Abstract
Description
The invention relates to the field of ophthalmology and more specifically relates to a scleral distension ring comprising locating means, suitable for the treatment of presbyopia (ADSTP), with possibly additional means for enhancing the effect of said ring .
Suction rings are known parts and used in corneal surgery techniques. They are formed by two vertical circular bands joined by a horizontal crown, the bands thus delimiting an annular chamber, the open part of which is applied to the sclera of the eye. The chamber is in communication with a vacuum pump via a small metal tube and a flexible plastic fitting. Before an intervention on the cornea, the practitioner places this suction ring on the sclera of the eye. Then, when the vacuum pump is started, a vacuum is established in the annular chamber which sucks the sclera on contact, causing an increase in intraocular pressure. The eyeball is therefore held firmly and allows the surgeon to intervene more easily on the cornea for example for the placement of intra-corneal rings intended for the treatment of weak myopias.
The function of this suction ring is therefore well known and its use has been generalized for this type of intervention.
However, the Applicant has found that, under certain conditions, in particular by positioning such a suction ring very precisely on the sclera, and more precisely by using a ring of dimension and shape better suited to the morphology of the patient, the ring in question led, opposite the annular chamber, to a scleral expansion opposite the peripheral zone of the lens. The Applicant has also found that by maintaining the suction ring on the eye for a well-defined period of time, this scleral distension becomes remanent for a long time. This scleral distension which increases the space between the lens and the ciliary body gives the zonule more room and force to pull on the equator of the lens, which goes in the direction of an increase in its efficiency and its action on the change in the shape of the lens and accommodation. One of the merits of the Applicant has therefore been to imagine and develop a scleral distension ring which accentuates this phenomenon in a localized and persistent manner, inducing a modification of the convexity of the lens, that is to say a suppression at the less partial to presbyopia. Possible additional means have also been devised to reinforce these effects, by further accentuating the scleral distension caused by this ring, with a view to the treatment of more serious presbyopia.
The main object of the invention is therefore a scleral distension ring for the treatment of presbyopia, formed by a horizontal crown framed by vertical bands which delimit an annular chamber communicating by a tube with a vacuum pump the ring used being adapted to the morphology of the patient and having means for its location and positioning relative to the limbus and the cornea.
Possible additional means reinforce the effects of said scleral distension ring.
According to a particular characteristic of the invention, the means for locating and positioning the ring consist of markers and / or a diametral cross at the center of the ring, the markers advantageously being small diametrically opposite small plates extending the crown. inward of the ring.
According to another particular characteristic of the invention, the possible additional means for reinforcing the effects of the scleral distension ring consist of intra-corneal rings having an appropriate shape capable of inducing mechanical deformations of the sclera. Other characteristics and advantages of the invention will be better perceived on reading the description which follows of an embodiment taken by way of nonlimiting example in which reference is made to the appended drawings which represent FIG. 1, a schematic perspective view from below of a scleral distension ring Figure 2, the same ring seen from above The distension ring 1, presented in Figures 1 and 2, is a metal ring intended to be placed on the eyeball to about 2 mm from the limbus. I1 is formed by a horizontal crown 2 framed by two vertical circular bands 3. The crown and the two bands define an annular chamber 4. The outer edges 5 of the vertical bands 3 are slightly offset in height in order to be able to match the roundness of the globe eye when the ring is in place. In the crown 2 is provided an orifice 6 communicating with a small metal tube 7 connected by a flexible connector 8 to a vacuum pump not shown, to establish a vacuum in the chamber 4.
This scleral distension ring is equipped with locating and positioning means by which it can be placed precisely with respect to the limbus and the cornea. The means are essentially markers formed from small diametrically opposite small plates 9 extending the crown 2 towards the inside of the ring. These markers are used to center the ring relative to the limbus. We see in Figure 2 that another positioning means consists of a diametral spider 10, to place it in the center of the ring, symmetrically on either side of the cornea.
Such a scleral distension ring must be, in its shape, its dimensions and its size, adapted to the morphology of the patient, for the treatment of presbyopia. Thus the practitioner will have a plurality of rings suitable for each, including oval rings adapted to certain oval corneas with large horizontal axis. By way of example, it is indicated that the internal diameter of the rings varies from 11.5 to 12.5 mm, the external diameter varying from 15.5 to 17.5 mm, which saves a space of approximately 4 mm between the two. ring edges.
The scleral distension ring is therefore positioned precisely on the eyeball, relative to the limbus and the cornea using the aforementioned means. Then a vacuum is established in the chamber 4 which sucks the sclera on contact. This depression is maintained for about two minutes on the eye to cause a persistent distension of the sclera which attenuates presbyopia.
To accentuate this distension, in the case of the treatment of more serious presbyopia, complementary means can be used. Thus the implant of intra-corneal rings is advantageous not in terms of their optical effects on vision, but because of the mechanical deformations they induce on the sclera, which go in the direction of the scleral distension caused by the distension ring. The intra-corneal rings known for the treatment of myopia are modified accordingly by giving them an appropriate shape.
