PRÉSENTATION D'UN ÉCARTEUR SPÉCIFIQUE <B>APPELÉ</B> HÀIVIAC-ECARTEUR Le présent appareil est un dispositif pour faciliter les interventions chirurgicales effectuées sous vidéo-coeliochirurgie. Rappelons que ce type d'intervention est réalisée par l'intermédiaire de trocarts optiques et de trocarts opérateurs par lesquels sont introduits dans la cavité péritonéale insufflée sous pression constante, les différents instruments chirurgicaux. La chirurgie sous vidéo-coelioscopie est aisément réalisée dans l'étage sus-mésocolique qui correspond à la partie supérieure de l'abdomen. La nécessité d'avoir recours à des positions extrêmes en cours d'intervention est parfois impérative. Ce positionnement du patient allongé sur la table d'opération en position déclive correspond à ce que l'on appelle la position de Trendelenburg. Dans cette position, les anses intestinales suivent la déclivité et vont se positionner dans la partie supérieure de l'abdomen. Il est important de savoir que cette position de Trendelenburg est parfois gênante puisqu'elle interfère sur la respiration du patient, elle modifie l'hémodynamique vasculaire et cardiaque. Cette position de Trendelenburg ne doit pas dépasser 20 . Chez certains patients, il est même impossible de réaliser ce positionnement. On comprend aisément que la chirurgie sous vidéo-coelioscopie dans ces conditions, soit rendue difficile puisque les anses intestinales sont toujours présentes dans le champ opératoire. Par ailleurs, meure avec des positions satisfaisantes, la mobilité des anses intestinales, voire du grand épiploon telle que la gêne est fréquente. Certes, il est possible en rajoutant des trocarts et des pinces supplémentaires de pallier à cet inconvénient mais la gestuelle est toujours limitée gênant ainsi la bonne ergonomie du chirurgien. Admittedly, it is possible by adding extra trocars and forceps to overcome this disadvantage, but the gesture is always limited thus impeding the good ergonomics of the surgeon.

L'extrémité du HAMAC est ressortie sur la paroi abdominale controlatérale et est fixée. Bien entendu, le degré et l'importance de l'écartement suspension dépend de la manière dont est positionné le HAMAC à l'intérieur de la cavité péritonéale. Le HAMAC-ECARTEUR peut être effectué soit, dans du matériel restérilisable mais il est souhaitable qu'il soit effectué dans un matériel à usage unique. Le HAMAC lui-même, doit être confectionné dans un matériau compatible avec les viscères abdominaux (nylon, polypropylène, téflon...). Les mailles doivent être distantes de 1,5 cm environ permettant ainsi aisément le passage à travers les mailles d'éventuels instruments introduits en deçà de la suspension. Le HAMAC doit être préformé, de façon à avoir un certain galbe optimisant ainsi la suspension.
