Cette tige fémorale pour prothèse de hanche, destinée à être enfouie dans le fémur à prothéser, se prolonge par un col prothétique, la tige présentant une zone proximale (3), une zone diaphysaire (4), et une zone distale (5). De manière connue, une prothèse de hanche est constituée d'une tige fémorale destinée à être insérée dans le canal médullaire du fémur de l'articulation de la hanche considérée, et un col prothétique, surmonté d'une tête, faisant ou non partie intégrante de la tige, et destiné à coopérer à son extrémité 10 avec un noyau de frottement, le plus souvent réalisé en polyéthylène, lui-même destiné à coopérer avec la cavité cotyloïidienne de l'os iliaque de la hanche considérée, au niveau de laquelle est préalablement inséré ou mis en place un cotyle de forme appropriée.
La tige faisant l'objet de l'invention est plus particulièrement adaptée dans le cas de fracture du grand trochanter. Dans cette hypothèse en effet, il peut être utile de consolider la fracture par le biais de câbles de cerclage reliant la tige au grand trochanter. En d'autres termes, la tige doit être adaptée pour permettre à la fois l'appui du grand trochanter et le passage de câbles de cerclage. 20 Une telle prothèse est par exemple décrite dans le document US-A-4 163 292. Ce document décrit en effet une tige, dont la zone proximale est munie de cinq orifices traversant respectivement, trois orifices ménagés de la face antérieure à la face postérieure, et deux orifices agencés sur la face externe 25 de la zone proximale. Cette tige est plus spécifiquement adaptée pour les fractures intertrochantériques, dites "comminutives" nécessitant la résection de la tête du fémur, d'une partie du grand trochanter et de la totalité du petit trochanter. La tige présentée dans ce document présente une zone proximale de section supérieure à celle du reste de la tige, de sorte qu'elle occupe un volume important incompatible avec le traitement des fractures simples du grand trochanter. En outre, cet implant présente une forme telle, qu'elle rend difficile son impaction vers le grand trochanter, dans la mesure où l'implant est dépourvu d'une configuration propre à permettre l'insertion de la tige dans le 5 fémur à prothéser. Qui plus est, cet implant présente un encombrement antéropostérieur rendant difficile la synthèse du trochanter sur la tige. Enfin, sa partie distale n'assure pas une stabilité primaire suffisante avant cimentation.
En d'autres termes, le problème posé est de développer une tige qui soit 1o adaptée au traitement des fractures per-trochantérienne.
Pour ce faire, l'invention a pour objet une tige fémorale destinée à être enfouie dans le fémur à prothéser, se prolongeant par un col prothétique, la tige présentant une zone proximale, une zone diaphysaire, une zone distale, la zone 15 proximale étant munie d'orifices traversants destinés au passage de câbles de cerclage pour fixer ladite tige au grand trochanter.
Cette tige se caractérise en ce qu'elle est munie d'une collerette d'appui séparant la zone proximale de la zone diaphysaire, et en ce que les faces 20 antérieure et postérieure, au niveau de la zone proximale, sont situées dans le même plan que celles de la zone diaphysaire, dans sa section la plus large.
Selon une première caractéristique, la collerette d'appui est ménagée sur partie de la périphérie de la tige.
Dans un mode de réalisation avantageux, la collerette est ménagée uniquement sur tout ou partie des faces antérieure et postérieure de la tige.
Pour permettre le passage des câbles de cerclage, la zone proximale est munie de trois orifices traversants, ménagés de part et d'autre des faces antérieure et postérieure, et alignés sur toute la hauteur de la zone proximale. La répartition de ces orifices permet donc d'obtenir une fixation homogène de la tige au grand trochanter.
Dans un mode de réalisation particulier, les deux orifices inférieurs sont de forme oblongue, permettant le passage d'un passe-fil, facilitant ainsi la synthèse par cables ou fils métalliques.
Pour permettre une impaction optimale, la partie supérieure de la zone proximale présente une section moindre par rapport à la section de la partie inférieure. En d'autres termes, la zone proximale présente deux sections distinctes, respectivement une première section, supérieure à celle de la zone 15 diaphysaire dans sa partie de plus grande section, et une seconde section, de taille inférieure au niveau de la zone supérieure. Avantageusement, le troisième orifice le plus haut est ménagé dans la partie supérieure de la zone proximale, c'est-à-dire dans une zone de moindre épaisseur.
L'invention ressortira bien des exemples de réalisation suivants à l'appui des figures annexées.
La figure 1 est une représentation de la face antérieure d'une tige fémorale droite selon l'invention.
La figure 2 est une vue de profil de la tige de l'invention.
La figure 3 est une représentation schématique de la face interne de la tige de l'invention.
La figure 4 est une représentation schématique de la face externe de la tige de l'invention.
La tige fémorale de l'invention telle que représentée sur la figure 1 désignée par la référence générale (1) se prolonge au niveau de son extrémité supérieure par un col prothétique (2). Cette tige fémorale présente trois zones distinctes, respectivement une zone proximale (3), une zone diaphysaire (4) et une zone distale (5).
Selon une première caractéristique de l'invention, visible sur la figure 1, la zone proximale est séparée de la zone diaphysaire par une collerette d'appui (6).
Comme représenté sur la figure 2, cette collerette d'appui (6) est ménagée sur 0o les faces antérieure (7) et postérieure (7') de la tige pour permettre un appui diaphysaire optimal.
Toujours selon l'invention et comme il apparaît sur les figures 2, 3 et 4, les faces antérieure (7) et postérieure (7') de la zone proximale (3) sont situées dans 15 le même plan que celles de la zone diaphysaire dans sa section la plus large (8).
La zone proximale (3) présente en outre trois orifices distincts (9, 10, 11) alignés sur toute la hauteur de la zone proximale (3). Les orifices (9, 10) sont ménagés dans la partie inférieure de la zone proximale (3) et sont de forme oblongue pour permettre le passage des câbles de cerclage. L'orifice (11) est 20 quant à lui ménagé dans la partie supérieure de la zone proximale dans une zone de moindre épaisseur (12). La tige est par ailleurs munie de deux rainures opposées (13) favorisant sa tenue dans le ciment et ce, de manière connue.
En pratique, le grand trochanter est partiellement réséqué par le praticien, 25 puis la tige est mise en place dans le canal médullaire du fémur. La tige repose alors en appui par le biais de la collerette sur partie de la périphérie du grand trochanter. La qualité de positionnement de la tige est renforcée par la mise en place de câbles passant à travers les différents orifices et autour du grand trochanter.
La partie métaphysaire de la tige est conformée de telle sorte à permettre l'usage d'un impacteur, propre à favoriser le positionnement postérieur de la tige au contact du grand trochanter.
En outre, la partie la plus proximale (14) de la tige est de forme bitulipée, et ce, selon les deux plans perpendiculaires passant pas sa médiane. Ce faisant, cette forme confere à la tige une stéréo - stabilité primaire de l'implant.

Claims (8)

REVENDICATIONS
1/ Tige fémorale pour prothèse de hanche, destinée à être enfouie dans le fémur à prothéser, se prolongeant par un col prothétique, la tige présentant une zone 5 proximale (3), une zone diaphysaire (4), et une zone distale (5), la zone proximale (3) étant munie d'orifices traversants (9, 10, 11), destinés au passage de câbles de cerclage pour fixer ladite tige au grand trochanter dudit fémur à prothéser, caractérisée en ce qu'elle est munie d'une collerette d'appui (6) séparant la zone proximale (3) de la zone diaphysaire (4), et en ce que les faces 10 antérieure (7) et postérieure (7'), au niveau de la zone proximale, sont situées dans le même plan que celles de la zone diaphysaire (4) dans sa section la plus large.
2/ Tige fémorale selon la revendication 1, caractérisée en ce que la collerette S5 d'appui (6) est ménagée sur partie de la périphérie de la tige.
3/ Tige fémorale selon la revendication 1, caractérisée en ce que la collerette d'appui (6) est ménagée uniquement sur toute ou partie des faces antérieure et postérieure de la tige.
4/ Tige fémorale selon la revendication 1, caractérisée en ce que la zone proximale (3) est munie de trois orifices traversants (9, 10, 11), ménagés de part et d'autre des faces antérieure (7) et postérieure (7'), alignés sur toute la hauteur de la zone proximale.
5/ Tige fémorale selon la revendication 4, caractérisée en ce que les deux orifices inférieurs (9, 10) sont de forme oblongue.
6/ Tige fémorale selon la revendication 1, caractérisé en ce que la partie supérieure de la zone proximale (3) présente une section moindre par rapport à sa partie inférieure.
7/ Tige fémorale selon l'une des revendications 4 à 6, caractérisé ce que le troisième orifice (11) est ménagé dans la partie supérieure de la zone proximale.
8/ Tige fémorale selon l'une des revendications 1 à 6, caractérisée en ce que, io la partie la plus proximale (14) de la tige est de forme bitulipée, et ce, selon les deux plans perpendiculaires passant pas sa médiane, de telle sorte à conférer à la tige une stéréo - stabilité primaire.
