associated with it; and FIG. 3 is a front view, similar to FIG. 2, illustrating an alternative embodiment of a hip prosthesis belonging to a set of prostheses according to the invention.
Figure 1 illustrates a hip prosthesis belonging to a set of prostheses according to the invention. In a manner known per se, this prosthesis, designated as a whole by the reference 10, comprises a rod 12, of longitudinal axis Y'-Y, intended to be housed and retained in a diaphyseal cavity, hollowed in the medullary canal of a femur not shown. The upper end of the rod is extended by a neck 14, in a direction inclined relative to the aforementioned axis Y'-Y. Finally, a truncated spherical head 16, of axis of symmetry AA, is provided at the free end of the aforementioned neck 14. The prosthesis 10 is intended to cooperate, in a manner known per se, with an acetabulum 18, made in the form of a a substantially hemispherical metal cup, adapted to be secured to the pelvic bone. As variants, it can be provided that the acetabulum is made integrally, or consists of several components. Inside this cup can be housed, in a fixed manner, an insert 20 also hemispherical, 4 made of a plastic or ceramic material. The inner surface of the insert 20 is shaped substantially complementary to the outer surface of the head 16, so that the latter is articulated in the manner of a patella relative to the insert 20 and the acetabulum 18. Figure 2 illustrates the prosthesis 10, shown in Figure 1, which is now seen from the front. We find the rod 12, as well as the neck 14, while the head 16 is not shown. In this figure 2, C is the center of this head. In this figure 2, there is also the main axis of the rod 12, namely Y'-Y, and another axis X'-X, perpendicular to this axis Y'-Y and passing through the zone 22 d cortical support of the prosthesis. This zone 22 corresponds in a conventional way to the limit of the intramedullary portion of the prosthesis. These two axes X'-X and Y'-Y form an orthogonal coordinate system, in which the coordinates of the various elements of the prosthesis 10 can be defined. Thus, in particular x and y are the abscissa and the ordinate of the center. C of the head. According to the invention, this center C is substantially coincident with that of the reconstructed acetabulum, which corresponds in known manner to the center of the prosthetic acetabulum, once implanted. The coordinates of this center C therefore increase with the size of the prosthesis, which is defined by the thickness e of the rod taken along the axis X'-x, namely for a zero ordinate. This means that the higher the value of the thickness e, the higher the value of the coordinates (x, y). Advantageously, it is possible to assign a small number of values to these coordinates, depending on the different values of e.
Thus, it is for example possible to provide three values for the y-coordinate of the center C of the head, depending on whether the value of the thickness e is low, medium or high. Moreover, for each value of this ordinate y, it is possible to provide several values of the abscissa x, for example three different values. Thus, according to a first embodiment of the invention. - if e is less than 26 mm, (x11, y1) = (45.95 mm, 28 mm) - if e is between 26 and 31 mm (X21, Y2) = (49.50 mm, 29.8 mm) and - if e is greater than 31 mm, (x31, y3) = (53.50 mm, 31.6 mm) According to a second embodiment of the invention: - if e is less than 26 mm, (x12, y1) = (46.55 mm, 28 mm) (x22, Y2) _ 25 mm, 28 mm) - s_L e is between 26 and 31 mm, (x23, Y2) = (50.90 mm, 29.80 mm); and - if e is greater than 31 mm, (x33, y3) = (55.00 mm, 31.6 mm).
The latter differs from that shown in FIG. 2, in that it is provided with a collar 24, intended in a manner known per se to allow the prosthesis to take support on the cortical bone. If e is between 26 and 31 mm, (50.25 mm, 29.8 mm); and - if e is greater than 31 mm, (x32, y3) = (54.20 mm, 31.6 mm). Finally, according to a third embodiment of the invention. - If e is less than 26 mm, (x13, y1) = (47,15 Note 1 the length, or principal dimension of this collar 24. Advantageously, the value of this length 1 increases with the thickness e of In this regard, it is possible to provide for example three values of 1, depending on the different thicknesses - if e - if e 8.2 mm, is less than 26 mm then 1 = 7.2 mm; between 26 and 31 mm, then 1 = - if e is greater than 31, then 1 = 9.2 mm.
