Description

L'invention concerne les implants cotyloïdiens destinés à se substituer au
cotyle naturel d'un patient sur lequel on implante une prothèse totale de hanche.
Ces implants se composent habituellement d'une cupule métallique, de forme extérieure sensiblement hémisphérique, et d'un insert garnissant l'espace intérieur de la cupule. L'insert, qui est généralement en céramique ou parfois en polyéthylène, définit un logement à surface sphérique dans lequel vient se loger, et peut se déplacer en rotation, la tête sphérique de la pièce fémorale de la prothèse.
On utilise couramment des implants cotyloïdiens qui ne sont pas ci- mentés dans le bassin du patient.
La majorité sont du type dit Press Fit (emboîtement sous pression entre des volumes à interfaces concentriques), c'est-à-dire qu'ils sont introduits à force dans une cavité de dimensions légèrement inférieures, fraisée dans l'os iliaque à l'emplacement du cotyle naturel. On obtient ainsi une fixation primaire à frottement dur de l'implant cotyloïdien, qui est ensuite complétée naturellement et de façon définitive par la repousse osseuse pénétrant dans des dépressions ménagées sur la surface de la cupule rendant cette surface ru-gueuse. Cette fixation peut éventuellement être complétée par des vis traver- sant le fond de l'implant.
La structure géométrique externe de la cupule de l'implant est donc un élément fondamental pour garantir le succès de l'installation de l'implant cotyloïdien. On considère habituellement qu'une forme hémisphérique de la cupule procure la meilleure tenue pour ce qui est de la fixation primaire.
Toutefois, cette forme hémisphérique présente un défaut fréquent lié à l'asymétrie de développement qui existe entre les parois cotyloïdiennes osseuses antérieures et postérieures. Le développement de la paroi postérieure est presque toujours suffisant pour assurer un bon arrimage de l'implant. En revanche, il est fréquent que le développement de la paroi extérieure soit défi- cient, ce qui entraîne que le bord de la cupule hémisphérique fait saillie par rapport à sa couverture osseuse.
Si cela se produit, il y a un risque de conflit entre le bord antérieur de la cupule et le muscle psoas qui frotte contre lui lors, notamment, des mouve- ments de flexion. Les tendinites par conflit du psoas sont ainsi fréquentes et de traitement difficile.
Le but de l'invention est de proposer une conception d'implant cotyloïdien pour prothèse de hanche qui élimine les risques de conflit entre son bord antérieur et le muscle psoas.
A cet effet, l'invention a pour objet une cupule pour implant cotyloïdien de prothèse totale de hanche, du type comportant une surface extérieure sensiblement hémisphérique sur la périphérie de laquelle sont ménagées de deux à cinq saillies suivant des parallèles de la cupule, définissant des stries et un espace intérieur pour la réception d'un insert destiné à recevoir la tête fémorale de la pièce fémorale de la prothèse, caractérisée en ce que le bord extérieur de la cupule présente une surface arrondie et lisse.
Le bord extérieur arrondi présente de préférence une section transversale en forme de 114 de cercle de rayon compris entre 2 et 4 mm, l'arrondi débute au droit du bord de la strie la plus proche de l'équateur de la cupule, et le centre de courbure de l'arrondi est situé sur la perpendiculaire à la tangente au bord de ladite strie.
De préférence, une partie de la surface extérieure de la cupule n'incluant pas la surface arrondie du bord extérieur présente une rugosité favorisant l'accrochage de la cupule dans le cotyle du patient.
Ladite surface extérieure est recouverte de préférence d'un matériau favorisant la croissance osseuse dans ses parties rugueuses.
La cupule peut comporter des encoches sur sa surface extérieure, lesdites encoches étant absentes sur le bord extérieur arrondi de la cupule.
Lesdites encoches sont de préférence orientées sensiblement selon des méridiens de la surface extérieure de la cupule.
Lesdites saillies ont de préférence des sections transversales sensiblement triangulaires.
La cupule peut comporter des orifices pour le passage de vis d'ancrage osseux.
L'invention concerne également un implant cotyloïdien du type comportant une cupule et un insert garnissant son espace intérieur destiné à recevoir la tête fémorale de la pièce fémorale d'une prothèse de hanche, caractérisé en ce que la cupule est du type précédent.
L'invention concerne également une prothèse totale de hanche du type comportant une pièce fémorale destinée à se substituer à la partie supérieure du fémur d'un patient et un implant cotyloïdien, caractérisée en ce que ledit implant est du type précédent.
Comme on l'aura compris, l'invention consiste essentiellement à pré-voir une surface arrondie et lisse sur le bord antérieur de la cupule, au lieu de la zone anguleuse habituelle. Les risques de contacts entre ce bord antérieur et le muscle psoas sont ainsi limités, et si de tels contacts se produisent leurs conséquences sont moindres.
L'invention sera mieux comprise à la lecture de la description qui suit, donnée en référence aux figures annexées suivantes: - la figure 1 qui montre vue de face une cupule d'implant cotyloïdien selon l'invention; la figure 2 qui montre de façon agrandie le bord antérieur de cette cupule.
De manière classique, la cupule 1 de type press-fit selon l'invention possède une forme extérieure sensiblement hémisphérique, de façon à s'inscrire totalement dans une cavité osseuse cotyloïdienne normale, si nécessaire rectifiée par fraisage au préalable. De manière également connue, son espace intérieur 2 destiné à recevoir un insert en céramique ou en polyéthylène possède, dans l'exemple représenté, une forme sensiblement tronconique dans le fond de la cupule et une forme sensiblement cylindrique avec une lé- gère conicité dans la partie supérieure de la cupule. II doit être entendu que cette forme de l'espace intérieur 2 n'est qu'un exemple, et que toute forme connue serait acceptable pour une cupule selon l'invention, cette caractéristique étant fonctionnellement indépendante de ce qui constitue l'essence de l'invention et sera décrit plus loin.
De manière également connue, la cupule comporte sur sa périphérie une série de quatre saillies de section transversale sensiblement triangulaire suivant des parallèles de la cupule 1. Ces saillies 3, 4, 5, 6 ont pour fonction de participer à l'amarrage de la cupule dans le cotyle, car du fait de leur forme de demi-flèche, elles tendent à pénétrer dans la paroi osseuse. Elles sont réparties, par exemple, entre sensiblement les 5ème et 20ème parallèles de la cupule 1 hémisphérique. Le nombre de quatre est un exemple, ce nombre peut aller de deux à cinq. Une saillie unique serait insuffisamment efficace. Un nombre supérieur à cinq serait inutile: il obligerait soit à ménager des saillies dans une zone plus proche du pôle de la cupule 1, soit à réduire la section des saillies. Dans la première hypothèse, l'efficacité de l'amarrage conférée par les saillies les plus proches du pôle serait médiocre; dans la seconde hypothèse les saillies seraient fragilisées et ne pourraient plus jouer leur rôle de façon satisfai- sante.
Ces saillies 3, 4, 5, 6 permettent de définir des stries 7, 8, 9, 10 sur la surface extérieure de la cupule 1.
Selon l'invention, au lieu de présenter un angle vif comme habituellement, le bord extérieur 11 de la cupule 1 présente une surface arrondie à sec- tion transversale en forme de 1/4 de cercle de rayon R de l'ordre de 3 mm (entre 2 et 4 mm). Cet arrondi débute au droit du bord 12 de la strie 10 la plus proche de l'équateur de la cupule 1.
Le centre de courbure 13 de l'arrondi se trouve sur la perpendiculaire P à la tangente T au bord 12 de la strie 10.
De manière préférée et connue en elle-même, des encoches 14, 15 (au nombre de quatre dans l'exemple représenté, et dont deux sont visibles sur la figure 1) sont ménagées sur la surface de la cupule 1, de préférence selon des méridiens de cette surface. Elles contribuent au maintien de la cupule 1 É dans le cotyle en constituant des zones dans lesquelles s'effectuera une re- pousse osseuse, de façon à prévenir une possibilité de rotation de la cupule autour de son axe polaire. Ces encoches 14, 15 ont une longueur de quelques cm (1 à 2 cm par exemple), une largeur de l'ordre de 2 à 3 mm et une profondeur de l'ordre de 1 mm.
Il est impératif que ces encoches 14, 15 ne débordent pas sur la partie arrondie 11 du bord extérieur de la cupule 1. Dans le cas contraire, on forme-rait des angles vifs sur cette partie arrondie 11, ce qui risquerait de provoquer des conflits avec le muscle psoas que, précisément, l'invention cherche à évi- ter. Dans l'exemple représenté, les encoches 14, 15 s'arrêtent au niveau de la saillie 6 la plus proche de l'équateur de la cupule 1.
La surface extérieure de la cupule 1 présente très préférentiellement une forte rugosité de manière à offrir une multiplicité de sites d'accrochages lors de la croissance osseuse. La figure 2 montre cette rugosité 16. Il est toute-fois impératif que cette rugosité 16 ne soit pas présente sur le bord arrondi 11 de la cupule 1, car elle pourrait blesser le muscle psoas, et ainsi conduire aux problèmes que l'invention cherche à éviter. La surface du bord arrondi 11 doit rester lisse. Les parties rugueuses de la cupule 1 peuvent être recouvertes d'hydroxyapatite ou de toute autre matière favorisant la croissance osseuse.
Comme on le voit sur la figure 1, la cupule 1 peut également comporter des orifices 17, 18 permettant le passage de vis d'ancrage osseux, qui viennent compléter la fixation procurée par les saillies 3, 4, 5, 6, les encoches 14, 15 et la rugosité 16.
Comme on l'a dit, la cupule 1 selon l'invention reçoit classiquement un insert en céramique ou polyéthylène pour former un implant cotyloïdien, qui est à son tour associé à une pièce fémorale pourvue d'une tête sphérique pour former une prothèse totale de hanche.

Claims (10)

REVENDICATIONS
1. Cupule (1) pour implant cotyloïdien de prothèse totale de hanche, du type comportant une surface extérieure sensiblement hémisphérique sur la périphérie de laquelle sont ménagées de deux à cinq saillies (3, 4, 5, 6) suivant des parallèles de la cupule (1), définissant des stries (7, 8, 9, 10), et un espace intérieur (2) pour la réception d'un insert destiné à recevoir la tête fémorale de la pièce fémorale de la prothèse, caractérisée en ce que le bord extérieur de la cupule (1) présente une surface (11) arrondie et lisse.
2. Cupule (1) selon la revendication 1, caractérisée en ce que son bord extérieur arrondi (11) présente une section transversale en forme de 1/4 de cercle de rayon compris entre 2 et 4 mm, en ce que l'arrondi (11) débute au droit du bord (12) de la strie (10) la plus proche de l'équateur de la cupule (1), et en ce que le centre de courbure (13) de l'arrondi (11) est situé sur la perpendiculaire (P) à la tangente (T) au bord (12) de ladite strie (10).
3. Cupule (1) selon la revendication 1 ou 2, caractérisée en ce qu'une partie de sa surface extérieure n'incluant pas la surface arrondie (11) du bord extérieur présente une rugosité (16) favorisant l'accrochage de la cupule (1) dans le cotyle du patient.
4. Cupule (1) selon la revendication 3, caractérisée en ce que ladite sur20 face extérieure est recouverte d'un matériau favorisant la croissance osseuse dans ses parties rugueuses (16).
5. Cupule (1) selon l'une des revendications 1 à 4, caractérisée en ce qu'elle comporte des encoches (14, 15) sur sa surface extérieure, lesdites en-coches étant absentes sur le bord extérieur arrondi (11) de la cupule (1).
6. Cupule (1) selon la revendication 5, caractérisée en ce que lesdites encoches sont orientées sensiblement selon des méridiens de la surface extérieure de la cupule (1).
7. Cupule (1) selon l'une des revendications 1 à 6, caractérisée en ce que lesdites saillies (3, 4, 5, 6) ont des sections transversales sensiblement 30 triangulaires.
8. Cupule (1) selon l'une des revendications 1 à 7, caractérisée en ce qu'elle comporte des orifices (17, 18) pour le passage de vis d'ancrage osseux.
9. Implant cotyloïdien du type comportant une cupule (1) et un insert garnissant son espace intérieur (2) destiné à recevoir la tête fémorale de la pièce fémorale d'une prothèse de hanche, caractérisé en ce que la cupule (1) est du type selon l'une des revendications 1 à 8.
10. Prothèse totale de hanche du type comportant une pièce fémorale destinée à se substituer à la partie supérieure du fémur d'un patient et un implant cotyloïdien, caractérisée en ce que ledit implant est du type selon la revendication 9.
