Abstract

Starting at the external face (2a), recesses (5) are formed into the outer cup wall. Polyethylene forming the molding (3) is cross-linked and heated to softening temperature of 130 deg C. The cup (2) is engaged into its cavity. The outer molding is then clamped radially inwards, whilst still at temperature. Polyethylene flows into the recesses (5) of cup. The assembly is cooled, maintaining clamping. Preferred features: Additional operations include subjecting the molding to hard vacuum long enough to desorb oxygen contained in the polyethylene, e.g. for 48 h. Once irradiation is complete, the molding is placed in an atmosphere of pure oxygen, to saturate it with oxygen. The recesses are notches with rounded bases, distributed equally around the circumference of the cup, orientated towards its meridian lines. A flange (8) covers part of the cup face (2a), delimiting the cavity opening. The flange prevents any direct contact between the neck of the femoral shaft and the cup. The flange has a circular internal face of conic form, angled at 100 deg -120 deg . The face is formed with a cylindrical portion in the side of the cavity opening.

Description

The present invention relates to a method of manufacturing a cotyloid implant for a hip prosthesis.

Such an implant generally comprises a shell of more or less hemispherical shape and a polyethylene piece intended to be engaged and retained in the shell. The shell is intended to be placed in the acetabular cavity of the joint to be fitted, and the polyethylene piece is intended to receive with possibility of articulation, in a cavity which it delimits, the spherical head of an implanted medullary rod. on the upper end of the femur.

Such a polyethylene piece allows, by the relative flexibility of its constituent material, to give damping properties to the prosthetic joint. However, it has the drawbacks of wearing out and creeping in the long run under the effect of the repeated stresses undergone by the joint. The wear results mainly from micro-movements of the part relative to the shell. It leads to a clearance of your femoral head in said part, which adversely requests the anchoring of the shell and can lead to a separation of the latter. Creep results from the play that may exist between the shell and the polyethylene part.

To remedy these wear and creep problems, it has been designed to equip a polyethylene part with a metal or ceramic cup, this cup itself delimiting the articulation cavity.

However, it appears that the problems of micro-movements of the cup relative to the polyethylene part or of residual play between this part and the cup have not been satisfactorily resolved to date, so that there still exists a risk of wear and creep of this part.

The object of the present invention is to remedy this essential problem.

-aménager des évidements dans la paroi de la cupule, à partir de la face extérieure de cele-ci ;
-réaliser la réticulation du polyéthylène constituant ladite pièce ;
-chauffer cette pièce jusqu'à une température de malléabilité du polyéthylène, c'est-à-dire sensiblement 130 ;
-engager la cupule dans la cavité de cette pièce ;
To this end, the process which it concerns comprises the operations consisting in:
- arrange recesses in the wall of the cup, from the outer face thereof;
performing the crosslinking of the polyethylene constituting said part;
-heating this part up to a malleability temperature of the polyethylene, that is to say substantially 130;
-engage the cup in the cavity of this part;
-exercise on said part, while the latter is still at the aforementioned temperature, a radial tightening inwards, so as to cause a creep of the polyethylene in the recesses of the cup, and
-allow cooling of the polyethylene piece while maintaining this tightening or during this cooling.

The crosslinking of the polyethylene makes it possible to increase) the temperature at which this polyethylene becomes malleable; the additional heating necessary to obtain this malleability makes it possible to obtain an additional expansion of the polyethylene, due to this heating.

This additional expansion has the effect, after cooling, of correlatively increasing the shrinking stress exerted by the polyethylene piece on the cup and consequently ensures a perfect connection of this piece with this cup.

Crosslinking also makes it possible to "freeze" the stresses caused in the polyethylene by tightening and to significantly reduce the deformation capacity of this polyethylene by creep.

The creep of the polyethylene around the cup and in the recesses thereof makes it possible to obtain large contact and support surfaces between these two parts as well as a perfect complementarity of shape between them.

The implant obtained by the method according to the invention thus has perfect cohesion of the polyethylene part and of the cup, which eliminates the risks of wear or creep of the polyethylene while retaining damping properties at the joint. prosthetic thanks to the polyethylene piece.

The crosslinking of the polyethylene is preferably carried out by irradiation from an appropriate radioactive source, in particular by irradiation with cobalt between 35 and 100 kilo-grays.

-avant l'irradiation, à mettre ladite pièce sous un vide poussé pendant une durée suffisante pour la désorption de l'oxygène contenu dans le polyéthylène, à savoir une durée de 48 heures ;
Advantageously, the method further comprises the operations consisting,
before irradiation, putting said part under a high vacuum for a period sufficient for the desorption of the oxygen contained in the polyethylene, namely a period of 48 hours;
- once the irradiation has been carried out, putting said part in an atmosphere saturated with oxygen to allow the saturation of the polyethylene with oxygen.

The vacuuming of the part makes it possible to eliminate the oxygen absorbed naturally by the polyethylene and therefore to eliminate any risk of chemical reactions or harmful physicochemical modifications likely to be generated by the irradiation.

The method advantageously comprises the operation consisting in arranging said recesses in the form of notches with rounded bottoms, arranged equidistantly around the periphery of the cup and oriented along meridians thereof.

The recesses thus formed promote the creep of the polyethylene and allow a perfect anti-rotation connection of the cup relative to the polyethylene part.

The method also advantageously comprises the operation consisting in arranging an at least partial covering flange of the face of the cup delimiting the opening of the cavity of this cup, this flange being shaped to prevent any direct contact with the neck of the femoral stem. with the cup.

This collar is advantageously arranged so as to have an inner circular face of conical shape, the slope of which is of the order of 100 to 120.

This face makes it possible to guide the femoral head reliably in the cavity of the cup in the event of slight dislocation of the prosthetic joint.

In addition, the face of said part which delimits the cavity intended to receive the cup can be made with a cylindrical portion on the side of the opening of this cavity.

The increased thickness of the part resulting from this cylindrical portion makes it possible to achieve the above-mentioned creep of the polyethylene in an optimal manner.

For a good understanding, the invention is again described below with reference to the appended diagrammatic drawing representing, by way of nonlimiting example, an acetabular hip prosthesis implant obtained by the manufacturing process which it relates to.

La figure 1 est une vue en perspective de deux pièces destinées à tre assemblées pour constituer un sous-ensemble de cet implant ;
FIG. 1 is a perspective view of two parts intended to be assembled to constitute a sub-assembly of this implant;
FIG. 2 is a front view of this sub-assembly placed in a clamping tool, and
Figure 3 is a sectional view of said sub-assembly after assembly.

FIG. 3 represents the insert 1 of an acetabular implant of hip prosthesis. This insert 1 is intended, according to a conventional technique, to be engaged and retained in the cavity of a shell (not shown) of more or less hemispherical general shape, this shell being intended to be placed in the acetabular cavity of the joint to be fitted.

The insert 1 is constituted by a cup 2 made of metal or ceramic and by a piece 3 of crosslinked polyethylene.

As the figures show, the cup 2 has a general hemispherical shape and delimits a hemispherical cavity 4. This cavity 4 is intended to receive, with possibility of articulation, the spherical head of a medullary rod implanted on the upper end of the femur.

This cup 2 comprises three notches 5 with rounded bottoms, arranged from its outer face, equidistantly around its periphery. These notches are arranged along meridians of the cup 2 and open into the face 2a thereof which delimits the opening of the cavity 4.

The part 3 defines a substantially hemispherical cavity 6 for receiving the cup 2 and comprises three protrusions 7 of its constituent material, engaged in the three notches 5.

The part 3 also has a collar 8 for partially covering the face 2a. This flange 8 is shaped to prevent any direct contact of the neck of the femoral stem of the prosthesis with the cup 2 and has an inner circular face 8a of conical shape, the slope of which is of the order of 110. 3This face 8a makes it possible to reliably guide the femoral head in the cavity 4 in the event of slight dislocation of the prosthetic joint.

In addition, the part 3 comprises a smooth outer face and a rim 9 projecting outwardly for its support against the metal shell.

-mettre la pièce 3 sous un vide poussé pendant une durée suffisante pour la désorption de l'oxygène contenu dans le polyéthylène, à savoir une durée de 48 heures ;
-réaliser la réticulation du polyéthylène par une irradiation au cobalt entre 35 et 100 kilo-grays ;
-mettre la pièce 3 dans une atmosphère saturée en oxygène pour permettre la saturation du polyéthylène en oxygène ;
-chauffer cette pièce 3 jusqu'à une température de malléabilité du polyéthylène, c'est-à-dire sensiblement 130 ;
-engager la cupule 2 dans la cavité ;
-placer l'ensemble dans un outil de serrage radial de la pièce 3 contre la cupule 2, cet outil comprenant, ainsi que le montre la figure 2, quatre mors 10 dont chacun présente une face de serrage en quart d'hémisphère ;
-exercer sur ladite pièce 3, alors que celle-ci est toujours à la température précitée, un serrage radial vers l'intérieur, de manière à provoquer un fluage du polyéthylène dans les encoches 5, pour constituer les saillies 7 ;
The process for manufacturing the insert 1 comprises the operations consisting in:
put the part 3 under a high vacuum for a period sufficient for the desorption of the oxygen contained in the polyethylene, namely a period of 48 hours;
-realize the crosslinking of the polyethylene by irradiation with cobalt between 35 and 100 kilo-grays;
put the part 3 in an atmosphere saturated with oxygen to allow the saturation of the polyethylene with oxygen;
-heating this part 3 to a malleability temperature of the polyethylene, that is to say substantially 130;
-engage the cup 2 in the cavity;
place the assembly in a radial clamping tool of the part 3 against the cup 2, this tool comprising, as shown in FIG. 2, four jaws 10 each of which has a clamping face in quarter of a hemisphere;
-exercise on said part 3, while the latter is still at the aforementioned temperature, a radial tightening inwards, so as to cause a creep of the polyethylene in the notches 5, to constitute the projections 7;
fitting out the flange 8 by molding produced by clamping using another tool, and
-allow the cooling of part 3 while maintaining this tightening or during this cooling.

The implant obtained by the method according to the invention thus has perfect cohesion of the part 3 and the cup 2, which eliminates the risks of wear or creep of the polyethylene while retaining damping properties at the joint. prosthetic thanks to part 3.

It goes without saying that the invention is not limited to the embodiment described above by way of example but that it embraces, on the contrary, all the variant embodiments. Thus, the part 3 can be designed to be fixed directly to the bone by cement, and then comprises an outer face suitably shaped, in particular provided with grooves for the creep of the cement.
